In general cantons regulate and control the Swiss health service system; patient flows within and between cantons are thereby partially disregarded. This paper develops an alternative spatial model, based upon the construction of orthopedic hospital service areas (HSAOs), and introduces indices for the analysis of patient streams in order to identify areas, irrespective of canton, with diverse characteristics, importance, needs, or demands. HSAOs were constructed using orthopedic discharge data. Patient streams between the HSAOs were analysed by calculating three indices: the localization index (% local residents discharged locally), the netindex (the ratio of discharges of nonlocal incoming residents to outgoing local residents), and the market share index (% of local resident discharges of all discharges in local hospitals). The 85 orthopedic HSAOs show a median localization index of 60.8%, a market share index of 75.1%, and 30% of HSAOs have a positive netindex. Insurance class of bed, admission type

This paper uses spatial data of cases of intimate partner violence against women (IPVAW) to examine neighborhood-level influences on small-area variations in IPVAW risk in a police district of the city of Valencia (Spain). To analyze area variations in IPVAW risk and its association with neighborhood-level explanatory variables we use a Bayesian spatial random-effects modeling approach, as well as disease mapping methods to represent risk probabilities in each area. Analyses show that IPVAW cases are more likely in areas of high immigrant concentration, high public disorder and crime, and high physical disorder. Results also show a spatial component indicating remaining variability attributable to spatially structured random effects. Bayesian spatial modeling offers a new perspective to identify IPVAW high and low risk areas, and provides a new avenue for the design of better-informed prevention and intervention strategies.
This study developed a method for measuring change in socio-economic equity in health care utilisation using small-area level administrative data. Our method provides more detailed information on utilisation than survey data but only examines socio-economic differences between neighbourhoods rather than individuals. The context was the English NHS from 2001 to 2008, a period of accelerated expenditure growth and pro-competition reform. Hospital records for all adults receiving non-emergency hospital care in the English NHS from 2001 to 2008 were aggregated to 32,482 English small areas with mean population about 1500 and combined with other small-area administrative data. Significant efforts have been undertaken in medicine to identify hospital and primary care service areas (eg, the Dartmouth Atlas of Health Care) using patient origin information. Similar research in dentistry is nonexistent. The goal of this dissertation was to develop and refine methods of defining dentist service areas (DSAs) using dental insurance claims. These service areas were then used as spatial units of analysis in studies that examined relationships between utilization of oral health services, dentist workforce supply, and service area characteristics. Enrollment and claims data were obtained from the Iowa Medicaid program for children and adolescents ages 3-18 years during calendar years 2008 through 2010. The first study described rates of treatment by orthodontists in children ages 6-18 years. Orthodontic DSAs were identified by small area analysis in order to examine regional variability in utilization. The overall rate of utilization was approximately 3%; 19 DSAs were delineated.
Background Several European studies have found significant small area variation in the risk of childhood onset (type 1) diabetes (T1D) which has been interpreted as evidence for contextual determinants of T1D. However, this conclusion may be fallacious since the limited number of newborn infants and the low risk for T1D is a source of spurious variability not properly handled by usual statistical methods. Selection of individual variables for any measure should be based on the theoretical fit with the concept we are interested in measuring, such as material deprivation, and on the particular context of a country. Most deprivation measures include domains relating to income, employment, socioeconomic status or class (often based on job type), education, housing and ownership of specific goods or items. Some measures also include domains relating to access to various services (schools, shops, doctors) or information on the environment (street lighting, crime rates).. There are strong theoretical grounds to include each of the listed domains in deprivation measures. For example, low-income and unemployment reflect deprivation as they limit material resources, while low levels of education disadvantage people in accessing many resources, such as better jobs or services. Different indicators of morbidity for chronic disease may not necessarily be available at a disaggregated spatial scale (e.g., for small areas with populations under 10 thousand). Instead certain indicators may only be available at a more highly aggregated spatial scale; for example, deaths may be recorded for small areas, but disease prevalence only at a considerably higher spatial scale. Nevertheless prevalence estimates at small area level are important for assessing health need. An instance is provided by England where deaths and hospital admissions for coronary heart disease are available for small areas known as wards, but prevalence is only available for relatively large health authority areas. Downloadable (with restrictions)! This paper exploits rich SOEP microdata to analyze state-level variation in health care utilization in Germany. Unlike most studies in the field of the Small Area Variation (SAV) literature, our approach allows us to net out a large array of individual-level and state-level factors that may contribute to the geographic variation in health care utilization. The raw data suggest that state-level hospitalization rates vary from 65 to 165 percent of the national mean. Ambulatory doctor visits range from 90 to 120 percent of the national mean. Interestingly, in the former GDR states, doctor visit rates are significantly below the national mean, while hospitalization rates lie above the national mean. The significant state-level differences vanish once we control for individual-level socio-economic characteristics, the respondents health status, their health behavior as well as supply-side state-level factors.

When they threw the technology at ACE, it quickly became known as the blob: because everything was linked to everything else, you couldnt make out the individual edges in the graph. Self-poisoning is one of the most common methods of suicide worldwide. The intentional ingestion of pesticides is the main contributor to such deaths and in many parts of rural Asia pesticide self-poisoning is a major public health problem. To inform the development of preventive measures in these settings, this study investigates small-area variation in self-poisoning incidence and its association with area-based socioeconomic and agricultural factors. Ecological analysis of intentional self-poisoning in a rural area (population 267,613) of Sri Lanka in 2002. The geographic distribution of cases was mapped to place of residence. Using administrative division (GN), median population size 1416, as unit of analysis, associations with socioeconomic and agricultural indicators were explored using negative binomial regression models. Defining a CA allows a cancer center to describe its primary patient population and assess how well it meets the needs of cancer patients within the CA. A CA definition is required for cancer centers applying for National Cancer Institute (NCI)-designated cancer center status. In this research, we constructed both diagnosis and diagnosis/treatment CAs for the Massey Cancer Center (MCC) at Virginia Commonwealth University. Pentaclethra macroloba is a dominant tropical canopy tree found throughout Central America and Amazonia. The annual mortality rate of P. macroloba seedlings is 60% in the tropical wet forest at the La Selva OTS biological station in Costa Rica. A possible cause of this dramatic mortality rate is insect herbivory, which can reduce seed production, plant growth, and survivorship, as well as cause leaf abscission. During the summer of 2009, new leaves of P. macroloba seedlings were selectively herbivorized by geometridae caterpillars to remove 25% of leaf material. Rachis growth rates were calculated over a 15-day period to compare differences between controlled and herbivorized leaves. In addition, 115 P. macroloba seedlings were tagged and photographed for leaf area analysis. The objective of this study was to evaluate the influence of biofield treatment on physical and thermal properties of amide group containing compounds (AM and CA). The study was performed in two groups (control and treated). The control group remained as untreated, and biofield treatment was given to treated group. The control and treated compounds were characterized by X-ray diffraction (XRD), differential scanning calorimetry (DSC), thermogravimetric analysis (TGA) and surface area analysis. XRD of treated AM showed decrease in intensity of peaks as compared to control sample. 1. The effective volume of distribution of labelled glycerol was studied in conscious young adult rabbits provided with in-dwelling cannulae in the femoral blood vessels. This could be estimated after sampling arterial blood throughout an intravenous infusion of [2-3H]glycerol. The volume was calculated by using an algebraic method of graphical area analysis over 100 min of equilibration, and is symbolized 100V e or 100V e%. It occupied 34.1 +/- 2.2% (mean +/- S.E.M.; n = 13) of the body weight. The pool of endogenous glycerol occupying this space is distinguished in the present paper by calling it the transit pool, symbolized 100Me. 2. The median time of transit of glycerol through this pool was approx. 6 min in these conscious rabbits with normal (less than 0.2 mM) blood glycerol concentrations. 3. The study of selective metal oxide-based binary/ternary systems has received increasing interest in recent years due to the possibility of producing efficient new ceramic materials for relative humidity (RH) detection, given the superior properties of the mixed compounds in comparison with pristine ones. The aim of this work was focused on preparation and characterization of non-doped and Nb|sub|2|/sub|O|sub|5|/sub|-doped TiO|sub|2|/sub| : WO|sub|3|/sub| pair (in the pellet form) and evaluation of corresponding humidity-dependent electrical properties. The microstructure of the samples was analyzed from scanning electron microscopy, X-ray diffraction patterns, Raman spectra, BET surface area analysis, and porosimetry. The catalytic conversion of methanol to hydrocarbons of gasoline range has been studied over HZSM-5, ZnO/HZSM-5, CuO/HZSM-5 and CuO/ZnO/HZSM-5. The selectivity of methanol conversion to hydrocarbon has been found to be a function of CuO/ZnO/HZSM-5 concentration. A comparison has been done to study the conversion and yield of different metal oxides doped on the catalyst. The catalysts were prepared by means of impregnation technique followed by heat treatment at 550 °C for 5 h. The copper and zinc oxide loading over HZSM-5 (Si/Al = 45) catalyst were 7 wt.% each. These catalysts were characterized by X-ray diffraction and surface area analysis. The performances of the catalysts were evaluated by conducting experiments in a fixed bed reactor (i.d.: 19 mm) made of stainless steel. We study the conditions under which discontinuous mechanical properties of a collapsible tube can induce transcritical flows, i.e. the transition through the critical state where the speed index (analogous to the Mach or the Froude numbers for compressible and free surface flows, respectively) is one. Such a critical transition may strongly modify the flow properties, cause a significant reduction in the crosssectional area of the tube, and limit the flow. General relationships are obtained for a short segment using a one-dimensional model under steady flow conditions. Marginal curves delimiting the transcritical regions are identified in terms of the speed index and the cross-sectional area ratio. Since there are many examples of such flows in physiology and medicine, we also analyse the specific case of prosthesis (graft or stent) implantation in blood vessels. The annual incidence of injury admissions coded for maltreatment syndrome declined in infants and in 1-3 year olds, while admissions coded for maltreatment-related features increased in all age groups. The combined incidence of these categories remained stable and represents 1.5-6.4% of all acute injury admissions in children under 5 years old. There was wide variation between hospitals in recording of maltreatment-specific and maltreatment-related codes, which was not explained by age, deprivation indices or case load. Compared with a national trauma registry, maltreatment-specific codes or assault were recorded less frequently in HES..
