CMS released a proposed rule that would update the Hospital Outpatient Prospective Payment System and the Ambulatory Surgical Center Payment System.
This week APTA responded to 4 specific questions posed by the Centers for Medicare and Medicaid Services (CMS) in its proposed rule for Medicare hospital outpatient prospective and ambulatory surgical center payment systems (OPPS) for calendar year 2013. The questions are in relation to implementation of the Medicare Part A to Part B Rebilling Demonstration. In this demonstration, participating hospitals are allowed to receive 90% of the allowable Part B payment for Part A short-stay claims that are denied on the basis that the inpatient admission was not reasonable and necessary. The hospitals can rebill denied Part A claims as Part B services and be paid additional reimbursement when an inpatient admission is found not reasonable and necessary. CMS questions involve setting parameters regarding the amount of time a patient can remain on observation status, establishing specific clinical criteria for admission and payment, aligning hospital payment rates more closely with the resources used to ...

ASCs offer improved patient quality and care, while also reducing healthcare costs. With hospitals increasingly facing challenges such as budgetary issues, inefficient operating room turnovers, and limited operating room capacities. Increasing shift of surgical procedures to outpatient settings with improvements in medical technologies is expected to provide a fillip to the market.. Moreover, growing incidence of chronic disorders can play a positive role in the growth of the market. INTRODUCTION: The value proposition of surgery at freestanding ambulatory surgery centers (FSASCs) in terms of efficiency, safety, and patient satisfaction is well established and has led to increased FSASC utilization. However, there are comorbid conditions that disqualify certain patients from surgery at FSASCs. Understanding the percentage of patients whose comorbid conditions exclude them from FSASCs is important for the proper planning and utilization of operating room assets. We aim to understand the percentage of excluded patients, and we predict that certain procedures have higher rates of disqualification due to the types of patients who undergo them ...

San Antonio abortionists Alan Braid and Eduardo Aquino recently spent $3M to move their abortion practice into an ambulatory surgical center (ASC). With the passage of House Bill 2, the Pro-Life Omnibus Bill of 2013, Texas state law now requires that medical and surgical abortions be committed at ASCs rather than small, office-like abortion mills. The standards do not make abortion one iota safer for preborn children, but they offer crucial protections to pregnant women, especially in the case of an abortion-related emergency.. The Texas Observer ran a story bemoaning the common sense safety standards to which Texas abortion mills are now bound. The piece suggests that the strict regulations governing the construction of ASCs are trivial: "The standards dictate numerous structural details, down to the width of doors and hallways, as well as fire and safety features and administrative requirements.". The real issue is not that ASC standards are overly burdensome to abortionists or pregnant women, ...

Although either facility is considered safe, there are key differences that you may want to consider. One of those differences, according to a recent study, is the efficiency of the ambulatory surgery center.. The study in the American Journal of Surgery compared the hospital to the surgery center and showed that total facility time and the time intervals before and after surgery were longer in the hospital setting.. The authors had the opportunity to compare both facilities as their breast operations were relocated - from an ambulatory surgery center to a hospital. The records of 92 hospital patients and 92 ASC patients were then retrospectively analyzed to compare time intervals and other information.. Time intervals for surgical care were measured and reviewed by the authors and they found that on average, total facility time was 69 minutes shorter in the ambulatory surgery center. Most significant was the 55-minute difference in the preoperative time period - "the time from the entrance into ...

As of 2003, there are about 3,700 ambulatory surgical centers in the United States, compared with 275 in 1980 and 1,450 in 1990. This rapid increase reflects a general trend toward surgeries performed on an outpatient basis. According to American Medical News , 70% of all surgical procedures performed in the United States in 2000 were done in outpatient facilities, compared to 15% in 1980. As of 2003, over seven million surgeries are performed annually in American ASCs. Between 1990 and 2000, the number of operations performed annually in these centers rose 191%, from 2.3 million procedures in 1990 to 6.7 million in 2000. The types of surgical procedures performed in ASCs have also undergone significant changes in recent years. Many of the early ASCs were outpatient centers for plastic surgery. Advances in minimally invasive surgical techniques in other specialties, however, led to the establishment of ASCs for orthopedic, dental, and ophthalmologic procedures. According to the Federated ...
