This is a single-blinded study with parallel arm design, stratified sampling and blocked randomization. 120 individuals with post-stroke UE hemiparesis will be enrolled; 40 participants per 3 UE impairment levels as defined by the investigators measurement framework. Participants will engage in 12 sessions (3x/wk for 4 weeks, 2 hrs/session) of reach-to-grasp task-practice. Within each impairment level, subjects will be randomized to a treatment group; 20 to targeted task-practice and 20 to non-targeted task-practice. Targeted task-practice is personalized to patient-specific UE motor deficits with initial targets (goals) defined according to baseline measures and targets continually progressed after every 3 therapy sessions so that the goals match the patients evolving level of ability. Non-targeted task practice is defined as a standard of care task-practice intervention intended to increase UE use but not alter specific impairments. Data analyses will include (1) ANOVA to test the effects of ...
Background: Based on the available long-term results of cardiac stem cell therapies, it seems, that it offers short-term moderate benefits, but the long-term outcome is still matter of debate. In 2008, the Austrian arm of the MYSTAR study (a prospective multi-center single-blind trial) including patients with recent AMI and treated with combined (intramyocardial and intracoronary) delivery of autologous BM-MNCs has been completed with 1-year FUP. The MYSTAR results showed moderate but significant improvement in infarct size and LV function similar to other trials, and confirmed safety, feasibility and efficacy of BMC treatment in AMI patients. The patients enrolled in the study reach the 5 to 8 years FUP at 2011, raising the question whether the combined delivery of autologous BM-MNCs results in a long-term benefit for these patients.. Aim of the study: To investigate the long-term, 5 years clinical outcome of patients enrolled into the MYSTAR study.. Study design: Prospective non-randomized ...

Children conceived after IVM did not show differences during embryonic development, at birth nor in their neuropediatric development at the age of 2 compared to their counterparts after IVF and after ICSI (Bayley score 91.3 ± 21.0 for IVM, 96.8 ± 13.2 for IVF and 103.9 ± 13.1 for ICSI) and of the pooled ART group compared to children after spontaneous conception (96.6 ± 16.4 ART and 103.2 ± 9.4 spontaneous conception). When analyzing singleton pregnancies only, again no differences during pregnancy, at birth and at their 2-year evaluation were detected between IVM versus IVF and IVM versus ICSI. ...
OBJECTIVE To determine whether an activity specific exercise program could improve ability to perform basic mobility activities in long-term care residents with Alzheimer disease (AD). DESIGN Randomized, controlled, single-blinded clinical trial. SETTING Residents of 7 long-term care facilities. PARTICIPANTS Eighty-two long-term care residents with mild to severe AD. INTERVENTION An activity specific exercise program was compared to a walking program and to an attention control. MEASUREMENTS Ability to perform bed mobility and transfers was assessed using the subscales of the Acute Care Index of Function; functional mobility was measured using the 6-Minute Walk test. RESULTS Subjects receiving the activity specific exercise program improved in ability to perform transfers, whereas subjects in the other 2 groups declined.
# 161 Scoliosis Classification and Specific Exercises Scoliosis affects up to 5% of the population. That means 5 out of every 100 patients a therapist treats will have scoliosis. In addition, more patients with scoliosis will complain of back pain than those without scoliosis. It can be theorized that an even higher population of patients with scoliosis will present to a therapist for back pain. Few therapists have received training to recognize and classify scoliosis both clinically or with imaging and do not incorporate a scoliosis specific treatment plan into their plan of care. This workshop will teach therapists how to clinically classify scoliosis and incorporate scoliosis specific exercises into a plan of care. Information is applicable to all ages of children and adults.Participants will take home a skill set they can start using the next Monday they return to clinic as well as a solid base to further their exploration of a scoliosis specific exercises. Transcranial anodal stimulation (tDCS) improves manual dexterity in healthy old adults. The underlying changes in finger force behavior for this improved dexterity have not been reported. Here, we investigated the effects of tDCS (20-min) over primary motor cortex (M1) combined with repeated practice on the Grooved pegboard test (tDCS+MP) on the fingertip forces applied to an object during grasp and manipulation. Eight right-handed able-bodied individuals (60-85 years) participated in a sham-controlled, single-blinded study. Each participant received anodal and sham intervention in two sessions at least 5-day apart. Before and after intervention, they performed a key-slot task that required inserting a slot on an object onto a stationary bar, an isometric force production task using a pinch grip, and the Grooved pegboard test. Anodal relative to sham tDCS+MP allowed participants to better retain the improved performance on the pegboard test. For the isometric task, anodal tDCS+MP significantly ...

Context: Neural mobilization is commonly used in sports, and previous studies have suggested that it has a positive impact on lower-limb flexibility and performance. However, studies exploring the effect of neural mobilization dosage are almost nonexistent. Objectives: This study aimed to assess whether 2 distinct dosages of neural gliding mobilization (4 and 8 sets of 10 repetitions) impact the flexibility and performance of both the mobilized and nonmobilized lower limb in basketball athletes differently. Design: Randomized, parallel, and single-blinded study. Setting: Amateur and professional basketball clubs. Participants: Fifty-two basketball athletes (40 men and 12 women), who were distributed into 2 groups; one received 40 (n = 28) and the other 80 repetitions (n = 24) of neural gliding mobilization. Intervention: Neural gliding mobilization applied to a single limb (the dominant limb). Main Outcome Measures: Knee extension angle for hamstring flexibility; hop tests and single-leg ...
Felix, Craig imagines my point totally accurately! Encouraged by this research, timoptic ocudose cost which confirmed the rationale for oral L-citrulline supplementation as a donor for the L-arginine/NO pathway in patients with ED, researchers in the Department of Urology at the University of Foggia, Foggia, Italy, conducted a single-blind trial, in which 24 men with mild ED (erection hardness score of 3), mean age 565 years, received a placebo for 1 month and L-citrulline, 15 g/d in 2 doses, for another month? Both standing and supine blood pressure are reduced following administration of lisinopril, timoptic ocudose cost and although symptomatic hypotension is rare, it occurs more commonly in patients who are hypovolemic or hyponatremicThe mechanism by which lisinopril improves the outcome following an acute MI involves reduced peripheral vascular resistance, improved perfusion and a direct action on the myocardium. People develop herpetic whitlow when they come into contact with areas ...
We included randomised clinical trials with blinded and non-blinded assessment of the same binary outcome. We excluded trials where it was unclear which group was experimental and which was control as such trials would not allow us to determine the direction of any bias; trials in which only a subgroup of patients had been evaluated by blinded and non-blinded assessors, unless they were selected at random; trials in which blinded and non-blinded assessors had access to each others results (for example, blinded assessments were provided to non-blinded assessors as a quality enhancement procedure); and trials where initially blinded assessors clearly had become unblinded-for example, when radiographs showed ceramic material indicative of the experimental intervention. Finally, we excluded trials with blinded end point committees adjudicating the assessments made by non-blinded clinicians because such adjudication often involves previous knowledge of the non-blinded assessment or is restricted to ...
Summary-Among more than half of patients with heart failure who have a preserved ejection fraction, treatment is largely empirical and predominately focused on the cardiovascular phenotype. A majority of patients with heart failure who have a preserved ejection fraction are older adult women with hypertension and several other comorbidities, including obesity, renal dysfunction, diabetes mellitus, and anemia, among others. Whether treating these comorbidities will have clinical benefits has been suggested but as yet has not been subject to rigorous study. Accordingly, we conducted a prospective, randomized, single-blind clinical trial of treating anemia with epoetin alfa in older adults with heart failure who have a preserved ejection fraction. The trial recruited an older adult cohort (mean age, ,75 years), predominately women (67%) with multiple comorbidities (including chronic pain and depression), which mimics what is seen in community-based studies of heart failure, but has, until now, not ...

Knowing which muscles you will call upon to complete any specific exercise - as a start I would recommend - Strength Training Anatomy by Frederic Delavier. If you are serious about getting fit and avoiding crippling injury you need to focus on this information. When you know what muscles you will be using then you can test them by targeting with specific exercises. By following this slow and progressive approach you will often discover major deficiencies in particular muscles. Once safely identified you can then focus on strengthening these weaker muscles before progressing onto complex exercises that call upon many individual muscles and groups.. Whenever you start a new exercise no matter it it is body weight like pull-ups, dips or push-ups or machine or free weights start with low number of repetitions and or low weight. You need to focus completely on the way the exercise is using your muscles. Be tuned to any pull or strain while carrying out an exercise and listen to your body ...

The Sea Tribes Olrich was that characters only repaint save for the standard black version. Its OK. While the others have little details that just bring them so much life, Olrich has a black suit of armor, purple skin, and some gold around his eyes while the eyes remain unpainted. Less was done here, and it shows - you get a swell clear purple dorsal fin, and the armor looks good, but its decidedly un-orca-like in how he sits on your desk. Blackfish, he is not.. With no weapons and sort of blah deco, hes the sets main shortcoming. Perhaps clear black skin and purple armor would have been cooler, but I guess well never know will we? Background: Irritable bowel syndrome (IBS) is among the most common gastrointestinal disorders, and a substantial proportion of patients have an insufficient response to treatment. In a single-blinded study, sacral nerve modulation (SNM) significantly reduced symptoms of IBS. The aim of the present double-blinded, randomized, placebo-controlled study was to evaluate the effect of SNM for IBS. Methods: We included patients with diarrhea-predominant or mixed IBS and a minimum baseline IBS-specific symptom score of 40 points (Gastrointestinal Symptom Rating Scale-IBS version). Patients underwent a 6-week period of SNM. In the first 4 weeks, the patients were randomized 1:1 to have the neurostimulator set subsensory or OFF for 2 weeks and then the opposite for another 2 weeks. Patients and investigators were blinded to settings. In the remaining 2 weeks, the stimulation was set suprasensory. IBS-specific symptoms and quality of life were assessed with validated questionnaires and bowel diaries. Key ...
Superior target lesion primary patency at 12 months with the IN.PACT AV DCB versus standard angioplasty, new data show. In addition to Holden, Robert Lookstein (Ichan School of Medicine at Mount Sinai, New York, USA) and Hiroaki Haruguchi (Haruguchi Vascular Access Clinic, Tokyo, Japan) are principal investigators of the IN.PACT AV Access investigational device exemption (IDE) study, which evaluated the IN.PACT AV DCB versus PTA.. They conducted a global, prospective, 29-centre, 1:1 randomised, single-blinded study evaluating the safety and effectiveness of DCB angioplasty versus PTA in dysfunctional AVF, following 330 patients out to five years. Each patient enrolled in the study had lesions up to 10cm in length in the native AVF.. The primary safety outcome was measured by the serious adverse event rate involving the AVF access within 30 days of the procedure. The primary effectiveness outcome was defined as freedom from clinically-driven target lesion revascularisation (CD-TLR) or access ...
This is a multi-center single-blinded study evaluating the effectiveness of a rigid TLSO for the treatment of adolescent idiopathic scoliosis. The study began as a randomized controlled trial; however, low enrollment in the randomized group caused a shift in the study design to allow patients who chose a specific treatment to be followed and included here. A successful outcome was defined as skeletal maturity without curve progression to 50 degrees or more. The combined group had a success rate of 72% compared to 48% success in the observation group. This study also included data from temperature sensors used to monitor time spent in the orthosis. There was a high correlation of successful outcomes to the time spent in the orthosis. The large disparity between success rates in treatment and observation groups along with the significant dose response to bracing led the researchers to terminate the study early. The researchers concluded that brace wear significantly decreases progression of ...
The present randomized, placebo-controlled, single-blind trial demonstrates that 4 weeks of treatment with rosiglitazone reduces inflammatory cell composition and cell activation in carotid artery specimen of nondiabetic patients. Moreover, rosiglitazone increased collagen content in the plaque, suggesting that TZD treatment renders these plaques more stable and less vulnerable.. Previous studies have shown that TZDs exhibit antiinflammatory and antiatherogenic properties in vascular cells in vitro, and data from animal models of arteriosclerosis demonstrated a reduction in lesion development on TZD treatment.8 Moreover, clinical studies have shown that TZDs reduce serum levels of inflammatory arteriosclerosis markers in treated patients11,12,22 and induce beneficial morphological changes in the vessel wall by reducing IMT of the carotid artery13 as well as neointima formation after coronary stent implantation.15,16 The present study extends our knowledge on the effect of TZDs in the vessel wall ...

Barker-Collo and colleagues (2009) evaluated the effectiveness of Attention Process Training (APT) in improving attention and broader outcomes in stroke survivors 6 months after stroke. In this single-blinded clinical trial, 78 stroke survivors were randomized to receive either APT or standard rehabilitative care. The participants were evaluated in four aspects of attention: sustained, selective, divided, and alternating, as well as the auditory and visual aspects of attention. The study participants were randomly assigned to receive standard care plus up to 30 hours of APT or standard care alone. At 6 months the participants who had APT had an average improvement of 2.49 standard deviations higher than the standard care participants on full-scale attention scores. The researchers acknowledged that APT appears to be a viable and effective means of improving attention deficits in stoke victims, but cautioned that further studies with larger samples and longer follow-up periods are needed to ...

Blinding (masking): in an experimental study, refers to whether patients, clinicians providing an intervention, people assessing outcomes, and/or data analysts were aware or unaware of the group to which patients were assigned. In the design section of Evidence-Based Nursing abstracts of treatment studies, the study is identified as blinded, with specification of who was blinded; unblinded, if all parties were aware of patients group assignments; or blinded (unclear) if the authors did not report or provide us with an indication of who was aware or unaware of patients group assignments.. Concealment of randomisation: concealment of rando-misation is specified in the design section of Evidence-Based Nursing abstracts of treatment studies as follows: allocation concealed (deemed to have taken adequate measures to conceal allocation to study group assignments from those responsible for assessing patients for entry in the trial [ie, central randomisation; sequentially numbered, opaque, sealed ...

Exercise Therapy is a set of repetitive stretching or strengthening maneuvers designed to develop weakened or injured muscle groups that support the spine. Specific exercises help rehabilitate and stabilize the spine. The individual bones of the spine are held in place by muscles and ligaments. Proper motion and position of these bones are crucial for proper nervous system function, correct posture and good health. Exercises and repeated maneuvers help re-pattern and condition these connective tissues.. Read More  ...
