We have shown that altered Notch signalling in the paraxial mesoderm of mouse embryos changes vertebral identities. Our results provide direct evidence that the coordination of segmentation and positional specification of mesoderm-derived tissues along the anteroposterior body axis requires both sufficient levels of Notch signalling as well as cyclic Lfng activity.. The type of patients who would have been declined surgery 20 yr ago for medical reasons, are now being offered extensive procedures. They commonly have preoperative co-morbid conditions such as serious cardiovascular and respiratory impairment. Airway management may be difficult. Surgery imposes further stresses of significant blood loss, prolonged anaesthesia, and problematical postoperative pain management. The perioperative management of these patients is discussed. The advent of techniques to monitor spinal cord function has reduced postoperative neurological morbidity in these patients. We here aim to evaluate the clinical application of a 3D classification system by the use of only posterior-anterior and lateral radiographs in Lenke 1 adolescent idiopathic scoliosis (AIS). Forty Lenke 1 AIS were classified by five observers following a three-step flowchart, developed based on our previous 3D classification system. This 3D classification characterizes the curve in the frontal and sagittal views and infers the third dimension with rules based on prior data to determine the 3D subtypes of the curve. Repeated rating was performed for 20 randomly selected patients in the same cohort. Complex spinal anomalies involved in congenital scoliosis consist of a jumble of vertebral defects. Progressive scoliotic curves are frequently encountered in these patients. We evaluated the results of hemi-epiphysiodesis for this patient population. A series of ten patients with unclassified scoliosis, involving multiple hemivertebrae and/or bars, was reviewed retrospectively. Hemi-epiphysiodesis was the primary treatment in all patients. Mortality, complications and wound healing problems did not occur. The average Cobb angle for all patients changed from 54° (range: 40°-74°), preoperatively, to 58° (range: 30°-104°), postoperatively. Applying the criterion of a minimum change of 20°, to take into account measurement variability, an epiphysiodesis effect was achieved in two procedures, progression was arrested in six procedures and failure occurred in two procedures. Additionally, due to the complexity and variability in the mechanics of spinal dysfunction, implants could potentially benefit from incorporating a customizable stiffness into their design. This paper presents a spinal implant with contact-aided inserts that provide a customizable nonlinear stiffness. An analytical model was utilized to optimize the device design, and the model was then verified using a finite element model. Validation was performed on physical prototypes, first in isolation using a tensile tester and then using cadaveric testing on an in-house spine tester.
