INTRODUCTION:. Wernickes dysphasia and formal thought disorder are regarded as distinct diagnostic entities although both are linked to pathology in the left superior temporal gyrus (STG). We describe a patient with focal pathology in the left STG, giving rise acutely to a fluent dysphasia, which gradually evolved into formal thought disorder.. METHOD:. Clinical, neuropsychological, neuropsychiatric, and neuroradiological assessment.. RESULTS:. A right-handed patient, AJ, presented acutely with a fluent dysphasia. His speech output gradually evolved from undifferentiated jargon, through neologistic jargon, to an intelligible but bizarre form of discourse. Comprehension was relatively well preserved. Radiology revealed an arteriovenous malformation in the left middle, and inferior temporal gyri, with reduced perfusion of the left STG. Purpose : To generate tomographic retinal cross-sections using en-face images acquired from non-interferometric Adaptive Optics (AO) Ophthalmoscopes, including Flood-Illumination (FIO) and Scanning Laser (SLO). We call this method "Optical Incoherence Tomography"(OIT). We explore the focus sensitivity of the OIT to guide focus positioning, improving the imaging protocol in en-face ophthalmoscopes. Methods : The OIT method consists of three steps: acquiring en-face images from different focal planes (forming a Z-stack), filtering out the image low-spatial frequency content, and computing the image energy. But we should remember that there are acute and chronic schizophrenic patients who respond more easily to our analytic approach. They gain insight, co-operate in the analysis and seem to improve from the beginning. In these cases there seems to be a part of the personality not completely involved in the psychosis. So in spite of their severe psychotic manifestations they do not completely lose touch with reality once the analysis is going ahead.
