Ehlers-Danlos syndrome (EDS) is a relatively rare, inherited connective tissue disorder and skin hyperextensibility, joint hypermobility, chronic joint and limb pain, blood vessel and tissue fragility are frequent symptoms. The aims of this study were to describe the degree to which individuals with EDS accept and cope with perceived limitations related to the syndrome, and to determine the relationships between acceptance of disability (AD), sense of coherence (SOC), functional health status and background data. Four questionnaires were mailed: the AD scale Modified (ADM), the SOC Scale, the Sickness Impact Profile (SIP) and a background form. The respondents with EDS were recruited through the EDS National Association in Sweden and the response rate was 74% (69 females and eight males). The Ethical Committee at Huddinge University Hospital (ref. no. 152/95) approved the study. This study has shown that the level of AD and SOC in individuals with EDS is similar to other patient groups, e.g. ...
Background: The BWCS is a structured, self-report questionnaire that the patient can generally complete with little or no intervention from an interviewer. The BWCS is based on items from the Bowel-Bladder Function Scale (Turnbull et al, 1992) and the Sickness Impact Profile. This five-item instrument provides a brief assessment of bowel control and the extent to which bowel problems have an impact on everyday activities. The BWCS is one of the components of the MSQLI. ...

Midwives can play a key role in promoting the oral health of pregnant women and assessing their oral health status. A maternal oral assessment tool (MOS) was developed and pilot tested by the study investigators to assist midwives in this role and the results were promising. The aim of this study was to undertake further sensitivity and specificity assessment of the MOS tool using two-comparison approaches- the longer oral health screening tool known as the Oral Health Impact Profile (OHIP-14) and an oral assessment by trained study dentists. Pregnant women were recruited for this study as part of a larger randomised controlled trial of a Midwifery Initiated Oral Health (MIOH) program. Pregnant women completed the MOS and OHIP-14 as part of their initial assessment undertaken by 38 trained and accredited midwives. A dental assessment was conducted for all women in the intervention group using three trained study dentists with high inter rater reliability. Two hundred and eleven pregnant women

NEI-VFQ-25 is independently associated with severity of DR. Visual acuity only accounts for 20% of the variance seen in NEI-VFQ-25. Therefore, in clinical trials, the NEI-VFQ-25 may be used in preference to visual acuity as a better assessment of overall patient outcome as it evaluates different gross aspects of visual acuity (contrast sensitivity, near acuity, distance acuity) and quality of life rather than distance vision only and so should be considered as an independent outcome measure.. All individuals in a Swedish county afflicted with any type of hereditary muscular dystrophy (MD) were identified and 57 (85 percent) of eligible individuals in the age range 16 to 64 were included in the study. Respiratory disturbances were estimated by means of spirometry and analysis of arterial blood gases, and 58 percent yielded abnormal results on at least one of these examinations. Elevated PCO2 was found more commonly than reduced forced vital capacity (FVC) and there was a moderate association between these parameters. Respiratory symptoms, most commonly breathlessness, were encountered in 79 percent. Pathologic ECG recordings were found in 21 individuals (37 percent). Conduction disturbances and affection of the myocard were most frequent in myotonic dystrophy. Quality of life was assessed by means of the Sickness Impact Profile instrument and the Kaasa test. The results showed that quality of life was significantly related to FVC and to the symptom of abnormal fatigue. Respiratory and ...
Retinas from our earliest vertebrate ancestors had cone-like photoreceptors, presumably allowing them to see in daylight, but little ability to see at night. Then, millions of years ago in the Mesozoic era, and in relatively short order, mammals emerged that had retinas with predominantly rod photoreceptors, allowing for them to see at night perhaps to hunt for food while their dinosaur predators were dozing. Now a new study led by researchers the National Eye Institute suggests how the genesis of rod photoreceptors may have occurred to give rise to nocturnal mammals. ...

The National Eye Institute (NEI) was established in 1968 and is located in Bethesda, Maryland. The NEI is one of 27 institutes and centers of the US National Institutes of Health (NIH), an agency of the US Department of Health and Human Services. The mission of NEI is to prolong and protect the vision of the American people. The NEI conducts and performs research into treating and preventing diseases affecting the eye or vision. Before the NEI was established, primary responsibility for vision research at NIH was done by the National Institute of Neurological Disease and Blindness (NINDB) (which is now known as the National Institute of Neurological Disorders and Stroke). NINDB was established in 1950, after President Harry S. Truman signed the Omnibus Medical Research Act. The bill agreed to establish new separate institutes within NIH. This marked the beginning of vision research at a federal level. Organizing, structuring, and separating vision and neurological research was a challenge at ...
In an NIH-supported clinical trial comparing three drugs for diabetic macular edema (DME), Eylea (aflibercept) provided greater visual improvement, on average, than did Avastin (bevacizumab) or Lucentis (ranibizumab) when vision was 20/50 or worse at the start of the trial. However, the three drugs resulted in similar average improvement when starting vision was 20/40 to 20/32. Investigators found no major differences in the safety of the three drugs. The trial was funded by the National Eye Institute (NEI), part of the NIH.. [Read More] ...

TY - JOUR. T1 - Health-related quality of life. T2 - A guide for the health professional. AU - Nanda, Upasana. AU - Andresen, Elena. PY - 1998. Y1 - 1998. N2 - The importance of evaluating health care in terms of patients quality of life has only recently been acknowledged in the context of outcomes research and the measurement of quality of care. As a health professional, trying to choose the appropriate health status or health-related quality of life (HRQOL) measure can be daunting, as one is faced with a bewildering array of choices. The purpose of this article is to describe the process by which professionals might make a choice about measures in outcomes research. The article includes examples and rationale for using generic or disease- specific HRQOL measures. In addition, examples are provided of HRQOL assessment in two adult chronic disease applications: asthma and congestive heart failure.. In a longitudinal study reported in the Journal of Clinical Oncology, Husson et al found that health-related quality of life improved between diagnosis and 2 years after diagnosis in adolescent/young adult (AYA) patients with cancer, but it remained impaired compared with population norms.. Study Details. The study involved 176 patients aged 15 to 39 years with various cancers from 5 U.S. institutions who completed the Short Form-36 Health Survey (SF-36) within 4 months after diagnosis and at 12 and 24 months later. The SF-36 summary physical component scale (PCS) and mental component scale (MCS) scores were converted to T scores ranging from 0 to 100, with a higher score indicating better health-related quality of life.. Changes Over Time. At baseline, AYA patients had significantly poorer mean PCS scores (38.7 vs 52.9, P , .001) and MCS scores (42.9 vs 48.9, P , .001) vs population norms (ages 18-40 years). AYA patients had significant improvements in PCS and MCS scores from baseline at 12 ...

To describe the development of a proctitis-specific quality-of-life module to supplement the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30). The module was developed according to EORTC guidelines, which consisted of an extensive literature review to identify previously described issues and interviews conducted with seven health professionals and 10 patients to rationalize the item list for construction into a provisional module. The module developed was then pretested with 28 patients and five health professionals. The final module contains 21-items that are suitable to obtain information about the patients quality of life after high-dose pelvic irradiation. The questionnaire has now been translated into four languages and commenced field testing in late 2007. The EORTC QLQ-C30, supplemented by EORTC QLQ-PRT21, will enable health professionals to more accurately monitor the side effects that patients experience after pelvic ...

purpose. To examine the responsiveness of the National Eye Institute Visual Function Questionnaire-25 (NEI VFQ-25) by using data from the MARINA and ANCHOR trials in neovascular age-related macular degeneration (AMD) and to establish the change in the NEI VFQ-25 associated with a 15-letter change in best corrected visual acuity (BCVA).. methods. In MARINA, 716 patients were randomized to monthly intravitreal ranibizumab (0.3 or 0.5 mg) or sham injections. In ANCHOR, 423 patients were randomized to monthly ranibizumab (0.3 or 0.5 mg) with sham photodynamic therapy (PDT) or sham ocular injections with verteporfin PDT. Patients had follow-up interviews and BCVA measurements over 24 months. Data were analyzed separately for MARINA and ANCHOR, and treatment groups were pooled within each trial. The clinically relevant difference in NEI VFQ-25 was estimated based on regression models of change from baseline to month 12 in BCVA.. results. Subgroups categorized by BCVA change (≥15 letters gained, ,15 ...
Increasing research attention and funding has been directed to the assessment of patient-centered outcomes, most prominently QoL. 7,25 This evaluation is important for obtaining a more complete understanding of the effects of treatments and the natural history of disease on an individuals daily routine. Miljanovic et al. 6 evaluated the impact of dry eye on vision-related QoL in the Womens Health Study and Physicians Health Study by sending a supplementary questionnaire asking how much everyday activities were limited by symptoms of dry eye, and to what degree eye problems limited reading, driving, working at the computer, professional activity, and watching television. Mertzanis et al. 7 assessed the general health related QoL of dry eye patients with Short Form 36, which contains questions that are not specific to dry eye. Schiffman et al. 5 assessed dry eye patient QoL with a time trade off method. Although the definition of dry eye and the survey instruments used between studies were ...

Background Our aim was to investigate the existence of a reciprocal relationship between patients assessment of quality of life and their appraisal of health. If present, this relationship will interfere with the interpretation of heart surgerys effect on overall quality of life. Methods Path analysis was used to investigate reciprocal causal relationships between general health perceptions and overall quality of life before and after heart surgery. Longitudinal data from a study of coronary artery bypass surgery were used to model lagged, cross-lagged, and simultaneous paths over four time-points of assessment from before surgery to one year afterwards. The conceptual framework for the analysis was the Wilson and Cleary causal pathway model. General health perceptions were measured with the Short Form 36. Overall quality of life was measured with i) a single question regarding life satisfaction and ii) the multi-item Quality of Life Survey. Results Acceptable model fit was obtained for ...
Objective To determine factors associated with work in patients with ankylosing spondylitis (AS). Methods Three hundred fifteen patients with AS were enrolled in a 24-week, randomized controlled study of adalimumab with a longterm, open-label, adalimumab extension phase. Patient-reported outcome (PRO) measures included the Medical Outcome Study Short Form 36 Health Survey (SF-36), AS Quality of Life Questionnaire (ASQOL), Health Utilities Index Mark 3 (HUI-3), and Work Productivity and Activity Impairment-Specific Health Problem Questionnaire (WPAI-SHP). Multivariate logistic regression was used to analyze differences between working and nonworking patients. The relationships between PRO and WPAI-SHP scores were assessed using Pearson correlation coefficients. Multivariate modeling was applied to determine factors associated with productivity while at work. WPAI-SHP was assessed through 3 years of adalimumab exposure. Results Younger age (p = 0.002) and male sex (p , 0.001) were significantly ...
The older Hispanic population of the U.S. is growing at a tremendous rate. While ethnic-related risk and complications of diabetes are widely-acknowledged for older Hispanics, less is known about how health related quality of life is affected in this population. Cross-sectional study assessing differences in health related quality of life between older Mexican Americans with and without diabetes. Participants (n = 619) from the Hispanic Established Population for the Epidemiological Study of the Elderly were interviewed in their homes. The primary measure was the Medical Outcomes Study Short Form (SF-36). The sample was 59.6% female with a mean age of 78.3 (SD = 5.2) years. 31.2% (n = 193) of the participants were identified with diabetes. Individuals with diabetes had significantly (F = 19.35, p | .001) lower scores on the Physical Composite scale (mean = 37.50, SD = 12.69) of the SF-36 compared to persons without diabetes (mean = 43.04, SD = 12.22). There was no significant difference between persons
