The sella turcica is considered an important landmark in orthodontics as it is used extensively in various cephalometric analyses be it for diagnosis, evaluation of growth or treatment results. In order to recognize deviations from the norm, one needs to be familiar with normal radiographic anatomy as well as morphologic variability. A review of the literature was conducted regarding the norms and variations in size, shape, morphology and bridging of the sella turcica as evidenced by cephalometric evaluation. Literature search was carried out using the following keywords: Sella Turcica, Sella Bridging, Sella Size and Morphology. Search engines: PubMed and Google Scholar were utilised, followed by hand search. The purpose of the review is to provide an insight into detection of subclinical and potentially pathologic conditions during regular orthodontic pretreatment assessments.

In this series there were 89 pituitary adenomas, 8 meningiomas, 9 craniopharyngiomas, and one mucocele. Transcranial, transphenoidal, and combined transcranial-transsphenoidal approaches were used for surgical therapy. Three patients died postoperatively. We evaluated the implementation of the endoscopic (E) technique by an interdisciplinary ENT/neurosurgeon team as compared to the established classical microscopic technique (M) performed by one experienced neurosurgeon for pituitary adenomas in a single center. A retrospective analysis of patients operated for newly diagnosed pituitary adenomas was performed between November 2004 and August 2012. Outcome and complications are presented. A total of 116 patients were operated, 64 microscopically (M) and 52 endoscopically (E). Mean follow up was 35 months (range 1.4-95), 1 patient was lost to follow-up. Most frequent pathology was hormone inactive adenoma (60% E, 51% M). Operating time was stable in the M-group (± 94 min). DISCUSSION Surgical treatment of chordomas has been challenging to neurosurgeons for many years.[9] Recently, radical resection of the tumor has been recommended for better outcomes.[2] When complete resection of the tumor is not achieved, radiation treatment for residual tumor is recommended.[1,4,7] Conventional microscopic transsphenoidal techniques have been used in the treatment of clival chordomas.[5] While performing endoscopic transsphenoidal pituitary surgeries, we have observed the panoramic anatomical exposure provided by these techniques.[3] Endoscopic transsphenoidal techniques expose the planum sphenoidale, the optic protuberances covering the optic nerves, the carotid protuberances, and the clivus as well as the sella turcica. When we encountered a patient with a tumor based at the clivus, we were convinced that the tumor could be resected using an endoscopic technique.
