PY - 2017/6/1. Y1 - 2017/6/1. N2 - Purpose: Pediatric laparoscopic herniorrhaphy has rare complications, but recurrence might occur. The purpose of this manuscript is to evaluate the efficacy of linear suture technique of laparoscopic pediatric herniorrhaphy in reducing recurrences. Methods: Laparoscopic surgery was performed on 2223 pediatric patients (under 10 years old) from September 2012 to December 2014 in Damsoyu Hospital, Seoul, Republic of Korea. The causes of recurrence were investigated case by case. The patients were categorized into two groups according to the suture method used in closing the hernia orifice: Group 1 (purse-string suture, 1009 patients) and Group 2 (linear suture, 1214 patients). Results: There were 1413 (63.6%) male and 810 (36.4%) female patients. Mean age was 30.5 ± 29.2 ...
The simple interrupted suture is the most common skin closure method. Horizontal mattress sutures facilitate tissue eversion with the use of 50% fewer sutures. Vertical mattress sutures are useful in wounds under significant tension. Running sutures speed the closure of uncomplicated linear wounds. Subcuticular running sutures yield cosmetically pleasing results in wounds under mild tension.Optimal wound healing and epithelial migration occur in a moist environment free of coagulum or scab interposed between the healing surfaces. Epithelialization takes 2-3 days with air exposure, but only 18-24 hours with occlusion. So a light coat of antibiotic ointment and an occlusive dressing for 24-48 hours is beneficial. Most incisions may be safely cleansed with soap and water after this time. Sutures are removed after 3-5 days in the face and neck, 7-10 days in most other sites, and 10-14 days in the hands and feet. Suture marks are reduced by minimal tension on the skin closure and timely suture ...

Letter to the Editor. Suture Techniques during Laparoscopic and Robot-Assisted Partial Nephrectomy: A Systematic Review and Quantitative Synthesis of Peri-Operative Outcomes. Dear Sir,. We would like to congratulate the authors of this systematic review [1] highlighting the evolution of suture techniques for partial nephrectomy in the era of minimally invasive surgery. The authors note the significant technical modification for the replacement of intracorporeal free-hand knot tying with a sliding clip technique [2]. This technique has revolutionised the practice of PN and reduced the risk of the cheese cutting effect with the conventional suturing techniques. It is worth noting that this laparoscopic technique was first described by Agarwal et al in the BJUI in 2007 [3]. Indeed one of the authors of this SR also published on the robotic application of this technique in a publication in European Urology in 2009 (2), which also was remiss in referencing the original description of the ...
Postoperative anastomotic suture line complications, such as hemorrhage and pseudoaneurysm, are often encountered in thoracic aortic surgery. To minimize these complications different anastomotic techniques have been developed. We hereby describe a new distal anastomotic technique, which involves positioning the graft inside the aorta at the distal end, reinforcing the suture line with an externally placed Teflon felt strip, and finishing the anastomosis with a circumferential and continued suture technique called backstitch.

Another locking running stitch is the interrupted stitch, which is the running stitch with the thread knotted after each suture. It is usually not the end of the world if one interrupted suture should loosen or dehisce as the remainder of the interrupted sutures will keep the wound closed. The ends are snipped flush with the skin at 10 days. It can be used as an initial layer for a longer, higher-tension wound in order to create a lower tension environment for closing the wound with deep dermal sutures within the wound followed by either simple interrupted or running percutaneous sutures closer to the wound margins. In essence, it is a â combo-interrupted subcutaneous and skin closure suture.â . Polypropylene (Prolene; Ethicon) suture (0, 2-0, 3-0, 4-0, and 5-0) was used throughout this study. This suture will be the anchor for the rest of the suture line. The suture needles, materials, and techniques selected are at the discretion of the surgeon or medical professional assessing the wound. An ...

Thirty pairs of dental roll were sutured using only peripheral sutures with 6-0 polypropylene. An initial cyclic load of 5 N for 500 cycles was applied and increased by 5 N for an additional 500 cycles at each new load until rupture. The fatigue strength of an interlocking cross-stitch suture was 113% greater than a running suture and 36% greater than a standard cross-stitch suture. Interlocking the cross-stitch prevented shortening of the transverse portions under load and appears to be a useful technique for increasing ...

Results. A total of 1 331 consecutive patients with CLI were treated using BTK PTA and 135 (10.1%) were approached with the pedal-plantar loop technique in order to recanalize the foot arteries. Target lesions were mostly occlusive and diffusely diseased, involving in most cases the tibial arteries as well as the in-flow and out-flow vessels. Acute success was achieved for tibial PTA in 100% of the cases, with ability to position and inflate the balloon and achieve adequate angiographic results without peri-procedural complications in all, whereas acute success for the pedal-plantar loop technique was 85%. Clinical improvement in functional status was obtained and maintained after an average of 12 months, with a significant improvement of transcutaneous oxygen tension after 15 days, 59±16 mmHg in the group of patients in which the foot arteries revascularization was successfully feasible, versus 42±12 mmHg in patients achieving patency of two BTK vessels at the ankle level with partial ...

Minimally invasive procedures such as diagnostic or operative laparoscopy has increasingly becoming the initial access of choice with benefits such as improved appearance such as less scarring and less pain than open procedures. More small surgical cuts made abdominally have been used to assist with the surgery, and the cosmetic outcome has become one of the main factors in determining the closure method. There are various techniques available including conventional suture, skin glue or adhesive tape, but it is mainly based on the surgeons preference and ultimately patients satisfaction and to date there is no gold standard on the skin closure method for laparoscopic port-site wound. This research aim to compare the closure method for small 5mm laparoscopic wound skin closure between conventional suture versus non-closure method where the surgical incision wound is simply covered with simple dressing after the operation. 5mm laparoscopic wound is a small surgical incision where disruption of ...

Renal biopsy is regarded as the gold standard for the diagnosis, prognosis, and management of patients with renal disease. The first renal biopsy dates back more than a century [4]. Advancements in the needle, as well as use of real-time ultrasonography, have gradually improved the technique and the yields of the biopsies [5]. The safety of the procedure has improved considerably, even in the hands of trainee physicians [5, 6]. However, renal biopsy can cause minor to serious complications. Bleeding manifestations include hematuria, hematoma, and profuse bleeding with a consequent need for blood transfusions, coil embolization, or surgical procedures such as nephrectomy; rarely, death can result [3]. Serious complications are rare when risk factors such as hypertension, bleeding, and clotting tests are within acceptable ranges [7, 8]. Our patient had severe hypertension periprocedurally, which is a well-known risk factor for bleeding. His platelet count and coagulation were within reference ...

Efficiency of post-operative sutured tissues depends on the state stress and strain, affecting the rate of wound healing. The objective of this study was to examine, the effect of suture size and stiffness, on the deformation of an incised continuous tissue. A simple running suture configuration was modeled and analyzed using 3D Finite Element Method (FEM). Prolene suture thread (Ethicon Inc.) of size 7-0 and 8-0 with varying material stiffness in the range of 6240 MPa to 30 MPa have been used whereas CryoValve aorta graft (Cryo Life Inc.) is considered as tissue material. Results show that the suture size has major impact on the deformation state of the tissue than the suture stiffness. Even though the difference in material stiffness caused a negligible change in strain, there is 55% increase in max principal strain away from suture line. Moreover, altering the suture size from 7-0 to 8-0 resulted in a 12% reduction in max tissue stresses. Because of the localization of max stress around the ...
Now, that may sound a bit weird, to shoot for a somewhat extensive number of stitches or stitch combinations in one alphabet. But its not, really. To explain: You have an alphabet - that is, a collection of 26 designs (A-Z). Following some basic principles of stitch placement, each letter (or design) can be worked in myriad ways, using any variety of possible stitch combinations out of the 25 or so stitches and combinations explored.. I used the leftovers of the burnout velvet that I made a kimono with - the fabric is so pretty I could not waste it . What I did not think about was the nap of the velvet so the tank fabric is cut different to the kimono - my astute sewing friend noticed this when we went out for tea ( Hello Kelly ) . Also I used the leftover silk from the kimono bands to make my own bias binding instead of using the facings supplied with the pattern. My bias bindings were wibbly wobbly and at one point I did not sew them properly onto the fabric so I had to cut them off and start again ( no way I was unpicking this fabric which frays easily where the burnout part is ) .But overall the tank top was a success and has made up a nice `twin set ...
Because the fabric is lightweight my friend wisely suggested I should add a lining. I had to buy some lining from the local craft shop which has a very small selection of dress fabrics and luckily they had some closely matching chiffon. I chose to underline the fabric and did hongkong seams especially as they may be in view from the back. The body of the pattern fit quite well but the sleeves were massive. I did realise this when looking at the pattern but went ahead hoping some Harry Potter magic might occur when I was sewing ( a bit like sewing faster when you are running out if thread hoping that will get you through ! ) When I put the jacket on I thought I was in for a wadder but after taking in about 4 cms from each seam on the sleeves and removing about 10cms off the length it starting looking a bit more hopeful. I used two vintage buttons and my nickyknack loop turner ...

6. Methods: From February 2019 to May 2019, we included 50 patients who received uni-portal VATS for lung disease or mediastinal disease and simple continuous suture to strengthen the closure of intra- muscle combined with removal-free stitches on the skin. After an episiotomy or a second-degree tear, continuous suturing techniques, compared with interrupted suturing methods, for perineal closure are associated with less short-term pain, less need for analgesia, and less need for suture removal, according to an intervention review conducted by the Cochrane Pregnancy and Childbirth Group. Purse-string Suture - This method is a type of continuous suture that can be tightened, just like the strings in the drawstring bag. The continuous suture technique with absorbable suture does not require suture removal and provides support for the wound for a longer period of time. protrusion of viscera through all tissue layers. Objective: To compare the cosmetic results 12 months after treatment and complications associated with simple interrupted sutures (SIS) versus running subcuticular sutures (RSS) in facial surgery. In a recent publication, we proposed that the subarticular terminology is a misnomer, and this technique should be described as intradermal instead. I would suggest consulting with an expert in scar revision for optimal results. Running subcuticular suture The benefit of this suture is the minimal epidermal puncture points allowing the suture to be left in place longer without suture-track scarring. Which type of needle is preferred for suturing skin? Straight needles are very convenient for efficient and neat sub-cuticular sutures. The aim of this study is to compare aesthetic results of two different type of sutures (subcuticular versus synthetic glue) in post-thyroidectomy incisions. It is a randomized clinical trial (single blinded) designed as a ...
Although the laparoscopic myomectomy has benefits over the abdominal approach, such as less blood loss, less hemoglobin drop post-operatively, decreased post-operative pain and fewer overall complications, it is a difficult procedure, which often requires greater operative time to perform. In the current climate of attempting to reduce health care expenditures, even though the minimally invasive approach offers the advantage of reduced hospital stay, a deterrent for hospital administration and surgeons may be the increased operative time it may require. Barbed suture is a relatively new material available in gynecologic surgery. Barbs are cut into the suture with the barbs facing in a direction opposite that of the needle. The barbs allow for anchoring of the suture in tissues, which prevents migration and allows suturing without knot tying. Suture devices, primarily for use in endoscopic surgery, include a suture needle made of bioabsorbable material for pulling a length of suture material through bodily tissue allowing the suture needle to be inadvertently or intentionally left in the tissue, and a suture needle having a length of suture material attached thereto with a contractible loop or passage at the proximal end of the suture material to allow the suture needle to be passed therethrough, the loop or passage contracting to clamp or grip the suture material to function similar to a conventional tied suture knot.

On the face, a 6-0 or 7-0 monofilament suture maybe used, though fast-absorbing gut may be used on the eyelids and ears to obviate the need for suture removal. When the goal of the running alternating simple and vertical mattress suture placement is solely to encourage wound-edge eversion, fine-gauge suture material may be used on the extremities as well. Otherwise, 5-0 monofilament suture material is used if there is minimal tension, and 4-0 monofilament suture is useful in areas under moderate tension where the goal of suture placement is relieving tension as well as epidermal approximation. In select high-tension areas, 3-0 monofilament suture may be utilized as well. ...

The purpose is to test a coated polyglycolic acid multifilament suture in gastrointestinal surgery as part of a controlled randomized single blinded parallel group trial. The applicability and safety of the strand material used in two suture techniques (2-layer suture technique vs. 1-layer running) are to be tested on the basis of handling characteristics, the operative time consumed for the construction of the 1st anastomosis, the average time of the hospital duration for both patient groups, and the frequency of postoperative complications after discharge, 1 month and a maximum of 3-4 months. The used suture units are counted and the length of sutures determined which will be an indirect measurement of the costs of suture material for the two techniques ...

When tying suture knots, properly squaring successive throws is important. That is, each tie must be laid down perfectly parallel to the previous tie by reversing the loops in each successive throw. When tying rope, this is accomplished using the memory aide left over right and twist, and then right over left and twist. With instrument ties, this is accomplished by alternating sides as the suture is twisted around the needle driver. This procedure is important in preventing the creation of a granny knot, which tends to slip and is inherently weaker than a proper square knot. The first throw in the knotting sequence is often looped or twisted twice, producing the surgeons knot. When the desired number of throws is completed, the suture material is cut (if interrupted sutures are used) or the next suture may be placed (if running sutures are used). An absolute minimum of three throws are needed for knot security, but some sutures require more throws to remain tied. When in doubt, five throws ...
SUTURE REMOVALTo remove the interrupted sutures, grasp the suture at the knot with a toothed forceps and pull it gently to expose the portion of the stitch under the skin. This will delay the healing process. 1 Early removal of sutures can minimize the effect of cross- 9. The visible part of the suture opposite the knot should be cut and the suture is removed by pulling it in the direction of the knot.If a continuous suture is applied, it is cut through, close at each skin orifice on one side and the cut sections are removed through the opposite side by gentle traction. 14. After performing an aortotomy, there are a variety of techniques utilized for suture closure. There is no published data comparing the efficacy of various suture techniques. The goal of this study is to provide an ex-vivo quantitative assessment of resistance to leakage and dehiscence for three aortotomy closure techniques. An ex-vivo model was developed utilizing explanted porcine aorta. Aortotomies were closed using one of three techniques: 1) single layer baseball stitch 2) double layer baseball stitch 3) horizontal mattress stitch with a top layer baseball stitch. The aorta was pressurized with saline using an apparatus which captured all leaked fluid. The intra-aortic pressure was adjusted over 8 increments from 110 to 375 mmHg. Leakage rates were determined at each pressure level. Ten aortotomies were performed for each technique, resulting in 240 calculated leakage rates. At all pressures, the horizontal mattress group was measured to have significantly less leakage when compared to single or

Suture Materials •Primary purpose for suture -Approximate wound margins -Enhance tissue healing •Early role of suture -Wounds do not gain strength until 4-6 days after injury -Approximating of tissues depending on suture strength •The relationship between gradual loss of suture strength and slow increase of wound strength must be considered . In general, the higher suture number indicates a lower cross-sectional diameter of the suture, however because the USP grading system is based on suture strength, as opposed to suture diameter, two 4-0 rated sutures from different materials may have a different ...
Background During operations surgeons make a cut (incision) in the skin to gain access to the surgical site. Incisions are closed with sutures (stitches), staples, tissue adhesives or tapes. Sutures can be continuous or interrupted. Continuous sutures are usually inserted underneath the skin surface using absorbable or non-absorbable suture material. Interrupted sutures involve the full thickness of the skin and are usually non-absorbable (but not always).. Impaired wound healing increases costs of health care and leads to poor cosmetic results. We investigated whether it is better to use continuous or interrupted sutures to close wounds after non-childbirth surgery by performing a thorough search of the medical literature for randomised controlled trials (RCTs) that compared these two methods of skin closure (in August 2013).. Study characteristics We identified five RCTs with a total of 827 participants. Seven hundred thirty participants (384 received continuous sutures and 346 interrupted ...

Because every patient is different, the AMC uses several different surgical techniques to repair cranial cruciate ligaments. Two common techniques are known by acronyms: TPLO, which stands for tibial plateau leveling osteotomy, and TTA, or tibial tuberosity advancement. Both of these techniques cut and replace a section of the tibia using special bone plates. The bone is replaced in a fashion to improve joint biomechanics. The postoperative x-ray shown here demonstrates the TPLO repair.. Another type of surgical repair involves using suture material as a stand in for the torn ligament. The extracapsular lateral fabellotibial suture technique and the Tightrope® procedure are examples of a suture method of repair. Board certified surgeons at AMC prefer the TPLO and the extracapsular lateral fabellotibial suture techniques. Postoperatively, rehabilitation therapy is often prescribed to improve the rate of recovery through AMCs Tina Santi Flaherty Rehabilitation & Fitness Service.. Dogs with an ...

6 Looped running stitch. It can be used along straight or curved lines to outline a shape. Suggestions. This stitch is one of many line stitches that can be used for bold or delicate outlines, depending on your choice of thread. Although the running subcuticular suture is well described in surgical textbooks, 1 the four-step method is unique in that it separates and quantifies each component. Midway through the closure, the suture is intentionally looped in to the deep dermis and then out of the epidermis before continuing the running subcuticular suturing. 2. Sutures are used to close wounds and speed up the natural healing process, but they can also complicate matters by causing damage to soft tissues with their stiff fibers. To remedy the problem, researchers from Montreal have developed innovative tough gel sheathed (TGS) sutures inspired by the human tendon.. These next-generation sutures contain a slippery, yet tough gel envelop, imitating the structure of soft connective tissues. In putting the TGS sutures to the test, the researchers found that the nearly frictionless gel surface mitigated the damage typically caused by traditional sutures.. Conventional sutures have been around for centuries and are used to hold wounds together until the healing process is complete. But they are far from ideal for tissue repair. The rough fibers can slice and damage already fragile tissues, leading to discomfort and post-surgery complications.. ...

Dr. Robert F. LaPrade operated on my right knee in May of 2010. I was hit by a car on my bicycle near Horsetooth Reservoir in CO. I was life flighted to MCR in Loveland, CO. My orthopedic injuries were severe, but totally missesd by the orthopedic team at Poudre. I could not bear weight on my right side though I tried repeatedly, but finally I went and got an MRI and one of the orthopedic surgeons that I worked with was shocked when he saw the MRI result. My right knee was totally destroyed; ACL, MCL, PCL all severely torn; the patella was the only thing intact in my right knee. Dr. La Prade had just moved to Vail and I was his 2nd patient @ The Steadman Clinic. I was told by one of the orthopedic surgeons that I worked with that I would never run again and would be lucky if I could ever hike again. This was devastating news after being a top triathlete (3rd in the world in my age group in 1989 & 1st nationally in my age group) and a big marathon runner. All I can say is Dr. La Prade did an ...
Which knot?. It has been shown that different knot configurations suit particular suture materials better than others, according to a new study published in Polymers by Abellan et al. (April 2016). It included two suture materials available from Ostralos, silk and polyamide (nylon). As the knot is the weakest part of any suture, it deserved a closer look.. Five different suture materials using three different knot configurations were tested mechanically and physically in vitro. The mechanical method involved a Quasar 5 Universal Testing Machine to separate the sutures at an established mandible speed. The physical method exposed sutures to either pH conditions, ranging from pH 4 to 9, or thermal cycle fluctuations to simulate oral conditions. The total sample size amounted to 1350 specimens. The knots employed are as follows.. Knot A - three-throw forward, one forward and one reverse. Knot B - two-throw forward, one reverse and one forward. Knot C - one-throw forward, two forward and one ...
Suture selection is a crucial and integral factor to ensuring successful surgical outcomes. Suture materials are considered surgical implants because they remain inside in the patients body upon discharge. Suture material choices must be based on the anticipated wound healing times and tissue types as well as considerations unique to each patient. Using the wrong suture material may lead to unintended consequences such as wound dehiscence, abscess formation or tissue reaction. This session will enable veterinary technicians to understand the basis for suture material selection and usage. Learning Objectives: Upon completion of this course, the participant should be able to: ...
