Two of 20 placebo- and 19 of 40 antibiotic-treated patients improved. At 1 month, the placebo-treated patients could elect to receive ceftriaxone. Altogether, 58 patients were treated with ceftriaxone and followed up for 13 to 24 months. Improvement was noted in 27 of the 58 antibiotic-treated patients. Patients with a wide diversity of inflammatory arthritis were studied. Response to ceftriaxone was seen in all groups, including 5 of 12 with rheumatoid arthritis, 5 of 8 with psoriatic arthritis, 3 of 5 with vasculitis, and 14 of 33 with less well-differentiated chronic inflammatory arthritis. HISTORY. A five-years old male intact Boxer was referred to our VTH with a history of intermittent lameness during the last year and swelling of the tarsus since the last three months. The dog had a diagnosis of leishmaniosis (two years ago) and hypothyroidism (one year ago). He had been treated with allopurinol and levo-thyroxine since then. Serology to E. canis and Lyme was negative. Bone biopsy and synovial fluid analysis revealed bone neoformation with signs of mixed arthritis. At physical examination, a mild popliteal lymphadenopathy on the left hind limb and cutaneous lesions of the ears and nose were observed. Our diagnostic plan included a CBC, biochemistry, urinalysis, protein/creatinine ratio, T4, TSH, leishmaniosis serology, tarsal and thoracic radiographs, immunohistochemistry of leishmania from the bone biopsy and a bone cytology. The distribution of T gamma lymphocytes in the peripheral blood of one group of rheumatoid patients and in the synovial fluid in a second group was determined. The results were compared to those found for peripheral blood (PB) lymphocytes of normal subjects and for synovial fluid lymphocytes of osteoarthrosis and meniscitis patients. Besides recording percentage and absolute number, we also used cytofluorographic analysis to determine individual capacity of PB T gamma cells to bind heat-aggregated IgG (agg-IgG). The following results were found: 1) there is no significant difference between the percentage and absolute number of PB T gamma lymphocytes of rheumatoid arthritis (RA) patients and those of controls, 2) individual RA PB T gamma cells had a greater number and/or avidity of Fc receptor for IgG than those cells of controls, and 3) the percentage of RA T gamma lymphocytes in synovial fluid, revealed by IgG-EA ox rosetting, is significantly lower than that found in control patients. Objective: A Th1 biased immune response in synovial fluid has been reported in children with polyarticular and extended oligoarticular-type juvenile idiopathic arthritis (JIA). We investigated T cell phenotypes including Th1, Th2, Th17, and Treg with emphasis on Th17 and Treg, in order to differentiate cytokines in the enthesitis-related arthritis (ERA) form of JIA. Methods: The frequencies of Th1, Th2, Th17, and Treg cells were determined by flow cytometry in peripheral blood (PB) and synovial fluid from patients with ERA and healthy subjects. Levels of interleukin 1β (IL-1β ), IL-6, IL-21, IL-23, and transforming growth factor β (TGF-β ), cytokines that influence Th17 lineage cells, were measured in paired plasma and synovial fluid (SF) samples by ELISA.
