Although scrub typhus is uncommon in pregnant women, when present, it can have serious repercussions for the mother and developing fetus. Since it is uncommon, the clinical impact of scrub typhus on pregnancy has not been elucidated and an effective and safe therapeutic regimen has not been validated. The medical records of pregnant women whose scrub typhus were treated at Chungnam National University Hospital were reviewed and their clinical outcomes were evaluated. A review of the literature was also performed on pregnant women with scrub typhus and their clinical outcomes. Eight pregnant women with scrub typhus were treated successfully with a single 500-mg dose of azithromycin, and no relapses were reported. They all delivered healthy babies at term, without congenital or neonatal complications. In the reviews, azithromycin was effective against scrub typhus and had favorable pregnancy outcomes. Ciprofloxacin and cefuroxime failed to treat scrub typhus and fetal loss resulted. A single 500-mg dose
Scrub typhus is an acute and febrile infectious disease caused by the Gram-negative α-proteobacterium Orientia tsutsugamushi from the family Rickettsiaceae that is widely distributed in Northern, Southern and Eastern Asia. In the present study, we analysed the serum proteome of scrub typhus patients to investigate specific clinical protein patterns in an attempt to explain pathophysiology and discover potential biomarkers of infection. Serum samples were collected from three patients (before and after treatment with antibiotics) and three healthy subjects. One-dimensional sodium dodecyl sulphate-polyacrylamide gel electrophoresis followed by liquid chromatography-tandem mass spectrometry was performed to identify differentially abundant proteins using quantitative proteomic approaches. Bioinformatic analysis was then performed using Ingenuity Pathway Analysis. Proteomic analysis identified 236 serum proteins, of which 32 were differentially expressed in normal subjects, naive scrub typhus patients and
Scrub typhus is a common and underdiagnosed cause of febrile illness in Southeast Asia, caused by infection with Orientia tsutsugamushi. Inoculation of the organism at a cutaneous mite bite site commonly results in formation of a localized pathological skin reaction termed an eschar. The site of development of the obligate intracellular bacteria within the eschar and the mechanisms of dissemination to cause systemic infection are unclear. Previous postmortem and in vitro reports demonstrated infection of endothelial cells, but recent pathophysiological investigations of typhus patients using surrogate markers of endothelial cell and leucocyte activation indicated a more prevalent host leucocyte than endothelial cell response in vivo. We therefore examined eschar skin biopsies from patients with scrub typhus to determine and characterize the phenotypes of host cells in vivo with intracellular infection by O. tsutsugamushi, using histology, immunohistochemistry, double immunofluorescence confocal laser
Received - 19 May 2017; Reviewed - 9 Aug 2017; Corrected & Accepted - 12 Aug 2017. INTRODUCTION. Scrub typhus is an acute febrile illness caused by Orientia tsutsugamushi, transmitted to humans by bite of larval stage of Trombiculid mite.1] There are few case reports of scrub typhus2 in pregnancy advocating high suspicion while dealing with a case who has returned or residing in endemic areas as overall outcome depends on early diagnosis and management. We describe an unusual case of scrub typhus in a postpartum patient who had complicated sequelae. Septate pleural effusion and clinical picture suggestive of puerperal sepsis was the cause for delayed diagnosis of scrub typhus. However, prompt treatment once diagnosed, led to a favorable outcome.. CASE REPORT. A 26 year old female from North India developed fever, abdominal pain and respiratory distress from second postpartum day after vaginal delivery in a nursing home. In view of fever in postpartum period with infected episiotomy wound and ...
We conducted a diagnostic surveillance study to identify Plasmodium, dengue virus, chikungunya virus, and Orientia tsutsugamushi infections among febrile patients who underwent triage for malaria in the outpatient department at Ispat General Hospital, Rourkela, Odisha, India. Febrile patients were enrolled from January 2016-January 2017. Blood smears and small volumes or vacutainers of blood were collected from study participants to carry out diagnostic assays. Malaria was diagnosed using rapid diagnostic tests (RDT), microscopy, and PCR. Dengue, chikungunya, and scrub typhus infections were identified using rapid diagnostic test kits and ELISA. Nine hundred and fifty-four patients were prospectively enrolled in our study. The majority of patients were male (58.4%) and more than 15 years of age (66.4%). All 954 enrollees underwent additional testing for malaria; a subset of enrollees (293/954) that had larger volumes of plasma available was also tested for dengue, chikungunya and scrub typhus by either
Scrub typhus, murine typhus, and spotted fever group rickettsia all occur in the Lao PDR (Laos) [1], [2]. Scrub typhus and murine typhus account for ∼16% and 10%, respectively, of acute undifferentiated fever in blood culture-negative adults admitted to hospital in the capital city, Vientiane [1]. However, typhus-like illnesses are significant diagnostic challenges; patients with leptospirosis, dengue, typhoid, and malaria are also common and can present with similar symptoms and signs. Although these pathogens are common and mixed (or concurrent) infections are expected, the laboratory diagnosis of mixed infection is a vexed subject. Reports of mixed infections often use only serological criteria. The problems of antibody persistence and interspecies cross-reaction raise uncertainty as to whether these results represent true mixed infections, sequential infections, or cross-reactions. We report a patient with concurrent scrub typhus and murine typhus, demonstrated by dual PCR positivity, and

|jats:p|The enzyme-linked immunosorbent assay (ELISA) has been proposed as an alternative serologic diagnostic test to the indirect immunofluorescence assay (IFA) for scrub typhus. Here, we systematically determine the optimal sample dilution and cutoff optical density (OD) and estimate the accuracy of IgM ELISA using Bayesian latent class models (LCMs). Data from 135 patients with undifferentiated fever were reevaluated using Bayesian LCMs. Every patient was evaluated for the presence of an eschar and tested with a blood culture for|jats:named-content content-type=genus-species|Orientia tsutsugamushi|/jats:named-content|, three different PCR assays, and an IgM IFA. Abstract. Scrub typhus is transmitted by trombiculid mites and is endemic to East and Southeast Asia and Northern Australia. The clinical syndrome classically consists of a fever, rash, and eschar, but scrub typhus also commonly presents as an undifferentiated fever that requires laboratory confirmation of the diagnosis, usually by indirect fluorescent antibody (IFA) assay. We discuss the limitations of IFA, debate the value of other methods based on antigen detection and nucleic acid amplification, and outline recommendations for future study.
Scrub typhus is associated with outbreaks of acute encephalitis syndrome in Uttar Pradesh, India. A case-control study indicated that children residing, playing, or visiting fields; living with firewood stored indoors; handling cattle fodder; and practicing open defecation were at increased risk for scrub typhus. Communication messages should focus on changing these behaviors.
Scrub typhus (Tsutsugamushi disease) ,is an acute febrile infectious disease endemic in eastern asia is caused by Orienta Tsutsugamushi, transmitted by bite of a Leptotrombidium mite (chigger). 45 year old lady presented with progressive worsening of fever (102F), myalgias, headache and tachycardia (120 /min). Leucopenia with thrombocytopenia was noted with elevated liver enzymes. Provisional diagnosis of malaria and dengue was suspected and serological tests were carried out. Paracetamol, IV fluids was given and was patient was monitored for hypotension and bleeding manifestaions. Serology for malaria and dengue were negative. Despite management fever continued to spike. Black eschar was noted on the left shoulder tip and scrub typhus was suspected because of characteristic eschar. Doxycycline was started and serology was positive for Orienta Tsutsugamushi antibodies. Patient recovered with fever and platelet counts normalizing. Recovery was uneventful. The spotting of the black dot i.e ...
Scrub typhus (tsutsugamushi fever) is a zoonosis of rural Asia and the western Pacific islands that is caused by the obligate Gram-negative intracellular bacterium Orientia (formerly Rickettsia) tsutsugamushi, which is transmitted (typically from rats) to humans by the bite of a larval leptotrombidium mite (chigger). Scrub typhus is an acute, febrile, infectious illness that was first described in China in 313 AD. It is caused by Orientia (formerly Rickettsia) tsutsugamushi, an obligate intracellular gram-negative bacterium, which was first isolated in Japan in 1930.
There is a sudden spurt in the number of cases in Chennai with patients presenting with fever not responding to conventional antibiotics for three weeks and associated with brown mark in the skin.. Scrub typhus spreads when chiggers - mites found in forests and, more recently, in urban shrubs - bite the person and inject a microorganism Rickettsia tsutsugamushi into the blood. The bite area turns black, the major symptom of the disease. The fatality rate is 8%. The disease shows symptoms similar to dengue, typhoid and malaria.. ...
Juleps Glycolic Hand Scrub has the double exfoliating action of finely milled natural apricot seeds plus glycolic acid. This scrub has a gel like base with a fair amount of apricot seeds in it. Julep suggests using a quarter sized amount of their hand scrub to exfoliate damp hands, but I feel like I need much less than that. This scrub is kinda similar to the St. Ives Apricot Scrub in the that it feels. Juleps scrub is fairly harsh, but not irritating. This scrub really softens the skin and removes any dry bits you may have. Juleps Glycolic Hand Scrub is a nice treat at the end of a work day thats been rough on your hands. This hand scrub is a great companion for Juleps Rock Star Hand Cream. I got curious about using this scrub on my face, since Julep half suggests it, so I tried it a few times and didnt really notice much of an improvement in my skin, but there was a pleasant tingle afterwards to let me know that the glycolic acid was working. Julep warns that glycolic acid can cause ...
The coastal sage scrub plant community is divided into three geographical subtypes - northern coastal scrub, southern coastal scrub, and maritime succulent scrub.. The coastal scrub communities are divided into three regions: Northern Coastal Scrub and Coastal Prairie, which lies in San Luis Obispo to Oregon, Coastal Sage scrub which lies in San Diego to Monterey, and Maritime Succulent Scrub, which can be found in the San Diego County to Baja. The northern coastal scrub consists of prairie, which is terraces with deep alluvial soils, and scrub, which is found on steeper slopes and ravine areas. Evergreen shrubs and subshrubs, which are soft leaves. They are found in semi-open with a height of 2-3 meters, with multiple layers. Some examples of the plant species that can be found are bush monkeyflower (Mimulus aurantiacus), poison oak (Taxicodendron diversilobum), coffee berry (Frangula californica), golden yarrow (Eriophyllum confertiflorum). Artemisia californica can be found in Coastal sage ...

A study comparing the aerobic bacterial bioburden associated with surgical scrub attire shows significantly greater contamination among home-laundered attire than scrubs laundered by the healthcare facility, scrubs sent out by the facility to a third-party company for laundering, or single use/disposable scrubs. In fact, home-laundered scrubs cleaned and ready to wear had as much bacteria present as facility-laundered, third-party laundered and single-use scrubs which had been worn for one day. Conducted in the spring of 2009, the study was performed by Bioscience Laboratories, Inc., of Bozeman, Mont., and sponsored by Molnlycke Health Care U.S., LLC. A total of 80 surgical scrub garments, tops and bottoms, prior to use and after use in the operating room for a day, were collected from multiple healthcare facilities across the U.S. They comprised 10 sets of scrubs in each category: single-use; home-laundered; facility-laundered; and third-party laundered. The study showed no statistically ...
To understand the pathogenesis of scrub typhus, we examined chemokine and cytokine production in susceptible (C3H/HeN) and resistant (BALB/c) mice after infection with O. tsutsugamushi Gilliam. C3H/HeN mice produced high levels of chemokines macrophage inflammatory proteins 1 alpha (MIP-1 alpha ), MIP-2, monocyte chemoattractant protein 1 (MCP-1), and cytokines gamma-interferon (IFN-gamma ), interleukin-12 (IL-12), IL-10, and tumor necrosis factor alpha (TNF-alpha ) in response to O. tsutsugamushi infection, compared to BALB/c mice. Chemokine profiles in infected mice correlated well with the kinetics of inflammatory cell infiltration. Hyperproduction of chemokines and cytokines was observed in another susceptible-infection model (BALB/c-Karp). These results suggest that hyperproduction of chemokines and cytokines are associated with susceptibility during O. tsutsugamushi infection ...
Being old school, I dont like to see people wearing scrubs outside of the hospital. But there is no evidence that bacteria on scrubs spread disease, and a large number of ancillary hospital personnel wear scrubs.. Every few months when things are slow, someone publishes an article about the imaginary dangers associated with doctors wearing scrubs in public. A recent version is from The Atlantic. An associate editor saw some people in scrubs having lunch in a restaurant and was, of course, horrified. She questioned the magazines medical editor, Dr. James Hamblin, whose response was remarkably reasoned (until the end).. He pointed out that it might not have been doctors because everyone, including secretaries (and even custodial people in my hospital), now wears scrubs to work. Dr. Hamblin rightly added that there is a lot of debate about the issue. He speculated that some guys wear scrubs in public as a signal to women that they are doctors.. But at the end of the piece, he said it was OK if ...
Gone are the days when scrubs were only open in a lone tone - green. Starting late, the clinical administrations region has gotten a handle on the improvement of the style business. This change has seen the overall clinical industry where high style clothing that synchronizes well with their consistently plan attire. One near apparel thing that has changed during the time is clinical scrubs. Despite the a ton of decisions, plans, and shadings, scrubs get introduced to a lot of stains. With the disordered step by step timetable and control of nurses, genuine thought of clinical scrubs is a troublesome task. Scrubs are basically the most delicate attire in the entire clinical wear, and they can without a doubt get depleted. Thusly, in case you are on a tight spending plan and cant remain to buy new clinical scrubs every so often, it is basic to keep it unblemished and alive and well. Notwithstanding your monetary arrangement, it is basic to make them prop up long. Washing clinical scrubs like ...
The tiny chiggers (mite larvae) attach themselves to the skin. During the process of obtaining a meal, they may either acquire the infection from the host or transmit the rickettsieae to other mammals or humans. In regions where scrub typhus is a constant threat, a natural cycle of R. tsutsugamushi transmission occurs between mite larvae and small mammals (field mice and rats). Humans enter the cycle of rickettsial infection only accidentally. The seasonal occurrence of scrub typhus varies with the climate in different countries. It occurs more frequently during the rainy season. Certain areas such as forest clearings, riverbanks, and grassy regions provide optimal conditions for the infected mites to thrive. These small geographic regions are high-risk areas for humans and have been called scrub-typhus islands. More than 1 billion people living in the area of endemicity and travelers are at risk from scrub typhus[1-4]. Approximately 48%-82% of individuals develops an eschar[5] at the site of ...
Background Scrub typhus is an acute febrile disease due to infection. area. Despite continuous initiatives to build up a vaccine against scrub typhus over the last many decades, all approaches possess didnt induce effective immunity. The primary issue for the introduction of a scrub typhus vaccine may be the selection of correct antigens that cover a wide selection of antigenic strains and induce long-lasting immunity. Right here, we examined the usage of ScaA proteins being a vaccine antigen. Our results demonstrate that ScaA proteins functions being a bacterial adhesion aspect and an antibody against ScaA considerably inhibits infection into web host cells. Furthermore, ScaA vaccination provides defensive immunity Rabbit Polyclonal to Nuclear Receptor NR4A1 (phospho-Ser351). against lethal issues from the homologous stress, and confers better security against heterologous strains when coupled with TSA56 also, the major outer membrane protein that was used being a potential vaccine antigen ...

Spa enthusiasts around the globe are raving about it. With its unique approach and various health and beauty benefits, the Korean body scrub treatment is making its way onto many spa menus, proving to be one of the most popular services requested for rejuvenation and indulgence.. Although slightly different from traditional spa services, the Korean body scrub allows guests to enjoy its numerous benefits, while relishing in the pampering that goes along with it. Traditionally, clients are asked to soak in a warm bath or sit in a sauna or steam room prior to the scrub. This helps to soften the skin and prepare it for exfoliation. Clients are then asked to lay on a massage table, where the scrub will begin.. The therapist uses mitts, made of slightly coarse material, to scrub the body, periodically rinsing away the dead skin. The scrub is finished when all of the dead skin is washed away. First-timers often take longer to ensure that all of the dead skin is removed from the body. They may even ...

Update on the scrub bar: even though it was soft, I took it to shower after I had cured it for a few days. First, I was quite thrilled, the level of scrubbiness was very nice, and it went on well. The problem arose when I tried to rinse it off - I felt like I was covered in thick layer of waxy butter that would not let go of me. I eventually ended up using a different scrub to scrub this butter off of me. And that worked out fine. Except that now I had spent twice the time and twice the scrub to clean myself so that sure wasnt what I expected. So to recap, I did the recipe as written, except used a 60% strength sodium lactate, and upped the amount to have the right amount of it in the scrub, and I did only have BTMS-25, so used that. But from one of your articles I gathered that I could use it in a bar, just would get slightly stickier product. So what went wrong ...

My take on VLCC Fade Tan scrub: Okay, so this is one of the most raved about tan-removal products, or so it seems, when you read around! The scrub is a gel-based product, with fine orange scrubbing granules and some yellow beads thrown in. The orange beads are rounded in shape, and they scrub the skin well. I especially love how this scrub works on the white-heads around the nose. The other beads, the yellow ones, are quite soft and rubbery, and you need to specially break those beads with manual pressure to be able to use them. Once broken, they just give out some color and I don;t think they contribute to the scrubbing effect. The scrub has a fresh, sweet-orange fragrance ...

Orientia tsutsugamushi, an obligate intracellular bacterium, is the causative agent of scrub typhus. TNF-α and IFN-β mRNAs were induced after infection with O. tsutsugamushi. Western blot analysis shows that O. tsutsugamushi-stimulation leads to phosphorylation of ERK1/2, JNK1/2, and p38 MAPKs. Selective inhibitors of ERK1/2, JNK1/2, p38 MAPKs could all completely prevent TNF-α secretion. Host TNF-α production via p38 & JNK pathways by this bacterium was found to be regulated by post-transcriptional mechanism. In contrast, ERK pathway mainly control the transcription step of TNF-α gene regulation. Orientia inactivation by heat did not abolish induction of TNF-α production. However, heat inactivation of bacterium or treatment of host cell with cytochalasin D led to a complete loss of IFN-β induction, suggesting requirement of internalization and escape into cytoplasm by bacteria for host IFN-β response. Selective inhibitors of ERK1/2, p38 MAPKs, or NF-κB could all completely prevent ...
To date more than 20 antigenically distinct strains of Orientia tsutsugamushi (OT) reported within the tsutsugamushi triangle that cause an undifferentiated acute febrile illness in humans. Genotypic characterization of OT in different geographic regions or within the same country, is important in order to establish effective diagnostics, clinical management and to develop effective vaccines. Genetic and antigenic characterization of OT causing human disease in OT-endemic regions is not known for Sri Lanka. Adult patients and children who were admitted with an acute febrile illness and presumed to having acute scrub typhus based on presence of an eschar and other supporting clinical features were recruited. Eschar biopsies and buffy coat samples collected from patients who were confirmed having OT by IFA were further studied by real time PCR (Orientia 47 kD) and nested PCR (Orientia 56 kD) amplification. DNA sequences were obtained for 56 kD gene amplicons and phylogenetic comparisons were analyzed

Studying emerging or neglected pathogens is often challenging due to insufficient information and absence of genetic tools. Dual RNA-seq provides insights into host-pathogen interactions, and is particularly informative for intracellular organisms. Here we apply dual RNA-seq to Orientia tsutsugamushi (Ot), an obligate intracellular bacterium that causes the vector-borne human disease scrub typhus. Half the Ot genome is composed of repetitive DNA, and there is minimal collinearity in gene order between strains. Integrating RNA-seq, comparative genomics, proteomics, and machine learning to study the transcriptional architecture of Ot, we find evidence for wide-spread post-transcriptional antisense regulation. Comparing the host response to two clinical isolates, we identify distinct immune response networks for each strain, leading to predictions of relative virulence that are validated in a mouse infection model. Thus, dual RNA-seq can provide insight into the biology and host-pathogen interactions of a

Background To date more than 20 antigenically distinct strains of Orientia tsutsugamushi(OT) reported within the tsutsugamushi triangle that cause an undifferentiated acute febrile illness in...
A type-specific antigen (54- to 56-kilodalton polypeptide) in the envelope of Rickettsia tsutsugamushi was purified from each of three prototype strains (Gilliam, Karp, and Kato) by a combination of mild anionic detergent treatment, gel filtration, and reverse-phase high-performance liquid chromatography. The purified antigens from the three strains were shown to have similar amino acid compositions: primarily aspartic acid, glutamic acid, and glycine, with lesser amounts of cysteine, methionine, and tyrosine. The N-terminal amino acid sequences of the antigens were 74.3% homologous among the three strains. ...
Studies of geographical distributions and relative population densities of the vector mites of tsutsugamushi disease were carried out in October 1996 at 12 locations of the mid-south inland of the Korean peninsula, where chigger mites have been never studied. Of 177 field rodents and insectivores collected, 154 (87.0%) were Apodemus agrarius. Total 25,707 chigger mites were collected and 14 species were identified, of which Leptotrombidium pallidum was predominant (79.8%) and L. palpale the next (8.9%). L. pallidum, the vector species, was widely distributed in all study areas, showing the highest density at Cho-o 2-dong, Sangju-si (chigger index 201.8), and the lowest at Tanwol-dong. Chungju-si (chigger index 40.7). The other vector species, L. scutellare was found only at the southern part of the study area such as Yobae and Mipyong, Kumrung-gun and Unsu, Kimchon-si. The northernmost areas of the L. scutellare distribution were coincided with the areas where anual mean air temperature is above ...
During Napoleons retreat from Moscow in 1812, more French soldiers died of typhus than were killed by the Russians.[30]. A major epidemic occurred in Ireland between 1816 and 1819, during the famine caused by a worldwide reduction in temperature known as the Year Without a Summer. An estimated 100,000 people perished. Typhus appeared again in the late 1830s, and yet another major typhus epidemic occurred during the Great Irish Famine between 1846 and 1849. The Irish typhus spread to England, where it was sometimes called Irish fever and was noted for its virulence. It killed people of all social classes, as lice were endemic and inescapable, but it hit particularly hard in the lower or unwashed social strata. In the United States, a typhus epidemic broke out in Philadelphia in 1837 and killed the son of Franklin Pierce (14th President of the United States) in Concord, New Hampshire, in 1843. Several epidemics occurred in Baltimore, Memphis and Washington, D.C. between 1865 and 1873. Typhus ...

The citrate cycle (TCA cycle, Krebs cycle) is an important aerobic pathway for the final steps of the oxidation of carbohydrates and fatty acids. The cycle starts with acetyl-CoA, the activated form of acetate, derived from glycolysis and pyruvate oxidation for carbohydrates and from beta oxidation of fatty acids. The two-carbon acetyl group in acetyl-CoA is transferred to the four-carbon compound of oxaloacetate to form the six-carbon compound of citrate. In a series of reactions two carbons in citrate are oxidized to CO2 and the reaction pathway supplies NADH for use in the oxidative phosphorylation and other metabolic processes. The pathway also supplies important precursor metabolites including 2-oxoglutarate. At the end of the cycle the remaining four-carbon part is transformed back to oxaloacetate. According to the genome sequence data, many organisms seem to lack genes for the full cycle [MD:M00009], but contain genes for specific segments [MD:M00010 M00011 ...

The California Scrub Oak is a small evergreen or semi-evergreen shrubby oak in the white oak section. It grows up to 6 meters in height. It has variable, dull green leaves which are 1.5-3 centimeter long and 1-2 centimeter broad, leathery on their top surfaces and somewhat hairy underneath. Flowers are yellow but inconspicuous. The solitary or paired brown acorns are 1-3 centimeters long and 1-2 centimeters broad, and pointed or egg-shaped with thin caps when mature; they mature in about 6-8 months after pollination. It is a native of the scrubby hills of California. It is a common member of chaparral ecosystems; in fact, the word chaparral is derived from the Spanish word for scrub oak, chaparro. Many other scrub-type oaks can be found in the region, notably Quercus dumosa. In cooler, more exposed areas, scrub oak is usually a small, compact shrub, but in warm or sheltered areas the plant can spread out and grow several meters high. Oaks generally do not like summer water which promotes the ...

For our study, Weil-Felix test (WFT) was performed using the commercially available kit (Plasmatech, UK) as per the manufacturers instructions which gives a positive result to agglutination titres of ≥160 [2]. We are completely aware that WFT is not a gold standard test to diagnose rickettsial diseases and hence, we used a term presumed rickettsial retinitis in the report.. Multifocal retinitis post febrile illness in Indian scenario is commonly due to chikungunya, dengue, West Nile virus (WNV) and rickettsial infections. Ruling out other causes (chikungunya, dengue, WNV), taking appropriate history of tick bite rather than mosquito bite and skin rashes will increase suspicion for rickettsial retinitis. Of late, we processed stored serum sample of our patients of presumed rickettsial retinitis for WNV serology and PCR which showed negative results. Although WFT is not a gold standard test, it helps to subclassify rickettsial diseases (Indian tick typhus, epidemic typhus and scrub typhus) ...
