I to had a mentor with the burning passion to look like a dork outside the confines of the theater. He insisted on wearing this nonsensical single breasted tweed-like jacket. It had pockets to hold his nonsensicals including my most hated of instruments the dreaded Zippo lighter. As a head and neck surgeon he would use the lighter to heat up his mouth/throat mirror to all but glowing red to prevent it from fogging up as he described. I asked him on numerous occasions why not simply use fog-off or for that matter hand soap or even an alcohol wipe but he insisted that not only was his way better it was more sanitary since the flame sterilized the mirror. I dont get why we seem to look for reasons to make more trouble. Surely there is no logical reason why a warm up or for that matter any cover-up is going to prevent disease and if the whole goal is to prevent creepy cruddies from getting on our clothes from outside the OR spaces then the one way is to prevent OR clothes from leaving the OR ...

Hi, Needing help. Patients with colonized MDROs such as MRSA, VRE, ESBL, and etc..... Do you isolate (Contact precautions). We isolate (contact) for all MDROs, c-diff, acinetobacter,
Application of isolation (=contact precautions) in hospitalized patients is recommended when patient is colonized (or infected) by micro-organism known for its outbreak capacity or high pathogenicity. It allows the reinforcement of universal precautions (=standard precautions) in order to control patient-to-patient micro-organisms transmission. Recently, the efficacy of this measure is questioned and its impact on patient care seems deleterious.. In a particular context of Infectious Disease Unit, where standard precautions are handled (favorable architecture and appropriate practice for hand hygiene), assessment of contact precautions to reduce the incidence of acquired bacteria during hospitalization would be of interest.. The investigators design a non-inferiority comparative study to measure the colonization pressure in patients hospitalized in two different parts of the department: one unit only with standard precautions applied (intervention) and one unit with contact precautions (control) ...

by noblenetwork , Jan 23, 2014 , News. The Society for Healthcare Epidemiology of America (SHEA) recently issued new guidance on attire worn by personnel in healthcare facilities. SHEAs recommendations concluded "attire should attempt to balance professional appearance, comfort, and practicality with the potential role of apparel in the cross-transmission of pathogens."The organization called for more clinical studies to determine the relationship between attire and healthcare associated infections (HAIs), however noted several studies that demonstrate various types of attire can be contaminated. Find the full guidance "Healthcare Personnel Attire in Non-Operating-Room Settings"published in the Journal of Infection Control and Hospital Epidemiology here: http://www.jstor.org/stable/10.1086/675066 This guidance is an important step in updating regulations concerning soft surface fabrics, but more work is needed.SHEA discusses daily laundering and a "bare-below-the-elbows" approach for attire. ...
BACKGROUND Extended-spectrum β-lactamase-producing Enterobacteriaceae (ESBL-E) are emerging worldwide. Contact precautions are recommended for known ESBL-E carriers to control the spread of ESBL-E within hospitals. OBJECTIVE This study quantified the acquisition of ESBL-E rectal carriage among patients in Dutch hospitals, given the application of contact precautions. METHODS Data were used from 2 cluster-randomized studies on isolation strategies for ESBL-E: (1) the SoM study, performed in 14 Dutch hospitals from 2011 through 2014 and (2) the R-GNOSIS study, for which data were limited to those collected in a Dutch hospital in 2014 ...
3 I have 2 standing macros set up now, 1 for gown, one for ixion. The one with gown is for if i have sublimation pull initiated, and want the pull speeded up (obvously right). However, Ill also have the ixion on stand by if say i need actual mp more than my charge, but still wanna keep a charge pulling instead of spending it yet (ie low mana but need some like during endurance matches like soloing Ullikumi. Also, Id keep ixion cloak for situations like being in sky when i wanna put up sublimation, but dont wanna hassle with putting up skin, or cant cuz of magic aggro i can toss up my ixion cloak, garden bangles, and nodowa for a 2-3 tick regen to negate the sublimation pull for a majority amount. I do a lot of things that require ALOT of running, like running all over the sea zones, palace, sky, etc, and sometimes i need the cloaks use more so than the gowns use. (situational gear go ...

Antibiotic-resistant infections are associated with 2 million illnesses and 23,000 deaths in the United States each year, according to the CDC. Despite efforts by various entities to tackle this problem, it continues to grow at a rapid pace. In response, President Obama signed an executive order on Sept. 18 directing key federal agencies to take comprehensive action to fight the rise in antibiotic-resistant bacteria.
The WHO has published a report this week listing the antibiotic-resistant bacteria that pose the greatest risk and should be the focus for new treatments.

I get that no one likes to don gowns and gloves before seeing patients, particularly infectious disease consultants who see a large number of patients colonized/infected with multi-drug resistant organizsms (MDRO). I understand that touching is important for healing and that somehow wearing gloves impedes the healing touch. I even see why some still think that contact precautions place patients at greater risk for medical errors, even if the data supporting that contention is lacking. What I dont get is why infection prevention folks are pillorying one of the most effective methods we have in MDRO transmission prevention right when we need it the most ...

A surgical scrub apparatus includes a liquid-containing rupturable cylindrical cartridge which is slidable within a tubular handle having two (2) hollow interior spikes projecting longitudinally from one end. The cartridge can be inserted sufficiently far to cause rupture of the cartridge by both spikes. Liquid from the ruptured cartridge flows through the hollow spikes to an applicator sponge having two (2) wide area applicator surfaces. The hollow spikes are oriented such that one is always disposed higher than the other when the applicator surfaces of the sponge are disposed substantially horizontally. The upper spike can be disposed at a position wherein the liquid pressure applied thereto is less than ambient such that the upper spike serves as a vent passage which admits air into the cartridge. The lower spike thereby serves primarily as an outflow for liquid from the cartridge and air replaces the egressing liquid through the upper spike. The
