What is Surgical Sperm Retrieval?. This is a treatment option for men with Azoospermia (absence of sperm in the ejaculate) due to obstruction or poor production (non-obstructive).. Sperm can be collected directly from the epididymis where sperm are usually stored and matured. The sperm can be collected using a fine needle and syringe. This is known as (percutaneous epididymal sperm aspiration) or PESA. Sperm can also be retrieved from the testicles, a procedure known as (testicular sperm extraction) or TESE.. Surgical sperm retrieval is carried out by an experienced surgeon, usually a Consultant Uroandrologist, or a fertility specialist. The procedure needs to be timed to coincide with the female partners egg collection. If enough sperm is retrieved, it may be possible to freeze small amounts for use at a later stage. The sperm collected is then used to inject the eggs using ICSI.. ...
Infertility is a very sensitive topic for various couples. Seeing other couples having babies without any hassle and youre trying so hard to conceive can be very disturbing and heart breaking. In recent times, several surgical procedures have been introduced to retrieve sperm. Lets have a look at surgical sperm collection (SSC) procedure in this post. What is SSR or Surgical sperm retrieval? SSR or surgical sperm retrieval has been developed to collect sperms from testis or epididymis. This treatment has helped million of couples to conceive genetic baby even when there is no ejaculation of sperm. In addition to this, its very simple. Percutaneous Epdidymal Sperm Aspiration (PESA) In this procedure sperm is collected from epididymis with the help of small needle (no cuts are involved). After sample collection, it is examined under microscope to check the presence of sperm. If PESA is not successful, then TESA procedure is…. ...
Several authors have examined the effectiveness of percutaneous procedures for sperm retrieval in OA. Sperm retrieval rates have been quoted at approximately 100% when percutaneous epididymal and testicular retrievals are combined (38-43) (Table 1). Glina et al. reported a series of 58 men with OA treated with ICSI who underwent percutaneous epididymal sperm retrievals (with rescue TESA whenever needed). The authors reported 100% recovery of motile sperm using these combined techniques. Successful repeated PESA was performed up to three times, with recovery of motile sperm in over 80% of the cases. Forty-three percent of PESA procedures yielded sufficient spermatozoa to allow cryopreservation (42). Esteves et al. reported a SRR of 97.9% among 142 men with OA. In these series, TESA as a rescue procedure after a failed PESA was performed in 17% of the cases. One-third of the retrievals yielded a sufficient number of spermatozoa for cryopreser-vation (32). Lin et al. analyzed 100 men with ...
To provide indicators for the likelihood of sperm retrieval in patients undergoing testicular sperm extraction is a major issue in the management of male infertility by TESE. The aim of our study was to determine the impact of different parameters, including testicular histopathology, on sperm retrieval in case of reoperation in patients undergoing testicular sperm extraction. We retrospectively analyzed 486 patients who underwent sperm extraction for intracytoplasmic sperm injection and testicular biopsy. Histology was classified into: normal spermatogenesis; hypospermatogenesis (reduction in the number of normal spermatogenetic cells); maturation arrest (absence of the later stages of spermatogenesis); and Sertoli cell only (absence of germ cells). Semen analysis and serum FSH, LH and testosterone were measured. Four hundred thirty patients had non obstructive azoospermia, 53 severe oligozoospermia and 3 necrozoospermia. There were 307 (63%) successful sperm retrieval. Higher testicular volume, lower
Information retrieval in the vector space model is based on literal matching of terms in the documents and the queries. The model is implemented by creating the term-document matrix, which is formed on the base of frequencies of terms in documents. Literal matching of terms does not necessarily retrieve all relevant documents. Synonymy (multiple words having the same meaning) and polysemy (words having multiple meaning) are two major obstacles for efficient information retrieval. Latent semantic indexing (LSI) and concept indexing (CI) are information retrieval techniques embedded in the vector space model, which address the problem of synonymy and polysemy. The method of LSI is an information retrieval technique using a low-rank singular value decomposition (SVD) of the term-document matrix. Although the LSI method has empirical success, it suffers from the lack of interpretation for the low-rank approximation and, consequently, the lack of controls for accomplishing specific tasks in information
TY - JOUR. T1 - Laboratory processing and intracytoplasmic sperm injection using epididymal and testicular spermatozoa. T2 - What can be done to improve outcomes?. AU - Popal, Wana. AU - Nagy, P.. PY - 2013. Y1 - 2013. N2 - There are two main reasons why sperm may be absent from semen. Obstructive azoospermia is the result of a blockage in the male reproductive tract; in this case, sperm are produced in the testicle but are trapped in the epididymis. Non-obstructive azoospermia is the result of severely impaired or non-existent sperm production. There are three different sperm-harvesting procedures that obstructive azoospermic males can undergo, namely MESA (microsurgical epididymal sperm aspiration), PESA (percutaneous epididymal sperm aspiration), and TESA (testicular sperm aspiration). These three procedures are performed by fine-gauge needle aspiration of epididymal fluid that is examined by an embryologist. Additionally, one technique, called TESE (testicular sperm extraction), is offered ...

A new study has shown that a surgical technique called microdissection testicular sperm extraction (TESE) can effectively locate and extract viable sperm in more than one-third of adult male childhood cancer survivors who were previously considered sterile due to prior chemotherapy treatment.
What exactly is surgical sperm retrieval? Heres our round-up the key techniques. These include PESA, TESE, TESA and MESA. Theyre not as scary as they sound.
PESA (Percutaneous Sperm Aspiration). Here, a fine needle is inserted through the scrotum into the epididymis and sperm obtained by gentle suction. After collection, each sample is examined under a microscope to confirm the presence of sperm.. TESA (Testicular Sperm Aspiration). Here a fine needle is inserted into the testis and tissue samples are obtained by gentle suction and examined under the microscope. If no sperm is present, a small testicular biopsy is taken through a small incision in the scrotum and testis, immature sperm can then be extracted from the tissue. The cut is then stitched back. Surgically retrieved sperm are immature and therefore not capable of fertilization by conventional means except by ICSI. After the procedure a firm scrotal support will be recommended as well as regular pain killers like paracetamol as you may feel some discomfort and bruising for about 48hrs.. If there are spare sperm or testicular tissue, these may be cryopreserved for later use. Surgical sperm ...
These procedures are done on men when semen analysis shows azoospermia or no sperms.. It is usually done under local anesthesia.. TESA: Testicular sperm aspiration. It is done by inserting a needle through the skin of the scrotum into the testis and aspirating or sucking out the fluid inside the testicle. The fluid is then checked for presence of any sperms.. PESA: Percutaneous epididymal sperm aspiration. The procedure is same as TESA but the needle is inserted into the epididymis.. MESA: microsurgical epididymal sperm aspiration. It is an open surgical procedure wherein a operating microscope is used to locate the tubules of the epididymis. The tubules are then examined for the presence of motile sperms.. TESE: Testicular sperm extraction. It is usually done in men with zero sperm count and without any anatomical blockage. The procedure involves making a small opening in the scrotum and excising bits of testicular tissue from different regions of the testis. A microscope is then used to ...
Surgical sperm retrieval is a way of collecting sperm in men who have little or no sperm in their semen. Doctors can use this sperm in fertility treatments.
Sperm retrieval refers to any procedure that is used to obtain sperm for fertility purposes. In general, sperm retrieval is performed when there is either no sperm present in the semen or if men are unable to ejaculate. In almost all cases, sperm retrieval must be utilized in combination with in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) for reasonable pregnancy rates to be obtained.
This page is the gateway to enter the knowledge of tissue retrieval technique. This presentation describe tissue extraction techniques used in laparoscopic surgery.
The method of choice for sperm retrieval (SR) is based on the type of azoospermia, which can be obstructive or non-obstructive, and the attending surgeons preferences and experience. Obstructive azoospermia (OA) is associated with the inability to detect spermatozoa in the ejaculate and post-ejaculate urine after centrifugation due to the bilateral obstruction of the seminal ducts.. Hence in such cases Percutaneous Epididymal Sperm Aspiration is done. We have got successful results with PESA and we can keep the extra sample frozen for future use.. Testicular Sperm Extraction (TESE)- The extraction of the testicular tissue (Fig 12) for sperm search and isolation is done usually in PESA failed patients or proven obstructive azoospermia cases. For conventional TESE, a standard open surgical biopsy technique is used to remove the testicular tissue without the aid of optical magnification. This is a day care and minimally invasive procedure.. ...
If no sperm is retrieved from the epididymis or where a non-obstructive problem, or a problem with the sperm-making process is diagnosed, then a biopsy of one or both testes may be necessary. In this case a small cut is made in the scrotal skin and in the surface of the testis so that a small amount of tissue can be removed. A small dissolving stitch will allow easy healing of these wounds. In this case a small portion of the tissue may be sent for laboratory assessment of its structure and the sperm production process. The rest will be retained for sperm retrieval. Although it is less likely, mature testicular sperm can sometimes be frozen for future use.. ...
The sperm aspiration methods from the epididymis (MESA, PESA) or the testes (TESA) are simple, but are usually blindly performed and can cause testicular tissue damage such as haematoma, cysts and testicular atrophy. Testicular biopsy (TESE) is recommended by the international scientific literature as a more effective method of sperm retrieval and it is associated with fewer complications. For this reason, in our Unit, the surgical sperm retrieval is performed by testicular biopsy (TESE) by specialized surgeons.. The procedure takes place on the day of egg collection with intravenous or local anaesthesia. The small testicular tissue pieces are processed by the embryologist in order to retrieve any available sperm. Following the retrieval, ICSI can be performed. Any excess sperm can be cryopreserved for future use to avoid performing another biopsy.. ...
May, 2019 - American Journal of Roentgenology. ABSTRACT :. OBJECTIVE. The objective of our study was to determine whether contrast-enhanced ultrasound (CEUS) perfusion measurements obtained before testicular sperm aspiration (TESA) can improve or predict sperm retrieval (SR) outcomes of TESA in patients with azoospermia.. SUBJECTS AND METHODS. Between May 2017 and January 2018, 70 patients with azoospermia (mean age, 29 years; age range, 22-41 years) underwent testes CEUS within 10 days before TESA. Major perfusion areas were visually chosen, and their ranges were recorded. The other areas were defined as minor perfusion. CEUS quantitative features were acquired for both the main perfusion area and whole testis. Testis tissue biopsies were taken for both major and minor perfusion areas by cognitive fusion, and SR outcomes were compared. Associations between testicular volume, quantitative CEUS features, and SR outcomes were analyzed.. RESULTS. Twenty-four men were found to have obstructive ...
At Yale Urology, we have a world class, state-of-the-art center for treatment of Azoospermia (no sperm in the ejaculate). Dr. Stanton Honig, director of the Male Sexual Medicine Program, will review your case and determine the best approach. There are two groups of patients that may be candidates for sperm retrieval: those with obstruction, and those without obstruction (i.e. a problem with sperm production). Both scenarios are treated differently, and in most cases, patient history, a physical examination, and blood work can determine whether your case is obstructive or non-obstructive. Cases of obstruction usually are a result of a prior vasectomy, absence of the vas deferens, or a blockage at the epididymis, a hernia repair site, or at the ejaculatory duct. Obstruction cases can be treated with either reconstructive surgery or sperm retrieval. In nearly all obstruction cases, sperm can be retrieved from the testis using a procedure called testis sperm aspiration or extraction (TESE), or ...

The alternative to surgical reversals or reconstruction / repair is sperm retrieval by needle aspiration, and the techniques commonly employed, are PESA (Percutaneous Epididymal Sperm Aspiration) and TSE (Testicular Sperm Extraction). Most men are familiar with their soft and squishies, but know little of the epididymis; sausage-like sacs attached behind each testicle that contain meters of tightly coiled miniscule tubes and canals. In essence, they connect the testicles bilaterally to the vas deferens, which constitute the main pipeline of the males reproductive tract. The epididymis is the subject of controversy, but it can be safely said that they act to accumulate and transport sperm. In the normal male, it takes 6 weeks for sperm to travel their path from genesis in the testicles to ejaculation, and a large part of that time is spent in the epididymis. The controversy though is intriguing. Some specialists believe that the very fine hairs within the epididymis literally stroke the sperm ...

Notice how the lines are flat early on, then midway the search retrieval rate starts to fall and the zero search rate starts to rise. This is what it looks like when more and more users are struggling to find content.. As the trends continued it became increasingly clear that it wasnt a self-correcting problem and we needed to find it and fix it.. We tried to find a pattern in the searches that explained the trends. The first question we asked was whether the metrics were going in the wrong direction for different reasons or for the same reason. A search can be unretrieved (blue line goes down) either when there are no search results, in which case its also a zero search (red line goes up), or when there are search results but the user doesnt click any of them (blue line goes down but red line holds steady).. In our case, we looked at the subset of unretrieved searches that were not zero searches and found that the retrieval rate held steady. The retrieval rate fell only when we included the ...

Background The dorsogluteal and ventrogluteal intramuscular injection sites both have their use in clinical practice; however, it has not been established in whom one or the other should be preferentially targeted or avoided. There is a need for an evidence-based approach towards site selection for a successful intramuscular injection outcome and to avoid unwanted injection outcomes of inadvertent subcutaneous injection or bone contact. Injection outcome is dependent on injection site subcutaneous fat thickness and muscle thickness; these are likely influenced by gender and anthropometry. Objectives To determine whether subcutaneous fat, muscle, and total tissue thicknesses differ between the dorsogluteal and ventrogluteal sites, and whether theoretical injection outcome (intramuscular, subcutaneous, or bone contact) can be predicted by demographic and anthropometric data and described by an algorithm. Design Cross-sectional study design. Settings University in Australia. Participants 145 volunteers

3. Due to variations in the retrieval and confirmation of isolates submitted to Sexually Transmitted Bacteria Reference Laboratory of the Health Protection Agency in 2005, the data on resistance prevalence are statistically weighted. This is done to avoid resistance estimates being under representative of sites that have a low retrieval rate and over representative of sites that have a high retrieval rate. Consequently, estimates for previous years have been recalculated in this way and so values presented here may differ slightly from to the unweighted estimates that have appeared in previous GRASP reports ...

The past decade has seen a dramatic increase in the amount of data captured and made available to scientists for research. This increase amplifies the difficulty scientists face in finding the data most relevant to their information needs. In prior work, we hypothesized that Information Retrieval-style ranked search can be applied to datasets to help a scientist discover the most relevant data amongst the thousands of datasets in many formats, much like text-based ranked search helps users make sense of the vast number of Internet documents. To test this hypothesis, we explored the use of ranked search for scientific data using an existing multi-terabyte observational archive as our test-bed. In this paper, we investigate whether the concept of varying relevance, and therefore ranked search, applies to numeric data - that is, are data sets are enough like documents for Information Retrieval techniques and evaluation measures to apply? We present a user study that demonstrates that dataset ...

It is estimated that about 2 million couples each year face the problem of infertility. While assisted reproductive technologies (ARTs) have helped many couples conceive, vasectomy reversal is the main option for restoring fertility for the vasectomized men. Small percentages of couples who fail in ARTs change their mind and would like to have vasectomy reversal. The common questions they ask are how the sperm retrieval techniques affect their qualification for the vasectomy reversal. This blog specifically answers this question.. ARTs requires the collection of both egg and sperm to increase the likelihood of conception. The sperm-extraction methods vary depending on the mens fertility, the ability of the surgeon to retrieve sperm and the needs of the couple. The five main types of sperm retrieval techniques are:. Microsurgical Epididymal Sperm Aspiration (MESA): a small incision in scrotum is made to identify the epididymal tubules, then the sperms in epididymis are aspirated with a small ...

One in every 500 men is born with an extra X chromosome, with a karyotype of 47, XXY rather than the typical 46, XY male karyotype. This is known as Klinefelter Syndrome and is the most common genetic variation which impacts a mans fertility. Men with pure Klinefelter Syndrome are nearly universally azoospermic with a sperm count of zero in the semen due to a lack of adequate sperm production in the testicles. Just nearly two decades ago these men were told they could not be fathers with their own sperm, but things have dramatically changed in our understanding since then.. The majority of men with Klinefelter Syndrome, approximately 70%, can have sperm retrieved through a highly meticulous microdissection testicular sperm extraction (microTESE) in the hands of highly skilled microsurgically trained reproductive urologists. Dr. Parviz Kavoussi performs microTESE regularly with excellent sperm retrieval rates. The embryologists from Westlake IVF join him in the operating room for this highly ...
The objectives of this study were: 1) To determine the effect of oocyte retrieval techniques (slicing and aspiration) on the quality and quantity of cattle oocytes, 2) To evaluate the effect of different concentrations of hormones on the maturational rate of cattle oocytes selected by brilliant cresyl blue staining, 3) To evaluate fertilization rate and cleavage/embryonic development of oocytes with or without cumulus cells, and 4) To compare the effect of fresh and frozen thawed semen on the fertilization rate of cattle oocytes. In Experiment 1: oocytes were recovered from abattoir derived ovaries using slicing and aspiration. The recovered oocytes were exposed for 90 minutes to 26μM of brilliant cresyl blue (BCB) stain and classified according to the colour of their cytoplasm: BCB+ (oocytes with blue cytoplasm) and BCB- (unstained oocytes). There was no difference (P,0.05) in the quality of oocytes recovered using slicing (60.7 %) or aspiration (53.7 %) techniques. In experiment 2: The BCB ...

Young boys with KS typically display various hormonal abnormalities and spermatogenic degeneration before the completion of puberty. There is controversy as to the optimal time to begin fertility treatment in KS patients. While early intervention has been proposed, treatment of KS adolescents is not warranted based on currently available data. It can be diagnosed before or immediately after a child is born. In other cases, Klinefelter Syndrome is identified during childhood when learning or behavioural difficulties develop, or around the time of puberty when expected physical changes are delayed or dont happen.. Because the symptoms arent always obvious, an adult might not be diagnosed until they seek medical help for infertility, a loss of sex drive, or a bone fracture. The majority of Klinefelter cases are diagnosed in adulthood.. We think that as many as 75% of men with Klinefelter Syndrome are never diagnosed, and remain untreated for life, possibly because doctors dont routinely check the size of testicles.. ...
In those patients with obstructive azoospermia or vasectomy, it is possible to obtain sperm from the epididymis so that they can have a child through the ICSI technique of in vitro fertilization.

In terms of the childs education, XXYs with Klinefelter Syndrome are typically well behaved in the classroom and are eager to please the teacher. However, if they are presented with material that they find difficult, they tend to withdraw. If it goes unnoticed by the teacher, the child may fall behind in the curriculum and may need to be held back a grade. Therefore, it is important, for the success of the child, to be in a small classroom where he can receive sufficient individual attention from the teacher. The childs chances of success are even greater if the parents and school cooperate to form an individualized education plan and provide related services if necessary ...
For several years, scientific, industrial, and biological fields have benefited from knowledge of phase information, which allows for the revealing of hidden features of various objects. An alternative to interferometry is single-beam phase retrieval techniques that are based on the transport of intensity equation, which describes the relation between the axial derivative of the intensity and the phase distribution for a given plane in the Fresnel region. The estimation of the axial intensity derivative is obtained from a series of intensity measurements, where the accuracy is subject to an optimum separation between the measurement planes depending on the number of planes, the level of noise, and the actual object phase distribution. In this Letter, a quantitative analysis of the error in estimated axial derivative is carried out and a model is reported that describes the interdependence between these parameters. The results of this work allow for estimation of the optimum separation between ...
Males diagnosed with Klinefelter Syndrome also often experience learning difficulties and speech and language disorders. Because of the host of problems that this syndrome presents, early diagnosis and treatment is crucial. There is no cure for this condition, but there are numerous treatments available. With the right medical treatments, most males can grow up to have normal lives. It is imperative for a patient to begin treatment as early as possible. Unfortunately, if a physician fails to diagnose, or misdiagnosis, Klinefelter Syndrome, it can considerably increase the number of medical problems that a patient experiences, thus affecting their quality of life.. Because this condition is not well-known, it is not included in the standard battery of tests that doctors usually perform on unborn children. The cause of Klinefelter Syndrome can generally be attributed to genetic factors, but the use of certain prescription medications, such as Wellbutrin and other antidepressants during pregnancy, ...
You and your wife/partner are in this together. Joining her on visits to the fertility doctor is one of the best ways to show your support. No matter the diagnosis, there are always options. Recent advances in fertility treatments provide solutions for practically any type of male infertility. ICSI - direct injection of sperm into the egg, is successful in creating a child even in cases with extremely low sperm numbers. TESE - testicular sperm extraction - can provide just enough sperm to fertilize the partners eggs - even if the sperm count is zero! These are only two examples of what can be done ...

Different babies develop at different rates. Sitting up and standing come at different times to babies and it doesnt necessarily mean that there is a problem. Usually, even those that are a bit slower than the others will catch up with the rest at some point.. Despite this, however, slower than usual developing can be a sign of Klinefelter syndrome. It might not be noticeable to begin with because babies do develop at different rates. In addition to sitting and standing, it can also cause babies to develop slower than others when it comes to speech. If you do notice a baby is developing slower than they should do, you should arrange to get it checked out.. Advertisement. ...
Complexities of Care in Klinefelter Syndrome: An APRN PerspectiveAutors:Sharron Close, PhD, MS, CPNP-PC, Amy Talboy, MD, Ilene Fennoy, MD, MPH
Hubby and I are 34 & 35 (Im 34). We have two biological kids (one via ivf). We have male infertility (plus, its been a few years since we had in vitro so its possible, though not confirmed, that my ovarian supply is on the decline...I turn 35 in 3 months.) We have a decent cash reserve...not completely loaded, but live quite comfortably. So, we thought we were done having kids and my husband had a vasectomy a few years ago. Well, now my biological clock has started pounding LOUDLY. My understanding is that for a few extra thousand (which is feasible for us) in addition to fresh ivf fees, we can have his sperm retrieved. So, we can def afford ivf plus sperm retrieval...but, what are the chances that will work, ya know, given my age? Plus, fresh cycles/stimulation are very taxing physically. Plus the fact that my husband and I are perfectly comfortable and happy to adopt an embryo(s) as a way to not only grow our family but also give these already-created babies a chance at life.
