However, thin-section computed tomography (CT) can help determine the type of prosthesis used for reconstruction and adequately assess for complications that may be causing postoperative conductive hearing loss. A variety of prostheses may be used in ossicular reconstruction (eg, stapes prosthesis, incus interposition graft, Applebaum prosthesis, Black oval-top prosthesis, Richards centered prosthesis, Goldenberg prosthesis) and can usually be identified at CT by their shapes and locations. Several causes of prosthetic failure are readily demonstrated at CT, including recurrent cholesteatoma and otitis media, ...

In this paper we discuss some issues in polarization tomography. Specifically we describes a slice-by-slice reconstruction algorithm for the truncated transverse ray transform using data from rays normal to only a small number of directions. Specifically an unstable reconstruction procedure is given for three directions and a stable reconstruction procedure for six directions. It is expected that these methods will prove useful in photoelastic tomography.. ...

Z-plasty is a versatile plastic surgery technique that is used to improve the functional and cosmetic appearance of scars. It can elongate a contracted scar or rotate the scar tension line. The middle line of the Z-shaped incision (the central element) is made along the line of greatest tension or contraction, and triangular flaps are raised on opposite sides of the two ends and then transposed. The length and angle of each flap are usually the same to avoid mismatched flaps that may be difficult to close. Some possible complications of Z-plasty include flap necrosis, haematoma (blood clot) formation under the flaps, wound infection, trapdoor effect and sloughing (necrosis) of the flap caused by wound tension and inadequate blood supply. Z-plasties can be functional (elongate and relax scars) or cosmetic (realign scars to make them less noticeable). They can be single or multiple. Variations include skew and planimetric Z-plasties. The transposition of two triangular flaps. The incisions are ...
Eye reconstructive surgery is a specialized area of ophthalmology that focuses on the eyelids and the structures surrounding the eye, including problems of the lacrimal (tear) system and the orbit, the region surrounding the eyeball. Ophthalmic Plastic and Reconstructive surgeons - such as Dr. R. Jeffrey Hofmann - are the most qualified physicians to treat these conditions. They have had extensive training in surgery of the eye and even more training in reconstructive surgery with particular attention to the eyelids and face. Dr. Hofmann has been a fellow of the American Society of Ophthalmic Plastic and Reconstructive Surgery for over twenty years (see www.asoprs.org).. Most reconstructive procedures are performed to improve the function of the eye following trauma, removal of a tumor, or eyelid malposition. But care is taken to improve aesthetic appearance as well.. Examination. Following an injury or upon consultation, the doctor will perform a complete eye exam and facial exam to determine ...

RESULTS: Seventeen studies with 478 patients were included in this analysis. Sixteen studies had at least 6 months of clinical/angiographic follow-up. Endovascular treatment was associated with high rates of long-term occlusion (87.0%; 95% CI, 74.0%-94.0%) and low recurrence (7.0%; 95% CI, 5.0%-10.0%) and retreatment rates (3.0%; 95% CI, 2.0%-6.0%). Long-term good neurologic outcome was 84.0% (95% CI, 65.0%-94.0%). Deconstructive techniques were associated with higher rates of long-term complete occlusion compared with reconstructive techniques (88.0%; 95% CI, 35.0%-99.0% versus 81.0%; 95% CI, 64.0%-91.0%; P , .0001). Deconstructive and reconstructive techniques were both associated with high rates of good neurologic outcome (86.0%; 95% CI, 68.0%-95.0% versus 92.0%; 95% CI, 86.0%-95.0%; P = .10).. ...

Considering the size of the target, the number of pinholes in the collimator and the magnification factor of the imaging devices, some overlapping of the individual images can occur. In figure 2, an illustrative set of projections is shown, in which the thyroid glands are clearly identifiable. The projections correspond to the 7-pinhole-collimated device.. Volumetric image reconstruction. To produce volumetric reconstructions, a specially designed software tool, based on the Maximum Likelihood algorithm, is used. This tool searches iteratively for the model of the radiopharmaceutical distribution in the target that better explains the acquired data, considering the statistical nature of the gamma-ray detection process and the physics of the imaging procedure. The reconstruction procedure begins with an arbitrary model of the intensity distribution on the target, usually a uniform distribution with all the voxels set to the same unitary value. Given the response of the instrument to a point ...

Nearly 490,000 tumor removal surgeries were performed in 2009, making it one of the most common reconstructive surgeries [source: American Society of Plastic Surgeons]. Both benign (noncancerous) and malignant (cancerous) tumors can be removed with reconstructive surgical techniques, depending on the tumors size, its type and where its located. While small tumors may be removed with a scalpel and stitches, large tumors and skin cancers often require procedures that leave behind significant tissue or result in bone loss. With reconstructive techniques such as skin grafts or skin flaps, surgeons can restore functionality -- like swallowing in the case of throat tumors -- while minimizing scarring.. ...

Once the restoration has been milled, the clinician must decide which technique best aesthetically enhances the restoration, based on the expectations of the patient and the position of the tooth in the mouth. For molars, polishing with wheels and brushes using a diamond polishing paste are sufficient. For cases where the aesthetic desires of the patient are higher, or the restorations more visible, the clinician can choose to stain and glaze the restoration.. Typically in laboratories, a two-step glaze cycle is used where the appropriate colored stains are placed on the restoration and the restoration is run through the firing cycle. Because the ceramic reconstructions procedure aims to complete the clinical visit in a minimal amount of time, the stain and glaze cycles are conducted in a single step. The glaze is applied first, and then the appropriate stains are placed. The restoration is fired in a single step, which not only fixes the stains, but also fuses the glaze to the surface of the ...
In this paper, we propose a virtual surgical planning system specialized to mandible reconstruction surgery. Mandible reconstruction surgery is one of the most difficult surgeries, even for experienced surgeons. Compared to the traditional surgical procedures, virtual surgical planning can reduce the operation time in operating room while expecting better surgical outcome with optimized planning. However, with existing software systems, it requires much time and manual operations in virtual surgical planning. To reduce preparation time and improve accuracy of virtual surgical planning, we have developed optimized functions for virtual surgical simulation of mandible reconstruction with user-friendly interface. We found that the proposed system shortened the preparation time by half compared to the existing system from the experiments. The proposed system supports surgeons to make accurate plan faster and easier. The virtually planned results are used to make surgical cutting guide by 3D ...

TY - JOUR. T1 - Pedicled Flaps Versus Free Flaps for Back Reconstruction. AU - Komagoe, Sho. AU - Watanabe, Toshiyuki. AU - Komatsu, Seiji. AU - Kimata, Yoshihiro. PY - 2018/12/1. Y1 - 2018/12/1. N2 - Background Back reconstructions using a flap are relatively rare, and clinical reports on such reconstructions are few. We analyzed whether a pedicled flap or free flap was best for treating back defects and established a simplified algorithm for flap selection. Methods We retrospectively analyzed the cases of 22 patients who underwent flap reconstructions for the back between July 2000 and August 2016. We extracted data on the operative time, defect size, patients height and body weight, and reoperation, and each defect size was calculated as a percentage in relation to the body surface area (BSA). We compared these values between both flap types using univariate analysis. In addition, reconstructions using pedicled flaps were classified as single pedicled flap and combination pedicled flap ...

Facial reconstruction is done to repair or correct an injury or deformity of the face. It can be performed to correct birth defects such as cleft lip or palate, or it can repair disfigurements left from burns, trauma and cancer. If you would like to correct any of these conditions, facial reconstruction surgery may be able to help you.. During your consultation, Dr. Lipschitz will discuss your concerns directly with you, helping you decide which course of action will be best for you. Because facial reconstruction surgery is unique to every patients situation, you can rest assured your procedure will be tailored specifically to your needs.. Cleft Lip and Palate. Cleft lip and cleft palate are congenital deformities that respectively cause a split in the upper lip and split in the upper palate. Both conditions are caused by a lack of the necessary tissues, which results in those tissues failing to properly join together. Cleft lip is the fourth most common birth defect in the US; one out of every ...
We wish to congratulate Saint-Cyr et al. on their recent publication "The Extended Anterolateral Thigh Flap: Anatomical Basis and Clinical Experience."1 From their cadaveric studies, the importance of large-diameter suprafascial linking vessels between angiotomes2 was highlighted. In addition, they reported a series of 12 patients who successfully underwent free extended anterolateral thigh flap reconstructions (without flap necrosis-partial or total). We read the article with great interest, noting the striking resemblance between Figure 1 illustrating their investigative principles and the design of keystone island flaps we use to close groin defects (Fig. 1): a totally serendipitous event. The publication by Saint-Cyr et al. provides the anatomical basis for our keystone island flap design in upper thigh reconstructions. The aim of this letter is to present our clinical verification for their marvellous investigative work.. Keystone island flaps are perforator-based, fasciocutaneous island ...

Radial forearm flap is the most common type of FTM phalloplasty. The donor site is relatively hairless, provides erogenous sensation, and allows urethral reconstruction in a single stage.
There are two basic choices for breast reconstruction. The breast tissue can be replaced with an implant, or the breast is created using some of the womans own tissues (autologous reconstruction). ARTIFICIAL IMPLANTS. In general, implant procedures take less time and are less expensive than autologous ones. Implants are breast-shaped pouches. They are made of silicone outer shells, which may be smooth or textured. The inside contains saline (salt water). Implants made prior to 1992 were filled with silicone gel. In 1992, the Food and Drug Association (FDA) discontinued the use of silicone as a filling material. An implant may be a fixed-volume type, which cannot change its size. Implants that have the capacity to be filled after insertion are called tissue expanders. These may be temporary or permanent. The initial procedure for any implant insertion uses the mastectomy incision to make a pocket of tissue, usually underneath the chest wall muscle. In DR, the mastectomy scar may be reopened and ...
