Apocrine carcinoma is a rare malignant adnexal neoplasm. The differential diagnosis between apocrine carcinoma and cutaneous metastasis is often difficult. Here, we report a case of locally recurrent penile apocrine carcinoma initially diagnosed as metastatic adenocarcinoma of the colon. A 75-year-old man with a history of surgical resection due to sigmoid colon cancer and penile metastasis two years prior to this study presented with a nodule at the left penile base. He underwent a wide local resection of the penile mass under a suggested preoperative diagnosis of extra-mammary Pagets disease (EMPD) associated with previous sigmoid colon cancer ...

Abstract:. Isolated involvement of rectosigmoid region is very rare. Such cases are difficult to diagnose clinically due to their nonspecific and atypical presentations. This may leads to misdiagnosis and sometimes unplanned treatment. A 65‑year‑old female had operated for suspected traumatic intestinal obstruction with operative findings suggestive of carcinoma rectosigmoid region. Later on, investigations had proved the case to be tuberculosis of that region without any other signs. Thus, a thorough knowledge of the various presentations of rectosigmoid tuberculosis is important in order to improve patient management.. ...

Our nationwide cohort study of 23 487 prospectively followed CC patients revealed that although patients with RCC were, overall, older, more often of female gender, had modestly higher ASA scores and more advanced UICC stages than patients with LCC, the picture was indeed more complex when assessing the underlying CC subsites. Mortality from RCC was only relatively higher than mortality from LCC during the first 2 years after CC diagnosis, probably due to the observation that the majority of recurrences in colorectal cancer occurs within the first 2 years after primary treatment.12 In analyses of CC subsites, the highest relative mortality was from cancer of the splenic flexure (ie, belonging to the LCC category), while the lowest relative mortality was from sigmoid cancer (also belonging to the LCC category). The differences in mortality from CC of different colon subsites was only to a minor extent explained by the ASA scores, number of harvested lymph nodes, number of lymph nodes with ...

In normal colonic circular muscle obtained from 21 patients, the frequency of spontaneous summation contractions ranged from 3 to 7 per 4 min, whereas in circular muscle from 13 patients with ulcerative colitis, the frequency of these contractions ranged from 1 to 9 per 4 min. Nonadrenergic, noncholinergic relaxation produced by electrical field stimulation was recorded in the majority of circular smooth ...
Distant metastases from colon cancer spread most frequently to the liver and the lung. Risk factors include positive lymph nodes and high grade tumors. Isolated metastases to the appendicular skeleton are very rare, particularly in the absence of identifiable risk factors. The patient was a 55 year old male with no previous personal or family history of colon cancer. Routine screening revealed a sigmoid adenocarcinoma. He underwent resection with primary anastomosis and was found to have Stage IIA colon cancer. He declined chemotherapy as part of a clinical trial, and eight months later was found to have an isolated metastasis in his right scapula. This was treated medically, but grew to 12 × 15 cm. The patient underwent a curative forequarter amputation and is now more than four years from his original colon surgery. Stage IIA colon cancers are associated with a high five year survival rate, and chemotherapy is not automatically given. If metastases occur, they are likely to arise from local
Fistula development after esophageal resection is considered as one of the most serious postoperative complications. The authors reported a case on clinical experiences in the postoperative diagnostic and successful therapeutic management of a tracheomediastinal fistula after esophageal resection, using endoscopic application of fibrin glue. The early approach of an anastomotic insufficiency after esophageal resection because of a squamous cell carcinoma (pT3pN0M0G2) below the tracheal bifurcation including transposition of a re-modelled gastric tube and end-to-side anastomosis 24 hours postoperatively in a 55-year old patient combined i) surgical re-intervention from the periesophageal site (reanastomosis, gastroplication, lavage, local and mediastinal drainage) and, later on, ii) extensive rinsing with consecutive endoscopic fibrin glue application into the tracheal mouth of the subsequently developed tracheomediastinal fistula as a consequence of the inflammatory changes within the ...

CLINICAL CONSIDERATIONS OF SIGMOID COLON 1. Colonic aganglionosis (Hirschsprung disease). It is caused by the arrest of the caudal migration of neural crest cells. The hallmark is the absence of ganglionic cells in the myenteric and sub-mucosal plexuses, most commonly in the sigmoid colon and rectum, resulting in a narrow segment of colon (i.e., the colon fails to relax). Although the ganglionic cells are absent, there is a proliferation of hypertrophied nerve fiber bundles. The most characteristic functional finding is the failure of the internal anal sphincter to relax following rectal distention (i.e., abnormal rectoanal reflex). Mutations of the RET proto-oncogene (chromosome 10q.11.2) have been associated with Hirschsprung disease. Clinical findings include a distended abdomen, inability to pass meconium, gushing of fecal material upon a rectal digital exam, fecal retention, and a loss of peristalsis in the colon segment distal to the normal innervated colon. The barium radiograph shows a ...
Intrauterine contraceptive device (IUD) migration from the uterus is a known but a relatively rare complication. Migration of the device in the surrounding viscera can be life threatening. During an end sigmoid diversion colostomy, the end of the sigmoid colon is brought through the abdominal wall, where it may be turned under, like a cuff. The edges of the colon are then stitched to the skin of the abdominal wall to form an opening called a stoma. Stool drains from the stoma into a bag or pouch attached to the abdomen.

From the small bowel, the residual material drains into the large intestine (or colon). The colon is about 6 feet in length and begins with the cecum, extends into the ascending colon, the transverse colon, the descending colon, the sigmoid colon and finally the rectum and anus. The function of the colon is to absorb water and salts and bacteria also interacts with the liquid waste matter in the colon assisting with the conversion to feces or stool. This interaction with bacteria contributes to the formation of gas or flatus. Stool is stored primarily in the descending colon and sigmoid colon where it moves into the rectum ...

Premalignant colonic growth. Endoscopic view of a benign (non-cancerous) but premalignant tubular adenoma (lower centre to lower right) in the sigmoid colon. Premalignant conditions are ones which, if left untreated, may develop into cancer. Adenomas are growths that arise from glands. In the colon these are an important early warning of potential cancer development. The sigmoid colon is the final part of the large intestine (colon) and joins to the rectum. The colon is mainly responsible for extracting water and salt from solid wastes before they are eliminated from the body. - Stock Image C001/2427
