With that clarification out of the way its time for me to update an old post which summarized recommendations on this topic. That post presented an evidence based method for daily extubation assessment of mechanically ventilated patients, focusing on the spontaneous breathing trial (SBT). Although I stand by the premise of that post it needs updating in light of this study which looked at integrating the daily SBT with a daily spontaneous awakening trial (SAT) as a protocol. Given that daily SATs (sedation interruptions) and SBTs are commonplace (or should be) in mechanically ventilated patients whats so new and different about this? I dont know for sure, but when those two procedures were organized in the form of an explicit protocol they out performed usual care in several metrics including mortality (NNT=7!). Fellows are responsible for organization of this conference, where an unknown case is presented to the attending physicians who then discuss differential diagnosis, diagnostic evaluation, and initial treatment recommendations. Following this discussion, the findings and outcome are presented by the fellow, after which he/she presents an in-depth review of the literature pertaining to the case.
Critical reviews of research and the medical literature form an integral part of the daily ICU bedside teaching rounds and didactic conferences. Each Monday both administrative and research issues are addressed at the Trauma/Critical Care Faculty Meeting. In addition, changes in practice and proposed research topics are discussed in detail at the monthly Practice Management Guidelines meeting. Recently implemented Surgical Critical Care Conference and the Trauma Operative Conference deliver Grand Rounds-quality lectures by the institutions (and visiting) Faculty that are aimed specifically at "fellow-level" education (PGY-6 and above). The SCC resident is expected to deliver at least one Grand Rounds lecture throughout the year. The exception to this rule is if the set_page_dirty caller is racing with truncate or invalidate. A buffer can not be set dirty if it is not uptodate. If either of these situations occurs, it indicates there could be some data loss problem. Some of these warnings could be a harmless one where the page or buffer is set uptodate immediately after it is dirtied, however we should fix those up, and enforce this ordering. Bring the order of operations for truncate into line with those of invalidate. This will prevent a page from being able to go !uptodate while were holding the tree_lock, which is probably a good thing anyway.
