Title: Endodontic retreatment of maxillary incisors, Author: Miguel Pizango, Name: Endodontic retreatment of maxillary incisors, Length: 5 pages, Page: 1, Published: 2011-04-21
Sometimes, a root canal doesnt fully heal and may become infected post-treatment. Endodontic retreatment picks up where prior procedures left off.
An endodontic retreatment is performed after an initial procedure such as a root canal to address any underlying issues that remain in the tooth.

In this prospective cohort study of previously treated pulmonary TB patients managed under programmatic conditions in Uganda, we found that 70%-80% of patients with and without HIV coinfection had a successful treatment outcome at the conclusion of antituberculous therapy (i.e., cured and completed treatment). Although this overall response compares well with many other studies, we found that the retreatment regimen has unacceptably low treatment response rates in certain subgroups of patients and is associated with poor long-term outcomes, particularly in MDR-TB and in HIV-infected patients. Initiation of treatment with ART in eligible patients mitigated much of the excess mortality associated with HIV infection alone, in agreement with a recent clinical trial showing that use of ART concurrently with antituberculous therapy in TB patients is safe and improves survival [34]. HIV-infected patients with MDR-TB had the highest death rate, but we could not ascertain if death was a consequence of ...
The best single predictor for patients who needed retreatment was aneurysmal anatomy (Table 2). Our results imply that it can be expected that a small aneurysm with a narrow neck will need no retreatment in about 97% of cases. From a practical standpoint, these aneurysms are definitely treated with conventional coil techniques. In turn, to show the benefit of bioactive coils for the clinically relevant end point rate of retreatment in these patients (who represent almost two thirds of our group), a very large study would be needed.. The risk of retreatment in the remaining patients with a neck width of ,4 mm or an aneurysm size of ,10 mm is probably increased about threefold. In contrast, Raymond et al14 found recurrences not significantly increased when the neck size was ,4 mm. One possible explanation is that recurrences in small aneurysms actually do occur rarely but are treated in an even smaller portion (Table 1). This observation might be explained, in part, by the fact that small ...
Among 676 patients enrolled in this phase III trial, 40 (6%) were retreated with their original treatment regimen following disease progression. Our analysis has some limitations, most obviously the small patient numbers which precluded a rigorous statistical analysis. Only 19 of 32 evaluable patients had their response status confirmed in the study database (Table 1), but it is likely that the investigators clinical judgment was accurate in most cases. Although the sample size is small, durable ORs and/or SD were achieved in some patients retreated with ipilimumab, with additional restoration of disease control with a second or, in the case of one patient, third retreatment schedule.. Most patients tolerated one full cycle of ipilimumab retreatment, and several tolerated a full second cycle of retreatment. The frequencies of irAEs observed during retreatment were similar to those observed during treatment, with no new types of toxicities, and most events were mild-to-moderate. Although patient ...
When patients present to the dental clinic for retreatment, there is little room for error. Cases requiring retreatment may have already resulted in complications, failures, and even iatrogenic damage. It is therefore in the best interest of patients and for the discipline that clinicians share protocols, techniques, and innovations used in retreatment to improve the quality of care and avoid further error. The clinical cases depicted in this DVD compendium all lay bare two things: first the medical mistake and then the innovative fix that allowed for a successful outcome at retreatment ...
Endodontic retreatment has many advantages, the most significant one being retaining your natural tooth. The investment you make in saving your tooth, rather than going for an extraction, may result in a healthy tooth that can remain functional for years.. In addition, advances in technology mean that new techniques in the treatment of root canals may be available, which might not have existed when the first procedure was done to your tooth. Endodontic retreatment also allows for the remedial of problems faced with complicated root canal anatomy, which might not have been treated adequately during the first procedure.. The cost of endodontic retreatment depends on the degree of complication of the procedure. Since it involves the removal of your restoration and filling material, as well as examination for unusual canal anatomy, the procedure will be more complicated than the initial root canal treatment. Consequently, retreatment generally costs more than the initial treatment. On the other ...
What does root canal retreatment involve?. On the day of the retreatment procedure a local anesthetic will be administered, unless another type of anesthetic has been selected. The affected tooth is isolated with a rubber dam. The dam protects the tooth during treatment from bacteria and saliva. The amount the dentist can do within a single appointment will much depend on the amount of inflammation present, and the complexity of the treatment.. The first step in a root canal retreatment is to gain access to the inner tooth. If a crown and post have been placed, these will be removed.. Next, filling material and obstructions that block the root canals will be removed. This removal is conducted using an ultrasonic handpiece. The advantage of using this tool is that any unwanted material is vibrated loose. Tiny instruments will then be used to clean and reshape the root canals. X-rays may be taken to ensure that the roots are thoroughly clean. If this part of the treatment proves to be complex, ...

This section provides guidance on the retreatment of persons with chronic HCV infection in whom prior therapy failed. The level of the evidence available to inform the best regimen for each patient and the strength of the recommendation vary, and are rated accordingly (see Methods Table 2). In addition, specific recommendations are given when treatment differs for a particular group (eg, those infected with different viral genotypes). Recommended regimens are those that are favored for most patients in that group, based on optimal efficacy, favorable tolerability and toxicity profiles, complexity, and treatment duration. Alternative regimens are those that are effective but, relative to recommended regimens, have potential disadvantages, limitations for use in certain patient populations, or less supporting data. In certain situations, an alternative regimen may be optimal for a specific patient. Specific considerations for pediatric patients and persons with HIV/HCV coinfection, decompensated ...

PURPOSE: The therapeutic ratio of reirradiation for recurrent or second primary (RSP) squamous carcinoma of the head and neck may be improved in the intensity modulated radiation therapy (IMRT) era. However, patient selection for reirradiation remains challenging. We performed a multi-institution cohort study to investigate modern outcomes after IMRT-based reirradiation and to identify prognostic subgroups. PATIENTS AND METHODS: Patients with RSP squamous carcinoma originating in a previously irradiated field (≥40 Gy) who underwent reirradiation with IMRT (≥40 Gy re-IMRT) were included. Locoregional failure and late toxicity were calculated using the Gray competing risk method. Cox proportional hazards regression was used to identify factors associated with overall survival (OS). Factors associated with OS were entered into a recursive partitioning analysis (RPA) for OS. RESULTS: From 7 institutions, 412 patients were included. The median dose of re-IMRT was 60 Gy, and the median time between RT
This study demonstrates that retreatment with durvalumab can be safe and effective in patients who stop treatment and progress while off therapy. Of the 70 retreated patients, more than 70% experienced clinical benefit (11.4% with PR and 60.0% with SD). This suggests that retreatment restored antitumor activity in a majority of patients, resulting in high rates of durable disease control with an acceptable safety profile. Despite its retrospective design, the study had multiple strengths, including the analysis of the largest retreatment cohort in this setting, the inclusion of diverse tumor types, and treatment homogeneity.. There are no established guidelines for optimal duration of initial therapy with ICIs. In clinical practice and in many research protocols, patients are treated through the first evidence of disease progression to account for tumor flare (pseudoprogression) and to provide adequate time for ICI activity to become apparent, which may take longer than with conventional ...

Sometimes a root canal treatment does not heal well the first time it is performed or the tooth becomes re-infected. This is typically due to complex root anatomy or a missed extra canal. In many cases, endodontic retreatment can be performed where the inside of each canal is cleaned or cleaned again for a second time. The prognosis will depend on the condition of the tooth prior to endodontic retreatment. We evaluate each and every tooth and develop a special treatment plan for your diagnosis. After endodontic retreatment is completed, a record of the procedure will be sent to your general dentist. You will then return to your general dentist for the final restoration or crown ...

|i|Background and Aim|/i|. The development of interferon- (IFN-) free regimens substantially improved efficacy of treatment for HCV, but despite excellent effectiveness the failures still occur. The aim of our study was to evaluate the efficacy of retreatment with genotype specific direct acting antivirals- (DAA-) based regimens in nonresponders to previous IFN-free therapy.|i| Materials and Methods|/i|. Analysed population consisted of 31 nonresponders to IFN-free regimen, which received second IFN-free rescue therapy, selected from 6228 patients included in a national database EpiTer-2.|i| Results|/i|. Age and gender distribution were similar, whereas proportion of genotype 1b was slightly higher and genotype 4 lower in the whole population compared to studied one. Patients included in the study demonstrated much more advanced fibrosis. Primary therapy was discontinued in 12 patients, which were recognized as failures due to nonvirologic reason, whereas virologic reason of therapeutic failure was
Results 60 patients (60% males), with a mean age of 68 ± 11 years and a mean disease duration of 11 ± 9 years were included. 69% had polyostotic disease and a mean percentage of skeletal involvement of 10.8 ± 7.6%. 68% were symptomatic: 71% of those referring bone and 54% joint pain attributed to PD. 48.3% had been previously treated with pamidronate (cumulative dose 234 ± 209 mg). The mean follow-up period after zoledronate infusion was of 37 ± 13 months. Only 4 patients (6.6%) required retreatment, on average 30 months after the first zoledronate infusion. A marked reduction of ALP) was observed at 3 (70%) and 6 months (74%) being maximal at 12 months (75%) (p , 0.001). At 3 and 6 months, 95% and 96% of patients respectively, achieved remission. Maximum effect was obtained at 12 months after treatment with 98% of patients being in remission. Significant reductions of the mean levels of BSALP, P1NP, and b-CTX (p , 0.001) were also verified at 3, 6 and 12 months after treatment. 47% of ...
Introduction: The aim of this study was to evaluate the frequencies of nonsurgical retreatment, root -end surgery, extraction, and further restoration of root -filled teeth in Sweden in 2009 during a follow-up period of 5 years and to compare the outcomes in teeth restored with direct or indirect restorations. Methods: Data from the Swedish Social Insurance Agency were analyzed, and the frequencies of nonsurgical retreatment, root end surgery, extractions, and further restoration were calculated for all teeth registered as root filled during 2009. Chi-square tests were applied to detect any significant differences in the frequency of further treatment in teeth registered as restored with either a direct or an indirect restoration within 6 months of root filling. Results: Of the 248,299 teeth reported root filled in Sweden in 2009, nonsurgical retreatment was registered in 2.2%, root -end surgery in 1.0%, and extractions in 9.2% during the follow-up period. Of the teeth restored with a direct ...
Hair rebonding allows breakage of hair bonds, rearranging those bonds and restructures it once again to the desired, straight, state. Using a flat iron, the hair bonds are restructured and then applied with a neutralizing agent that will preserve the newly structured hair bonds.. The tool usedis a flat-iron to flatten/straighten the hair and relatively takes longer than relaxing.. The effect relatively takes longer than relaxing but is more lasting than relaxing and the results of hair rebonding will be a pin-straight, flat-out stick straight hair.. If you like the sleek, super straight look, then this option will be suitable for you. But to me, after many years (and many times!) of rebonding my hair, I find that it looks rather unnatural and closer to a broomstick effect than a healthy straight hair effect. ...
58 patients with CRVO were randomized 1:1 to receive either a sham L-CRA plus intravitreal ranibizumab 0.5mg (29 patients) or L-CRA plus ranibizumab 0.5mg (29 patients). Selection criteria and the retreatment protocol were based on the CRUISE study of ranibizumab for CRVO. The L-CRA/sham was attempted at 2 sites at baseline above, and below the optic disc. Monthly injections of ranibizumab in both groups were commenced at month 1 and continued as per the CRUISE study for the next 6 months. From month 7-24 both groups received monthly prn ranibizumab as required according to the retreatment protocol. The 24 month analysis was conducted following the intent-to-treat principle on the full analysis set, with the last observation carried forward for missing data. Outcome measures included change from baseline BCVA and CFT, both measured by masked assessors and also the number of injections required in the prn phase according to predetermined reinjection criteria ...

They saw some pretty dramatic intracranial responses, Dr. Yushak reported. For the 76 patients who were asymptomatic, of good performance status, and without prior local therapy, the intracranial response rate was 58%, median progression-free survival rates were 44% and 19%, respectively. The lowest response rate-44%-was observed in patients with V600D, K, and R mutations, but still, 44% is impressive, she noted. This study is for patients that have a tumor that cannot be removed through surgery (unresectable) or may have spread (metastasized) to other parts of their body. In this study, ASP1951 will be given alone (monotherapy) or combined with pembrolizumab (combination therapy). Both ASP1951 and pembrolizumab will be considered study drug. Pembrolizumab has been approved by FDA for use in patients with skin, lung, cervical cancer, certain cancers of head and neck and other cancers but may not be approved to treat all types of cancer. However, the use of ASP1951 plus pembrolizumab has not been approved by regulatory authorities and is therefore investigational. The study consists of 3 periods: screening (up to 28 days), treatment (up to 48 weeks [16 cycles]) and follow up (up to 45 weeks), followed by an optional re-treatment period for participants that qualify. The re-treatment period will allow participants to receive study drug treatment again for up to 16 additional cycles (approximately 48 ...
