The goal of this study was to determine the safety and efficacy of retroperitoneoscopic approach used to remove iliopsoas hematoma. In this study we performed a retrospective analysis of patients treated with a transabdominal laparoscopic neurectomy for chronic pain after operations of the groin or right lower abdomen. Several therapeutic approaches have been recommended in order to treat chronic postoperative groin pain. To the best of our knowledge, none of the existing studies has evaluated the quality of life of those patients.. Background: Spontaneous perinephric hematoma with no associated pathology or provocation is a rare clinical phenomenon. The hematoma requires a two-year interval for a favorable hematoma resolution, and no associated hypertension or renal scarring. Aims: Evidence of the efficacy of conservative management for spontaneous perinephric hematoma with a 2-year follow up. Case Presentation: A previously healthy 38-year-old woman, presented with a sudden onset of left flank pain, associated with fatigue and pallor. The patient remained hemodynamically stable with no significant history or associated provocations identified. Conclusion: The acute management strategy is favorable in such condition, as the hematoma remains under tamponade in the retroperitoneal space, regardless of the size and organ displacement. Primary melanoma of the adrenal gland is exceptionally rare as demonstrated by the few cases reported in the medical literature, and it has a high fatality rate. We present the case of a patient with two relapses and survival to date. We describe the case of a 58-year-old Caucasian woman who consulted her doctor with symptoms of asthenia, anorexia and weight loss. A mass was palpated in her abdomen at the height of the left hypochondrium. A computed tomographic scan revealed a retroperitoneal mass measuring 10 cm × 15 cm originating in the left adrenal gland. A left nephroadrenalectomy and splenectomy were performed. Histopathologically, the retroperitoneal mass corresponded to a melanoma, and no primary melanoma was found in any other location. Primary and recurrent retroperitoneal tumors can involve the aortoiliac vasculature. They are often considered inoperable or incurable because of the locally advanced nature of the disease or the technical aspects involved in safely resecting the lesion. Safe resection of these lesions requires experience and extensive preoperative planning for success. Extra uterine deciduosis (EUD) is an histological benign condition which can mistaken macrospicaly as malignancy resembling peritoneal carcinomatosis. EUD can be discovered accidentally in pregnancy during caesarean sections but as well, during pelvic surgery in women under oral contraception (Tang et al. 1985) or for cervical cancer (Cobb 1988). Forth-one-year-old male admitted to emergency department with the complaint of abdominal pain. Wall of abdominal aorta seemed thickened on ultrasonography (USG). Abdominal computerized tomographic angiography showed soft tissues surrounding aorta, beginning 9 cmproximal to iliac bifurcation and continuing up to the level of common iliac artery (aortitis? retroperitoneal fibrosis?), and the patient was referred to rheumatology clinic. The patient was hospitalized for further investigations. He did not complain of fever, fatigue, or urinary symptoms. The blood tests revealed the following: Erythrocyte sedimentation rate (ESR) 57 mm/h, CRP 43 mg/L, HGB 14.1 g/dL, WBC count 9800/μL, PLT count 306000/μL, MCV 82.3 fL, creatinine 1.15 mg/dL and ALT 18 U/L. After recovering from such a challenging surgery, my patients often ask me, Am I cured now? I can only imagine the emotional roller coaster they must experience when, knowing what we went through and seeing the remarkably long abdominal scar, we necessarily discuss the fact that even with the best surgery at the best specialty centers, these cancers have a notorious reputation for coming back. Radiation and chemotherapy sometimes lower this risk, but given the rarity of this disease, we currently do not have enough information to know if these nonsurgical treatments are truly effective. Ultimately, when, not if - for most patients, unfortunately - the beast returns, and if it has not spread, we are back to surgery, with each subsequent operation harder and riskier than the last.
