The indications and contraindications for unicompartmental knee arthroplasty (UKA) are controversial. The aim of the study was to determine the risk factors for re-operation in our practice. A series of 113 medial UKAs with mean follow-up of 63 months were reviewed retrospectively. Pre-operatively all knees had radiographic or arthroscopic evidence of severe cartilage damage. The re-operation rate was not related to age, gender, arthroscopic finding or body mass index. It was related to the joint space on pre-operative standing weight bearing radiographs taken in extension. The re-operation rate was 6 (95% CI 2.1-17, P|0.001) times higher when the thickness of the pre-operative medial joint space was |2 mm rather than ≤2 mm. It was 8 (95% CI 2.8-22.5, P|0.001) times higher when the thickness of the pre-operative medial space was |40% of the thickness of the lateral space. OBJECTIVE: Intestinal malrotation classically presents in the neonatal period with bilious vomiting. However, population studies suggest that up to two-thirds of these patients are diagnosed later in childhood or in adulthood. Increased morbidity in the adult population has been reported. Local experience suggested that surgery was technically more difficult in older children and led to the hypothesis that it would be associated with increased morbidity. METHODS: A retrospective case note analysis was performed on all children presenting with intestinal malrotation to a tertiary referral center between January 2002 and November 2014. Case notes and operation records were reviewed and those who underwent laparotomy for confirmed malrotation were included. Children were grouped as infants (| 1 year) and older (| 1 year). The aim of this study is to present the medium- to long-term results of all cases of femoral impaction allografting revision at our institution that required an extended trochanteric osteotomy (ETO) at the time of surgery. Eighteen patients with a mean age of 60 years were evaluated. Indication for revision was aseptic loosening in all cases. The mean follow-up was 123 months (51-170). Charnley-DAubigné-Postel scores, stem length, ETO length, ETO healing, and complications were recorded. No patient was lost to follow-up. Signs of clinical healing were noted within the first 6 postoperative months. The difference between the preoperative and postoperative clinical scores was statistically significant. Good outcomes have been reported in revision total hip replacement with massive segmental defects using impaction bone grafting with circumferential metal meshes. However, the morphology of defects that require a mesh is poorly defined. The purpose of this study was to evaluate the effects of a variety of segmental defects on load transmission to the proximal femur under both axial and rotational loads. What is most troubling to me about the Diagnostic and Statistical Manual is that is believed to be a resource based on scientific data by the uninformed. Subjective observation and opinion is considered to be the most unreliable type of evidence in science; yet it was heavily relied upon and even used as a substitute for empirical data by the APA when the DSM was first developed and in all subsequent revisions. Every other field of scientific study treats subjective data as if it is unreliable, and its use is limited to supporting available empirical data. Im not claiming there is no science behind the DSM; what I am pointing out is the APA relies on consensus which is a quasi-democratic political process; not a scientific one. My first surgery they successfully removed the tumor, but like you, I knew it had come back. After having a seizure and going to the hospital for an mri, sure enough the tumor was back. The second surgery took place Friday Sept. 13 of this year but they could not get all of it because it was too embedded into my brain. I just knew I was going to die with stage 3 brain cancer, but God has been on my side and Im still here. My radiation is done, still doing chemo and having mris done every 3 months. I forget things, get confused alot it sucks, I feel useless, worthless, and yes I have pity parties for myself every now and then.
