DR MCQUAID: So the objectives of my talk today are hopefully that when were done here you will, first of all, be able to describe some ways in which to gain a childs trust, so that you can actually make a vision assessment, and perform some retinoscopy or refraction. Id like you to be able to name a few conditions which can be revealed by your first impression of seeing the child, and some very simple binocular assessments. You should be able to explain when were done here why retinoscopy is such an important, vital skill to have when it comes to dealing with children. And then you should also be able to mention a couple of ways to manage pediatric refractive error. So now the thing is - is Im not going to stand up here and teach you how to did retinoscopy or how to do refraction on a child or anybody else. What Im gonna do here is just give you some insight and some quick non-confrontational-type tests, and that sort of thing. Im going to assume that many, if not most of you, at least ...

Retinoscopy (Ret) is a technique to obtain an objective measurement of the refractive condition of a patients eyes. The examiner uses a retinoscope to shine light into the patients eye and observes the reflection (reflex) off the patients retina. While
Ive attempted for several years to learn retinoscopy. Ive even had a workshop where I thought I finally understood the with and against, however that and no other means of learning ever stuck. It felt like learning calculus … having to memorize complex formulas to apply every step of the way. This is the first explanation/demonstration that flipped the switch on … and it be brightly lit rather than a flickering dimmer bulb that even moths werent attracted to. It finally makes complete sense. Sense enough that Im wondering why it was so hard for me to get before. Thanks so much for putting this together! Im taking my COT in 5 days and this was the last thing for me to learn … the only thing that has been holding me back for years!. ...
Pavagada is one the most backward and underserved areas in Karnataka state from the health, economic and educational stand points. This region along with surrounding taluks in Tumkur district of Karnataka is devoid of eye care. The study has been launched with an aim of finding the prevalence of eye diseases in children below 15 years by screening and then treating eye conditions in the rural children using a house to house survey model. Till date approximately 30,000 children have been screened for eye diseases.. Shree Sharada Devi Eye Hospital & Research Centre, Pavagada, along with Narayana Nethralaya & Narayana Hrudalaya, Bangalore has taken up an epidemiological study, The Pavagada Pediatric Eye Disease Study. Pavagada & Madhugiri are taluks in Tumkur district of Karnataka state. The population of the 2 taluks is about 5 lakhs. 25 to 30% of the population is below 15yrs of age. It is one of the most backward and underserved areas from health, economic and educational standpoints. Due to ...
The Retinomax is a hand held instrument designed to provide a rapid estimate of refractive error. The portability and ease of use of the Retinomax suggest that it might be a useful tool for providing definitive gold standard measurements of refractive error under cycloplegic conditions for use in research studies, and may be useful for screening young children for high refractive errors under non-cycloplegic conditions.. We were able to achieve our goal of obtaining Retinomax measurements from all but two subjects between 1 and 8 years of age under cycloplegic conditions. Younger subjects tended to turn away from the Retinomax, as they do with any instrument that is placed close to the face. However, the ophthalmic technician was successful in obtaining Retinomax measurements in seven of nine children less than 3 years old. Retinoscopy was successfully completed in all children, and subjective refinement of the initial retinoscopy reading was successfully completed in 22 children.. The mean ...
Abstract. Objectives: To provide a description of refractive errors in healthy, term-born children, aged 1 through 24 months, and to test the hypothesis that spherical equivalent becomes significantly less hyperopic and less variable with increasing age. Methods: This is a prospective, cross-sectional design, cycloplegic retinoscopy was used to measure the refractive error in both eyes of 100 healthy, term- born children in four age groups. Spherical equivalent, cylindrical power and axis were analyzed as a function of age. Results: Spherical equivalents of right and left eyes did not differ at any age. Hyperopia declined significantly with increasing age. The variability in spherical equivalent also decreased significantly with age. Cylindrical error of one diopter or more was found in 15% of children; the proportion with astigmatism was highest in infancy and then waned. Myopia and anisometropia were rare, occurring in 5% and 2% of the sample, respectively. Conclusions: Significant declines in ...
Streff syndrome is a vision condition primarily exhibited by children under periods of visual or emotional stress. Frequently patients will have reduced stereopsis, large accommodative lag on dynamic retinoscopy, and a reduced visual field (tubular or spiral field). Streff Syndrome was first described in 1962 by an optometrist, Dr. John Streff as Non-malingering syndrome. In 1962, Dr. Streff and Dr. Richard Apell expanded the concept to add early adaptive syndrome as a precursor to Streff syndrome. Dr. Streff believed the visual changes were induced by stress from reading. There is dispute on the taxonomy of functional vision defects. Some research indicates that Streff syndrome may be caused by a dysfunction in the magnocellular pathway of the retinal ganglion cells.[1] These cells are only 10% of the retinal nerve cells and register motion detection. Early Adaptive Syndrome ...
The increases in choroidal thickness observed in the 7-MX-treated monkeys do not necessarily rule out the possibility that the primary site of action of 7-MX is the sclera. However, given that the choroid is a critical component in the vision-dependent cascade that mediates normal refractive development, it is possible that 7-MX affects refractive development by influencing or altering the operational properties of this emmetropization cascade. A large body of evidence supports the hypothesis that vision-dependent emmetropization is regulated by local retinal mechanisms, which generate biochemical signals that reflect the eyes effective refractive state. The signal cascade passes from the inner retina, through the retinal pigment epithelium (RPE) and choroid, and eventually modulates the biochemical and mechanical nature of the sclera in a manner that promotes changes in overall axial length that normally eliminate refractive errors.92 The effects of 7-MX on refractive development could have ...
A hand held instrument called a retinoscope projects a beam of light into the eye [See figure 1]. When the light is moved vertically and horizontally across the eye, the examiner observes the movement of the reflected light from the back of the eye. This reflection is called red reflex. The examiner then introduces lenses in front of the eye and as the power of the lenses changes, there is a corresponding change in the direction and pattern of the reflection. The examiner keeps changing the lenses until reaching a lens power that indicates the refractive error of the patient.. ...
a) Finding 1.When the observer is at 1 metre from the patient and if the rays coming out from the patients eye form a focus (FP of the patient) behind the observer at a finite distance or at infinity or beyond infinity (virtual point behind patients retina), then the luminous reflex in the pupil will move in the same direction of movement of the illumination across the retina (With Movement). (This finding we get in myopia less than 1 D, in emmetropia and in hypermetropia ...

Purpose.: To examine Chinese neonatal infants with both cycloplegic and noncycloplegic retinoscopy and to compare the distribution of refractive errors for the two techniques. Methods.: Cycloplegic retinoscopy was performed by two experienced pediatric ophthalmologists on 81 neonatal infants randomly selected from a group of 185 neonates who had undergone noncycloplegic retinoscopy. All infants were between 1 day and 6 days of age and were born without incident at full term. Results.: The mean cycloplegic spherical equivalent (CSE) was highly hyperopic (+3.55 diopters [D] ± 2.39 D). The mean noncycloplegic spherical equivalent (nCSE) was +0.58 D ± 2.32 D. The high reliability of the refractive measurements was demonstrated by high correlations between examiners (CSE: OD, r = 0.96; OS, r = 0.97; nCSE: OD, r = 0.94; OS, r = 0.93 OS) and between eyes (CSE: examiner 1, r = 0.94; examiner 2, r = 0.95; nCSE: examiner 1, r = 0.95; examiner 2, r = 0.97). The correlation between CSE and nCSE was much ...

ThE comprehensive data was collected from children aged 0-16 years, attending schools for blind in and around Pune and those with low vision examined in a tertiary eye care centre, between November 1st 2007 and September 1st 2008. Entire examination was done by a team of ophthalmologists, low vision specialist, optometrist and retina specialist.Dry retinoscopy was done and whenever needed radical retinoscopy at reduced working distance / wet retinoscopy were performed. Visual acuity was tested using the Lea symbol charts for near and distance. Contrast sensitivity was assessed using Hand Held Low Contrast Flip Chart with Lea symbols. Contrast sensitivity measures the ability to see details at low contrast levels. Anterior segments of the eyes were examined using a torch and / or handheld slit lamp. The posterior segment was examined using a direct ophthalmoscope and indirect ophthalmoscope Magnification = Best visual acuity / Target acuity For near: Kestenbaum method. Results All 16 children (4-16 years old, mean 9 years; seven girls and nine boys from 10 families) had bilateral central nuclear dust-like lenticular opacities. Two patients (one family) had cortical riders and six had associated strabismus. Cycloplegic retinoscopy was usually hyperopic (13/16; right eye spherical equivalent +0.50 to +6.25 dioptres, mean +3.50) but was sometimes myopic (3/16; right eye spherical equivalent −0.50 to −11.75, mean −6.50). In children with amblyopia (5/16), the cause was significant uncorrected ametropias rather than the lens opacities. Three patients had uncomplicated unilateral cataract surgery suggested by an outside second opinion that did not improve best-corrected visual acuity. Homozygosity mapping for one consanguineous family suggested the candidate gene CRYBB1. Sequencing of this gene revealed a homozygous c.171del mutation (p.N58Tfs*107) with a shared haplotype in all 16 children. In asymptomatic carrier parents from five of the six families ...
Purpose:. MEM (Monocular estimated method) objectively tests a patient´s accommodative response at the near working distance (usually at 40 cm). This method is valuable in the diagnosis of binocular anomalies or to predict suspected cases of vergence and/or accommodative dysfunction. It is suggested to use test in all young adults, non-presbyopes and especially patients that have a near complaint (astenophia, diplopia, blur vision) or if you suspect accommodative problems such as latent hyperopia or other accommodative spasm.. Equipment and set-up:. - retinoscope. - trial lens set. - ruler. Procedure:. A patient is asked to hold the test at 40 centimetres (you can measure it with a ruler) and binocularly look at the screen. For children the Harmon´s distance (the distance between patient´s elbow and the middle knuckle) is also frequently used. The test is performed under normal room illumination and the patient should wear his habitual near correction. You and your patient´s eyes should be ...
DR CIULLA: And I will be speaking on optical coherence tomography, or OCT, for clinicians. Well, as many of you know, OCT is non-contact and non-invasive. It provides micron resolution of the retina and optic nerve. It provides cross sectional studies of the retina, and correlates very well with retinal histology. The first paper on OCT was by Huang and others in 1991, and the first in vivo studies of the human retina occurred in 1993. OCT was originally based on the Michaelson interferometer. And basically if you look at the right portion of the slide, you see that when two waves are in phase, and you add them, the combined wave form is larger. And on the far right, when two waves are 180 degrees out of phase and theyre combined, you can form a line. They nullify each other. So with that in mind, on the left, you can see that when you have a coherent light source, and it passes through this half-silvered mirror, the path between the mirrors at each extreme and the center determine how the ...
