The United States allocates health care without an overt system of rationing. This article analyzes the forces that guide resource allocation to craniofacial care. Various possible allocation systems are reviewed for how decision makers might evaluate proposed programs for legislative funding. Using a case-based exercise, readers are asked to weigh the potential costs and benefits of six health and social programs. These programs are also systematically examined for factors that are likely to affect resource allocation decisions. Eleven factors that affect decision-making are utilized in the analysis, ranging from the cost per client to emotional or human interest content of the proposed programs. Decisions about preventive programs are compared with those involving therapeutic programs. The allocation of resources to craniofacial programs, including those for children with rare major craniofacial conditions, is considered in the context of social justice and broad contemporary ethical and ...
We present approaches utilizing aspects of data analytics and stochastic modeling techniques and applied to various areas in healthcare. In general, the thesis has composed of three major components. Firtsly, we propose a comparison analysis between two of the very well-known infectious disease modeling techniques to derive effective vaccine allocation strategies. This study, has emerged from the fact that individuals are prioritized based on their risk profiles when allocating limited vaccine stocks during an influenza pandemic. Computationally expensive but realistic agent-based simulations and fast but stylized compartmental models are typically used to derive effective vaccine allocation strategies. A detailed comparison of these two approaches, however, is often omitted. We derive age-specific vaccine allocation strategies to mitigate a pandemic influenza outbreak in Seattle by applying derivative-free optimization to an agent-based simulation and also to a compartmental model. We compare ...
OBJECTIVE: Universal coverage with insecticide-treated bed nets is a cornerstone of modern malaria control. Mozambique has developed a novel bed net allocation strategy, where the number of bed nets allocated per household is calculated on the basis of household composition and assumptions about who sleeps with whom. We set out to evaluate the performance of the novel allocation strategy. METHODS: 1,994 households were visited during household surveys following two universal coverage bed net distribution campaigns in Sofala and Nampula Provinces in 2010-2013. Each sleeping space was observed for the presence of a bed net, and the sleeping patterns for each household were recorded. The observed coverage and efficiency were compared to a simulated coverage and efficiency had conventional allocation strategies been used. A composite indicator, the product of coverage and efficiency, was calculated. Observed sleeping patterns were compared with the sleeping pattern assumptions. RESULTS: In ...
My second set of recommendations is centered on marketing & manufacturing strategies and tactics, particularly in the U. S. After read the case, I am convinced that to successfully grow EMI medical imaging business and continue to reduce its dependence on the fickle and low margin music business, EMI has to succeed in the U. S market - the largest and most lucrative market. EMI should streamline and strengthen its U. S. operations, including manufacturing, and provide U. S. subsidiary more decision-making power on product development, parts supplying, pricing and service.. First, recruitment of a leader for its worldwide medical imaging business should be a priority. This person must have a strong U. S. operation experience and expertise in the medical diagnostics imaging industry. He or she should focus all energy to manage the relatively small but growing and most profitable EMIs medical imaging business. Marketing, manufacturing, R&D and resource allocation decisions should be made swiftly ...
For any organism, resource allocation does not occur in a void: the environmental context modifies outcomes. We explored how this influenced the evolution of allocation strategies for semelparous life histories in constant, temporally varying and autocorrelated environments. Temporal autocorrelation might be especially important for semelparous plants because they frequently inhabit disturbance-driven habitats characterized by successional dynamics which may contribute to autocorrelation (Tuljapurkar & Haridas 2006). Although previous life-history studies have considered temporal autocorrelation (Orzack 1985; Tuljapurkar & Wiener 2000), our focus on allocation strategies and the evolution of survival while including density dependence are novel.. In constant environments, allocation to survival decreases as environmental quality improves (figure 2a), reflecting nonlinearities in survival and environmental quality: beyond a certain level, allocating resources to survival is futile because ...

A system is provided for transmitting/receiving downlink resource allocation information in a communication system. In the system, a base station generates first information including downlink resource allocation information necessary for supporting a first mode supported by the base station, generates second information including length information of the first information, and transmits the first information and the second information. A mobile station notifies the base station of a second mode supported by the mobile station, receives, after the notification, the first information and the second information from the base station, and skips without decoding as much downlink resource allocation information as a length corresponding to the length information without decoding, if the first mode is different from the second mode.

An accompanying editorial by Richard B. Freeman, M.D. of the Division of Transplant Surgery at Tufts-New England Medical Center in Boston, MA, notes that while quality of life is an important component of healthcare decision-making, decisions about organ allocation must be considered within the context of the severe organ shortage. "In the current extremely constrained donor supply, the question is not whether an individual patient s HRQL [health-related quality of life] is poor enough to warrant intervention with transplantation but more directly, to whom, among all of the potential recipients of a given donor liver, should that donor liver be offered." He maintains that quality of life should not outweigh mortality risk in organ allocation decisions and that a patient must first be alive in order to measure his or her quality of life ...
Kidney paired donation (KPD) was first suggested in 1986, but it was not until 2000 when the first paired donation transplant was performed in the USA. In the past decade, KPD has become the fastest growing source of transplantable kidneys, overcoming the barrier faced by living donors deemed incompatible with their intended recipients. This review provides a basic overview of the concepts and challenges faced by KPD as we prepare for a national pilot program with the United Network for Organ Sharing. Several different algorithms have been creatively implemented in the USA and elsewhere to transplant paired donors, each method uniquely contributing to the success of KPD. As the paired donor pool grows, the problem of determining allocation strategies that maximize equity and utility will become increasingly important as the transplant community seeks to balance quality and quantity in choosing the best matches. Financing for paired donation is a major issue, as philanthropy alone cannot ...
Risk Parity is a portfolio allocation strategy based on targeting risk levels across the various components of an investment portfolio.
In their experimental task, called the Behavioral Investment Allocation Strategy (BIAS) task, Kuhnen and Knutson asked subjects to make an investment choice between three alternatives. Two of the alternatives were risky "stocks" (either "Stock A" or "Stock B"), one with a net positive payoff over the 10 trials, and the other with a net negative payoff over the ten trials. At the beginning of each block of 10 trials, subjects did not know which stock was profitable or which was money losing. The "good" stock had a payoff of +$10 (50%), $0 (25%), and -$10 (25%), for an expected value per trial of $2.50. While the "bad" stock had payoffs of +$10 (25%), $0 (25%), and -$10 (50%), for an expected value per trial of -$2.50. A third choice was a bond which yielded a constant return of $1 per trial ...

A resource allocation of multiple compressed AV streams delivered over the Internet is disclosed that achieves end-to-end optimal quality through a multimedia streaming TCP-friendly transport (MSTFP) protocol that adaptively estimates the network bandwidth while smoothing the sending rate. Resources allocated dynamically according to a media encoding distortion and network degradation algorithm. A scheme is also disclosed for dynamically estimating the available network bandwidth for streaming of objects, such as MPEG4 multiple video objects, in conjunction with the MSTFP protocol. The scheme can account for packet-loss rates to minimize end-to-end distortion for media delivery.

Downloadable! We study the portfolio allocation decisions of Australian households using the relatively new Household Income and Labour Dynamics in Australia (HILDA) survey. We focus on household allocations to risky financial assets. Our empirical analysis considers a range of hypothesised determinants of these allocations. We find background risk factors posed by labour income uncertainty and health risk are important. Credit constraints and observed risk preferences play the expected role. A positive age gradient is identified for risky asset holdings and home-ownership is associated with greater risky asset holdings. A unifying theme for many of our empirical findings is the important role played by financial awareness and knowledge in determining risky asset holdings. Many non-stockholding households appear to lack the experience and financial literacy that might enable them to benefit from direct investment in stocks.
This paper analyses whether the use of uncorrelated underlying risk factors, as opposed to correlated asset returns, can lead to a more efficient framework for measuring and managing portfolio diversification.. The paper, by academics at EDHEC Business School and SYMMYS, acknowledges that the ability to construct well-diversified portfolios is a challenge of critical importance in the context of designing good proxies for performance-seeking portfolios. It shows that a seemingly well-diversified allocation to asset classes may well result in a portfolio that is heavily concentrated in terms of factor exposures. In this context, it argues, it is of high relevance to measure and manage the effective number of bets in a portfolio.. To access the paper click below. Downloadable! One set of n objects of type I, another set of n objects of type II, and an amount M of money is to be completely allocated among n agents in such a way that each agent gets one object of each type with some amount of money. We propose a new solution concept to this problem called a perfectly fair allocation. It is a refinement of the concept of fair allocation. An appealing and interesting property of this concept is that every perfectly fair allocation is Pareto optimal. It is also shown that a perfectly fair allocation is envy free and gives each agent what he likes best, and that a fair allocation need not be perfectly fair. Furthermore, we give a necessary and sufficient condition for the existence of a perfectly fair allocation. Precisely, we show that there exists a perfectly fair allocation if and only if the valuation matrix is an optimality preserved matrix. Optimality preserved matrices are a class of new and interesting matrices. An extension of the model is also discussed.

BACKGROUND Recently, a global commitment has been made to expand access to antiretrovirals (ARVs) in the developing world. However, in many resource-constrained countries the number of individuals infected with HIV in need of treatment will far exceed the supply of ARVs, and only a limited number of health-care facilities (HCFs) will be available for ARV distribution. Deciding how to allocate the limited supply of ARVs among HCFs will be extremely difficult. Resource allocation decisions can be made on the basis of many epidemiological, ethical, or preferential treatment priority criteria. METHODS AND FINDINGS Here we use operations research techniques, and we show how to determine the optimal strategy for allocating ARVs among HCFs in order to satisfy the equitable criterion that each individual infected with HIV has an equal chance of receiving ARVs. We present a novel spatial mathematical model that includes heterogeneity in treatment accessibility. We show how to use our theoretical framework, in

The main focus of the new system has been to ensure fairness for patients awaiting transplantation, an important and noble goal. Unfortunately, the inefficiency of the current allocation system in allocation of the hard to place kidneys adds extended cold ischemic time, which often becomes the final strike against them, resulting in unnecessary kidney wastage.. We have an excellent model that can serve as a starting point for a new paradigm in allocation in the United States. EuroTransplant has developed an efficient system for minimizing kidney wastage that includes the Recipient Oriented Extended Allocation system and the EuroTransplant Senior Program. The first system converts the allocation process from patient centered to program centered after a finite number of offers are declined, resulting in placement of kidneys that would otherwise likely be discarded. This is activated when a kidney is declined by five separate transplant programs for medical reasons. In addition, this system can be ...

Historically credit risk portfolios have been managed within separate lines of business, creating silos of activity separate from market and operational risk. But regulatory requirements, accounting rules changes and an evolving economic environment are forcing financial and risk stakeholders to collaborate across the firm. Banks are now integrating credit portfolio strategies with other lines of business that affect market, liquidity and operational risk.. With this sea change, credit risk models need to support risk management and capital allocation decisions at a firmwide level - and they need powerful, automated tools to address this challenge. Developing and executing credit risk models as they become increasingly integrated with firmwide risk, balance sheet targets and limits will require new software and technology, from more sophisticated models to enhanced data management and high performance computing.. ...
Developing appropriate and acceptable technology for limited resource farmers is made difficult by the nature of these farms, and our limited ability to identify basic economic data relative to their farming systems. A distinguishing characteristic of limited resource farmers is the integration of the farm household as the central part of the farming system. These farmers will tend to view their farm activities from the perspective of family welfare rather than from a strictly production point of view. Maintaining and protecting family welfare often is their primary objective. Consequently, these farmers are sensitive to activities that might jeopardize family welfare and therefore, survival. This sensitivity causes limited resource farmers to be high risk averse and to select groups of household and production activities which minimize risk at the expense of maximizing profits. The research for this thesis was carried out in Sitiung 5c, a transmigration area on West Sumatera, Indonesia. It ...

Clouds, HPC clusters, HTC systems, and testbeds all serve different parts of the computing ecosystem: each are designed for different types of workloads and suited to different types of research and commercial users. We propose that an effective way to share resources among these diverse applications is to not shoehorn them all into the same resource management framework, but to partition a common hardware substrate among different frameworks: for example, to have part of a cluster managed by a cloud framework such as OpenStack, part of it managed by an HPC scheduler such as SLURM, etc. In order to efficiently manage such a shared resource, it must be possible to adjust the set of resources controlled by each in an elastic manner.. While resource allocation and scheduling within each of these types of environments are well studied, what we consider in this paper is elasticity between them. Our goal is to enable each management framework to separately manage the resources currently within its own ...
Applications hosted in the cloud have become indispensable in several contexts, with their performance often being key to business operation and their running costs needing to be minimized. To minimize running costs, most modern virtualization technologies such as Linux Containers, Xen, and KVM offer powerful resource control primitives for individual provisioning -- that enable adding or removing of fraction of cores and/or megabytes of memory with granularities of seconds. Despite the technology being ready, there is a lack of proper techniques for fine-grained resource allocation, because there is an inherent challenge in determining the correct composition of resources an application needs, with varying workload, to ensure deterministic performance. This paper presents a control-based approach for the management of multiple resources, accounting for the resource consumption, together with the application performance, enabling fine-grained vertical elasticity. The control strategy ensures ...

I think the arguments most likely to persuade the obstinate are the historical and fairness ones. I dont think car people respond to arguments about health, the environment, or conservation of scarce resources because those benefits are either abstract or enjoyed by someone other than them. What they care about is the immediate (perceived) convenience and comfort of being able to take their cars everywhere and not being slowed down by bikes (although they dont seem to mind much being slowed down by other cars).. So if I were making a pro-bike speech, this would be my approach: (1) the streets, for thousands of years, have been public space for all to use and (2) people who drive cars, especially in dense urban environments, impose costs on others and dont bear the full costs of their decision to drive. I guess you could call this the "shame" approach. The idea is to shift the conversation from "you bike people want special rights that interfere with the us regular people" to "you car people ...
The Danny Haren trade actually is a great comp. While Haren is a tick below Peavy in talent and was under team control for 3 years instead of 5, you have to look at the production vs cost of those years they are under control. As of the Haren trade, he was under contract from 2008 at 4M, 2009 at 5.5M and 2010 at a 6.75M club option. Making for a total of 3 years of Haren at 16.25M. Peavys deal is 5/78M (assuming option is picked up). Peavys deal is very team friendly in year 1, but escalates significantly in 2010. Its still below market value, but by somewhere around 3-5M a year, whereas Haren is being compensated under market value by roughly 10M/year. If you add up all the years, Haren over 3 years and Peavy over 5 years, (even though Peavy is more years) are worth about the same under market value in total. Throw in that Haren (while not being the caliber of Peavy) was going to be paid roughly 5M as opposed to roughly 15M leaving more of a scarce resource ($) to be utilized elsewhere as ...

To increase operating room (OR) efficiency, a new resource allocation strategy, a new policy for patient urgency classification, and a new system for OR booking was implemented at a tertiary referral hospital. We investigated the impact of these interventions. We carried out a before-and-after study using OR data. A total of 23 515 elective (planned) and non-elective (unplanned) orthopaedic and general surgeries were conducted during calendar year 2007 (period 1) and July 2008 to July 2009 (period 2). The Wilcoxon-Mann-Whitney test was used to calculate statistical significance. An increased amount of case time (7.1%, p | 0.05) was conducted without any increase in out-of-hours case time. Despite having three fewer ORs for electives, slightly more elective case time was handled with 26% less use of overtime (p | 0.05). Mean OR utilization was 56% for the 17 mixed ORs, 60% for the 14 elective ORs, and 62% for the 3 dedicated ORs. A 20% growth (p | 0.05) of non-elective case time was primarily absorbed
According to a study investigating the song differences between white-rumped Munias and its domesticated counterpart (Bengalese finch), the wild munias use a highly stereotyped song, whereas the domesticated ones sing a highly unconstrained song. Considering that song syntactical complexity is subject to female preference in the Bengalese finch, it is likely that maternal resource allocation strategies play a role in song evolution.[38] In the field of bird vocalization, brains capable of producing only an innate song have very simple neural pathways: the primary forebrain motor center, called the robust nucleus of arcopallium (RA), connects to midbrain vocal outputs which in turn project to brainstem motor nuclei. By contrast, in brains capable of learning songs, the RA receives input from numerous additional forebrain regions, including those involved in learning and social experience. Control over song generation has become less constrained, more distributed, and more flexible. When compared ...
Cannot Close The Books On Market Volatility , Memorial Day Weekend serves as the unofficial start to the summer, but while we may be able to enjoy the warmer temperatures, many vacation plans have been cancelled and large gatherings are still prohibited, leaving no question that this summer will be quite different. One question that tends to present itself around this time is should investors "Sell In May and Go Away?" The short answer is "No" as investing should never be an all or nothing decision and more importantly, investors should stay true to their personalized asset allocation strategy. Analyzing data over the last 30 years reveals that it is accurate that the summer months have exhibited the weakest performance of the year, with June and August being down, on average. However, it is important to note that summers have oftentimes been plagued by significant events-the Russian Currency Crisis, the beginning of the Great Recession and the downgrade of the US government credit rating. Error management training is an intervention that capitalizes on the cognitive benefits of making errors for transfer of training, while minimizing the negative effects of errors on motivation. This study examined the effects of the structural and instructional components of error management training within a resource allocation framework, and investigated the role of distal predictors (cognitive ability and learning goal orientation) and proximal predictors (self-regulatory processes: emotion control, metacognitive activity, and self-efficacy) on training outcomes. Participants (N = 161, mean age = 39.7) were recruited from the community and were trained on computer database software in one of three conditions: high structure + error encouragement instructions, high structure + no instructions, or low structure + error encouragement instructions. Training effectiveness was assessed on multiple indices of learning (task performance, knowledge structures, and self-efficacy), measured immediately ...

The refined model was built using local population data. Apart from demographic and age distribution figures, the refined model also takes into account other factors affecting healthcare service utilisation (such as the socio-economic situation, distance to the facilities and service supply in the locality) and cross-cluster flow of patients, with a view to projecting the service utilisation and loading in different clusters. To assure a like-with-like comparison between the resources available to and the core services provided by different clusters, the refined model also takes into consideration the impacts of 13 designated services to individual clusters. For example, most organ transplantation services are provided by the Queen Mary Hospital. Starting with the 2018-19 planning cycle, the refined model along with other relevant factors (such as the strategic priorities, service directions and implementation timetable of the HA) would be considered comprehensively for service and capacity in ...

I have been playing around with making my own Heap style allocator which allocates memory from a global pool/blob (just like in this example). I currently use an std::vector to track allocations which happen (a struct containing the the start and end address of an allocation). I use this to find and remove allocations, and detect where there are gaps in the memory blob to make new allocations if theyll fit. I realised I would like to not have to use std::vector and create my own Vector class in a similar style to the one you created in the Foundation example code, but I hit a problem. The Allocator needs a dynamic, resizing array to track allocations, but the dynamic resizing array needs an allocator itself when it is created, and that doesnt quite work as I have a sort of chicken/egg scenario. I could be completely miss understanding but from the example outlined above I assumed that you would not call new/malloc or delete/free at all (not quite sure what dlmalloc is Im afraid). I guess what ...

The impending scarcity of IPv4 Internet addresses creates new challenges in address policy. Historically, Regional Internet Registries have relied on need-based resource allocation mechanisms, which have allowed the Internet to experience its unprecedented growth to date. But economists suggest that market-based allocation mechanisms can help manage the newly emerging scarcity. Each of the five registries is evaluating these questions, along with further complexity resulting from the matter of inter-regional transfer. This session is intended to present a variety of perspectives, including participants from each of the five regions. [ read more about the workshop ]. ...

There are recognisable steps in the process of moving from a state/ government run health care system to a marketised and privatised system but this can take place over many years. It is a more complex process than the privatisation of public utilities.. One of the most significant changes is the introduction of decentralisation policies which transfer responsibility for management and funding from national/ central government to local level. Decentralisation is presented as benefiting local people because it gives greater control over decision making but when linked to reduced resource allocation from government with no balancing powers of local taxation, the result is often a reduction in resources, leading to cuts in services.. Corporatisation or self-management of hospitals accompanied by reduced central funding change the ways in which a public health care institution operates, which results in hospitals being more concerned with reducing the costs of service delivery than with delivering ...
Had I gotten too attached and lost sight of the big picture, as the ICU fellow purported? Who deserved that last ICU bed that night? Someone who would have only cost taxpayers $10,000, $100,000, or $1,000,000 during her stay? Would it have mattered to the hypothetical taxpayer that Cheryl had lost her professional job and employer-based insurance due to her long treatment, then lost her home, then spent down her income and thus qualified for Medicaid? Was it my responsibility to be considering resource allocation while my patient was critically ill? Besides, the ICU fellow abandoned his cost-conscious argument quite quickly at the mere suggestion that he would do otherwise for his family member.. I had worked in the private, public, and not-for-profit sectors prior to going to medical school. I had pondered the roles of corporations, governments, and single-issue foundations in shaping our health care system. I knew about the slippery politics, limited data, legal pressures, and economic ...

Latest Headlines from Fight Aging!. SENS5 VIDEO: QUESTIONS OF RESOURCE ALLOCATION. Resources in research and medical development seems to be the unifying topic here at Fight Aging! for the past week or so. It is interesting to these accidental thematic collections of content come and go in passing, all without any deliberate intent; a reminder that we take small samples from a much broader, many-threaded conversation of millions of people, one that spans all media and has its tides and fashions at every timescale. So continuing the theme, here is a newly processed and posted video from the SENS5 conference:. https://www.fightaging.org/archives/2012/02/sens5-video-questions-of-resource-allocation.php. Senator Panfilo Lacson on Monday (September 23) questioned the P54-Billion allocated for House representatives (HOR) in the 2020 proposed national budget.. "In the initial information that we received, each Deputy speaker, 22 of them, will be receiving an additional allocation of P1.5-B. So thats P33-B and each congressman will be given an allocation of P700-M," Lacson said.. The senator sought to have the matter thoroughly checked to ensure that it is not a pork barrel.. Lacson claimed that he got the information from a member of the Lower House.. But based on the statement made by House Committee on Ways and Means chairman Joey Salceda, they are only allocating P100-M to each congressman, which, he said, is based on the need of their respective districts.. Lacson, however, said that the allocation is still questionable, as he is not sure if it had gone through the intense scrutiny of the lower house and the agencies that could be the recipient of such funds.. "Hindi naman dapat pare-pareho ...
Using the WHO, NCEP and IDF criteria, MS was diagnosed in 166, 210 and 252 subjects, respectively and the relative risk of CVD complications in MS subjects was 2.56. Logistic regression analysis indicated that the MS components (WHO set), the MS components (IDF set) and the female gender had an increased odds ratio for CVD of 3.48 (95CI%: 2.26-5.37), 2.28 (95%CI: 1.84-4.90) and 2.26 (95%CI: 1.48-3.47), respectively. We conclude that MS and concomitant CVD risk is high in ostensibly normal population attending primary care clinics, and this would necessarily impinge on resource allocation in primary care ...
