Retropharyngeal abscess - Wikipedia
Retropharyngeal abscess (RPA) is an abscess located in the tissues in the back of the throat behind the posterior pharyngeal wall (the retropharyngeal space). Because RPAs typically occur in deep tissue, they are difficult to diagnose by physical examination alone. RPA is a relatively uncommon illness, and therefore may not receive early diagnosis in children presenting with stiff neck, malaise, difficulty swallowing, or other symptoms listed below. Early diagnosis is key, while a delay in diagnosis and treatment may lead to death. Parapharyngeal space communicates with retropharyngeal space and an infection of retropharyngeal space can pass down behind the esophagus into the mediastinum. RPAs can also occur in adults of any age. RPA can lead to airway obstruction or sepsis - both life-threatening emergencies. Fatalities normally occur from patients not receiving treatment immediately and suffocating prior to knowing that anything serious was wrong. Signs and symptoms may include stiff neck ...

Endoscopic Transection for Intramural Esophageal Dissection Caused by a Retropharyngeal Abscess
Question: A 66-year-old man was admitted with severe pharyngeal pain, dysphagia and dyspnea. Laryngoscopy revealed severe laryngeal edema (Fig. 1A), and neck CT showed a non-collecting retropharyngeal abscess (Fig. 1B). He underwent tracheostomy, and despite antibiotic therapy, his fever and chills persisted with worsening dysphagia. Endoscopy showed diffuse swelling between the upper esophagus and the gastric cardia and an upper esophageal opening discharging pus (Fig. 2A, B). EUS revealed that the swelling of the esophagus and the gastric cardia was caused by submucosal pus accumulation (Fig. 2C), and esophagography showed esophageal dissection between the upper and lower esophagus with a small amount of leakage from the false lumen of the lower esophagus (Fig. 2D ...

Retropharyngeal abscess - Psychiatry Advisor
Deep neck infections can progress rapidly, and patients usually require hospitalization and close observation for complications. Treatment of RPA depends on severity of presentation.. Patients with airway compromise need immediate intubation by an experienced physician. Parenteral antibiotics should be started once IV access is secured, along with otolaryngology consultation for surgical drainage. Parenteral hydration and analgesia are important adjuncts to initial care.. Patients without airway compromise should be evaluated by an otolaryngologist, started on parenteral antibiotics, hydrated, and treated adequately for pain.. Initial parenteral antibiotics should provide treatment for common pathogens causing RPA, including group A Streptococcus, Staphylococcus aureus and respiratory anaerobes. Empiric therapy can then be modified based on perioperative pus culture (aerobic and anaerobic) and Gram stain results. In general, RPAs are polymicrobial infections, and the isolation of respiratory ...

Two compartments: Medial and lateral retropharyngeal. Medial retropharyngeal receives drainage from nasal pharynx and oral pharynx and drains to high internal jugular chain. Lateral retropharyngeal (nodes of Rouvierre) are located just medial to the internal carotid artery. Drainage is the same as for the medial retropharyngeal nodes. Early sign of nasal pharyngeal cancer may be presence of abnormal lateral retropharyngeal node in the patient over forty. ...

Pains in the Neck
Things often come in threes, but not usually all in a row. You just had an elderly patient with neck pain from an odontoid fracture sustained from a fall, followed by a 28-year-old with neck and throat pain that was due to a retropharyngeal abscess. Your next patient also has neck pain and since your hospital is full and your ED is boarding 14 inpatients currently, you are seriously hoping that this is going to be torticollis or a cervical strain that you can eventually send home.. Your high hopes start to drift downward almost immediately. She is a 50-year-old female who is on extended home IV antibiotics for tarsal osteomyelitis via a recently placed PICC line. Retropharyngeal course of the internal carotid artery | Journal of the Royal Army Medical Corps
A 77-year-old male patient presented with dysphonia. Endoscopic examination demonstrated a distortion of the posterior pharyngeal wall, which was pulsatile. The axial CT showed left internal carotid artery in a retropharyngeal location. An anomalous course of the carotid artery in the retropharyngeal space is an unusual finding that poses a risk of vascular injury during pharyngeal surgery and intubation. Such an anomaly may be congenital (incomplete descent of the third aortic arch) and may be more pronounced in older patients secondary to atherosclerosis and hypertension as occurred in our case noted here. ...

The second stripe has fat attenuation and represents the areolar and adipose connective tissues of the retropharyngeal and retroesophageal spaces.23 The retropharyngeal space is technically considered to extend from the base of the skull to the origin of the esophagus, where it continues inferiorly as the retroesophageal space. These 2 spaces are enclosed by the middle (visceral division) and deep (alar division) layers of the deep cervical fascia and contain a variable amount of lymph nodes superiorly.24 The fat stripe normally lies in close proximity to the anterior surface of the vertebral bodies and displacement can help detect and localize injuries to cervical spine.23 Variation in the thickness and conspicuity of this fat plane can be seen among individuals, but it is always present.25 This could be explained by differences in a patients body habitus, because patient weight has been shown to relate proportionally to the thickness of the PVST on radiographs.26,27 In some patients, this fat ...

What Is Torticollis?
Torticollis, also called wry neck or loxia, represents a category of neck conditions which exhibit a twisting position of the head and neck outside of a normal neutral position. There are many potential causes for torticollis from sleeping in a faulty position to an injury to the neck, like whiplash. However, its commonly labeled as idiopathic, which basically means, We dont know what caused it. Torticollis often occurs out of the blue with no obvious cause or history such as, When I woke up, I couldnt turn or move my head! It can occur at any age and may disappear on its own in a few days to weeks, but in rare cases, it can persist for months or years. Because it can be quite alarming, most people quickly seek healthcare services, which can include visiting a doctor of chiropractic. Once the more serious causes of torticollis are ruled out (such as a severe type of infection like a retropharyngeal abscess or bacterial meningitis, fracture, neoplasm, cervical dystonia, etc.), ...

Airway obstruction - Wikipedia
Lower airway obstruction is mainly caused by increased resistance in the bronchioles (usually from a decreased radius of the bronchioles) that reduces the amount of air inhaled in each breath and the oxygen that reaches the pulmonary arteries. It is different from airway restriction (which prevents air from diffusing into the pulmonary arteries because of some kind of blockage in the lungs). Diseases that cause lower airway obstruction are termed obstructive lung diseases. Lower airway obstruction can be measured using spirometry. A decreased FEV1/FVC ratio (versus the normal of about 80%) is indicative of an airway obstruction, as the normal amount of air can no longer be exhaled in the first second of expiration. An airway restriction would not produce a reduced FEV1/FVC ratio, but would reduce the vital capacity. The ventilation is therefore affected leading to a ventilation perfusion mismatch and hypoxia. ...
Continuous venovenous hemodiafiltration using cytokine-adsorbing hemofilters as adjuvant therapy for anaerobic descending...
We describe a combinatorial intensive care approach and discuss the critical factors that allowed us to successfully manage a life-threatening case of acute anaerobic septic shock triggered by descending necrotizing mediastinitis. We admitted a 38-year-old critically ill Kosovar Albanian man to our intensive care unit because of clinical manifestations of severe sepsis. His condition had worsened in the previous 2 weeks following unsuccessful antibiotic therapy for tonsillitis complicated by retropharyngeal abscesses. Computed tomography and intraoperative observations identified abscesses in the anterior and middle mediastinum regions and the distal part of the neck, directly on the border with the left lobe of the thyroid gland. Cultures indicated infections with α-hemolytic Streptococcus and Clostridium species: High procalcitonin and lactate levels, blood gas analysis, poor peripheral capillary oxygen saturation, and severe hemodynamic instability pointed to a case of acute septic shock. Lateral retropharyngeal lymph node metastasis from squamous cell carcinoma of the upper gingiva: A case report
Lateral retropharyngeal lymph node (LRPLN) is located between the internal carotid artery and the prevertebral muscles. Metastasis to the LRPLN is frequent in nasopharyngeal cancer, but is rare in oral cancer. The prognosis of patients with oral cancer with LRPLN metastasis is usually poor. The present study reported a patient with LRPLN metastasis from squamous cell carcinoma of the upper gingiva, who survived for >7 years. Docetaxel, cisplatin and fluorouracil (TPF) therapy was performed as induction chemotherapy and it was planned to subsequently conduct chemoradiotherapy or surgery. As the tumor only exhibited a transient response to TPF, surgery was selected. Postoperatively, only radiotherapy was performed and a favorable outcome was achieved ...
