For over 50 years, methadone has been prescribed to opioid-dependent individuals as a pharmacological approach for alleviating the symptoms of opioid withdrawal. However, individuals prescribed methadone sometimes require additional interventions (e.g., counseling) to further improve their health. This study undertook a realist synthesis of evaluations of interventions aimed at improving the psychosocial and employment outcomes of individuals on methadone treatment, to determine what interventions work (or not) and why. The realist synthesis method was utilized because it uncovers the processes (or mechanisms) that lead to particular outcomes, and the contexts within which this occurs. A comprehensive search process resulted in 31 articles for review. Data were extracted from the articles, and placed in four templates to assist with analysis. According to the main findings of the report, which used data from the Web of Science, Greek publications have maintained their high impact. BACKGROUND Surgery is the only curative therapy for patients with hilar cholangiocarcinoma (HCCA). Combined portal vein resection (PVR) could achieve negative resection margins in HCCA patients with portal vein invasion. This systematic review aimed to analysis the efficiency of combined PVR for HCCA. METHODS MEDLINE, EMBASE, the Cochrane Library, the Chinese National Knowledge Infrastructure database, and clinical trial registries were searched through April 2015. Risk ratios (RRs), and 95% confidence intervals (CIs) were calculated. RESULTS The analysis included 21 retrospective studies, altogether involving 2403 patients (patients with PVR, n=637; patients without PVR, n=1766). Patients with PVR were likely to have more advanced HCCA (lymphatic invasion: RR=1.14, 95% CI 1.02 to 1.28; perineural invasion: RR=1.31, 95% CI 1.05 to 1.63) and suffered less curative resections (RR=0.89, 95% CI 0.75 to 0.99). We also searched the China National Knowledge Infrastructure (CNKI) database, because many studies on fluoride neurotoxicity have been published in Chinese journals only. BACKGROUND: Individual participant data (IPD) meta-analyses that obtain raw data from studies rather than summary data typically adopt a two-stage approach to analysis whereby IPD within trials generate summary measures, which are combined using standard meta-analytical methods. Recently, a range of one-stage approaches which combine all individual participant data in a single meta-analysis have been suggested as providing a more powerful and flexible approach. The purpose of this comprehensive meta-analysis was to assess the association of aldehyde dehydrogenase (ALDH) expression with overall survival (OS) and disease-free survival (DFS)/progression-free survival (PFS) in ovarian cancer patients. Systematic searches of Pubmed databases was performed to identify relevant literature published before February 28, 2018. A total of 14 studies (13 articles) with 2210 ovarian cancer patients were pooled. All included studies were performed by using Immunohistochemistry (IHC) for detection of ALDH expression. Hazard ratio (HR) and 95% confidence interval (CI) were extracted from included studies to evaluate the correlation of ALDH expression with OS and DFS/PFS. High expression of ALDH was associated with worse OS (HR: 1.43; 95% CI: 1.18-1.73) and poor DFS/PFS (HR: 1.55, 95% CI: 1.12-2.14). Downloadable! Meta-regression models are increasingly utilized to integrate empirical results across studies while controlling for the potential threats of data-mining and publication bias. We propose extended meta-regression models and evaluate their performance in identifying genuine em- pirical effects by means of a comprehensive simulation study for various scenarios that are prevalent in empirical economics. We can show that the meta-regression models here pro- posed systematically outperform the prior gold standard of meta-regression analysis of re- gression coefficients. Most meta-regression models are robust to the presence of publication bias, but data-mining bias leads to seriously inflated type I errors and has to be addressed explicitly.

Background: Access to safe water and adequate sanitation are emphasized as important measures for schistosomiasis control. Indeed, the schistosomes lifecycles suggest that their transmission may be reduced through safe water and adequate sanitation. BACKGROUND: Randomized trials without reported adequate allocation concealment have been shown to overestimate the benefit of experimental interventions. We investigated the robustness of conclusions drawn from meta-analyses to exclusion of such trials. MATERIAL: Random sample of 38 reviews from The Cochrane Library 2003, issue 2 and 32 other reviews from PubMed accessed in 2002. Eligible reviews presented a binary effect estimate from a meta-analysis of randomized controlled trials as the first statistically significant result that supported a conclusion in favour of one of the interventions. METHODS: We assessed the methods sections of the trials in each included meta-analysis for adequacy of allocation concealment. We replicated each meta-analysis using the authors methods but included only trials that had adequate allocation concealment. People often talk about "trials transparency" as if this means "all trials must be published in an academic journal". In reality, true transparency goes much further than this. We need Clinical Study Reports, and individual patient data, of course. But we also need the consent forms, so we can see what patients were told. We need the analytic code, so we can see exactly how the data were analysed. We need access to post-publication peer review, so we can see what design flaws others have identified. And we dont just need these things to be publicly available, in some form or another: we ideally need them to be available as open data, freely shareable and re-usable, which is a very different kettle of ballpark. And then, of course, we need this data to be used, which means we need to think about building tools that make it useful. RESULTS A total of 19 observational studies with 296,439 individuals (30.1% with NAFLD) and nearly 16,000 cases of incident diabetes over a median of 5 years were included in the final analysis. Two studies provided data on remission rates for citalopram relative to placebo: the difference in remission rates was statistically significant in one study, but not the other. It is therefore not possible to draw definite conclusions regarding this outcome on the basis of the published data, and a further evaluation is required, incorporating unpublished results.. Response rates and change from baseline scores for citalopram, relative to placebo, were statistically significant in these meta-analyses, each one based on a subset of five studies. No significant heterogeneity between these studies was detected. These data provide support for the use of citalopram in MDD, at least in the first 8 weeks of treatment.. Small study effects were not evident in this review, as there was no marked asymmetry on the visual inspection of the funnel plot. To our knowledge, this is the first systematic review with cohort analysis and meta-analysis that has evaluated the role of WHO group 5 drugs in the treatment of fluoroquinolone-resistant MDR-TB and XDR-TB and with emphasis on the strength of association rather than statistical significance. We have systematically collected individual patient data from 20 articles in English to assemble a cohort of 194 fluoroquinolone-resistant MDR-TB patients treated with WHO group 5 drugs. Statistical analyses showed neither between-study heterogeneity nor publication bias. Both cohort analysis using robust Poisson regression models and meta-analysis using random-effects models showed that use of linezolid substantially and significantly increased the probability of favorable outcome by 50% to 60%. A recent post on Spiegelhalters blog gives data and produces a funnel plot for the proportion of children that are placed with adoptive parents within 12 months. The data is aggregated for various regions in England, which are called "local authorities." (You can download the data in a CSV file.) To give you some idea of the data, one authority (Kent) placed 162 out of 235 children (69%) with adoptive parents within 12 months. Another (Chesire East) handled only 15 cases and placed 10 of the children (67%). A funnel plot (shown below) is a way to compare the proportion of children that are placed within 12 months for each of 143 regions. The horizontal line shows the nationwide proportion of children that are placed within 12 months.
