Purpose Phosphodiesterase (PDE) terminates the signaling pathway of myometrial relaxation by degradating cAMP to the inactive 5 '-AMP. The PDE4 family is one of the most predominant PDE families that display high affinity to cAMP. The objective of this study was to evaluate the effects of PDE4 gene polymorphisms on tocolytic effects and adverse drug events (ADEs) of ritodrine therapy in patients with preterm labor. Methods A total of 170 preterm labor patients were included in this study. To elucidate the effects of genetic polymorphisms on the inter-individual variability of ritodrine efficacy and ADEs, 8 single nucleotide polymorphisms (SNPs) were genotyped: PDE4D (rs1544791, rs983280, rs1504982, rs10940648, rs829259) and PDE4B2 (rs598961, rs2180335, and rs17128809). Additionally, rs1042719 of the ADRB2 gene was included for multivariate analysis. The primary endpoint of this prospective study was the time to delivery (hr). The secondary endpoint was ritodrine-induced ADEs. Results The ...

TY - JOUR. T1 - Effect of labetalol infusion on uterine and fetal hemodynamics and fetal cardiac function. AU - Jouppila, Pentti. AU - Rasanen, Juha. PY - 1993/10/15. Y1 - 1993/10/15. N2 - The effect of intravenously-administered labetalol (1 mg/kg) on uterine and fetal hemodynamics and fetal cardiac function was studied in 13 women with pregnancy-induced hypertension. Maternal mean blood pressure had decreased significantly by the end of the labetalol infusion and 30 min later. The pulsatility indices reflecting peripheral vascular resistance did not change in the main uterine, placental arcuate, umbilical, and fetal middle cerebral and renal arteries. nor did the parameters reflecting fetal cardiac function. A subgroup of seven patients with a more pronounced decrease in maternal blood pressure and three cases with original signs of fetal asphyxia did not react hemodynamically in a different manner from the group as a whole with respect to the circulatory effects of labetalol. These findings ...

AU - Hanson, Russell B.. AU - Low, Phillip A.. PY - 1997. Y1 - 1997. N2 - Our aims were to assess the role of adrenergic modulation in the hyperventilation-induced increase in colonic tone. Of 40 healthy volunteers, 12 received placebo (saline) and the remaining 28 received either clonidine, yohimbine, phenylephrine, or ritodrine. Time-frequency mapping of heart rate based on Wigner distribution assessed variations in parasympathetic and sympathetic activity during hyperventilation. Tone in the descending colon was recorded by a barostat balloon before, during, and after 5 min of hyperventilation. Heart rate spectral analysis suggested diminished sympathetic and vagal activity during hyperventilation and increased sympathetic and vagal activity after hyperventilation. Adrenergic agents influenced (P = 0.01) the tonic ...
The aim of the present study was to evaluate the efficacy and β2-adrenoceptor (AR) selectivity of KUR-1246, a new uterine relaxant. Inhibition of spontaneous or drug-induced uterine contractions by KUR-1246 was evaluated in pregnant rats and rabbits by an organ bath method or by a balloon method. The selectivity of KUR-1246 was assessed simultaneously in organs isolated from late-pregnant rats. The affinity of KUR-1246 for human β1-, β2-, and β3-ARs was determined using two radioligands. KUR-1246 suppressed both spontaneous and drug-induced contractions in isolated uteri, the rank order of potency being isoproterenol , KUR-1246 , terbutaline , ritodrine. ICI-118551 (selective β2-AR antagonist) competitively antagonized the KUR-1246-induced inhibition of spontaneous uterine contractions, but CGP-20712A (selective β1-AR antagonist) and SR-58894A (selective β3-AR antagonist) did not. All β-AR agonists tested produced significant inhibition of spontaneous uterine contractions in vivo: ED30 ...

According to the results presented here, when the participants were placed on the prototype stretcher in the PP, maternal HR, SBP, DBP, RR, SpO2, and baseline fetal HR were within normal limits, independent of the position adopted. All the women felt comfortable in all positions.. When a pregnant woman is positioned on a traditional stretcher, some factors must be considered. First, the chosen position can influence the maternal-fetal hemodynamic parameters, mainly after the 20th gestational week 14. Second, the selected position can cause discomfort for women whose abdomens are larger due to pregnancy.. These factors led us to build a prototype stretcher that allows pregnant women to remain in a number of varied positions comfortably and without changes to maternal or fetal hemodynamics. We tested this specially designed stretcher and found that all parameters remained within the limits of normality, even though all the study participants were at more than 20 weeks gestation.. There was a ...
Do not pass the signs of premature labor off as "normal" or "nothing to worry about." It is very important that you seek immediate medical attention if you think you may be going into labor early.. If the symptoms do not seem severe, but are moderate, call your prenatal caregiver. He or she will advise you on whether to come into the office or go to the hospital. If the symptoms are sudden and seem severe, stop what you are doing and go straight to the emergency room. It is advisable for you to drink a few glasses of water or fruit juice to ensure you are well hydrated prior to treatment. Do not substitute coffee, tea or soft drinks for water or juice.. When you arrive for treatment, the emergency room doctors or your prenatal caregiver will perform a physical examination of your cervix to see if it is starting to open up. This is the only way to tell for sure whether your symptoms are being caused by preterm labor, or if they are due to another condition. If it is determined that you are ...

Oxytocin analogues (pitocin, Syntocinon®) are used to induce labour and support labour in case of non-progression of the parturition. It has largely replaced ergotamine as the principal agent to increase uterine tone in acute postpartum haemorrhage. An oxytocin nose spray may be a useful adjunct to stimulate breastfeeding. Inhibition of oxytocin action is the mode of action of the new tocolytic agent atosiban (Tractocile®), which is registered in many countries to suppress premature labour between 24 and 33 weeks of gestation. It has fewer side-effects than drugs previously used for this purpose (ritodrine, salbutamol and terbutaline). A 2005 study by Fehr et al suggested that nasally administered oxytocin worked to generate trust in humans. They demonstrated that in a risky investment game, experimental subjects given the hormone displayed what the researchers deemed "the highest level of trust" twice as often as the control group who were given placebos. The same experiment with the subjects ...

Fetal cardiac interventions are currently performed for critical aortic stenosis (CAS), pulmonary atresia intact septum (PA/IVS) and closed atrial septum in hypoplastic left heart syndrome (HLHS). The rational is to change fetal hemodynamics, prevent secondary damage and improve long term outcome at an acceptable risk for mother and fetus.. In fetal CAS the left ventricle (LV) dilates, shortening decreases and endocardial fibroelastosis develops. The most important issue is to identify those patients, who will benefit from this procedure.. Intervention has been shown to improve fetal hemodynamics. The risk of intrauterine death is around 12%. A biventricular outcome has been reported in 1/3 to 2/3 of these fetuses.. At the Childrens Heart Centre Linz, 57 procedures were performed in 50 fetuses, with a success rate of 83% and a biventricular outcome after a median follow-up period of 43 months of 56%.. Intervention in fetal PAIVS is technically more challenging. In our centre 12 attempts have ...

As many as 50% of premenopausal women regularly suffer from menstrual pain. Many of these women may be incapacitated for one to three days during each menstrual cycle. Primary dysmenorrhoea (PD) is where women suffer from menstrual pain but lack any pathology in their pelvic anatomy. A wide range of treatments are available and some of these, such as beta2-adrenoceptor agonists, have been used to treat women with primary dysmenorrhoea but their effects are unclear. Five studies involving 187 females with an age range of 15 to 40 years were included in this review. Oral isoxsuprine was examined in two studies; terbutaline oral spray, ritodrine chloride and oral hydroxyphenyl-orciprenalin were compared with placebo in a further three studies. All of the studies were conducted over 30 years ago and none were of high quality. None of these medications, other than isoxsuprine combined with acetaminophen and caffeine, were reported to have any beneficial effect. Side effects with these medications ...
Thanks for your interest in my blog post! Substituting my talk of abortion for your talk of induced premature labour for maternal convenience helps clarify the issue. I think that Doms point about induced labour involving a medical procedure might go some way towards explaining why inducing premature labour (solely for maternal convenience) is illegal but formula-feeding is not. Since pregnant women cannot induce their own premature labour without medical assistance, the law that prohibits it controls the behaviour of pregnant women via controlling the conduct of the relevant medical professionals. (Presumably, if a doctor were to induce premature labour simply because the woman in question had requested it, the law would punish the doctor rather than the patient.) This law, then, is one step removed from - to borrow your term - the intimate and personal area of a womans pregnancy. On the other hand, a law enforcing breastfeeding would aim directly to control mothers behaviour, which is ...

The lack of β-adrenergic modulation in VEDS cardiomyocytes suggests that one or more components of the signaling cascade were missing or functionally inactive. To identify these components, we assayed the functional activity of different elements in the β-adrenergic enzymatic cascade. A key finding was that ICaL was strongly stimulated by cell dialysis with the catalytic subunit of PKA in both VEDS and LDS cardiomyocytes. In contrast to a previous report6 in which fast current rundown was observed using Ba2+ as a charge carrier, we could accurately measure the stimulating effect of the catalytic subunit of PKA on ICaL in VEDS cardiomyocytes. The catalytic subunit of PKA produced a maximal ICaL stimulation. Importantly, ICaL stimulation by the catalytic subunit of PKA was found to be similar (≈3 times) in VEDS, LDS, and ventricular cardiomyocytes isolated from the adult mouse heart. These data provide strong evidence for similar structural properties of the Ca2+ channel or a closely related ...

Preterm birth (PTB), defined as birth between 20 and 36 6/7 weeks, is responsible for the majority of the neonatal morbidity and mortality in the United States, and 35% of all U.S. healthcare spending on infants. Globally, about 28% of the 4 million annual neonatal deaths are directly attributable to PTB.. Preterm labor (PTL) is the final pathway for about 50% of all PTB. Tocolytic agents are drugs that can slow or stop labor contractions in the attempt to delay births preceded by PTL. Primary tocolysis is defined as tocolysis given on initial presentation of women with PTL. In most of these women, PTL stops, but as their risk of PTB remains high, some have advocated use of maintenance tocolysis, i.e. tocolysis after arrested PTL. So far, no maintenance tocolytic agent has been shown to be beneficial in preventing PTB.. The aim of this study is to evaluate the efficacy of maintenance tocolysis with Arabin pessary compared to standard care in singleton gestations with arrested PTL and with short ...
Dihydropyridine Ca2+ channel blockers (CCBs) are widely accepted in the treatment of premature labour. Their mechanism of action in tocolysis involves the blockade of L-type Ca2+ channels, influenced by the Ca2+-activated K+ channels, beta-adrenergic receptors (β-ARs) and sexual hormones. In clinical practice, most experience has been gained with the use of nifedipine, whose efficacy is superior or comparable to those of β-agonists and oxytocin antagonists. Additionally, it has a favourable adverse effect profile as compared with the majority of other tocolytics. The most frequent and well-tolerated side-effects of CCBs are tachycardia, headache and hypotension. In tocolytic therapy efforts are currently being made to find combinations of tocolytic agents that yield better therapeutic action. The available human and animal studies suggest that the combination of CCBs with β-AR agonists is beneficial, although such combinations can pose risk of pulmonary oedema in multiple pregnancies and maternal

Coxiella burnetii is an intracellular bacterial pathogen that causes Q fever. Infected pregnant goats are a major source of human infection. However, the tissue dissemination and excretion pathway of the pathogen in goats are still poorly understood. To better understand Q fever pathogenesis, we inoculated groups of pregnant goats via the intranasal route with a recent Dutch outbreak C. burnetii isolate. Tissue dissemination and excretion of the pathogen were followed for up to 95 days after parturition. Goats were successfully infected via the intranasal route. PCR and immunohistochemistry showed strong tropism of C. burnetii towards the placenta at two to four weeks after inoculation. Bacterial replication seemed to occur predominantly in the trophoblasts of the placenta and not in other organs of goats and kids. The amount of C. burnetii DNA in the organs of goats and kids increased towards parturition. After parturition it decreased to undetectable levels: after 81 days post-parturition in goats and

Read the original question here.. Hi Anonymous,. I resonate with you. While I will admit that I personally love babies and hope to be married and a mom one day - I, too, get irked by the overwhelming abundance of Catholic wife and mommy blogs and the unspoken yet pervasive sense that "mommy-hood" is what it means to be a fully realized Catholic woman. I have other passions, abilities, and callings in life too besides pushing out babies. Ultimately, what it means to be a holy Catholic woman today is to follow Christ to the best of my ability, strengthened by the grace of God. I want to live out my apostolate, my call to be Christs hands and feet in the world, now, today, each day. God has not yet allowed me to become a wife or a mother, and for all I know, that might never happen for me. So in the meantime, what does it mean to live out my vocation in my current state of life? While you say "Im not maternal", I would ask - what does the word "maternal" mean anyway? What does it mean to be a ...

The purpose of this study was to evaluate the fetal cardiovascular function during prolonged magnesium sulfate tocolysis. We performed a fetal ultrasonographic examination in 15 patients (Mg group) during magnesium sulfate tocolysis for the treatment of preterm labor. The maternal serum magnesium concentration was 5.7 ± 0.5 mg/dl at the time of the examination. Sixteen fetuses in normal pregnancies at similar gestational ages were used as the control group. The fetal heart rate and the middle cerebral artery pulsatility index in the Mg group were lower than in the control group (p , 0.01). Fractional shortening (FS) of the right ventricle in the Mg group was lower (p , 0.01), while FS of the left ventricle was higher (p , 0.01) than in the controls. The calculated blood flow through the tricuspid orifice in the Mg group was lower than in the control group (p , 0.01). In contrast, the blood flow through the mitral orifice in the Mg group was higher than in the control group (p , 0.01). In ...
According to the American College of Obstetrics and Gynecology, the safest time for a pregnant woman to travel is during the second trimester, when she usually feels best and is in least danger of spontaneous abortion or premature labor. A woman in the third trimester should be advised to defer overseas travel because of concerns about access to medical care in case of problems such as hypertension, phlebitis, or premature labor. Pregnant women should be advised to consult with their health care providers before making any travel decisions. Collaboration between travel health experts and obstetricians is helpful in weighing benefits and risks based on destination and recommended preventive and treatment measures. Table 8-05 lists relative contraindications to international travel during pregnancy. In general, pregnant women with serious underlying illnesses should be advised not to travel to developing countries ...
That is exactly what happened to young Annie Boulo, only 27 at the time she became ill. Two days after first contracting the flu she came down with "Broncho-Pneumonia (Septic)". While suffering the painful effects of the virus, Annie went in to premature labor sometime after 6 a.m. on the 18th. She delivered two boys, who soon died. On the death certificate of one son, it was written the baby lived less than 2 hours. Still fighting to live, Annie survived the births and deaths of her much anticipated babies. Unfortunately the next day Annie, too, died at the home she shared with her husband and 3-year-old son - 853 Dauphin Street. October 1918 was the deadliest month in U.S. history - during the month there were 195,000 fatalities from the flu. Annie was one of 11,000 Alabamians to die during the global pandemic ...
