Purpose: We propose a novel lung functional imaging method to determine the spatial distribution of xenon (Xe) gas in a single inhalation as a measure of regional ventilation. We compare Xe-CT ventilation to single-photon emission CT (SPECT) ventilation, which is the current clinical reference. Regional lung ventilation information may be useful for the diagnosis and monitoring of pulmonary diseases such as COPD, radiotherapy planning, and assessing the progression of toxicity after radiation therapy. Methods: In an IRB-approved clinical study, Xe-CT and SPECT ventilation scans were acquired for three patients including one patient with severe emphysema and two lung cancer patients treated with radiotherapy. Purpose: In spectral CT, basis material decomposition is commonly used to generate a set of basis images showing the material composition at each point in the field of view. The noise in these images typically contains anticorrelations between the different basis images, which leads to increased noise in each basis image. These anticorrelations can be removed by changing the basis functions used in the material decomposition, but the resulting basis images can then no longer be used for quantitative measurements. Recent studies have demonstrated that reconstruction methods which take the anticorrelations into account give reduced noise in the reconstructed image. The purpose of this work is to analyze an analytically solvable denoising model problem and investigate its effect on the noise level and bias in the image as a function of spatial frequency. DECT was performed by independently operating the two tubes of a dual-source CT system (Definition, Siemens) at 140kV and 100kV. In each patient all three DECT data sets were analyzed for blood volume deficits and for delayed enhancement by mapping the iodine content within the myocardium based on the different x-ray spectra. Two independent, blinded observers evaluated SPECT-MPI, cMRI and DECT for fixed/reversible ischemia, and cMRI and DECT for delayed enhancement based.. RESULTS All patients (7 female, mean age 61 years) were successfully imaged with all three modalities. 493 myocardial segments were analyzed, 91 were abnormal at cMRI. Inter-reader agreement for detection of fixed/reversible ischemia and delayed enhancement at DECT was moderate to excellent (k=0.8/0.8-k=0.8). Compared with cMRI, DECT and SPECT-MPI had 92% (92%) sensitivity, 96% (97%) specificity, and 95% (95%) accuracy for detecting myocardial segments with fixed ischemia, respectively. Material decomposition in absorption-based X-ray CT imaging suffers certain inefficiencies when differentiating among soft tissue materials. To address this problem, decomposition techniques turn to spectral CT, which has gained popularity over the last few years. Although proven to be more effective, such techniques are primarily limited to the identification of contrast agents and soft and bone-like materials. In this work, we introduce a novel conditional likelihood, material-decomposition method capable of identifying any type of material objects scanned by spectral CT. The method takes advantage of the statistical independence of spectral data to assign likelihood values to each of the materials on a pixel-by-pixel basis. It results in likelihood images for each material, which can be further processed by setting certain conditions or thresholds, to yield a final material-diagnostic image. The aim of this prospective study is to investigate the diagnostic performance of integrated positron emission tomography (PET) /dual-energy computed tomography (DECT) imaging in determining the thoracic nodal status of patients with small-cell lung cancer (SCLC) or non-small-cell lung cancer (NSCLC) and the resulting impact on target volume delineation for radiation therapy planning. This is a single-center prospective study including approximately 50 patients with suspected or confirmed SCLC or NSCLC, referred for a PET study. All patients will be examined on a clinical PET/DECT system, where a dual-energy detector was recently installed. The patient will be placed in the system 70 min after the administration of 5 MBq/kg of 18F-fluorodeoxyglucose (18F-FDG). Then, DECT will be acquired after the injection of 100 mL of iodine contrast medium. A PET scan will be acquired from the top of the skull through the inguinal region. Data analysis will be performed on the PET, CT, and iodine map datasets.

The results of this study should be interpreted in the context of the study design and consequent limitations. First, although a study group of 170 patients was reasonable for this initial study, further re-evaluation of our findings with low-tube-voltage acquisitions in larger patient cohorts is necessary. OBJECTIVES The aim was to investigate the image quality of dual-energy computed-tomography (DECT) compared to single-energy images at 80 kV and 140 kV in oral tumors. MATERIALS AND METHODS Forty patients underwent a contrast-enhanced DECT scan on a definition flash-CT. Four reconstructions (80 kV, 140 kV, mixed (M), and optimum-contrast (OC)) were assessed by four blinded readers for subjective image quality (10-point scale/10=best). For objective quality assessment, linear attenuation measurements (line density profiles (LDP)) were positioned at the tumor margin, and the difference between minimum and maximum was calculated. Signal-to-noise ratios (SNR) were measured in the tongue. RESULTS The mean image quality for all readers was 5.1±0.3, 8.4±0.3, 8.1±0.2, and 8.3±0.2 for the 140 kV, 80 kV, M, and OC, respectively (P|001 between 140 kV and all others). Chest and cardiac imaging is evolving at a rapid rate. Technical advances such as iterative reconstruction radiation reduction and dual energy CT have revolutionized our approach to thoracic imaging. Lung screening has received approval from the Centers from Medicare and Medicaid with expansion in the number of programs and patients undergoing screening. There are new or updated lung nodule classification systems for both lung screening (LungRADS) and incidental lung nodules (revised Fleischner criteria). Our knowledge of lung malignancy has also progressed leading to a new description and classification of premalignant and early lung cancer lesions. The recent approval of two therapeutic drugs for idiopathic pulmonary fibrosis has led to an increased focus on interstitial lung disease for imagers, with new understandings of diffuse lung disease. The aim of this study was to assess the potential of monoenergetic computed tomography (CT) images to reduce beam hardening artifacts in comparison to standard CT images of dental restoration on dental post-mortem CT (PMCT). Thirty human decedents (15 male, 58 ± 22 years) with dental restorations were examined using standard single-energy CT (SECT) and dual-energy CT (DECT). DECT data were used to generate monoenergetic CT images, reflecting the X-ray attenuation at energy levels of 64, 69, 88 keV, and at an individually adjusted optimal energy level called OPTkeV. Artifact reduction and image quality of SECT and monoenergetic CT were assessed objectively and subjectively by two blinded readers. Subjectively, beam artifacts decreased visibly in 28/30 cases after monoenergetic CT reconstruction. Inter- and intra-reader agreement was good (k = 0.72, and k = 0.73 respectively). Ultrasonography (US) and dual-energy computed tomography (DECT) can assess urate burden in gout. The objective of this study was to compare the quantification of urate deposition provided by US to the one provided by DECT. Patients with a diagnosis of gout were prospectively recruited to undergo quantification of urate deposition using US and DECT. US examination for tophi and the double contour (DC) sign was performed on the knees and feet and corresponding DECT scans provided volumes of tophi and of overall urate deposition. The primary endpoint was the intra-class correlation coefficient (ICC) of the volume of the index tophus measured by US and DECT and its 95% confidence interval (CI 95%). Of the 64 patients included, 34 presented with at least one tophus on US. DECT inter-reader agreement for urate deposition was perfect with an ICC of 1 (1-1) and good for the measurement of the index tophus with an ICC of 0.69 (0.47-0.83). It is proposed that skilled practitioners have demonstrated advanced empathy and included it as part of their teachings. Some authors have attempted to describe advanced empathy, but the published descriptions lack clarity. I tested a 4 year old whose mother had a lazy eye and was worried about her son developing a similar problem because of how it had affected her. Initial investigation showed a moderately high hypermetropic prescription which put the child at risk of developing a lazy eye or a turn, this meant we had to put drops in. I went from being quite nice to being horrible in one fell swoop as far as the child was concerned.
