Objectives: This study aimed to describe the epidemiology of ankylosing spondylitis (AS) in rheumatology practice at the beginning of the anti-TNF (tumour necrosis factor) era, and to evaluate the initiation of anti-TNF therapy in a clinical setting where prescription is regulated by the authoritys imposed reimbursement criteria.. Methods: Between February 2004 and February 2005, all Belgian rheumatologists in academic and non-academic outpatient settings were invited to register all AS patients who visited their practice. A random sample of these patients was further examined by an in-depth clinical profile. In a follow-up investigation, we recorded whether patients initiated anti-TNF therapy and compared this to their eligibility at baseline evaluation.. Results: 89 rheumatologists participated and registered 2141 patients; 1023 patients were clinically evaluated. These 847 fulfilled the New York modified criteria for definite AS and 176 for probable AS. The profile of AS in rheumatology ...

Pediatric Rheumatology is a medical subspecialty concerned with diagnosis and therapy of rheumatic diseases. Physicians specializing in rheumatology are rheumatologists who deal with clinical problems involving joints, soft tissues, autoimmune diseases, vasculitis, and heritable connective tissue disorders. Pediatric Rheumatology disease influences the joints, muscles and connective tissues of Child, Infant and Adolescent. Numerous rheumatic ailments are chronic conditions. A few, similar to osteoarthritis, are the consequence of wear and tear. Others, for example, rheumatoid arthritis, are immune system problems. Common Rheumatic disorders are Lyme disease, Systemic lupus Erythematosus, Behcets Disease, Polymyalgia Rheumatica, Rheumatic fever. ...

Perhaps a debate for another time, but nonetheless, there are front-runners out there and in addition to this article I would urge you to seek out TPMP - Rheumatology Webinar with Dr Karl Gaffney, Consultant Rheumatologist, and Jack March, Senior Physiotherapist in Rheumatology, for an in depth look at rheumatology masqueraders. For many rheumatology conditions there are considerable delays to diagnosis. These delays consequently affect accurate, timely treatment, subsequent intervention and, most importantly, impact the quality of life of the people living with these burdensome conditions. The British Society for Rheumatology (BSR, 2015) published a policy report outlining key challenges and opportunities facing rheumatology services in the United Kingdom. They highlighted the importance of early diagnosis and proposed a 12-week window of opportunity in which to positively affect patient care by referring appropriately to a rheumatologist within this time.. Moreover, a national clinical audit ...
A small number of children were thought to be clinically extremely vulnerable (CEV) due to the medications taken or the combination of illnesses they have. They were placed on the CEV list (or Shielding List), held by NHSE. The decision about who should be on this CEV list was made following national guidance early in the pandemic. You will have been notified if your child was on this list, and you were advised to take extra precautions during the first peak of the pandemic in England. This is known as shielding.. UK paediatric rheumatology consultants discussed the scientific evidence collected during the pandemic so far in the summer of 2020, following which the national advice changed. It was agreed that no child or young person needs to be on the clinically extremely vulnerable (CEV) list because of their rheumatology disease. This applies both to the old and the new variants of SARS Cov2. This was communicated prior to the return to school in September 2020. A few CYP remained on the ...

Objective: To ask rheumatologists about the likelihood of performing a formal joint count at each visit of a patient with rheumatoid arthritis (RA) in standard clinical care.. Method: Direct query of rheumatologists at an international meeting of about 600 rheumatologists from 17 European countries.. Results: Overall, 14% of rheumatologists reported performing a formal joint count at each visit of each patient, and 44% of rheumatologists reported performing a formal joint count at more than 50% of visits of patients with RA. Therefore, 56% of rheumatologists reported performing a joint count at fewer than 50% of visits, including 45% at fewer than 25% of visits. One in eight rheumatologists (13%) reported never performing a formal joint count.. Conclusion: Although the joint count remains the most specific measure for RA, most visits of most patients with RA to most rheumatologists do not include a formal quantitative joint count.. ...

A quick web search about rheumatology is not fruitful either. Rheumatology is defined as: the medical specialty that manages rheumatic diseases. My rheumatology textbook doesnt even try to define what field is all about.. In addition, were not a popular specialty. I dont know of one famous rheumatologist. Were not usually in the news. Were not the heroes in any movie. In fact, I dont think Ive ever seen a TV character who is a rheumatologist (even though there is at least one paleontologist on TV!). The TV character that most closely approximates what a rheumatologist does is my hero Dr. House (no relation, unfortunately) on the TV show House, MD. He is actually board certified in nephrology and infectious diseases, but he gets consulted on very complicated cases, much like rheumatologists often do. However, hed make a lousy rheumatologist because, according to him, its never lupus!. As I see it, rheumatology is the study of inflammation (swelling, redness, warmth, and pain) ...

In a publicly funded universal healthcare system, we studied trends in encounters with rheumatologists over the past decade and observed increasing rates of access to rheumatologists within 6 and 12 months after diagnosis by a family physician. However, no such improvements were observed among patients seen within 3 months, a more favourable benchmark. We also explored whether receipt of rheumatology care was associated with patient and family physician characteristics, and measures of rheumatology supply. We found that patients of higher SES were more likely to receive timely rheumatology care, which has also been demonstrated in other Canadian provinces.29 ,30 Further, proximity to and density of rheumatologists were important determinants of timely rheumatology care.. While our results appear encouraging, 41% of patients are still not seen within 3 months of a primary care diagnosis as recommended by current guidelines. Thus, an important proportion of patients are not receiving optimal care. ...
Background/Purpose: American rheumatology practice patterns vary considerably. One challenge is practitioner shortage, thus a patient-self-administered screening tool would fill an unmet need if it identified patients with the highest likelihood of having an inflammatory disease (ID). Objective: To assess the appropriateness of referral patterns and concurrently to evaluate a previously published screening tool, Early Inflammatory Arthritis-3 (EIA-3)1. Methods: The US rheumatology database, JointMan® captures inflammatory/non-inflammatory diagnoses of referred patients at a large Washington State rheumatology practice. All patients at the practice fill in an electronic version of the EIA-3 tool. This published screening questionnaire estimates the likelihood of having an inflammatory disease [JIA, RA-fulfilling ACR criteria, Unspecified inflammatory arthritis, SpA, CTDs, SLE] vs non-inflammatory disease [OA, FM, CPPD]. The evaluating clinician was not aware of the EIA-3 results. The results ...

When our patients do not look like us, sound like us, speak the same language, or share a similar life experience, does it affect the way we care for them? While we might like to think it does not, a growing body of evidence suggests that it can. We all make unconscious, split-second decisions about the people we encounter throughout the day. As healthcare professionals, we are not immune to the effects of bias. Indeed, we may be more susceptible because we often operate under tight time constraints, with limited information, and under a high degree of stress and fatigue.. In a typical clinical setting, many patients require our attention, each with their own unique personal and medical story. Combine a diverse patient population with a wide variety of needs with a busy physician who is constrained by time and a wide variety of inadequate resources, and you have a challenging situation. This can be magnified for us as rheumatologists, dealing with patients with complex medical conditions-like ...
Patient-centred care is widely advocated in rheumatology. This involves collaboration among patients, caregivers and health professionals and is particularly important in chronic rheumatic conditions because the disease and treatment can impair patients health and wellbeing. Qualitative research can systematically generate insights about peoples experiences, beliefs and attitudes which patients may not always express in clinical settings. These insights can address complex and challenging areas in rheumatology such as treatment adherence and transition to adult healthcare services. Despite this, qualitative research comprises one per cent of studies published in top-tier rheumatology journals. A better understanding about the impact and role, methods and rigor of qualitative research is needed. This overview highlights recent contributions of qualitative research in rheumatology, summarizes common approaches and methods used, and outlines key principles to guide appraisal of qualitative studies ...
In a QI program and study on rheumatoid arthritis (RA), PRIME® assessed the impact of QI-focused education on community-based rheumatologists performance on national quality measures for RA, along with patient-centered measures aligned with National Quality Strategy priorities. Clinical performance was assessed through baseline and post-education chart audits. As reported in the peer-reviewed journal Rheumatology and Therapy, the study demonstrated a significant impact of QI-focused education in improving the rheumatologists performance on clinical and patient-centered RA quality measures. Applying PRIMEs Framework for Promoting and Evaluating Sustainability and Scalability, we incorporated benchmarking data from this QI study into a scale-up educational program, comprising a national series of RA webinars for interprofessional rheumatology clinicians. The scale-up program outcomes demonstrated education-associated gains in the clinicians ability to deliver evidence-aligned RA care, and to ...

Our data demonstrates that rheumatology publications from the AL countries still lag behind other world regions. Several factors may influence the underrepresentation of rheumatology research in the region. These may include bias from journals toward publishing articles from their country of origin in favor of articles of local interest, WoS may not include journals from middle to low-income countries [12] or articles may be published in local Arab journals that are not indexed on WoS [13, 14]. In addition, the productivity of a country may be skewed based on its number of indexed journals, which are relatively few in the Arab world [15, 16]. Within the region, however, and over the period of 1976 to 2014, we found a consistent and significant growth in the number of publications. This is not unexpected as the world overall is seeing an increase in rheumatology publications, particularly from emerging economies [4, 17]. The Arab world in particular, has developed significantly over the past ...

Referred patients with FM symptoms (n = 193) were randomized to a study group diagnosed by a specialized rheumatology nurse (SRN group, n = 97) or to a control group diagnosed by a rheumatologist (RMT group, n = 96). SRN patients were seen within 3 weeks by a nurse who took structured history and initiated routine laboratory tests. During a 5-minute supervision session, the rheumatologist was informed by the nurse about medical history, performed a brief physical examination, and confirmed or rejected the nurses diagnosis. RMT patients were seen by a rheumatologist after a regular waiting period of 3 months. Outcome measures were initial agreement between the nurse and rheumatologist in the SRN group, final diagnosis after 12-24 months of followup, patient satisfaction, and diagnostic costs. ...
ORTHOPAEDIC/RHEUMATOLOGY:-Derived from the Greek words orthos (correct, straight) and paidion (child), when he published Orthopedie (translated as Orthopedia: or the Art of Correcting and Preventing Deformities in Children[1]) in 1741. One of the fastest growing diseases in the world after diabetes & Blood pressure is orthopedic diseases.. Rheumatology:devoted to diagnosis and therapy of rheumatic diseases. Background and objectives High-normal thyroid stimulating hormone (TSH) (2.6-4.5 μIU/ml) is associated with metabolic syndrome (MetS) in population studies. We hypothesized that euthyriod polycystic ovary syndrome (PCOS) with TSH of higher than 2.5 had altered anthropometric, metabolic, and endocrine parameters as well as higher percentage of MetS compared with those with lower TSH levels. Patients and methods The present study included 60 young euthyroid PCOS women without any thyroid risk factors and 60 age-matched and BMI-matched healthy, fertile women. Anthropometric measurements were obtained, biochemical and hormonal assay were evaluated, and the homeostatic model assessment-insulin resistance was calculated. PCOS women were divided into high-normal TSH (group 1) and low-normal TSH (group 2) groups. MetS was defined according to National Cholesterol Education Program/Adult Treatment Panel III. Results Group 1 had significantly higher waist circumference, systolic blood pressure and ...
