INTRODUCTION: Adherence to diabetes mellitus treatment regimens among Type 2 diabetes patients in Tanzania has not been well documented. This study sought to assess adherence to antidiabetic drugs and associated factors among patients with Type 2 diabetes mellitus. METHODS: A cross-sectional study was conducted among type 2 diabetes mellitus patients who were attending the Diabetic clinic of Muhimbili National hospital between May 2009 and February 2010. Assement ofadherence to antidiabetic medications was based on patients self-reported recall of skipped days without taking medications, over the past one week and three months. Data wereentered and analyzed using Statistical Package for Social Sciences (SPSS Inc. Chicago, Illinois version 16). The crude and adjusted odds ratio (COR/ AOR) and 95% Confidence Interval (CI) were performed to determine factors associated with anti-diabetic medications adherence and a p-value of 0.05 or less was considered statistically significant. RESULTS: Adherence rates

We conducted a stratified, cluster-designed, serial cross-sectional household study from 2014-2015 in the Kilimanjaro Region, Tanzania. We used a three-stage cluster probability sampling method to randomly select individuals. To estimate prevalence, we screened individuals for glucose impairment, including diabetes, using hemoglobin A1C. We also screened for hypertension and obesity, and to assess for potential complications, individuals with diabetes were assessed for retinopathy, neuropathy, and nephropathy. We enrolled 481 adults from 346 urban and rural households. The prevalence of glucose impairment was 21.7% (95% CI 15.2-29.8), which included diabetes (5.7%; 95% CI 3.37-9.47) and glucose impairment with increased risk for diabetes (16.0%; 95% CI 10.2-24.0). Overweight or obesity status had an independent prevalence risk ratio for glucose impairment (2.16; 95% CI 1.39-3.36). Diabetes awareness was low (35.6%), and few individuals with diabetes were receiving biomedical treatment (33.3%). ...
The study findings indicate that the overall patients level of satisfaction on the quality of care at the OPD in Mwananyamala hospital was relatively low. This is in line with findings of patients satisfaction studies elsewhere [20, 21]. However, this is contrary to a study done in Muhimbili National Hospital in Dar es Salaam, whereby a high proportion of patients were satisfied with quality of care [22]. In fact, it is not generally proven that patients satisfaction is related to quality as reported by Leonard (2008) in a study done in Arusha Tanzania. In this study Leonard showed that, in the Tanzanian context, satisfaction is not directly associated with quality, but changes in quality do lead to changes in satisfaction, as patients notice improvements [5]. Of all five dimensions used to assess patients level of satisfaction, respondents were least satisfied with the assurance dimension, followed by reliability, tangibles, empathy, and responsiveness. According to Donabedian model [12] ...

An assessment was done in Tanzania to determine the extent to which the primary health care services have contributed to reducing the burden of malaria since the system was initiated in the 1980s. Seven descriptive processes and outcome indicators of effectiveness were used: changes of malaria transmission and incidence over time; use of facility-based care services for malaria; patients access to professional advice; the trend of treatment failure over time of sulfadoxine-pyrimethamine and chloroquine; survival rates of severe cases at the district hospital; a district malaria control strategy; number of malaria specific training for care providers; and the number of activities carried out on mosquito control measures. The data were collected from 1996 to 2003 in the Muheza district northeastern Tanzania. It covered household interviews with a stratified sample of 1,250 respondents, and in-depth interviews with all 175 health care providers in the 35 health facilities within the district. All six

Income poverty in Tanzania as elsewhere in developing countries is predominantly a rural phenomenon and affects largely households relying on subsistence farming. This is despite the fact that poverty reduction strategies have devoted increasing attention on the role farm employment in enhancing household income. This paper argues that, off-farm employment may have potential to contribute to reduction of rural households income poverty. Hence the main objective of the paper is set to examine effects of off-farm employment on income poverty. Data was collected from a random sample of 309 households in the first quarter of 2014 in five villages of Kilombero Valley, Tanzania using a structured questionnaire. Income poverty was analysed using the Foster-Greer-Thorbecke (FGT) poverty index and two stage least square (2SLS) regression. Households with off-farm employment income were found to be less poor based on all the three FGT parameters which are incidence, depth and severity of poverty. ...
Objective. To identify factors associated with perinatal mortality in northeastern Tanzania. Design. Prospective cohort study. Setting. Northeastern Tanzania. Population. 872 mothers and their newborns. Methods. Pregnant women were screened for factors possibly associated with perinatal mortality, including preeclampsia, small-for-gestational age, preterm delivery, anemia, and health-seeking behavior. Fetal growth was monitored using ultrasound. Finally, the specific causes of the perinatal deaths were evaluated. Main outcome measure. Perinatal mortality. Results. Forty-six deaths occurred. Key factors associated with perinatal mortality were preterm delivery (adjusted odds ratio (OR) 14.47, 95% confidence interval (CI) 3.2364.86, p , 0.001), small-for-gestational age (adjusted OR 3.54, 95%CI 1.1810.61, p = 0.02), and maternal anemia (adjusted OR 10.34, 95%CI 1.8956.52, p = 0.007). Adherence to the antenatal care program (adjusted OR 0.027, 95%CI 0.0030.26, p = 0.002) protected against perinatal ...

The Leather sector in Tanzania, despite having a history of over twenty years is for a variety of reasons not achieving its tremendous potential with only a few companies still operating. Tanzania by the early 80s had a total investment of over 100 million USD in 3 leather tanneries and one shoe-manufacturing unit. The main motivation for the government to invest in this sector (aside from the more obvious employment benefits) was to add value to Tanzanian exports, as well as develop products that could substitute imports. As with most parastatal organizations in Tanzania, economic liberalization greatly affected the survival of sector as most of these investments were never run commercially and concentrated more on employment generators rather than profit. Poor management, lack of technical expertise, as well as lack of working capital resulted in most mills achieving in some cases less than 10% capacity utilization. This trend continued until the mid 90s when most mills were shut down ...
This study focuses on the experiences of trafficked children in Tanzania. Trafficking of children deprives them of human rights and freedoms; it may also pose a public health risk. Migration of children who are fostered by extended family is a long-standing customary practice in Tanzania, but while the circumstances of fostering have changed, given increasing rural poverty and the impact of the HIV/AIDS, this has not been recognized in Tanzanian society. The government enacted the Anti-Trafficking Act 2008, and the Law of Child Act 2009, but children who migrate are increasingly being exploited and abused; this has transformed specific cases from migration into trafficking. Very little research has been conducted on the phenomenon of trafficking in East Africa and it was difficult to find any studies on child trafficking. This study explored the experiences of trafficked young girls in Dar es Salaam city. The specific objectives were: to determine the factors, motives and people involved in ...

Intimate partner violence (IPV) is one form of gender-based violence that concerns people in intimate relationships. Those involved may be current or former spouses, boyfriends, or girlfriends in heterosexual or homosexual relationships [1]. The perpetrators can be either men or women [2]. However, this paper concentrates on IPV by men against women because of its commonness and the serious negative effects on a womans health.. IPV by men against women is a worldwide public health and human rights concern. According to the WHO multi-country study, performed in 10 countries using a standardized methodology, the prevalence of different types of IPV vary between 15% and 71% among women aged 15-49 years [3, 4]. Studies from developing countries that were not involved in the WHO study, such as Haiti, Nigeria and Uganda, have estimates with a similar variation (11-52%) [5-7]. In urban Dar es Salaam, Tanzania, estimates of lifetime prevalences are 33% for physical and 23% for sexual violence against ...
Currently, family planning metrics derived from nationally-representative household surveys such as the Demographic and Health Surveys (DHS) categorise women into those desiring to space or limit (permanently stop) births, or according to their age in the case of young women. This conceptualisation potentially ignores a large and growing group of young women who desire to delay a first birth. This study uses household survey data to investigate the characteristics and needs for family planning of women who want to delay their first birth. The research was conducted in two rural districts in southern Tanzania (Tandahimba and Newala), and nested within the Expanded Quality Management Using Information Power (EQUIP) study. Data were collected as part of a repeated cross sectional household survey conducted between September 2013 and April 2014. The socio-demographic characteristics, including parity, contraceptive practices and fertility intentions of 2128 women aged 13-49 were analysed. The association
Testing for HIV remains an approach for preventing HIV and for accessing treatment for infected individuals. There is little information on predictors of HIV testing in men who have sex with men (MSM) in Sub-Saharan Africa, where there is often restricted access to treatment for stigmatised minorities. To understand factors associated with HIV testing in MSM in Tanzania, we analysed data from 271 MSM in Dar es Salaam. Tanzanian NIMR IRB approval was duly obtained (NIMR/HQ/R.8A/Vol.IX/822).. Sample median age was 24 years; the majority had completed secondary education, with a fifth of them educated beyond this level; … ...

This study was carried out with 1,857 poor children from 17 schools, living in low-income areas of Dar Es Salaam, Tanzania. All children took the Student Multiple Intelligences Profile (SMIP) questionnaire as part of a bigger project that gathered data around concepts and beliefs of talent. This paper sets out two aims, first to investigate the structural representation of the self perceived multiple intelligences for this set of children and second to discuss how the best fit model might reflect childrens culture and their school experiences. After carrying out exploratory factor analysis, a four factor first order model was shown to have a good fit. A higher order factor solution was investigated owing to the correlation of two latent constructs. In order to provide some insight into the multiple intelligences construct the relationship between the SMIP items, student test outcomes and attitudes to learning were examined. The item groupings were explored through African cultural beliefs ...

The stable isotope compositions of organic carbon (OC) and nitrogen, contents of OC and nitrogen, and C/N ratios for four short cores, which were recovered from Msansani Bay (WO 57), off Msimbazi River (WO 76) and Dar es Salaam harbour (WO 62 and WO 74), are used to document temporal variation in the sources and diagenetic alteration of organic matter (OM). The δ15N and δ13C values at sites WO 57, WO 62 and WO 74 show a trend of down-core decrease to the base of the core, and such a down-core trend could be related to diagenetic loss of compounds enriched in 13C and 15N. At site WO 76, the δ13C values show two zones that are depleted in 13C. These two zones correspond to black-greasy layers that were observed during sampling, and low δ13C values probably is an indication that the material was derived from the continent. Furthermore, these two zones have elevated values of OC, nitrogen and C/N ratios. The stable isotopes of OC and nitrogen show inter-sites differences with enrichment in 15N ...

On-farm experiments were conducted in farmers fields at 12 different sites in the 2 districts of Moshi and Rombo in northern Tanzania during the 2000-01 cropping season to study the effects of (brady)rhizobial inoculation in combination with P supply on growth and grain yields of soybean and common bean, and to assess the economic returns of these different technologies to farmers. A low level of N was included as an indicator of endogenous soil N status. The treatments included (brady)rhizobial inoculation, N fertilisation (30 kg N/ha as urea), P application [26 kg P/ha as triple super phosphate (TSP)], (brady)rhizobial inoculation + P fertilisation (26 kg/ha as TSP) and unfertilised uninoculated control. The study was conducted as a randomised complete block design with each of the 12 farmers fields as a replicate. At harvest, plant growth of soybean and common bean was significantly (P=0.05) greater with (brady)rhizobial inoculation compared with N and P supply or uninoculated control in ...
