A second-look procedure is sometimes performed to determine if a cancer patient has responded successfully to a particular treatment. Examples of cancers that are assessed during second-look surgery are ovarian cancer and colorectal cancer. In many cases, before a round of chemotherapy and/or radiation therapy is started, a patient will undergo a surgical procedure called cytoreduction to reduce the size of a tumor. This debulking increases the sensitivity of the tumor and decreases the number of necessary treatment cycles. Following cytoreduction and chemotherapy, a second-look procedure may be necessary to determine if the area is cancer-free. An advantage to second-look surgery following cancer treatment is that if cancer is found, it may be removed during the procedure in some patients. In other cases, if a tumor cannot be entirely removed, the surgeon can debulk the tumor and improve the patients chances of responding to another cycle of chemotherapy. However, second-look surgery cannot ...
PRIMARY OBJECTIVES:. I. Determine the failure-free survival of patients with newly diagnosed low-risk rhabdomyosarcoma treated with vincristine (V), dactinomycin (A), cyclophosphamide (C), and radiotherapy.. SECONDARY OBJECTIVES:. I. Determine local control rates in patients treated with this regimen. II. Determine the rate of second-look surgery in patients with bulk residual tumor at diagnosis (clinical group III) and the proportion of second-look surgeries that render patients treated with this regimen tumor-free or with microscopic tumor only and evaluate the pathologic significance of that residual tumor.. III. Determine the local control rates in patients with clinical group III disease treated with response-adjusted radiotherapy doses after second-look surgical resection.. OUTLINE: This is a nonrandomized, multicenter study. Patients are assigned to 1 of 2 treatment regimens according to disease stage and clinical group.. Background AMG 386, an investigational peptibody, blocks the interaction of angiopoietin-1 and -2 with the Tie2 receptor, thereby inhibiting tumor angiogenesis. We evaluated the tolerability of AMG 386 plus paclitaxel and carboplatin in ovarian cancer patients who had primary or interval debulking surgery (PDS or IDS, respectively).. Methods Women (≥ 18 yrs, GOG ≤ 1) with high-risk stage I (grade 3 or aneuploid grade 1 or 2) or II-IV ovarian cancer received 6 cycles of the combination AMG 386 (15 mg/kg IV QW) plus paclitaxel (175 mg/m2 IV Q3W) and carboplatin (AUC 6 IV Q3W) followed by AMG 386 maintenance monotherapy up to 18 months. Patients had PDS; patients with disease stage IIIC or IV had the option of planned IDS. AMG 386 dosing was withheld for > 4 weeks after PDS or before IDS. The primary endpoint was the incidence of dose-limiting toxicities (DLTs), which determined cohort expansion to n = 25; secondary endpoints included the patient incidence of adverse events (AEs), the incidence ...
My life ended when I was diagnosed with ovarian cancer. The doctors at the hospital where I was treated always called it "ovary cancer." It seems like a small point, but to me, it symbolized how far I had strayed from the normal world-even the normal world of medicine where I work. I was treated in this alien place for 9 interminable months, surrounded by other words I had had no personal connection with before: optimal debulking, stem-cell support, second-look procedure, peritoneal port. Having had no personal experience with serious illness of my own, I found it horrific beyond description-one revolting sensation after another accompanied by fear and despair. The fatigue alone was nightmarish; it was not the familiar and pleasant aftereffect of exertion but a strange sensation of being trapped and held in place by a smothering force. I am in awe of those, like my patients, who can undergo medical interventions on a long-term basis ...

OBJECTIVE: Previous reports evaluating primary tumor extirpation (hereafter, surgery) in patients presenting with de novo stage IV breast cancer describe mixed results regarding overall survival (OS). In this modern era of treatment, the impact of surgery was assessed, both controlling and adjusting for potential confounders, including comorbidities, tumor burden, vitality impact of distant metastatic site, hormonal therapy of ER/PR+ disease, and targeted therapy of HER-2+ disease. METHODS: Women presenting with de novo stage IV breast cancer during 2000-2015 were retrospectively studied using a single institutions cancer registry data. Patients with severe competing comorbidities (heart failure, chronic kidney disease) were excluded, as well as those missing data for patient, tumor, or treatment variables used in matching or analysis. As primary tumor extirpation was of principal interest, patients who underwent surgery as a first course of treatment were 1:1 matched with those treated without surgery

Turkey Tails are a common mushroom that grows throughout the woods of Tennessee and all over the world. Our interest in the science behind the anti-viral and anti-cancer properties of Turkey Tails began shortly after seeing preliminary clinical trial results and an anecdotal story in a TED lecture last year by mycologist Paul Stamets. In the last two minutes of the speech, Stamets described his 84 year old mothers successful fight against stage IV breast cancer that included taking Turkey Tail mushrooms. She was a deeply religious person who hadnt been to the doctor since 1968. According to Stamets, it was the second worst case of stage IV breast cancer her doctor had ever seen, and she was given three months to live. Shes now cancer free.. The early clinical trial data and Pauls hopeful story resonated, and Vince didnt hesitate to start his mother on Turkey Tails. At the same time, we began cultivating this amazing mushroom on our farm with a deeper sense of purpose.. We realize mushrooms ...

Hi Monica, let me introduce myself. Im a caregiver for my Mom who was dx with OVCA stage IIIB october 7th 2005, she is currently in remission that we know of... she will be having her doctors visit this Wednesday to see what he says about all the tests she has been taking. I have been reading lots and lots of the postings here for about 2 months but I couldn`t find out how to reply.. so Im familiar with your case and admire the strength you have had for the past 6 years right? You are so lucky to have found your cancer at an early stage. My Moms cancer was dx to be on the liver, diaphragm, near colon at the time. She had a big tumor which was removed but no debulking surgery, she had 6 round of carboplat plus bristaxol for 6 months then a 2nd look surgery were out of 10 biospsies 2(diaphragm and somewhere in abdomen)turned out positive for inplant not actually tumors but cancerous cells is what I understood.. so she had 2 more chemos(same kind)her last one was July 5th 2006 and from that day ...

The fetal period begins on day 57 (first day of the ninth week) and concludes at birth. It is characterized by rapid growth of the fetus and further differentiation of the tissues and organs.11 During weeks 10 to 20, the fetus grows primarily in length. During weeks 21 to 40, the fetus grows primarily in weight. Toward the end of pregnancy, the fetus can distinguish its mothers voice. It also responds to music and loud noise and is able to see light through the abdominal wall. The expected date of delivery is 38 weeks after fertilization; i.e., 280 days or 40 weeks after LMP (onset of a womans last menses).12 ...
The Giants must cope without second baseman Jeff Kent again. Kent, who spent time on the disabled list the past two seasons, is sidelined with a strained rib-cage muscle. Other options are Wilson Delgado, a shortstop by trade, and Jay Canizaro, who hit .280 with 26 homers and 78 RBIs at Triple-A Fresno and went 8-for-18 for the Giants in September. In spring training, starters throw every four days. If Baker decides to keep the same order for the regular season, Ortiz would throw the first home game at Pacific Bell Park. The Giants open with seven road games in Florida and Atlanta.

Mastectomy is unnecessary in many women with stage IV breast cancer, according to a study from Indian investigators, reported at the 2013 San Antonio Breast Cancer Symposium.1. The study randomly assigned 350 patients with metastatic breast cancer to mastectomy, complete axillary dissection, plus radiation therapy, vs no locoregional treatment, and found that the nonsurgical group had no worse survival than those who underwent mastectomy.. "The clinical conclusion would be that locoregional treatment of the primary tumor in women presenting with metastatic breast cancer did not result in any overall survival benefit, and hence should not be offered as a routine practice, but should be restricted as an option for palliation," according to Rajendra A. Badwe, MD, Director of the Tata Memorial Hospital, Mumbai.. All patients received anthracyclines, with or without taxanes, and were stratified by metastatic site, number of metastases, and hormone receptor status, then randomized to either ...
To the editor: We wish to call to your readers attention an unexpectedly severe and disabling neurotoxicity due to very-high-dose cisplatin therapy for cancer of the ovary. The attainment of complete remission, proven by second-look surgery, is the first step toward achieving prolonged survival and cure of stage III cancer of the ovary. Recently, Ozols and associates (1) have reported that administration of very high doses of cisplatin (200 mg/m2 body surface area over 5 consecutive days) has achieved frequent tumor regression in patients with advanced cancer of the ovary previously refractory to conventional doses of cisplatin. To improve complete ...
