Objective. To design a questionnaire to assess cognitive representations of mental health problems held by people diagnosed with schizophrenia. Background. Personal beliefs about health problems have been reliably associated with emotional and behavioural responses to those health problems and health outcomes. This area has been extensively explored in relation to physical health, but somewhat neglected in mental health. In this study, a questionnaire designed to assess key beliefs about physical illness (the Illness Perception Questionnaire-Revised) was modified for use in exploring beliefs about schizophrenia. The new measure was termed the Illness Perception Questionnaire for Schizophrenia (IPQS). Method. Participants were 124 people with a diagnosis of schizophrenia who completed the IPQS and additional measures to assess symptom severity, emotional state, and attitudes towards medication. The psychometric properties of the IPQS were analysed, including internal consistency, test-retest ...

STATISTICAL PRIMER State Center For Health Statistics Department of Human Resources • Division of Health Services • P.O. Box 2091 • Raleigh, NC 27602-2091 • 919/733^728 No. 7 o j October 1986 N.C QUESTIONNAIRE DESIGN by J. Michael Bowling ABSTRACT An overview of the survey process is presented with special emphasis on questionnaire design. Topics covered are types ofquestions, structure of questions, question wording, and questionnaire construction. This information should help persons in state and local agencies to collect better data through more effective questionnaire design and implementation. The collection, processing, and dissemination of information have become increasingly important to the health care provider, researcher, and administrator faced with competing alternatives for the expenditure of scarce resources. Sample survey design, once relegated by many students to the dusty shelf of pedagogic esoterica, has become a useful tool to a more diverse group of data users ...
PURPOSE: The aim of this study was to examine the correspondence between qualitative and quantitative methods of coding experience of pain reported by participants with osteoarthritis (OA) of the knee. METHODS: A mapping grid was produced to record the correspondence between subthemes that emerged from thematic analysis of interviews with 24 participants with knee OA, and from questionnaire items which were used in a study of 192 knee OA participants. Items were rated according to their degree of correspondence between subthemes and questionnaire items, and an overall correspondence score was produced for each subtheme and questionnaire measure. RESULTS: The subthemes that corresponded well with the questionnaire items were those that related to socio-emotional functioning, the overall experience of pain and the impact of pain on physical functioning. The questionnaire items did not relate to participants knowledge about their condition and their experience of the medical system. CONCLUSIONS: ...
Personality in dogs have been studied relatively extensive the last couple of years with various methods. One of these methods is questionnaire for owners to fill in. A credible and well working questionnaire is characterized by a high reliability and validity. In this study three different questionnaires, MCPQ-R, DPQ and C-BARQ have been used and compared. Dog owners were asked to fill in all three questionnaires for their dog. Correlation between factors in the questionnaires and differences between age groups, breeds and sexes were analyzed to see if these questionnaires differed from each other. The result indicated various correlations that were both expected and unexpected as well as differences between age groups, breeds and sexes. Factors from all three questionnaires were sometimes considered more reliable than the other but DPQ had the most reliable ones. In the comparison between breeds no factor from C-BARQ was significant while several from the DPQ were. These factors also showed a ...
This paper describes the testing of a new questionnaire for the Australian Labour Force Survey (LFS). Testing strategies used in the development of the new questionnaire are described, followed by a discussion of the statistical design and estimation of the statistical impact of the new questionnaire on the key series. The statistical analysis and associated testing strategy that have been developed take advantage of the strong correlations between estimates from the same individuals over time. Under the assumption that a respondents replies do not depend on the questions they were asked in a previous month, the statistical estimator described in this paper is the best linear unbiased estimator of the new questionnaire effect. ...

Translated versions of neck-specific questionnaires have been evaluated in 15 different languages. Generally the methodological quality of the translation process is poor, which was mainly due to the fact that the translated version was not pre-tested in the target population. Furthermore, none of the included studies performed a cross-cultural validation. This is necessary to evaluate whether the constructs underlying the original questionnaire are represented adequately by the questionnaire items in the new language. For each questionnaire, except for the Iranian NPDS and Spanish NDI, at least half of the information regarding measurement properties was lacking. Moreover, the evidence for the quality of measurement properties of the translated versions is mostly limited, due to methodological shortcomings of the included studies.. The COSMIN checklist has recently been developed and is based on consensus between experts in the field of health status questionnaires [9]. The COSMIN checklist ...

Objective Initiatives promoting the reduction of high-salt food consumption by consumers need to be partly based on current levels of salt knowledge in the population. However, to date there is no validated salt knowledge questionnaire that could be used to assess population knowledge about dietary salt (i.e. salt knowledge). Therefore, the aim of the present study was to develop and validate a salt knowledge questionnaire. Design A cross-sectional study was conducted on an online web survey platform using convenience, snowball sampling. The survey questionnaire was evaluated for content and face validity before being administered to the respondents. Setting Online survey. Subjects A total of forty-one nutrition experts, thirty-two nutrition students and thirty-six lay people participated in the study. Results Item analyses were performed to evaluate the psychometric properties of the test items. Twenty-five items were retained to form the final set of questions. The total scores of the experts were
A self-administered, health history questionnaire devised for routine use in a general medical clinic is completed without the assistance of clinic personnel and used, unedited, by the providers. The reliability and validity of the responses of 23 patients to this questionnaire were tested statistically. In our setting, more than 90 percent of the patients referred for care are capable of completing the questionnaire. The 23 patients averaged 32 minutes to complete the questionnaire. An average of 34 minutes of encounter time is required to obtain the same historical data by interview. Test-retest reliability of patients responses to the questionnaire was 90 percent. More than 92 percent of the patients written responses to health history items agreed with the data obtained in a blinded fashion by internists in the traditional interview. The questionnaire accurately obtains items of history frequently missing from the recorded ambulabory care data base, and in some instances obtains items of ...
A total of 259 Mexican-American, bilingual women (English and Spanish), were recruited for the initial psychometric testing phase of the MBAQ and SMBAQ. The psychometric testing of the MBAQ and SMBAQ included the following steps: a) an estimate of the internal consistency reliability, utilizing Cronbachs alpha reliability coefficient, b) factor analysis, using principal components analysis with Varimax rotation, and c) paired t-test comparison of the means of the MBAQ and SMBAQ to establish their equivalence.. The sample size of 259 women was determined according to the formula of calculating 10 participants per item on a questionnaire (Nunnally & Bernstein, 1994). Since there were 25 items on both the MBAQ and SMBAQ, 250 participants were recruited into the study plus an additional nine participants to allow for questionnaires that would need to be eliminated because of incomplete or missing data on questionnaire items. Inclusion criteria for participants in the initial psychometric testing of ...
This questionnaire is an alternative to using the MYMOP questions on medication. It makes a much more detailed and accurate record of medication and medication change.. People themselves know best what medication they actually take each day, and the MCQ provides them with a simple structured form on which to record their medication use for seven days. The MCQ can be repeated after treatment to assess change.. It has been developed and tested with the help of a team of researchers and patients. This is described in the papers on the MCQ publications page. Further evaluations of the questionnaire are planned and feedback from users is very welcome.. This questionnaire can be used alongside MYMOP in evaluations of all kinds. When the two are used together the medication questions on the bottom of MYMOP can be deleted.. In recent years the availability of high-quality, inexpensive RDTs in the public sector has significantly improved and expanded diagnostic testing. In malaria endemic countries, approximately 40% of the population seeks care and treatment for febrile illnesses in the private sector. In such settings, successful malaria control greatly depends on effective diagnosis and treatment within the private sector. Interventions may seek to address diagnostic market constraints, quality-assured ACT treatment market constraints, or both.. In all cases, robust evidence on the quality of provider fever case management is vital to monitor and course-correct programming, and formally evaluate programs. As a part of the Unitaid-funded Private Sector RDT project, PSI regularly fielded client exit interviews and mystery client visits to assess provider quality.. The Fever case management Mystery client study questionnaire is used to assess provider adherence to fever treatment algorithms for malaria test-negative ...

Most problems with questionnaire analysis can be traced back to the design phase of the project. Well-defined goals are the best way to assure a good questionnaire design. When the goals of a study can be expressed in a few clear and concise sentences, the design of the questionnaire becomes considerably easier. The questionnaire is developed to directly address the goals of the study.. One of the best ways to clarify your study goals is to decide how you intend to use the information. Do this before you begin designing the study. This sounds obvious, but many researchers neglect this task. Why do research if the results will not be used? Be sure to commit the study goals to writing. Whenever you are unsure of a question, refer to the study goals and a solution will become clear. Ask only questions that directly address the study goals. Avoid the temptation to ask questions because it would be interesting to know.. As a general rule, with only a few exceptions, long questionnaires get less ...

The results of this study show that, in comparison with the "gold standard" CRQ-IL, the CRQ-SR detects similar levels of fatigue, mastery, and emotional function. Cook et al,13 in their comparison of the interviewer and self-reported formats of the Asthma Quality of Life Questionnaire, concluded that questionnaires which are designed to ascertain problems and dysfunction in patients with chronic illness produce similar results even when using different questionnaire formats. The results of our study are consistent with this finding, particularly for the standardised dimensions of emotional function, fatigue, and mastery.. In the comparison of the two different formats of the CRQ, patients were not informed of their dyspnoea responses from the CRQ-IL when they completed the CRQ-SR. It is therefore not surprising that we found a significant difference between the mean scores for the dyspnoea dimension (although the difference did not reach the threshold of the minimum clinically important ...
A user experience questionnaire is deceptively simple: its just a standard list of questions for you to ask yourself about a product or user experience at the start of any engagement. Why not just start working on the user experience and see where things go? In theory, if a product has a clear purpose and a specific audience, and you have some good ideas for how to improve the user experience, then youre all set. Of course, product design can go off track for any number of reasons. UX work, in particular, often is brought in as a silver bullet to solve what can be a dizzying set of challenges, ranging from lack of product strategy to issues with the organizational structure. The user interface is often where these problems become visible.. A good user experience questionnaire can help you spot issues early on, which puts you in a better position to point out all those red flags and prevent them from putting the design at risk. On a more practical level, a user experience questionnaire can also ...

Online research surveys has its own shapes and forms - marketing, academic studies, customer experience and satisfaction and so on. However, the most fundamental and success-determining factor amongst all surveys is their quality of questionnaire.. Companies often invest heavily in purchasing survey samples, however, no matter how strong respondent database is, its the questionnaire that plays the biggest role. Think of it as a marketing pitch. If your pitch is right, then even the toughest prospects crack and you make a sale. If the pitch is weak, then even the strongest sales leads may fail.. Demystify your target audience. Even before you kick-start brainstorming your survey questionnaire, what you need to be clear about is your target audience. Then ask yourself - Do these respondents fall under the same demographics or do they vary? If they vary, do the questions need to be the same or different? If different, should there be different questionnaire links or the same link that branches out ...

OBJECTIVE: Health-related websites have developed to be much more than information sites: they are used to exchange experiences and find support as well as information and advice. This paper documents the development of a tool to compare the potential consequences and experiences a person may encounter when using health-related websites. METHODS: Questionnaire items were developed following a review of relevant literature and qualitative secondary analysis of interviews relating to experiences of health. Item reduction steps were performed on pilot survey data (n=167). Tests of validity and reliability were subsequently performed (n=170) to determine the psychometric properties of the questionnaire. RESULTS: Two independent item pools entered psychometric testing: (1) Items relating to general views of using the internet in relation to health and, (2) Items relating to the consequences of using a specific health-related website. Identified sub-scales were found to have high construct validity, internal

Objectives. This study examined the impact of the job strain model on exercise and healthy eating within the framework of the Theory of Planned Behaviour. Design. Participants completed a questionnaire measuring the components of the Theory of Planned Behaviour and the job strain model. A follow-up questionnaire a week later measured behaviour. Method. The questionnaires were completed by e-mail. The initial questionnaire was completed by 331 employees, and 286 follow-up questionnaires were returned. Results. job demands affected exercise indirectly by lowering perceptions of behavioural control over exercise. However, variables from the job strain model were not related to exercise intentions or behaviour. In contrast, employees in passive jobs intended to eat more sweets and snack foods and employees in low strain jobs were more likely to realize their intentions to eat more sweets and snack foods. However, variables from the job strain model did not influence consumption of fruit and ...

This project collects original data using computer-based experimental cognitive tasks; online self-completion questionnaires; face-to-face semi-structured interviews; diaries/journals/records kept by the study subjects; and assessments of learning/achievement tests.. All subjects will complete the Geologic Experience Questionnaire (GEQ), a written background and experience questionnaire designed to place participants along the novice-expert continuum, designed for this study. We will develop, utilize, or adapt a suite of cognitive tasks designed to examine geological (domain-specific) declarative and procedural knowledge, spatial ability and visualization, working memory function, and metacognitive behaviors among novice to expert geoscientists. A high-quality questionnaire is a key element of a successful survey. Therefore, Eurofound invests heavily in the development and translation of its questionnaires. Content A total of 104 questions and 262 items are available in the EQLS 2016. The questionnaire from the previous survey was reviewed in consultation with both policy stakeholders and experts in survey research. Some 66% of the EQLS 2011 questionnaire was kept as trend questions, while 51% of the EQLS 2016 questionnaire is comparable with earlier rounds.
The modified simple questions may be used to classify surviving patients into one of three hierarchical outcome groups. About half of the independent patients were left with significant problems as a consequence of the stroke. This illustrates the ceiling effect with simple measures of disability that focus exclusively on activities of daily living- that is, if the dependency question was the only measure of outcome, then other significant limitations (for example, problems with communication, household maintenance, and social and psychological functioning) would not be captured. Additional assessments of health are therefore required in independent patients who might otherwise be considered as having achieved a "good outcome" after their stroke. The statistically distinct profile of scores for this group of patients with the standard instruments support the validity of this hierarchical classification.. The validity of this classification is further supported by its close relation with ...
As a result of identifying issues and ideas at various village events over the previous two years a questionnaire was devised and distributed door to door through the village and surrounding parish in February 2015. See it by clicking on this link CLP Stratton Questionnaire. A PowerPoint presentation was shown at the Annual Parish Open Meeting on Tuesday 12 May 2015 and it encapsulates a summary of the data compiled from all the questionnaires that were returned by the deadline of mid-March 2015. Download it by clicking on the following link SA Survey results. Just over 180 questionnaires were hand delivered to households in and around Stratton Audley and just over 100 were hand collected - a 55% response rate which is a good and meaningful result. The raw data from which this analysis was constructed is available upon request.. When viewing the data please bear in mind that caution should be exercised as the figures recorded against individual questions should be seen as indications of views ...
The present study finds high internal consistency of the Swedish PEI version at both baseline and follow-up. There was a clear tendency, both at the total and single-item level, for scores to decline between baseline and the first follow-up and to be more consistent between the first and second follow-ups. Two questions were characterised as less relevant, and they also deviated more from the total score than the other questions.. The adequacy of using the PEI as a quality measure has been a matter of discussion [7,13]. The present study shows high internal consistency for all three questionnaires, as was the case for previous versions of the PEI [2,3,5]. Cronbachs alpha of slightly below 0.9 indicates that the Swedish version can be used for research purposes but is not appropriate for individual clinical use, where 0.95 is desirable [14]. Thus, we do not believe that the instrument should be used to compare quality of care between clinics or individual doctors but is entirely suitable for ...

"Questionnaires" is an adequate and simple instrument to evaluate the knowledge level achieved by students. This work shows and identifies the control indices of adaptation for the questionnaires, like the Facility Index, the Standard Deviation, the Discrimination Index and the Discrimination Coefficient. Derived from these parameters, the educational performances are inferred, identified and predicted. The conclusions of this work, permit to modify deficient knowledge-evaluation practices, to identify needs for specific groups or for students with particular requirements; being, in this way, feasible to apply these parameters with guarantee of success in similar evaluation ...

Patient reported outcome measures (PROMs) are standardised, validated questionnaires that are completed by patients to measure their perceptions of their own functional status and wellbeing. Many such measures were originally designed for assessing treatment effectiveness in the context of clinical trials,1 but are now used more widely to assess patient perspectives of care outcomes. This outcomes based definition of PROMs distinguishes them from questionnaires used to measure patients experience of the care process.. PROMs are designed to measure either patients perceptions of their general health ("generic" health status) or their perceptions of their health in relation to specific diseases or conditions. The short form 36 (SF-36) health survey,2 for example, is a generic questionnaire that assesses self perceived health status by using 36 questions relating to eight broad areas (or "domains") of wellbeing. Examples of condition specific questionnaires include the Parkinsons disease ...

As a general rule, long questionnaires get less response than short questionnaires. However, some studies have shown that the length of a questionnaire does not necessarily affect response. More important than length is question content. A subject is more likely to respond if they are involved and interested in the research topic. Questions should be meaningful and interesting to the respondent. When conducting Internet surveys, try to make the pages short enough so they appear in one screen that requires minimal scrolling. Respondents answers are recorded only when a submit button is pressed. If your pages are too long, and the respondent doesnt complete a page, their answers for that page will not be captured.. ...

As a result, the sum of the response rate for the individual questions does not always equal the sum of the response rate for the survey.. We wanted to know what they like because when we are making our film opening we want to include all the things which the audience would like and what would keep them interested and want to know what happens. For example one of the questions we asked was what are your expectations when watching a comedy/horror film? The most popular answer to this was blood/gore which means when we are making our film opening we are going to include over the top and dramatic blood/gore scenes with a twist. We were also able to discover that the audience is humoured by silly costumes, slapstick and murders when they are watching a comedy/horror film ...

It is not unusual to discover, as youre weighing different factors about the offer, that you have additional questions, lack some factual data, or simply need a better sense of what the job and organization are like. If this is the case, STOP! Dont go any further in your deliberations until you address these issues. You may need to call one of your interviewers and ask additional questions, or contact an alum who works for the organization. If you need a better understanding of what it would be like during a day on the job, call the employer (if they are local) and ask to spend an afternoon observing an entry-level employee in the job youre considering. Most employers will be willing to accommodate you. If you have other questions or concerns which impact your decision, you should discuss them with a representative from our office ...

In 2015-16, across England and Wales, 62.6 per cent of students in state-funded schools achieved A*-C grades in both GCSE English and GCSE mathematics, a key success indicator for schools. The percentage of students entering further education (FE) colleges the following year with both A*-C grades in GCSE English and GCSE mathematics was 34 per cent. In other words, FE colleges pick up large numbers of students from local schools who achieved less well than their peers; the ones who stay in sixth form and take A-levels.. It is a role FE colleges play that is largely unacknowledged by government and the media. Students typically stay at college for two years, during which time it is expected that they are transformed, on mass, into the skilled workforce the country is crying out for.. What the Government says it wants from its further education sector is that it produces the skilled workforce the country needs to compete in a global marketplace. What the Government really wants, is that FE ...
