PURPOSE To study the use of fibrin sealant as a drug delivery system for the sustained transscleral delivery of dexamethasone and methotrexate. METHODS Scleral sections excised from moist-chamber-stored human globes were mounted in a perfusion chamber. Dexamethasone-fluorescein or methotrexate-fluorescein in either fibrin sealant or balanced salt solution (BSS) was applied to the episcleral surface. BSS was perfused to the choroidal side, fluorescence was measured in perfusate fractions, and an apparent scleral permeability P(A) was calculated for each solute-vehicle combination. RESULTS P(A) for both compounds was significantly lower with fibrin sealant delivery compared to delivery in BSS (p | 0.001). However, the fibrin sealant vehicle provided a more sustained release of both drugs through 24 hr. CONCLUSIONS Incorporating dexamethasone and methotrexate into a fibrin sealant provided a more gradual drug delivery and a more uniform delivery compared to dissolving these drugs in BSS. Fibrin

This study determined the in vitro permeability of cisplatin through isolated human sclera as delivered by a collagen matrix vehicle. Short-term and long-term intraocular levels of cisplatin were also measured in the rabbit eye after a subconjunctival injection. Cisplatin in either a collagen matrix vehicle or a control balanced salt solution (BSS) vehicle was applied to human sclera mounted in a specially designed in vitro perfusion chamber. The amount of cisplatin that diffused across the sclera was measured in hourly samples for 24 h using atomic absorption spectrometry. In vivo studies were also performed in Dutch Belted rabbits given subconjunctival injections of cisplatin in collagen matrix or in BSS. Eyes were enucleated at 1.5 h and 2 weeks after injection, frozen, and dissected to determine the intraocular cisplatin concentrations. Transscleral delivery of triamcinolone acetonide into the vitreous using sub-Tenons injections may be a safer alternative to reduce the sight-threatening complications of direct intravitreal injections. However, sub-Tenons injections have demonstrated low and poorly sustained vitreous drug levels in animal studies. To improve our understanding of the clearance mechanisms of corticosteroids, we evaluated vitreous drug levels following sub-Tenons injection of triamcinolone acetonide in rabbits with selective elimination of conjunctival lymphatic/blood vessels and the choroid. Pigmented rabbits were given a sub-Tenons injection of a preservative-free triamcinolone acetonide formulation of either a 10- or 20-mg dose in the superotemporal quadrant. Synchrotron X-ray diffraction patterns were obtained from the cornea and posterior sclera of control and myopic chicks. No significant difference was found in the interfibrillar or in the intermolecular spacing of the collagen fibrils from the corneas of control and myopic chicks. The intermolecular spacing of myopic sclera was shown to be significantly (p , 0.01) higher than in controls. Sclera and cornea from normal and myopic chicks were stained for proteoglycans using the critical electrolyte method of Scott and Orford (1981). In the sclera, two morphologically distinct types of proteoglycans could be distinguished; one small and usually elongated (approximately-equal-to 20 nm), the other larger and irregularly shaped. The small proteoglycans were seen binding preferentially to the d and e bands of the collagen fibrils. When doing a trabeculectomy, for example, you can gradually practice the skills needed to perform canaloplasty. During trabeculectomy we normally just create a scleral flap and then do the sclerostomy. But before doing the sclerostomy, you can do a deep sclerectomy, which means removing a deeper block of sclera so youre left with a thin layer of sclera overlying the choroid. You can dissect that anteriorly until you get a little bit of a Descemets window, and stop there; doing this will give you practice dissecting a deep sclerectomy. Each time you do it, make a bigger Descemets window and start looking for Schlemms canal. You can still go ahead with the trabeculectomy; just do what you would normally do. Methods: Five groups of five chicks were studied from day 0-12 post-hatching. One group remained untreated (C), and four were form-deprived by monocular light diffusers to induce myopia. Two groups (RL and A) wore diffusers for 9 days, and the other two groups (D and D + A) wore diffusers throughout the study. Group D received no further treatment (myopia positive control). In this study, the organization of collagen fibrils within the sclera of the eye was investigated using the 7 keV hard X-ray microscope of the Pohang light source and compared to images from electron and atomic force microscopy. From the captured X-ray images, individual collagen fibrils were observed clearly in a spatial resolution much better than 100 nm, both in longitudinal sections and in transverse sections. Some of the collagen fibrils showed evidence of axial periodicity. In some regions of the samples, we could see cross-bridge like structures between adjacent collagen fibrils. The X-ray microscope also allowed the observation of keratocytes and the lamella structure of the scleral stroma.
