Hemiplegic shoulder pain is common in stroke survivors, developing in up to 54% of patients. The underlying mechanisms include soft tissue lesions, impaired motor function and central nervous system-related phenomena. Hemiplegic shoulder pain has many underlying causes and is challenging to manage, requiring a team approach, including physicians, therapists and caregivers. The management strategy must target the underlying causes. Preventative measures, such as positioning and handling, can reduce the risk of developing hemiplegic shoulder pain. We have reviewed the literature on hemiplegic shoulder pain, and used this to categorise the underlying mechanisms, and to explore the best management strategies.. ...
How common is Shoulder Pain? In my office it comes in a close "third" to chronic neck pain/headaches and lower back pain/sciatic pain. According to current medical statistics found in the American Academy of Pain Medicine; shoulder pain affects nearly 1 in 10 American adults. According to Mario Pribicevis chapter (The Epidemiology of Shoulder Pain: A Narrative Review of the Literature) in the 2012s Pain in Perspective, there are numerous studies putting the incidence of Shoulder Pain higher than the published projection of one in ten. A recent scientific study demonstrated that 15% of men and 25% of women experienced shoulder pain which directly correlates with a larger study conducted overseas examining major population pools in many regions of Europe. The findings? They found that nearly 1 out of 5 people experienced varying degrees of Shoulder Pain. This is a huge statistic and numerous studies also demonstrate that Chronic Shoulder Pain is not only painful but highly debilitating as it ...
Chronic shoulder pain is a common, costly, and disabling problem for society. The identification of factors predictive of the development of chronic shoulder pain is necessary to develop innovative and effective treatments to reduce the societal impact of shoulder disorders. In previous work the investigators identified a genetic and psychological subgroup that robustly predicted heightened shoulder pain responses in a pre-clinical cohort and poor 12 month shoulder pain recovery rates in a clinical surgical cohort. In this follow-up study the investigator proposes to test how interventions tailored to the high risk subgroup affect pain responses in a pre-clinical cohort.. The optimal theorized match for the identified high-risk subgroup is a combination of personalized pharmaceutical and education interventions. This combined personalized intervention versus a placebo pharmaceutical and general education intervention group is the primary comparison of interest. Also, an evaluation of the ...
Shoulder pain is a common secondary condition in people with spinal cord injury (SCI) that often results in loss of function and of independence and imposes limitations on self-care, work, and leisure activities, and leads to decreased quality of life. More than 40% of individuals with SCI report shoulder pain at the beginning of inpatient rehabilitation; this number increases to 50% at hospital discharge. The onset of shoulder pain within the first year after injury may lead to lifelong chronic shoulder pain. Although information is known about shoulder pain in patients with long-term SCI, little is known about the beginning of shoulder problems and how they progress early after the injury. In addition to physical problems, psychosocial factors are also associated with chronic pain.. This study will investigate the progression of musculoskeletal (shoulder muscle flexibility, muscle strength, movement coordination, and rotator cuff health) and psychosocial (fear of movement, pain ...
... can arise from a multitude of places-from joints, muscles, tendons, and bursa in and around the shoulder region as well as from more distant locations like the neck, upper back, or even referred pain from the gall bladder. The onset of shoulder pain is highly variable as it can arise without an obvious cause or be related to a specific mechanism of injury such as a work or sports injury. Shoulder pain can also occur as a result of repetitive trauma over time, such as a job requiring overhead reaching. Neurological injuries such as stroke or a pinched nerve in the neck can cause shoulder pain as well. Experts estimate that as much as half of the population experiences shoulder pain each year, though many people often decide to "just live with it" and therefore, dont seek treatment. However, shoulder and neck disorders do account for 18% of disability payments for MSK pain. These following factors contribute to shoulder pain, either alone or in combination with each other: • ...

OBJECTIVE To determine the frequency of shoulder pain in our amyotrophic lateral sclerosis (ALS) population and to explore potential associations with demographic and clinical features. METHODS We retrospectively reviewed the medical records of 193 patients with ALS patients seen at the Lahey Clinic between 2005 and 2009. Patients were categorized by the predominance of upper and lower motor neuron signs and the body regions initially involved. The frequency of shoulder pain was identified in each of these subgroups. RESULTS Forty-five (23%) of the 193 patients reported shoulder pain at some time during the course of their illness. Age, gender, manual labor, prior shoulder problems, ALS phenotype, and initial region of involvement were not correlated with shoulder pain. Patients with shoulder pain were more likely to develop proximal arm weakness during their illness and to report pain elsewhere. CONCLUSIONS Despite the limitations posed by this retrospective study, it underscores the prevalence
Objectives Recently, a rule was developed to predict sick leave related to shoulder pain during a period of 6 months after patients have consulted a general practitioner for a new episode of shoulder pain. The objective was to evaluate the generalizability of this prediction rule by testing it in two other populations of workers who had gone for a consultation in primary care for a new episode of shoulder pain.. Methods The prediction rule was derived in a prognostic cohort study (N=350). The outcome was sick leave related to shoulder pain during 6 months following the first consultation. The rule was tested on merged control groups from three trials on shoulder pain (N=128). In addition to this population, a recently conducted study on musculoskeletal disorders (N=224) was used to validate the prediction rule. The generalizability of the prediction rule was tested by studying calibration and discrimination in the validation cohorts.. Results The prediction rule showed reasonable calibration in ...
Comparable clinically important variables at baseline reflected successful randomization. Although there was a higher proportion of total anterior circulation strokes in the control group, the composite of total and partial anterior syndromes was evenly distributed (81.3% in control group, 84.4% in intervention group). It is possible that subjective pain report in participants with total anterior circulation strokes may have been influenced by higher cortical dysfunction, though the authors accounted for this in exclusion criteria. Although the difference of 4.12 mm in baseline VAS between groups did not reach clinical or statistical significance, it could indicate a potential confounding factor. The mean time between stroke onset and enrollment was similar between groups, in keeping with the typical nadir of hemiplegic shoulder pain at the 2- to 3-month mark.34. A single SSNB injection provides superior reduction in hemiplegic shoulder pain in comparison with placebo injection. The SSNB group ...
As an injury specialist I come across with all kinds of injuries and one of the top injuries that I come across is shoulder injuries and shoulder pain.. Now I want to show you the 5 worst exercises for your shoulder and then give you couple of solutions that you can do for those worst exercises.. So I want to go through exercise # 1.. A lot of times the common thinking out there for any type of shoulder injuries, shoulder pain or shoulder prevention program is to do some sort of rowing exercise or one arm row.. A lot of people end up doing it wrong and what happens is it makes their shoulder worse. It ends up reshaping their shoulder the wrong way. And it leads to their shoulder being in a shape that increases the risk of shoulder injury and shoulder pain.. Let me go through that One Arm Row movement. I can reach down, come up and then go through that rowing movement. Now hopefully you saw the mistake that I am making.. And how if I ...
Chronic shoulder pain refers to a pain or movement problem that has been present for more than six weeks. Early detection and intervention is the key in the prevention and management of chronic shoulder pain.
Mechanical stress over the years of our life is often considered as a primary contributor to the deterioration of joints, ligaments, and tendons. Commonly known as arthritis or osteoarthritis it primarily affects the larger joints of the body such as the hips, knees and the lumbar spine. The shoulder is also prone to undergoing arthritic change primarily due to its level of mobility. The trade off of having tremendous mobility in the shoulder under normal circumstances is a joint with inherent instability. This leave us prone to mild injuries because in the shoulder we sacrifice stability for mobility. With each subsequent mild injury to the shoulder joint our body adds more and more scar tissue in an effort to stabilize the shoulder and over time this can lead to a more serious chronic pain syndrome.. Whats Involved in Chronic Shoulder Problems?. Chronic pain of the shoulder typically involves a small group of shoulder stabilizing muscules known collectively as the rotator cuff. The four ...

Shoulder pain pumps are medical devices designed to alleviate the severe pain that accompanies the recovery process following certain shoulder surgeries. These pain pumps present an alternative method of pain management to oral or intravenous medication that allows the medication to be applied directly to the shoulder and the surrounding area. This is accomplished through the use of a small elastometric pump that delivers the medication through a catheter placed directly into the shoulder. Although this technology was used for decades without many reports of unusual or significant complications, healthcare professionals and patients have been alarmed in recent years at the spike in serious complications following the use of a post-operative shoulder pain pump.. The most common users of shoulder pain pumps are individuals who have undergone open surgery to their shoulders due to a sports-related trauma or other injuries. Athletes participating in certain sports such as football and baseball are ...

BACKGROUND: Shoulder pain is a very common presentation in primary care. Evidence of benefit for subacromial corticosteroid injection is inconclusive and confined largely to studies with short follow-up. We plan a large, definitive, primary-care-based trial to determine efficacy and safety in patients with rotator cuff tendinopathy, and conducted a pilot trial to explore feasibility. METHODS: Six general practitioners (GPs) from Oxfordshire, UK underwent update training in assessing painful shoulders and injecting the subacromial space. Each then recruited patients aged 35 to 74 years from primary care complaining of shoulder pain lasting no more than 6 months. Eligible participants were randomized to receive either methylprednisolone acetate 40 mg with lidocaine 1% (total volume 1 ml), or lidocaine 1% alone (total volume 1 ml), injected into the subacromial space. The participants were blinded to treatment allocation. Feasibility outcomes were rates of recruitment, withdrawal, adherence to the protocol

In around 10 percent of cases, I find trigger points in the armpits, which have a relation with shoulder pain. The TrPs are treated at each visit until their complete elimination.. Other Variables to Consider. Often the patient has a diagnosis of partial or complete tear of the rotator cuff. This could appear to be an important cause of the shoulder problem, but a trial by Sher5 showed that after 60 years of age, 26 percent of subjects had a partial tear and 28 percent had a complete tear (a hole all through the rotator cuff) without symptoms.. Research by Welfing6 showed that in 925 symptomatic patients, 6.5 percent had a calcium deposit. In 200 others, asymptomatic this time, there was a deposit in 7 percent. In another study,7 the authors concluded that the relationship between calcium deposits and shoulder pain is unclear.. Imaging such as radiography, arthrography, computed tomographic scanning, and magnetic resonance imaging should be reserved for difficult cases in which the diagnosis is ...

Shoulder Pain Treatment Depending on the source of the discomfort, shoulder pain treatment can involve anything from orthopedic surgery to simply resting the shoulder. The disparity in treatments is due to the complexity of the shoulder, which is comprised of several bones, muscles, ligaments, and tendons that give the arm a full 180-degree range of motion. This high level of mobility leaves it particularly susceptible to many types of injury and disease, including arthritis, fractures, and dislocation.
Many forms of arthritis and other diseases can cause shoulder pain and instability. Read about the common diseases that affect the shoulder.

OBJECTIVES: To investigate the association between shoulder region pain presenting in primary care and cervical spine magnetic resonance imaging (MRI) abnormalities. METHODS: A matched case-control study of 48 pairs of participants. Patients had presented to primary care with a new episode of shoulder pain. Controls had no history of pain in the shoulder region and were individually matched with cases by age, gender and referring clinician. All participants underwent a structured clinical assessment and cervical spine MRI. Scans were scored by experienced musculoskeletal radiologists blinded to case-control status. RESULTS: Median age of participants was 51 years (range 19-79) and 21 (44%) were female. Neck pain in the past week was reported by 25 (52%) cases and seven (15%) controls (odds ratio, OR, 10.0; 95% confidence interval, CI, 2.4, 88.2). Cervical spine MRI from C3/4 to C6/7 revealed: 18 (38%) of both cases and controls had disc height loss |/=50% at any level; 10 (21%) cases and eight (17%)
