TY - JOUR. T1 - The efficacy of laparoscopic intracorporeal linear suture technique as a strategy for reducing recurrences in pediatric inguinal hernia. AU - Lee, S. R.. AU - Choi, Sae-Byeol. PY - 2017/6/1. Y1 - 2017/6/1. N2 - Purpose: Pediatric laparoscopic herniorrhaphy has rare complications, but recurrence might occur. The purpose of this manuscript is to evaluate the efficacy of linear suture technique of laparoscopic pediatric herniorrhaphy in reducing recurrences. Methods: Laparoscopic surgery was performed on 2223 pediatric patients (under 10 years old) from September 2012 to December 2014 in Damsoyu Hospital, Seoul, Republic of Korea. The causes of recurrence were investigated case by case. The patients were categorized into two groups according to the suture method used in closing the hernia orifice: Group 1 (purse-string suture, 1009 patients) and Group 2 (linear suture, 1214 patients). Results: There were 1413 (63.6%) male and 810 (36.4%) female patients. Mean age was 30.5 ± 29.2 ...
The simple interrupted suture is the most common skin closure method. Horizontal mattress sutures facilitate tissue eversion with the use of 50% fewer sutures. Vertical mattress sutures are useful in wounds under significant tension. Running sutures speed the closure of uncomplicated linear wounds. Subcuticular running sutures yield cosmetically pleasing results in wounds under mild tension.Optimal wound healing and epithelial migration occur in a moist environment free of coagulum or scab interposed between the healing surfaces. Epithelialization takes 2-3 days with air exposure, but only 18-24 hours with occlusion. So a light coat of antibiotic ointment and an occlusive dressing for 24-48 hours is beneficial. Most incisions may be safely cleansed with soap and water after this time. Sutures are removed after 3-5 days in the face and neck, 7-10 days in most other sites, and 10-14 days in the hands and feet. Suture marks are reduced by minimal tension on the skin closure and timely suture ...

Results. A total of 1 331 consecutive patients with CLI were treated using BTK PTA and 135 (10.1%) were approached with the pedal-plantar loop technique in order to recanalize the foot arteries. Target lesions were mostly occlusive and diffusely diseased, involving in most cases the tibial arteries as well as the in-flow and out-flow vessels. Acute success was achieved for tibial PTA in 100% of the cases, with ability to position and inflate the balloon and achieve adequate angiographic results without peri-procedural complications in all, whereas acute success for the pedal-plantar loop technique was 85%. Clinical improvement in functional status was obtained and maintained after an average of 12 months, with a significant improvement of transcutaneous oxygen tension after 15 days, 59±16 mmHg in the group of patients in which the foot arteries revascularization was successfully feasible, versus 42±12 mmHg in patients achieving patency of two BTK vessels at the ankle level with partial ...

Renal biopsy is regarded as the gold standard for the diagnosis, prognosis, and management of patients with renal disease. The first renal biopsy dates back more than a century [4]. Advancements in the needle, as well as use of real-time ultrasonography, have gradually improved the technique and the yields of the biopsies [5]. The safety of the procedure has improved considerably, even in the hands of trainee physicians [5, 6]. However, renal biopsy can cause minor to serious complications. Bleeding manifestations include hematuria, hematoma, and profuse bleeding with a consequent need for blood transfusions, coil embolization, or surgical procedures such as nephrectomy; rarely, death can result [3]. Serious complications are rare when risk factors such as hypertension, bleeding, and clotting tests are within acceptable ranges [7, 8]. Our patient had severe hypertension periprocedurally, which is a well-known risk factor for bleeding. His platelet count and coagulation were within reference ...

Efficiency of post-operative sutured tissues depends on the state stress and strain, affecting the rate of wound healing. The objective of this study was to examine, the effect of suture size and stiffness, on the deformation of an incised continuous tissue. A simple running suture configuration was modeled and analyzed using 3D Finite Element Method (FEM). Prolene suture thread (Ethicon Inc.) of size 7-0 and 8-0 with varying material stiffness in the range of 6240 MPa to 30 MPa have been used whereas CryoValve aorta graft (Cryo Life Inc.) is considered as tissue material. Results show that the suture size has major impact on the deformation state of the tissue than the suture stiffness. Even though the difference in material stiffness caused a negligible change in strain, there is 55% increase in max principal strain away from suture line. Moreover, altering the suture size from 7-0 to 8-0 resulted in a 12% reduction in max tissue stresses. Because of the localization of max stress around the ...

However these limited number of stitches are used in an amazing number of combinations to create stitch patterns. We then repeat those stitch patterns to shape our items. I have been practicing the art of crochet for over 56 years and I have found that over the years stitch patterns as well as the items we make from those stitch patterns come in and out of fashion and that is why I maintain the idea everything old is new again. The same basic stitches worked in the same old patterns but with new names. I constantly find "old" stitch patterns with names that are new to me. I tend to use the "old names" but always advise the stitch pattern may have several other new names ...

I used the leftovers of the burnout velvet that I made a kimono with - the fabric is so pretty I could not waste it . What I did not think about was the nap of the velvet so the tank fabric is cut different to the kimono - my astute sewing friend noticed this when we went out for tea ( Hello Kelly ) . Also I used the leftover silk from the kimono bands to make my own bias binding instead of using the facings supplied with the pattern. My bias bindings were wibbly wobbly and at one point I did not sew them properly onto the fabric so I had to cut them off and start again ( no way I was unpicking this fabric which frays easily where the burnout part is ) .But overall the tank top was a success and has made up a nice `twin set ...
Because the fabric is lightweight my friend wisely suggested I should add a lining. I had to buy some lining from the local craft shop which has a very small selection of dress fabrics and luckily they had some closely matching chiffon. I chose to underline the fabric and did hongkong seams especially as they may be in view from the back. The body of the pattern fit quite well but the sleeves were massive. I did realise this when looking at the pattern but went ahead hoping some Harry Potter magic might occur when I was sewing ( a bit like sewing faster when you are running out if thread hoping that will get you through ! ) When I put the jacket on I thought I was in for a wadder but after taking in about 4 cms from each seam on the sleeves and removing about 10cms off the length it starting looking a bit more hopeful. I used two vintage buttons and my nickyknack loop turner ...

A side stitch is that sharp pain that hits below the ribs. Here are a variety of effective strategies to prevent this common running problem.
Defined by Merriam-Webster as "a local sharp and sudden pain especially in the side," stitch isnt a new word. "Tonight thou shalt have cramps, side-stitches that shall pen thy breath up," says Prospero in William Shakespeares "The Tempest." In a first-century mention of the stitch, Pliny the Elder recommended treating it with "the urine of a she-goat, injected into the ears ...

Sam and Rayna accompanied me to my appointment and I sat in the waiting room, as soon as they said my name I hopped out of my chair. When I say it was like Christmas, I really felt like it was Christmas. I was so ready to move forward with my life, and leave the stitches behind me. I sat down and the nurse started asking me questions, and all I could say was that I was so excited to get my stitches out. She said well the doctor wants to talk to you about the pathology report, and then we could talk about the stitches. The doctor came in, and looked at me, I could smell his lunch on him. All I was thinking was he just ate lunch, and I am getting my stitches out and then were getting lunch. He turned and looked at me and said "Well thyroid cancer was a concern, and thats what you have, so were going to do surgery tomorrow and remove the rest of your thyroid." I was like wait, you need to back up. No one has been allowed to say the C-word (being cancer), and he had just said it. I have cancer, in ...

Although the laparoscopic myomectomy has benefits over the abdominal approach, such as less blood loss, less hemoglobin drop post-operatively, decreased post-operative pain and fewer overall complications, it is a difficult procedure, which often requires greater operative time to perform. In the current climate of attempting to reduce health care expenditures, even though the minimally invasive approach offers the advantage of reduced hospital stay, a deterrent for hospital administration and surgeons may be the increased operative time it may require. Barbed suture is a relatively new material available in gynecologic surgery. Barbs are cut into the suture with the barbs facing in a direction opposite that of the needle. The barbs allow for anchoring of the suture in tissues, which prevents migration and allows suturing without knot tying. This study will be a single-centre non-blinded randomized controlled trial comparing laparoscopic myomectomy with barbed suture versus traditional suture ...
Suture devices, primarily for use in endoscopic surgery, include a suture needle made of bioabsorbable material for pulling a length of suture material through bodily tissue allowing the suture needle to be inadvertently or intentionally left in the tissue, and a suture needle having a length of suture material attached thereto with a contractible loop or passage at the proximal end of the suture material to allow the suture needle to be passed therethrough, the loop or passage contracting to clamp or grip the suture material to function similar to a conventional tied suture knot.

On the face, a 6-0 or 7-0 monofilament suture maybe used, though fast-absorbing gut may be used on the eyelids and ears to obviate the need for suture removal. When the goal of the running alternating simple and vertical mattress suture placement is solely to encourage wound-edge eversion, fine-gauge suture material may be used on the extremities as well. Otherwise, 5-0 monofilament suture material is used if there is minimal tension, and 4-0 monofilament suture is useful in areas under moderate tension where the goal of suture placement is relieving tension as well as epidermal approximation. In select high-tension areas, 3-0 monofilament suture may be utilized as well. ...

The purpose is to test a coated polyglycolic acid multifilament suture in gastrointestinal surgery as part of a controlled randomized single blinded parallel group trial. The applicability and safety of the strand material used in two suture techniques (2-layer suture technique vs. 1-layer running) are to be tested on the basis of handling characteristics, the operative time consumed for the construction of the 1st anastomosis, the average time of the hospital duration for both patient groups, and the frequency of postoperative complications after discharge, 1 month and a maximum of 3-4 months. The used suture units are counted and the length of sutures determined which will be an indirect measurement of the costs of suture material for the two techniques ...

When tying suture knots, properly squaring successive throws is important. That is, each tie must be laid down perfectly parallel to the previous tie by reversing the loops in each successive throw. When tying rope, this is accomplished using the memory aide "left over right and twist, and then right over left and twist." With instrument ties, this is accomplished by alternating sides as the suture is twisted around the needle driver. This procedure is important in preventing the creation of a granny knot, which tends to slip and is inherently weaker than a proper square knot. The first throw in the knotting sequence is often looped or twisted twice, producing the surgeons knot. When the desired number of throws is completed, the suture material is cut (if interrupted sutures are used) or the next suture may be placed (if running sutures are used). An absolute minimum of three throws are needed for knot security, but some sutures require more throws to remain tied. When in doubt, five throws ...

Bing-Rong Liu, Ji-Tao Song. the 2nd affiliated hospital of Harbin Medical University. Background/objective:. Successful closure of gastric defects is the key procedure following endoscopic full-thickness resection (EFR). We describe a new endoscopic closure method for large gastric defects after EFR procedure using endoclips, which we call endoscopic double-layer suture technique.. Methods:. Endoscopic double-layer suture was performed after EFR. The muscular and mucosal layers were sutured separately by using endoclips. The total suture operation time was 19 minutes (the muscular layer suture time was 9 minutes and the mucosal layer suture time was 10 minutes).. Results:. Liquid diet resumed 2 hour after the operation in this patient. Follow-up endoscopy 3 day after the procedure showed that the wound healed well without bleeding or fistula.. Conclusions:. The use of the double-layer suture method seems to be a safe and feasible procedure for repairing gastric defect left after EFR. We propose ...
Because every patient is different, the AMC uses several different surgical techniques to repair cranial cruciate ligaments. Two common techniques are known by acronyms: TPLO, which stands for tibial plateau leveling osteotomy, and TTA, or tibial tuberosity advancement. Both of these techniques cut and replace a section of the tibia using special bone plates. The bone is replaced in a fashion to improve joint biomechanics. The postoperative x-ray shown here demonstrates the TPLO repair.. Another type of surgical repair involves using suture material as a stand in for the torn ligament. The extracapsular lateral fabellotibial suture technique and the Tightrope® procedure are examples of a suture method of repair. Board certified surgeons at AMC prefer the TPLO and the extracapsular lateral fabellotibial suture techniques. Postoperatively, rehabilitation therapy is often prescribed to improve the rate of recovery through AMCs Tina Santi Flaherty Rehabilitation & Fitness Service.. Dogs with an ...

Which knot?. It has been shown that different knot configurations suit particular suture materials better than others, according to a new study published in Polymers by Abellan et al. (April 2016). It included two suture materials available from Ostralos, silk and polyamide (nylon). As the knot is the weakest part of any suture, it deserved a closer look.. Five different suture materials using three different knot configurations were tested mechanically and physically in vitro. The mechanical method involved a Quasar 5 Universal Testing Machine to separate the sutures at an established mandible speed. The physical method exposed sutures to either pH conditions, ranging from pH 4 to 9, or thermal cycle fluctuations to simulate oral conditions. The total sample size amounted to 1350 specimens. The knots employed are as follows.. Knot A - three-throw forward, one forward and one reverse. Knot B - two-throw forward, one reverse and one forward. Knot C - one-throw forward, two forward and one ...

A suture fixation system includes a suture assembly having an anchor, an introducer, and a delivery device. The introducer is attachable to a finger of a person and includes a platform attached to an exterior of the introducer and a zip line attached to the platform. The delivery device is movable along the zip line and configured to removably retain the anchor. The introducer allows the finger to identify a target landmark within a patient and the delivery device is movable along the zip line and attachable to the platform to position the anchor for insertion to the target landmark.
The self securing suture includes a suture having at least one terminal end and a toggle, formed of suture material, at the sutures terminal end. In one embodiment, the suture toggle is a bar. In another embodiment the suture is a wire and the toggle is a wire element toggle. The wire element toggle is a double strand segment or an open loop segment. The suture delivery system includes a slotted needle configured such that a protruding tab or leg of the toggle bar fits within slot of the needle. For wire sutures, the open loop wire element toggle fits within the slotted needle. In another embodiment, a depending wire tip, from the wire element toggle, fits within the slot in the needle. In either situation, after the needle pierces the bodily structure, tissue or organ, the tab, leg, depending wire tip or open loop wire segment coacts with the inboard portion of the bodily structure, tissue or organ, and, as the delivery needle is withdrawn, the toggle at the terminal end of the suture remains in the
On the third re-laparotomy, the silo was removed and the decision was made to definitively close his abdomen. Fascial suture was not possible because the central section remained with an insurmountable gap of 6 cm. We performed the posterior part of the components separation technique (CST) described by Ramirez et al. [4]. Longitudinal incisions of the two posterior rectal sheaths parallel and near his linea alba were made. We bluntly separated these sheaths from his rectus muscles, paying attention not to disturb the neurovascular bundles at the lateral rectal borders. This led to a good mobilization of the two rectal myofascial complexes (RMFCs), allowing apposition under acceptable tension. We used the Kloppel interrupted suture technique, and placed four Smead-Jones internal retention sutures. Our patient made an uneventful recovery, with the wound healing by primary intention.. The sample of the peritoneal exudate for culture and sensitivity taken during the initial operation did not grow ...
A surgical suture for ligating, tying or suturing two sutures simultaneously with a single maneuver. An eyeless needle is swaged to an end of a main suture section. The main suture section ends are fused portions of two separate strands of suture material. The separate strands form an intermediate portion of the main suture section between fused portions. Alternatively, the main suture section may have a plurality of intermediate separate suture strand portions connected to each other by fused portions. Marking bands are located at the junctions of the fused portions with the intermediate separate strand portions.
This non-incisional buried suture technique allows for the surgeon to perform double eyelidplasty without making large incisions. Instead, small pin holes are made barely large enough for the needles to pass through. Sutures are repeatedly passed from one hole to another underneath the skin so as to have its name "buried" under the skin and finally come out of the hole where it started to make a knot. The knot is also buried under the skin. Therefore, bleeding or swelling is minimal immediately after surgery. The operation is so simple that surgery itself takes only about 10 minutes. The sutures can either be left inside permanently or removed later. It is the natural fibrosis around the suture that maintains the form of the double fold. The main advantage of this technique is rapid recovery after the operation and technical ease.. Excellent candidates of this operation are those who are in their teens or in their twenties with good skin elasticity. They must also have moderate amount of muscle ...

The donor site was closed with a subcutaneous continuous suture of polydioxanone 4-0 and the skin was closed with simple interrupted nylon 4-0. The flap was sutured onto the wound edges with nylon 4-0 simple interrupted sutures.. Post-operative care. A light dressing and an Elizabethan collar were placed on all patients to prevent patient interference and/or damage to the skin flap. The dressing was changed after 48 h and the flap was clinically evaluated. A large number of the flaps took on a dark purple colour at 48 h. This was thought to be due to mild post-operative venous congestion of the flap post surgery, which had disappeared completely at four days post surgery, when the flap took on a normal pink colouration. This venous congestion did not affect flap survival or increase its dehiscence. The animals were sent home with strict instructions to maintain rest and pain control for four days. The skin flaps were checked at five days post surgery and the skin sutures removed at 14 days post ...

Inverted Nipples: Overview. Inverted nipples can detract from your breasts natural beauty and inhibit you from enjoying them to the fullest. So why not make them look as beautiful as they were meant to be with a visit to Dr. Michael W. Gray.. Inverted Nipples: Procedure. Inverted nipples occur when breast ducts dont grow and stretch at the same rate as the breast. Fortunately, theres an easy fix that Dr. Gray can perform in as little as 15 minutes under local anesthesia. A tiny incision is hidden at the border where the areola meets the skin. The connections inverting the nipple are released and the nipple is then everted. The location of this incision produces the most optimal results cosmetically. Absorbable sutures are then hidden under the skin, making suture removal unnecessary.. Inverted Nipples: Recovery. After the procedure, patients can return to work the same day. Most feel minimal discomfort during recovery and because the absorbable sutures are under the skin. Suture removal is not ...
Laparoscopy requires the development of technical skills distinct from those used in open procedures. Several factors extending the learning curve of laparoscopy include ergonomic and technical difficulties, such as the fulcrum effect and limited degrees of freedom. This study aimed to establish the impact of four variables on performance of two simulated laparoscopic tasks. METHODS: Six subjects including novice (n=2), intermediate (n=2) and expert surgeons completed two tasks: 1) four running sutures, 2) simple suture followed by surgeons knot plus four square knots. Task variables were suturing angle (left/right), needle holder type (standard/articulating) and visualisation (2D/3D). Each task with a given set of variables was completed twice in random order. The endpoints included suturing task completion time, average and maximum distance from marks and knot tying task completion time. RESULTS: Suturing task completion time was prolonged by 45-degree right angle suturing, articulating ...
