Our group of primary SS patients showed a 19% prevalence of anti-HCV antibodies by third generation ELISA while only 10% of the patients had HCV viraemia. Several studies have considered this question and detected prevalences ranging from 0-75%.4 5Discrepancies among the studies could be caused by: (1) different clinical criteria for the diagnosis of SS, (2) analysis of anti-HCV antibodies in serum, which can lead to false positive results because of hyperglobulinaemia or gammaglobulin treatment as noted by others4 and, (3) differences in the incidence of HCV among different populations.. It is of interest to note the presence of HCV infection exclusively in primary SS patients. This result could be attributed (1) to a pathogenic role of HCV in primary SS, as has been previously suggested with other viral agents,2 or (2) to the presence of a sicca syndrome similar to primary SS leading to the misclassification of asymptomatic HCV infected patients as primary SS.5 Morphological examination of the ...

Labial salivary gland biopsy (LSGB) is the most important diagnostic tool for the diagnosis of Sjögrens syndrome (SS), but its diagnostic value is rarely studied. This study assessed the sensibility and specificity of LSGB, and the clinical profiles of patients who were referred for biopsy. Retrospective analysis of the histopathological reports from LSGB and medical report data from patients who underwent LSGB between 2008 and 2011 was conducted. About 290 biopsies were performed and 74 were excluded due to insufficient clinical data. Of the 216 patients, 0.46% was carrier of hepatitis C virus, 30.1% had primary SS (pSS), and 8.8% had secondary SS (sSS). Of the samples, 94.3% presented dryness symptoms, 51.6% experienced dryness only, 42.7% had systemic manifestations, and 66.9% presented low unstimulated salivary flow and/or Schirmers test. LSGB was necessary in 67.6% to confirm the presence of SS based on the American-European Consensus Group 2002 criteria (AECG). Based on specialists opinion,

Background/Purpose: Salivary gland ultrasonography is a highly specific tool for the diagnosis of primary Sjögren´s syndrome (PSS). Real time sonoelastography (RTS) is a novel imaging option involving tissue stiffness assessment. Previous works have reported higher RTS values in PSS vs healthy and sicca controls. Herein we evaluated the stiffness of parotid and submandibular glands using RTS and correlated it with a multiparametric approach including morphologic ultrasonographic parameters, profibrotic chemokines and cytokines levels, serology and the presence of minor salivary gland fibrosis. Methods: We included 26 patients with PSS according to the AECG criteria, who had a salivary gland biopsy with no more than 5 years previous the ultrasonographic evaluation. B-Mode ultrasonography and RTS (ARFI methodology) were performed by a trained radiologist. Parotid and submandibular glands were individually rated for parenchymal echogenicity, homogeneity, hypo-echogenic areas, hyper-echogenic ...

I have made several trips to the dentist. He said it was a blocked minor salivary duct, and he could remove it surgically. He said there was no stone present, that it was just inflamed. I went to my doctor and he said it could be a sinus infection and gave me Penicillin which did nothing. Today I went back to the dentist, and he wants me to go to an oral surgeon for a diagnoses. I have used numerous mouthwashes and am drinking plenty of water. What could this pain be? I am so fed up with it and now am very concerned that it may be oral cancer ...

Most oral cancers start in the cells that line the mouth (the mucosa; called squamous cell carcinomas, the same as the common types of skin cancer). Oral cancers can also start to grow in minor salivary glands, bone, blood vessels, nerves, and other structures. These cancers are rare and only make up less than 5% of oral cancers. ...

Polymorphous low-grade adenocarcinoma is a rare malignancy arising from the minor salivary glands in the aerodigestive system, most frequently the hard palate. The treatment of choice is wide surgical resection, and the efficacy of radiotherapy has not been confirmed. A 54-year-old male presenting with a mass at the base of the tongue performed transoral laser microsurgery. The pathologic diagnosis was polymorphous low-grade adenocarcinoma. Complete surgical excision was performed via transoral robotic surgery without a flap reconstruction of the surgical defect. Without complications of bleeding or injury to the hypoglossal nerve, proper surgical margins were obtained, and no recurrence was found after 6 months after surgery. The patient did not complain of dysphagia or aspiration. We conclude that, in surgery for tongue base tumors with unknown malignant potential, transoral robotic surgery can be considered for achieving a definite resection avoiding a mandibulotomy without complications of dysphagia

We hypothesized that lymph node involvement in minor salivary gland cancers is associated with clinical and pathological factors commonly available to the clinician after a typical initial workup. Our aim was to identify these factors using a dataset that allowed us to compile the largest series of minor salivary gland cancers in the published literature. Using this dataset we also aimed to characterize the distribution of histological types by primary site, identify the predictors of the use of external beam radiation therapy and neck dissection, and examine the effect of lymph node involvement on survival. Using the SEER database, we identified 2667 minor salivary gland cancers with known lymph node status from 1988 to 2004. Univariate and multivariate analyses were conducted to identify factors associated with the use of neck dissection, the use of external beam radiation therapy, and the presence of cervical lymph node metastases. Kaplan Meier survival curves were constructed to examine the effect

Abstract: Pleomorphic adenoma also called benign mixed tumor, is the most common salivary gland tumor. Most of these tumors occur in major salivary glands; Parotid gland happens to be the commonly involved one (about 90 %) and about 10 % of these occur in the minor salivary glands. The most common sites for pleomorphic adenoma of minor salivary glands are the palate, followed by the lips, the cheeks, floor of the mouth, tongue, tonsil, pharynx, the retromolar area and the nasal cavity. FNAC of intraoral swellings is rarely advised. Here, we are reporting a rare case of pleomorphic adenoma arising from the palate diagnosed on FNA ...
New 2016 ACR/EULAR classification criteria for primary Sjogrens syndrome (SS) have been developed and endorsed by the ACR. The newly proposed criteria include simple-to-perform items. Two important points of the new criteria should be considered. Firstly, they indicate that either salivary gland biopsy or anti-Ro must be positive in order to corroborate the inflammatory and autoimmune nature of the disease. Secondly, the criteria recognize the systemic nature of SS, namely that patients without salivary or ocular glandular symptoms, but with extraglandular manifestations and B cell activation markers were also included in the SS classification. Additionally, the new criteria modified some technical points. The ocular staining score threshold was increased to 5 due to the higher specificity. The immunological profile includes only anti-Ro antibodies, while positivity for antinuclear antibodies and rheumatoid factor or isolated anti-La was excluded due to a lack of specificity. The 2016 ACR/EULAR

Biopsies of apparently normal buccal mucosa were taken from 14 patients with Crohns disease and 13 healthy controls who were matched for dental status. Most patients had an increased number of lymphocytes around vessels in the subepithelial tissue and two showed fibrosis with moderate atrophy of minor glands. Plasma cells which contained immunoglobulin, predominantly IgA, were only found around minor salivary glands in both patients and controls. Quantitative studies showed a significant increase in the number of cells containing IgA in patients compared with controls. No correlation was found between immunoglobulin pattern and disease activity, age, sex, or duration of Crohns disease. A significant correlation was found between the activity of the disease, as defined by the Crohns Disease Activity Index, and the number of plasma cells containing IgM.. ...

Although IL-17 and IL-23 produced by Th17 cells are associated with the pathogenesis of SS, the pathological role of Th17 cells remains obscure (11-15). The results of the present study shed light on the function of RORγt in the development of sialadenitis. Infiltrating mononuclear cells in the salivary glands of RORγt Tg mice were mainly CD4+ T cells at the early stage of the inflammation whereas B cells gradually increased with age. These changes resemble lymphocyte infiltration in the labial salivary glands of patients with SS. The results provide evidence for the role of RORγt overexpression in CD4+ T cells in the development of spontaneous sialadenitis-like SS through downregulation of CD4+CD25+Foxp3+ Treg cells.. Experimental evidence suggests that IL-17 produced from CD4+ T cells is directly involved in the onset of SS-like disease (11, 22). However, we demonstrated in the present study the development of sialadenitis in IL-17-deficient RORγt Tg mice, indicating that IL-17 is not ...
Only a handful of studies on OS in SjS are currently available, which are summarized in Table 1 [14,16,19-22]. Previous studies have mostly investigated human SjS samples [14,16,19-21] and only one study utilized a mouse model of SjS [22]. All studies have clearly shown increased OS biomarkers in SjS [14,16,19-22]. Ryo et al. [19] showed excess levels of two OS markers, 8-hydroxy-2-deoxyguanosine and hexanoyl-lysine (HEL), in saliva from SjS patients suggesting an involvement of OS in SjS pathogenesis. Others reported an increased level of OS in the labial salivary gland of primary SjS patients, measured by markers of DNA, and lipid oxidation [20]. Cejkova et al. [21] showed an increased the level of MDA in the conjunctival epithelium and Wakamatsu et al. [16] found that increased levels of OS utilizing HEL in tear, and HEL and 4-hydroxy-2-nonenal (4-HNE) in the conjunctival epithelium of SjS patients.. Analyses of T cell receptors (TCR) on T cells infiltrating labial salivary glands of patients with Sjögrens syndrome (SS) indicate that the cells expand by antigen stimulation in context of major histocompatibility complex (MHC). To elucidate the autoantigens recognized by T cells infiltrating in labial salivary glands from patients with SS, proteins derived from human salivary gland cDNA libraries were screened by West-Western method using TCR-CDR3 probe, which is antigen recognition region of TCR on T cells. 13 cDNA clones were detected as proteins binding to TCR-CDR3 region. One was a human alpha-amylase salivary precursor (AA54-407), suggesting that alpha-amylase might be a salivary gland-specific autoantigen. To examine whether alpha-amylase acts as an antigen in labial salivary glands, PBL from 11 patients with SS were incubated with 9 different synthetic amino acids of alpha-amylase or salivary alpha-amylase. SSCP analysis on TCR clearly showed that alpha-amylase reactive T cells were ...

Answer: Necrotizing sialometaplasia. Histology: The stroma surrounding the proliferation of bland squamous epithelium in this case is highly inflamed, and shows focal necrosis. The nests of squamous epithelium may appear to be invasive, but on low power have an overall lobular, branching architecture to them. Focally one may see a benign residual duct of native minor salivary gland. Significant cytologic atypia and mitotic activity are not evident. The findings are consistent with necrotizing sialometaplasia, representing squamous metaplasia of injured salivary gland ductal epithelium, typically in the setting of an ischemic insult. Discussion: Invasive squamous cell carcinoma would be highly unusual in this age group, and would demonstrate greater cytological atypia and an infiltrative growth pattern. Mucoepidermoid carcinoma could be suggested by the presence of proliferating squamous epithelium in associationwith the benign residual ducts of the minor salivary gland. The lobular nature of the ...
Objective. The present study examined the reliability and correlation of sialography, salivary gland biopsy, and ultrasonography for Sjögrens syndrome (SS), and evaluated the usefulness of ultrasonography as a diagnostic tool for SS in comparison with sialography and histopathology. Methods. Seventy-three patients who underwent sialography, ultrasonography, and salivary gland biopsy were included in this study. The study evaluated the diagnostic reliability and correlation of each kind of examination with SS. Results. There was a statistically significant difference in the sensitivities of sialography and histopathology, in the specificities of sialography and ultrasonography, and in the accuracies of sialography and both of ultrasonography and histopathology. The correlation coefficient (r) between sialography and ultrasonography was significantly higher than the others and indicated a good correlation. Conclusions. Ultrasonography can be used as a diagnostic tool for SS with its advantage of ...
Mucoepidermoid carcinoma is the most common type of minor salivary gland malignancy in adults. Mucoepidermoid carcinoma can also be found in other organs, such as bronchi, lacrimal sac and thyroid. Mucicarmine staining is one stain used by pathologist for detection. Occurs in adults, with peak incidence from 20-40 years of age. A causal link with cytomegalovirus (CMV) has been strongly implicated in a 2011 research. Presents as painless, slow-growing mass that is firm or hard. Most appear clinically as mixed tumors. This tumor is not encapsulated and is characterized by squamous cells, mucus-secreting cells, and intermediate cells. Mucoepidermoid carcinomas of the salivary and bronchial glands are characterized by a recurrent t(11;19)(q21;p13) chromosomal translocation resulting in a MECT1-MAML2 fusion gene. The CREB-binding domain of the CREB coactivator MECT1 (also known as CRTC1, TORC1 or WAMTP1) is fused to the transactivation domain of the Notch coactivator MAML2 PMID 16444749. A possible ...

Radiotherapy of malignancies in the head and neck often involves the major and minor salivary glands in the radiation field. Adverse effects, such as dry mouth symptoms, are common after such therapy. The aim of this thesis was to study longitudinal effects of fractionated irradiation to the head and neck on salivary gland function in man and in the rat and to test radioprotection from antioxidant vitamins (retinol, a-tocopherol and ^-carotene).. A sharp decrease in parotid saliva flow rate was seen after one week of irradiation in cancer patients, but in some patients recovery was seen two months after completed treatment. Loss and recovery of salivary gland function were dependent on the total dose given. Irradiation with doses of ,65 Gy to the parotid glands led to permanent loss of function in the majority of parotid glands, while recovery could be seen after irradiation with doses of ,52 Gy. Concentrations of some proteins and electrolytes in saliva were increased during irradiation but no ...
Adenoid cystic carcinoma (ACC) is an uncommon malignancy that arises in secretory glands. The most common sites for the disease are the major and minor salivary glands but these tumors may also arise in the nasal cavity, lacrimal gland, tracheobronchial tree, breast or vulva. The mainstay of treatment for localized ACC is surgical resection often followed by post-operative radiotherapy. Although this leads to an initially high rate of local control, the 5-year disease-free survival rate is 50-75%. In addition, a significant proportion of the patients develop distant metastases, most frequently in the lung. Compared to other malignancies, ACC tends to grow more slowly. Thus, patients often do well in the short-term but long-term prognosis remains guarded and most succumb to the disease within 10-15 years. To date, systemic therapies have proven to be largely ineffective against recurrent and metastatic ACC. Dovitinib is a broad-targeted-profiled RTK inhibitor active against these three RTKs ...

The sicca components of Sjögrens syndrome were investigated in 14 patients with primary biliary cirrhosis with a battery of tests that included Schirmers test; rose bengal staining; parotid secretory sialography; scintillation scanning of salivary glands with 99mTc; radioactive counts in saliva at 15 and 60 minutes after intravenous injection of the same radionuclide, as an index of salivary gland function; and histologic study of minor salivary gland specimens obtained by lip biopsy. Abnormalities of lacrimal or salivary glands were detected by two or more tests in all patients. Presence or degree of sicca components of Sjögrens syndrome did not correlate with the duration or the degree of liver disease, the presence of autoantibodies, or the levels of immunoglobulins. Our findings indicate that primary biliary cirrhosis is a systemic disease and support the possibility that its pathogenesis is autoimmune. ...
To the editor: This letter has been evoked by Dr. Norman Cummings discussion of biopsy in the recently published NIH Conference, Sjögrens Syndrome-Newer Aspects of Research, Diagnosis, and Therapy [Ann Intern Med 75:937-950, 1971].. It was of considerable diagnostic and research interest when it was discovered that the ubiquitous minor salivary glands of the lower lip became involved in the same lymphoid infiltrative process that is seen in the major salivary glands. Thereafter, rather than risk facial nerve injury or parotitis by parotid biopsy, it was shown that a simple lower lip biopsy, performed under local anesthesia, provided ample material ...
Mucocele is a relatively common, uncomplicated, and truly benign oral lesion. It is frequently seen in young age and mainly affect the lower lip; it often occurs after trauma to a minor salivary gland, such as lip biting. We are presenting a particular case of a 33-year-old woman who repeatedly developed superficial swelling in the lower lip, the last one enduring from many weeks. Surgical removal revealed Mucocele at histological examination and, in this particular case, the presence of nine accessory ectasic salivary glands ...
The nonspecific and often vague nature of symptoms that are associated with AL amyloidosis frequently leads to delays in diagnosis such that organ dysfunction is advanced by the time treatment is initiated. The diagnosis of AL amyloidosis should be considered in patients with unexplained proteinuria, cardiomyopathy, neuropathy, or hepatomegaly and in patients with multiple myeloma that has atypical manifestations.. The diagnosis of AL amyloidosis requires (1) demonstration of amyloid in tissue and (2) demonstration of a plasma cell dyscrasia. Tissue amyloid deposits demonstrate apple-green birefringence when stained with Congo red and viewed under polarizing microscopy. Fine-needle aspiration of abdominal fat is a simple procedure that is positive for amyloid deposits in ,70% of patients with AL amyloidosis (15,16). Other tissues that allow for relatively noninvasive biopsy procedures are the minor salivary glands, gingiva, rectum, and skin. However, obtaining tissue from an affected organ may ...

The purpose of this study is to perform biopsies of one of the glands that make saliva. The biopsied tissue will then be analyzed to see if it has changes that occur in Parkinsons disease. This study will determine whether it is possible to do a second biopsy a few years after a previous biopsy and determine whether there are changes in the biopsy that would allow for analysis of disease progression.. ...

In recent years, an increase in the number of CT and MRI scans has been recorded for first-line diagnosis in patients with tumor-like lesions of the salivary glands at the outpatient care of an ENT-department (university hospital). 30.7% of the patients with tumor-like lesions in salivary glands underwent an imaging procedure - MRI or CT - prior to US and in about 11.0% of the patients CT scans lack standards like intravenous contrast or a slice thickness of 5 mm or less. Besides data presentation, the present study has been performed to highlight several issues explaining this phenomenon. Especially, one issue addresses the key question, whether MRI, CT and US devices of the newer generation were more valid to deduct the correct diagnosis with special focus on the nature of the tumor lesion (benign/malignant).. According to our study population, the percentage of CT and MRI scans with standard quality for first-line diagnosis in patients with tumor-like lesions of the salivary glands ranged ...

Sjogren syndrome likely results from a combination of genetic and environmental factors (multifactorial). Several different genes appear to affect the risk of developing the condition, however, specific genes have yet to be confirmed. Simply having one of these genes does not cause a person to develop the disease; some sort of trigger is also needed. That trigger may be a viral or bacterial infection. The genetic variations that increase susceptibility may reduce the bodys ability to turn off the immune response when it is no longer needed. The possibility that the endocrine and nervous systems may play a role in the disease is also under investigation.. ...
