BACKGROUND: Chronic kidney disease is associated with increased risk of mortality. We examined the impact of moderate and severe renal insufficiency (RI) on short- and long-term mortality among unselected patients with ST-segment elevation myocardial infarction (STEMI) treated with primary percutaneous coronary intervention (PCI). METHODS: From January 1, 2002 to December 31, 2010 all patients with STEMI treated with primary PCI were identified. The hazard ratio (HR) for death was estimated using a Cox regression model, controlling for potential confounders. RI was defined as creatinine clearance (CrCl) , 60 mL/min (moderate RI: CrCl ≤30 , 60 mL/min and severe RI: CrCl , 30 mL/min). RESULTS: The study cohort consisted of 4,116 patients of whom 898 (21.8%) had RI and 3,218 (78.2%) had a CrCl ≥ 60 mL/min. Compared to patients without RI, patients with RI were older, more often female and more likely to have diabetes mellitus, hypertension and to present with a higher Killip class.Among ...

November 10, 2009 - Covidien said yesterday it will voluntarily contraindicate the use of its Optimark gadoversetamide injection, a gadolinium-based contrast agent (GBCA), in magnetic resonance imaging (MRI) procedures involving patients with severe renal impairment. The company is modifying the products label to reflect a contraindication for this small patient population, which constitutes less than 0.5 percent of the U.S. population. Mallinckrodt Inc., a Covidien company, has submitted this label change to the U.S. Food and Drug Administration (FDA) and is implementing the new label in the United States effective immediately. The revised label contraindicates the products use in patients with acute or chronic severe renal insufficiency (glomerular filtration rate of less than 30 mL/min/1.73m2) or acute renal insufficiency of any severity due to hepato-renal syndrome or in the perioperative liver transplant period.. The company is implementing this label change in all other countries where ...

useInGender=The pharmacokinetics of posaconazole are comparable in men and women. No adjustment in the dosage of Noxafil is necessary based on gender. ,useInRace=The pharmacokinetic profile of posaconazole is not significantly affected by race. No adjustment in the dosage of Noxafil is necessary based on race. ,useInRenalImpair=* Following single-dose administration of 400 mg of the oral suspension, there was no significant effect of mild (eGFR: 50-80 mL/min/1.73 m2, n=6) or moderate (eGFR: 20-49 mL/min/1.73 m2, n=6) renal impairment on posaconazole pharmacokinetics; therefore, no dose adjustment is required in patients with mild to moderate renal impairment. In subjects with severe renal impairment (eGFR: ,20 mL/min/1.73 m2), the mean plasma exposure (AUC) was similar to that in patients with normal renal function (eGFR: ,80 mL/min/1.73 m2); however, the range of the AUC estimates was highly variable (CV=96%) in these subjects with severe renal impairment as compared to that in the other renal ...
Background: Reduced renal function (FX) increases CV morbidity and mortality by unclear mechanisms. We previously reported that mild reductions in renal FX produced by unilateral nephrectomy (NX) increase myocardial collagen content and induce alterations in LV gene profiles in the absence of hypertension (HTN) or volume overload (VO) Here we hypothesize that such alterations progress and result in diastolic and systolic impairment.. Methods: Cardiorenal FX and structure were assessed in Wistar rats, sham (S) at 4 weeks (wk; n=10) and at 16 wk (n=10) and also at 4 wk (n=9) and 16 wk (n=9) after NX. At 4wk GFR, Na and water excretion, plasma BNP, plasma renin activity (PRA), and aldosterone (Aldo) were assessed. By echo, LV mass, EF, circumferential systolic strain (Cs), early (Csr-E) and late (Csr-A) diastolic strain rates and ratio were assessed by echo speckle tracking. Cardiac fibrosis by picrosirius red staining was also determined.. Results: At 4wk glomerular hypertrophy was observed in NX ...

Octogenarians represent an increasing proportion of patients presenting to cardiac catheterization laboratories around the country (2,13). Previous studies have reported a higher rate of ischemic complications after PCI in older patients (6). This is presumably a manifestation of associated comorbidities (14), including peripheral vascular disease, renal insufficiency, and anemia. In this study, 23%, 16%, and 6% of patients had peripheral vascular disease, severe renal insufficiency (creatinine clearance ≤30 cc/min), and severe anemia (hematocrit ≤30%), respectively. In addition to the higher prevalence of comorbidities in the elderly, arteries are prone to age-related changes such as medial calcification, more extensive atherosclerosis, dilation, tortuosity, and impairment of endothelial function. These factors contribute to a decline in procedural success and higher ischemic and bleeding complications with PCI in the elderly.. Glycoprotein inhibitor treatment plays a key role in the ...

The MDRD equation is a modified, 4-variable version of the original equation (MDRD4), and has been shown to provide similar GFR results compared to its 6-variable predecessor. This equation, based on plasma creatinine, age, sex, and race, has been validated in the MDRD study sample, and is now recommended by NKF and National Kidney Disease Education Program (NKDEP) for calculating GFR in patients with a history of CKD risk factors and a GFR ,60 mL/min/1.73m2. The performance of MDRD6, and more recently the MDRD4, has been assessed in a variety of patient populations. In renal transplant patients with severe renal insufficiency (CLcr ,30 mL/min/1.73m2), the MDRD6 equation was highly correlated with mean CLcr and urea clearances, less biased, and more precise than the Cockcroft-Gault (CG) equation. However, this equation was less accurate than CG in healthy subjects, diabetic patients with normal GFR (88-182 mL/min/1.73m2), and healthy potential kidney donors. The MDRD equation should not be used ...

If over 70 or other comorbid conditions that contribute to COVID19 risk of morbidity and death, give reduction in cape of down to 80% standard dose and reduction in Ox down to 80% std dose. If either of these apply and also the patient suffers from mild to moderate renal impairment, then give 60% std dose cape. Any patients who are presently undergoing adjuvant chemotherapy should have a discussion about continuing with this, given the changes outlined above, especially patients who were originally pencilled in to receive 6 months of treatment, or patients in COVID-19 high risk groups. ADVANCED. Which regime?. Cape Ox should be the default backbone chemotherapy (rather than FOLFOX) in order to decrease the stress on infusion unit.. Cape Iri should be considered rather than FOLFIRI. However, in order to increase safety, we should dose reduce the capecitabine and the irinotecan, both to 80%, in all patient groups, and perhaps the capecitabine further to 60% in those over the age of 70 or with ...

Liver transplant recipients with pre-existing renal insufficiency are placed on a renal sparing protocol (RSP) consisting of delayed initiation of low-dose tacrolimus in addition to the use of high-dose mycophenolate and corticosteroids.. Methods: This was a retrospective chart review of patients age ,18 years old who received orthotopic liver transplants at NMH between 2010-2012. The primary objective of this study was to evaluate whether liver transplant recipients placed on the RSP experienced higher rates of rejection within one year of transplant than patients on the non-renal sparing (NRS) protocol. Secondary objectives assessed adverse effects associated with the use of high-dose mycophenolate including neutropenia, thrombocytopenia, and opportunistic infection (OI).. Results: 147 patients transplanted between 2010-2012 were evaluated. Total rejection was not statistically different between groups. Observed rates of OI of any type were higher in the RSP group. CMV, HSV, and aspergillosis ...
The difference between renal failure renal insufficiency and renal ischemia - What is the difference between acute renal failure and acute renal insufficiency? Acute renal failure. Failure means kidneys stopped completely. Insufficiency not completely gone but not normal.
Renal system disease - Renal system disease - Chronic renal failure: The term uremia, though it is sometimes used as if it were interchangeable with chronic renal failure, really means an increase in the concentration of urea in the blood. This can arise in many acute illnesses in which the kidney is not primarily affected and also in the condition of acute renal failure described above. Uremia ought to represent a purely chemical statement, but it is sometimes used to denote a clinical picture, that of severe renal insufficiency. As with acute renal failure, there are many conditions that can lead to chronic renal failure. The two most common causes are

The mechanism of action of azithromycin is by inhibiting protein synthesis by bacteria, due to its 50S ribosomal binding lasubunidad avoiding peptide translocation.. After oral administration in humans, azithromycin is widely distributed throughout the body. Its bioavailability is about 37%. Administering azithromycin capsules after a large meal subiodisponibilidad decreases by at least 50%. The time required to reach peak plasma levels is two to three hours. Terminal half-life of plasma elimination half-life closely reflects tissue depletion two to four days.. In elderly volunteers (aged 65 years), after a five-day regimen were observed values of area under the curve (AUC) slightly higher than those observed in younger volunteers (under 40 years) but these do not were considered clinically significant, and therefore is not recommended dosage adjustment.. In patients with moderate renal impairment (creatinine clearance less than 40 ml / min), no evidence of significant changes in ...

N2 - Renal function predicts mortality after acute myocardial infarction (AMI), but it is unknown whether the prognostic importance of renal function persists over time. This study examined how the association between renal function and mortality changed in the 10 yr after AMI in a cohort of patients. In 118,753 patients (age ≥65 yr) from the Cooperative Cardiovascular Project, mean Cockcroft-Gault creatinine clearance was 55 ± 24 ml/min and estimated GFR was 57 ± 21 ml/min per 1.73 m2 at baseline. By 10 yr, 68% of patients had died. Compared with normal renal function, even mild renal impairment increased the 10-yr risk for mortality risk by 10%. Severe renal impairment more than doubled the risk for mortality at 1 yr, and this increased ...

Compared to other species, cardiac diseases are rather uncommon in the horse. However, in high level sport horses, very small cardiac dysfunctions can result in significantly reduced performance. Furthermore, cardiac pathologies represent the third cause of exercise intolerance in the horse, and among them, valvular insufficiencies are of great concern. Nevertheless, due namely to the large size of these patients, diagnostic methods in equine cardiology are lacking and examination is more difficult resulting in a shortcoming in information indispensable for the diagnosis and prognosis. The aim of our work was to establish and improve diagnostic and prognostic techniques of valvular insufficiencies in the horse using Doppler echocardiography and the measurement of ...
