Objectives To update the incidence and prevalence of multiple sclerosis (MS) in Vasterbotten County, Sweden, and to compare this to previous investigations in the same area. Background Northern Sweden is a high-risk area for developing MS. Vasterbotten County has previously been surveyed in detail regarding the occurrence of MS. In several countries, increases in MS prevalence and incidence as well as a change in the sex ratio have been reported. Materials and methods Multiple sources were used to identify MS cases in Vasterbotten that either had their onset of the disease from 1998 to 2010 and/or lived in Vasterbotten, the two dates chosen for prevalence calculation: the 31st of December 2005 and 2010. Obesity is quite common in Swedish children, and we found that 12.9% of the five year old children were overweight and 4.3% were obese. It has been proposed that long-term breastfeeding may protect against obesity [15-19, 29, 30]. Both metabolic imprinting and behavioral aspects may potentially explain this association [31]. In Sweden, most mothers breastfeed their children [11]. In our study, 78.4% of the mothers exclusively breastfed for at least three months. The median duration of any breastfeeding was eight months. In comparison with other countries these figures are quite high [12]. The common and long-term breast-feeding duration could therefore be expected to reduce the risk of obesity in Swedish children. The present study reports for the first time on the experience of infliximab in the clinical setting of IBD in a strictly population based cohort. The study comprised all IBD patients treated in Stockholm County between 1999 and 2001. This enabled us to assess infliximab therapy in an epidemiological context with regard to severe adverse events, including postoperative complications, serious infections, malignancies, and deaths. In our clinical experience with infliximab in an unselected IBD population, we encountered more severe adverse events than expected from earlier reports.18-20 A total of six deaths were seen; all of those patients had received a single dose (5 mg/kg) of infliximab. We follow men and women born in nineteenth-century Northern Sweden and their descendants in contemporary Sweden. We examine absolute demographic success (number of great grandchildren), absolute socioeconomic success (number of great grandchildren with tertiary education), and average socioeconomic success (proportion of great grandchildren with tertiary education) from the perspective of our nineteenth-century cohorts. Our data comprise a population of predominantly farmers born in the Skellefteå region in Northern Sweden in the 1860s and 1870s, and we follow these lineages until 2007. We follow our lineages during the fertility transition, industrial revolution, and expansion of tertiary education. OBJECTIVE: To investigate whether childhood sociodemographic factors and parental psychopathology affect suicide risk differently in men and women. METHOD: Cox regressions were used to calculate interaction effects of gender for childhood and parental risk factors for 8815 suicides (27% women) in a national cohort of 2.47 million people born between 1946 and 1968. RESULTS: Low parental socioeconomic status increased suicide risk only for men, hazard ratio (HR) = 1.22 (P = 0.003 for gender interaction), while living in a metropolitan area increased the risk only for women, HR = 1.42 (P , 0.001 for gender interaction). Parental psychotic or affective disorder increased suicide risk more strongly for women (HR = 2.08), than for men (HR = 1.52) (P = 0.004 for gender interaction). Genetic variation at 1p13 modulates serum lipid levels and the risk of coronary heart disease through the regulation of serum lipid levels. Here we investigate if the interaction between genetic variants at 1p13 and serum lipid levels affects the risk of non-fatal myocardial infarction (MI) in the Stockholm Heart Epidemiology Program (SHEEP), a large population based case control study. Previous tick bite had been noted in 66% of these seropositive children but the majority (94%) had not previously been treated for LB. The need for a mobile workforce inevitably means that the length of the total work day (working and traveling time) will increase, but the health effects of commuting have been surprisingly little studied apart from perceived stress and the benefits of physically active commuting. We used data from two cross-sectional population-based public health surveys performed in 2004 and 2008 in Scania, Sweden (56% response rate). The final study population was 21, 088 persons aged 18-65, working | 30 h/week. Duration (one-way) and mode of commuting were reported. The outcomes studied were perceived poor sleep quality, everyday stress, low vitality, mental health, self-reported health, and absence from work due to sickness during the past 12 months. Childhood parental death has been associated with adverse health, social and educational outcomes. Studies on long-term outcomes are in general scarce and there is little evidence on the long-term impact on anti-social behaviour. This study takes advantage of high-quality register data to investigate risk of violent crime in relation to childhood parental death in a large national cohort covering the entire Swedish population born in 1983-1993 (n = 1,103,656). The impact of parental death from external (suicides, accidents, homicides) and natural causes on risk for violent crime from age 15 to 20-30 years, considering multiple aspects of the rearing environment (including parental psychiatric disorders and criminal offending), was estimated through Cox regression. Unadjusted hazard ratios associated with parental death from external causes ranged between 2.20 and 3.49. In southern Sweden, fungicide treatment of winter wheat is prevalent and recommended almost routinely against leaf blotch diseases. However, yield increases and hence the resulting net returns from fungicide use are highly variable within and between years. These variations raise questions about whether, when and how fungicides should be used. To help answer these questions, a thorough economic evaluation of fungicide use was carried out, based on results from untreated plots and fungicide-treated plots in trials in farmers fields, 1983-2007. Scenarios with varying grain prices and costs of fungicide treatment were evaluated and examined. Doubling and tripling the grain price led to the largest impact on the net return from fungicide treatment, followed by increasing cost of the fungicide. Other costs were of minor importance. Achalasia is a motor disorder of the lower esophageal sphincter, which fails to relax on swallowing. Although a greater risk of esophageal squamous cell carcinoma among achalasia patients is fairly well established, no epidemiological study has evaluated the risk of esophageal adenocarcinoma in these patients. We compiled a cohort of 2,896 patients recorded with a discharge diagnosis of achalasia between 1965 and 2003 in the Swedish Inpatient Register. Background: Families with parental intellectual disabilities are likely to need support in achieving a decent family life. In order to accurately plan for such support services, society needs data regarding the occurrence of those parents and their children. The aim of this study was to investigate the five-year incidence of children born to women with intellectual disabilities in a county in Sweden. Methods: Women born between 1975 and 1989 were identified from school registers for children and adolescents with intellectual disabilities in the county of Blekinge. The womens personal identification numbers were, in 2010, linked and matched with the Swedish Medical Birth Register. Results: In total, 98 women with intellectual disabilities were identified. Previous studies have indicated that survivors of childhood acute lymphoblastic leukemia (ALL) have an increased morbidity measured in terms of health care utilization. However, earlier studies have several potentially important limitations. To overcome some of these, we investigated hospital contact rates, and predictors thereof, among 5-year survivors of ALL in a population-based setting, and compared them to a control cohort regarding outcome measures from a comprehensive nation-wide health register. All individuals diagnosed with ALL before the age of 18 in Southern Sweden during 1970-1999 and alive January 2007 (n = 213; male = 107) were identified through the Swedish Cancer Register. The evidence on the impact of high sugar consumption on micronutrient dilution does not yet allow for the establishment of clear thresholds of consumption. To establish upper and lower limit intake thresholds for added sugar, more studies from different countries and multiple populations are needed. The aim of this study was to examine the association between the intakes of added sugar and various micronutrients among the adult Swedish population across almost two decades. The data were obtained from the samples from two populations: 1) Riksmaten Adults, a national dietary survey (n = 1797, 44% male, aged 18-80 years, data collection from 2010 to 11) that assessed dietary intake using a 4-day web-based food diary; and 2) the Malmö Diet and Cancer Study, a population-based cohort study (n = 12,238, 45% male, aged 45-68 years, data collection from 1991 to 1994) that assessed dietary intake via a combination of a 7-day food diary, a food frequency questionnaire and an interview. PURPOSE: The objective of the study was to compare patterns of survival 2001-2004 in prostate cancer patients from England, Norway and Sweden in relation to age and period of follow-up. SUBJECTS AND METHODS: Excess mortality in men with prostate cancer was estimated using nation-wide cancer register data using a period approach for relative survival. 179,112 men in England, 23,192 in Norway and 59,697 in Sweden were included. RESULTS: In all age groups, England had the lowest survival, particularly so among men aged 80+. Overall age-standardised five-year survival was 76.4%, 80.3% and 83.0% for England, Norway and Sweden, respectively. The majority of the excess deaths in England were confined to the first year of follow-up. CONCLUSION: The results indicate that a small but important group of older patients present at a late stage and succumb early to their cancers, possibly in combination with severe comorbidity, and this situation is more common in England than in Norway or Sweden.
Objectives This article investigates various reasons for sickness presenteeism (SP), that is, going to work despite illness. The research questions asked is: What are the main reported reasons for SP in Norway and Sweden? Design Cross-sectional survey in Norway and Sweden. Use of binomial logistic regression analysis. Participants A random sample of people aged between 20 and 60years was obtained from complete and updated databases of the Norwegian and Swedish populations. A postal questionnaire was sent to the selected individuals, with response rate 33% (n=2843). 2533 workers responded to questions about SP during the last 12months. From the Stockholm County in-patient register we selected all patients with schizophrenia, discharged from all hospitals in the county during 1971, in voluntary and compulsory care. The cohort of 1,190 patients was followed for 10 years in the national cause-of-death register. 231 deaths were identified and 68 came under the ICD chapter XVII, Injuries.... These 68 deaths were analysed through medical records and death certificates in order to find out detailed causes of death, evidence of misclassification, and information on suicides during psychiatric care. Of the 68 deaths 33 were classified as suicides, 14 undetermined, 20 accidents and one homicide. Suicides predominated among the younger patients and accidents among the elderly. By scrutinizing medical records we identified seven deaths we judged to be suicides but which had been classified as accidents or undetermined. 36% of all deaths and 44% of those classified as suicides occurred during psychiatric care. Objectives: Knowledge of the societal costs of psoriasis is limited. This study estimated the cost of care, psoriasis area and severity index (PASI), and quality of life in a defined patient population in Sweden. Methods: A prevalence-based prospective recruitment of patients visiting two Swedish dermatology clinics between September and December 2009 was performed, collecting resource utilization for health care contacts, treatment, travelling, and productivity loss during 1 month. Results: 164 patients were included. Mean total cost per patient-month was 994€. Main cost drivers were outpatient visits and light therapy (49%), biological drugs (20%) and productivity loss (22%). Total cost for topical treatment only (TT; 34%) was 369€, light therapy (LT; 24%) 1,274€, traditional systemic treatment (TST; 26%) 1,085€ and biological systemic treatment (BST; 16%) 1,709€ per patient-month.
