Methods. We evaluated 72 patients with EA and 43 controls for the study. All patients and controls underwent rheumatological and gastroenterological clinical examinations, and demographic features were recorded. For each patient, these factors were also recorded: duration of inflammatory bowel disease and arthritis from onset to enrollment, history of viral and bacterial infections, and occurrence of previous major trauma to the spine. These scores were taken: Bath Ankylosing Spondylitis Metrology Index (BASMI), Bath Ankylosing Spondylitis Functional Index (BASFI), Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Harvey-Bradshaw Index, and the Simple Clinical Colitis Activity Index. All subjects had MRI of the spine. ...
Measurement in cm of distance between marks originally placed while participant was standing erect 10 cm above and 5 cm below the midpoint of a line that joins the posterior superior iliac spines. Distance between marks was re-measured with participant maximally bent forward, knees fully extended, with supine in full flexion. The measurement was carried out two times and best of the two measurements which corresponds to the highest value were reported. Higher score indicated greater spinal mobility. Change: Week x observation minus Baseline observation. Baseline value was used when present, otherwise valid screening value used as baseline if within 14 days of first test article injection ...
Results Eighty-two (27%) of the 300 rheumatologists responded. Three of the responders (4%) stated that they measured chest expansion in every patient whereas 72 (87%) said that they performed the test only in patients suspected to have ankylosing spondylitis. Seven rheumatologists (9%) reported that they never measured chest expansion.. There was a good intra and inter-observer agreement in chest expansion measurements as judged by a difference of less than 1 cm in 80% of the calculations. A chest expansion of less than 2.5 cm had a sensitivity of 23% in patients with AS whereas it had a specificity of 49% (2 patients with RA, 1 with SLE and 5 HC had values below 2.5 cm). There was a significant correlation between vital capacity and chest expansion only in male patients with AS (r = 0.51, p = 0.005). Chest expansion decreased with age in all groups (r2 = 0.083, F = 0.0001). There was no relationship between the chest expansion either with gender or with BMI, both among the patients and the ...
Our previous studies have demonstrated that IL-37 plays as an immune mediator to restrain the inflammatory response of autoimmune diseases in SLE and GD [16,30]. Other published data have shown that IL-37 has a pivotal function in inflammatory regulation of inflammatory disorders [17,18]. Yet, it is still not clear whether IL-37 is involved in the pathogenesis of AS. Here, we for the first time reported that IL-37 concentrations in serum and IL-37 mRNA expression in PBMCs were dramatically higher in AS patients compared to HC, and the levels of IL-37 positively correlated with the disease activity, especially with the major pro-inflammatory cytokines related to AS. These data suggested IL-37 closely relates to AS, especially the activity of AS, and may play an important role in inflammatory regulation within the AS.. Several complications often present in AS patients, such as uveitis, inflammatory bowel disease, and psoriasis [4]. Osteoporosis (OP) is a common complication in the pathogenesis of ...
Results The endothelial function analysis demonstrated a significant reduction of post occlusive hyperaemia area in AS patients in relation to healthy donors, thus pointing at the presence of a prominent endothelial dysfunction in these patients.. The analysis of the genetic profile in PBMCs showed a significant alteration in the expression of genes related to inflammatory (CCL2, CCL5, CCR1, CCR2, IL1R2, IL1A, IL2, IL5, NOS3, SPP1), and oxidative stress response (APOE, SOD1). An altered expression of genes linked to cellular adhesion (CDH5, ICAM-1, SELL, SPP1, CTGF, FN1, THBS4, TNC), lipid metabolism and transport (ABCA1, APOA1, APOE, FABP3, LPL, LPA, MSR1), cell growth and proliferation (CSF2, SSP1, BCL2, FABP3, FGF2, LIF, CRGF), as well as transcription and apoptosis regulators (PPARA, PPARD, EGR1, BCL2, BCL2A1, BCL2L1) was further demonstrated.. In vitro treatment of HUVECs with PBMCs from AS patients with a high pro-inflammatory profile promoted a significant activation of the endothelial ...

Combining intensive group exercise with an educational-behavioral program can provide promising results in the management of patients with clinically stabilized ankylosing spondylitis (AS) on tumor necrosis factor (TNF) inhibitor treatment, say authors of an article available online April 1 in Journal of Rheumatology. Sixty-two outpatients (49 men, 13 women, mean age 47.5 ± 10.6 yrs) were randomized to rehabilitation plus an educational-behavioral (n = 20) program, to an educational-behavioral program only (n = 20), or to a control group (n = 22). The educational-behavioral program included 2 educational meetings and 12 rehabilitation exercise sessions (stretching, strengthening, chest and spine/hip joint flexibility exercises), which patients then performed at home. Outcome assessment at the end of rehabilitation training at 2 and 6 months was based on spinal pain intensity in the previous 4 weeks by self-report visual analog scale, Bath Ankylosing Spondylitis Metrology Index (BASMI), Bath ...
What is Anklylosing Spondylitis?. Ankylosing spondylitis is a member of a group of rheumatic diseases that affects the mobility of the spine, collectively known as spondyloarthropathies. Spondyloarthropathies are a group of related inflammatory joint diseases associated almost exclusively with the presence of a genetic marker called HLA-B27 and are characterized specifically by sacroiliac joint involvement in addition to other symptoms. Spondyloarthropathies affect the axial joints (vertebral column, ribs, and the sternum) and can also involve peripheral joints (e.g., feet and shoulders). The clinical features of the spondyloarthropathies are distinct from other rheumatologic conditions such as rheumatoid arthritis. Although ankylosing spondylitis affects each individual differently in symptom severity and disease progression, the primary symptoms of ankylosing spondylitis typically include back pain and back stiffness, especially in the morning. Chronic inflammation and irritation of the spinal ...

Exclusion criteria included spondylitis other than ankylosing spondylitis and clinical or radiologic evidence of complete spinal ankylosis. Follow-up was 93 ...
Evidence for ankylosing spondylitis was sought by clinical, radiologic, and ophthalmologic examination in HL-A W27-positive men, aged 18 or older, selected from a tissue-donor population. Back pain of 3 months duration or longer (P , 0.05), back stiffness, restricted lumbar flexion and chest expansion, sacroiliac erosions (P , 0.05) and sclerosis, and ophthalmologic sequels of anterior uveitis were found more often in the 24 men of the W27 group than in a control group of 31 men lacking this antigen. Based upon accepted criteria, 3 W27 persons had definite spondylitis and an additional 3 W27 persons and one control subject had findings strongly suggestive of spondylitis (P , 0.05). This striking frequency, if extrapolated to the general population, would place approximately 1 of 4 W27-positive men at risk for this disease. ...
Several recent studies have been performed to identify and measure the outcomes of treatment of axial SpA in both research and clinical practice. The development of valid, reproducible and objective tool for the evaluation of disease activity in axial SpA is difficult, although valuable instruments have been recommended by several researchers [37]. Among the proposed composite indices, BASDAI has acceptable properties as a measure of disease activity in axial SpA. Nevertheless, there are few issues with regard the items content and appropriateness of response formats of BASDAI [38]. Further, it has been shown that the BASDAI is an ambiguous measure of disease activity in patients with peripheral or axial disease activity and that reflects only patients perspectives and not necessarily captures the entire spectrum of disease activity [39]. For that reason ASAS group has tried to go a step further in the evaluation of disease activity in AS by developing the ASAS-endorsed disease activity score ...
If someone in your life has Ankylosing Spondylitis, this article is an excellent and succinct tutorial that will give insight into how the disease affects them. If you have AS, this article is an excellent tutorial and offers some very good information to consider. This is an outstanding article describing the impacts of Ankylosing Spondylitis. Many people - if they have heard of it at all - think AS affects only the lower spine. This article describes the process and progression of the disease, and the impacts on other areas of the body, fatigue and even chest pains caused by AS. I find myself in the position of having an advanced level of AS - impacts in the shoulders, hips, knees, back, and calcification (causing inflammation) where ligaments and tendons attach to bone. Because of injuries early in life, and the resulting early onset of osteoarthritis, the first two rheumatologists I saw simply noted the osteoarthritis and stopped looking there. Finally, I was referred to an excellent doctor ...

SOURCE: Steinberg CL, et al. (1955). Comparison of rheumatoid (ankylosing) spondylitis and crippling fluorosis. Annals of the Rheumatic Diseases 14: 378-384.. "During our anthropological studies we were puzzled by the high incidence of a peculiar type of arthritis and joint ankylosis that seemed to afflict a majority of the adult inhabitants of a number of small Miao villages in the neighbourhood. In fact, in many of the older people the disease had advanced to such an extent that they appeared to be set in semi-sitting positions, with almost complete loss of movement in the limbs… Later a copy of Roholms mongraph on fluorine intoxication came to hand and we were struck by the similarity in appearance between the photographs of the Danish workers in cryolite and the crippled Miao peasants in China… It is quite possible that endemic centres [of skeletal fluorosis] exist but that the cause of the disabling spondylitis or other joint affections has not been determined, and a diagnosis of ...
While genetics is believed to be a key player in the development of Ankylosing Spondylitis, the exact cause has not yet been determined. The majority of people who have AS also carry a specific gene that has been linked to the condition.. This gene produces HLA-B27, a protein or genetic marker, that more than 95% of Caucasians with ankylosing spondylitis have. However, there are people who dont have this protein who develop AS and there are many people who do carry this marker yet never develop the condition.. Researchers theorize that there may be other genes that may be involved, as well as environmental factors that trigger the gene activation, such as a bacterial infection, causing people who are susceptible to AS to activate it. Scientists have identified more than 60 genes that are believed to be associated with AS with only about 30% that are linked to HLA-B27 regarding overall risk. Other genes that have been identified as key to AS include IL-23, IL-17, IL-12, and ERAP.. It is also ...
Ankylosing spondylitis (AS) is a chronic, systemic, inflammatory form of arthritis that preferentially affects the spine leading to limitation of spine movement. The cause of AS is not fully known, but there is a strong genetic predisposition associated with a genetic marker called the human leukocyte antigen (HLA)-B27.

Background: HLA-B*27 is strongly associated with ankylosing spondylitis (AS). It represents a family of alleles that differ among ethnic groups. Objective: The aim of this study was to determine the distribution of HLA-B*27 alleles in AS patients and healthy controls in Isfahan (Iran). Methods: Sixty AS patients and 430 healthy blood donors were selected. All subjects were HLA-B*27 positive by flow cytometry. HLAB* 27 subtypes were determined by PCR-SSP. Results: Forty patients (66.7%) and 17 controls (3.95%) were HLA-B*27 positive. Subtypes detected by PCR-SSP were B*2705, B*2702, B*2704 and B*2707. One patient was B*2702/B*2710. No significant difference was found in the distribution of these alleles between AS patients and controls. Conclusion: Although Caucasian subtypes are predominant among Iranians, this population is characterized by a combination of both specific Caucasian and Oriental subtypes. However such results should be interpreted carefully because of the small sample size in our

Psoriasis is a multi-systemic chronic inflammatory disease that affects about 1.5-3% of the general population, of which almost 20% suffer from a moderate-severe form. Those patients can be treated with a systemic agent, and, in case of scarce response or contraindications, they may require a biologic therapy, such as tumor necrosis factor or interleukin-12/23 inhibitors. When also those agents fail, clinicians face a true therapeutic challenge. We report a case series of multi-failure 16 patients, successfully treated with secukinumab, a fully human monoclonal antibody that selectively neutralizes interleukin-17A, recently approved for the treatment of plaque psoriasis, psoriatic arthritis and ankylosing spondylitis ...

Objective. To assess whether there is a statistically significant difference in the frequency of common MEFV allele variants in patients with ankylosing spondylitis (AS) as compared with control patients with rheumatoid arthritis (RA) and with healthy control subjects. Methods. Sixty-two patients with AS, 50 healthy control subjects, and 46 patients with RA were assessed for the presence of MEFV variants. Exon 10 was analyzed by direct sequencing. E148Q was analyzed by restriction endonuclease enzyme digestion (REED) or by direct sequencing when REED analysis failed. Results. The allele frequency of all MEFV variants in the AS group was significantly higher than that in the pooled control group of healthy subjects plus RA patients (15.3% versus 6.8%; P = 0.021). M694V was the only variant that was significantly more common in the AS group than in the combined or individual control groups (P = 0.026 for AS patients versus healthy controls, P = 0.046 for AS patients versus RA patient controls, and ...
Ankylosing Spondylitis: NSAIDs. Nonsteroidal anti-inflammatory drugs (NSAIDs) reduce joint pain and inflammation. They do not prevent joint damage, but they can relieve pain and stiffness and improve joint function. NSAIDs may even slow the progression of the disease, as shown on X-rays. There are many NSAIDs available. NSAIDs commonly used for AS include: acetylsalicylic acid (Aspirin®, generics) flurbiprofen (Ansaid®, generics) ibuprofen (Advil®, Motrin®, generics) indomethacin (generics) naproxen (Naprosyn®, generics) Side effects associated with NSAIDs can include stomach upset, nausea, heartburn, abdominal pain, ulcers, and even stomach bleeding.

There have been marked improvements in treatment options but none have yet been shown to induce remission. The past decade has seen major advances in the diagnosis and management of ankylosing spondylitis (AS) and in research into its pathogenesis. It remains the case that no current treatments have been shown to lead to disease remissions or to halt the progression of the bony ankylosis that causes the major morbidity associated with this condition. Nonetheless, improved diagnostic methods and management have led to major benefits for patients, with marked improvements in quality of life with reduced treatment-associated side effects.. ...
