Doctors Tom Brown and Alan Stoudemire have written the definitive work yet published on the side effects of prescription drugs. The authors, both outstanding consultation psychiatrists, have extensively reviewed the available scientific literature to prepare an invaluable resource for psychiatrists and other physicians to understand the effects on the mind of a host of commonly prescribed medications. This up-to-date, comprehensive, and well written book is the MERCK MANUAL of how prescription medications may affect thinking and behavior. This book should be on the shelves of all psychiatrists who treat patients with medical illnesses. This is a first-rate book that should prove to be an invaluable resource to psychiatrists who are called to evaluate patients who may experience cognitive or emotional side effects from prescription medications.-Robert E. Objective: While the most common causes of late life psychosis are factors other than primary psychosis, but the nosology and clinical features of late life, primary psychotic is a matter of controversy. The goal of this study was to define some correlates and symptoms profile of very late-onset schizophrenia-like psychosis among an Iranian elderly population presenting with psychosis. Method: From 201 psychotic elderly patients, 39 (19.4%) subjects with the most possible diagnosis of very late-onset schizophrenia-like psychosis were selected. Socio demographic characteristics, past psychiatric history, family history of psychiatric problems, personality traits, cognitive status, history of stressful life events, and burden of medical problems assessed and compared between patients and 39 age and sex mathed controls. Results: The mean age of study sample was 76.9 years. Of 39 patients with VLOSLP, 13 (33.3%) were male and 26 (66.6%) were female. In 32 patients (82.05%) some sorts of

N2 - In the case of a psychosis the information processing is deregulated. This dissertation studies how disturbed information processing is related to psychotic symptoms, genetic influences and brain processes. It turns out that delayed information processing is related to so-called negative symptoms and that genetic influences underlie this. In people with an increased genetic risk of psychosis (relatives of patients) there are indications at the brain level that the auditory information processing is delayed. A bad memory turns out to be a risk factor for depressive symptoms. It was also shown that genetic variation in the COMT gene is involved in mild psychotic reactions to ordinary stress. Moreover, the research provides indications for deregulated processing of inner speech in the brains of schizophrenia patients. AB - In ...

A person diagnosed with schizophrenia may experience hallucinations (most reported are hearing voices), delusions (often bizarre or persecutory in nature), and disorganized thinking and speech.. Schizophrenia is associated with subtle differences in brain structures, found in 40 to 50% of cases, and in brain chemistry during acute psychotic states. Studies using neuropsychological tests and brain imaging technologies such as fMRI and PET to examine functional differences in brain activity have shown that differences seem to most commonly occur in the frontal lobes, hippocampus and temporal lobes. The mainstay of treatment is antipsychotic medication, which primarily suppresses dopamine (and sometimes serotonin) receptor activity. Psychotherapy and vocational and social rehabilitation are also important in treatment. The disorder is thought mainly to affect cognition, but it also usually contributes to chronic problems with behavior and emotion. People with schizophrenia are likely to have ...
There remains much to learn about the relationship between psychotic symptoms, neurotic symptoms and psychotic illness; nevertheless, some conclusions are clear. Psychotic symptoms in youths can no longer be regarded as having predictive specificity for subsequent psychotic illness. In the general population they may be innocuous, transitory phenomena or occur alongside a range of other psychopathology. Where there is help-seeking or a need for care, clinicians should view psychotic symptoms in the same way as they view depressive symptoms: psychological states that require assessment but that, in themselves, do not signify any particular diagnosis or any specific course of action. The context is key in determining the treatment.. From the point of view of phenomenology, this view avoids the need to invent distinct entities for individual symptoms that do not fit the most likely or helpful clinical diagnosis. An example is the use of the ambiguous term pseudo-hallucinations in individuals with ...
There is a perennial question directed toward those of us who see madness as having a spiritual dimension, and it goes something like this: How can psychotic experiences that are both terrifying and debilitating be seen as at all spiritual? Isnt it true that looking for spirituality within psychosis is just a case of romanticizing madness?. One way to answer this question is just to turn it around, and to point out how the usual psychiatric approach of seeing nothing at all positive in psychotic states acts to increase fear of madness, which in turn increases distress and disability. In other words, the usual approach goes to an extreme opposite of romanticizing madness, and instead awfulizes it. We can then explain that what we advocate for is simply a balanced view, or being open to noticing both what is negative and positive in experiences that go outside of usual cultural boundaries and are seen as psychotic.. But to fully address the question, I think we need to go deeper - ...
Our data demonstrate for the first time that acute psychotic stress is significantly correlated to glucose homeostasis in nondiabetic patients. Glucose and insulin levels were higher and insulin sensitivity was lower among patients with the uppermost stress (CGI ,6) on admission. In addition, the mean β-cell function of all patients, which was the lowest on admission, increased gradually during hospitalization in parallel to the decrease of the psychotic stress. These findings indicate that acute psychotic stress has adverse effects on glucose control in psychiatric nondiabetic patients. In contrast, the mean insulin sensitivity of the study group was directly correlated with psychotic stress, reaching its peak on admission and decreasing afterward. This suggests that acute psychotic stress may result in opposite effects on blood glucose levels that ultimately remain within normal limits. Nevertheless, it should be kept in mind that our data represent mean values of glucose, insulin, and the ...

The user, looking for the ultimate high, will generally inject the drug into the blood stream via a hypodermic syringe. In this way the user can induce a greater quantity of the drug into the body with a single, massive dose of up to a half gram, or roughly 100cc liquid meth in the syringe (Gard, 2004). Many who abuse the drug inject this much in a single dose in order to get as high as possible. This massive dosage effects the signal processes in the brain. Neurons transmit electrical signals in the brain during normal activity (Mastin, 2010).. After time, injecting methamphetamine in large doses several times per month for several years will cause a breakdown in the communicating structure of the brain. This breakdown may cause the user to suffer psychotic episodes. A psychotic episode, or psychosis, is a loss of contact with reality. The National Alliance of Mental Illness, in an article entitled, What is Psychosis, defines psychosis as The experience of loss of contact with reality, and ...

THE WITHDRAWAL. (1) Dosage tapering. There is absolutely no doubt that anyone withdrawing from long-term benzodiazepines must reduce the dosage slowly. Abrupt or over-rapid withdrawal, especially from high dosage, can give rise to severe symptoms (convulsions, psychotic reactions, acute anxiety states) and may increase the risk of protracted withdrawal symptoms (see Chapter III). Slow withdrawal means tapering dosage gradually, usually over a period of some months. The aim is to obtain a smooth, steady and slow decline in blood and tissue concentrations of benzodiazepines so that the natural systems in the brain can recover their normal state. As explained in Chapter I, long-term benzodiazepines take over many of the functions of the bodys natural tranquilliser system, mediated by the neurotransmitter GABA. As a result, GABA receptors in the brain reduce in numbers and GABA function decreases. Sudden withdrawal from benzodiazepines leaves the brain in a state of GABA-underactivity, resulting in ...
Nevirapine is a non-nucleoside reverse transcriptase inhibitor used to reduce the viral load in HIV infection. Its side effects include hepatotoxicity, gastrointestinal symptoms, and dermatological reaction.1 Efiravenz, another non-nucleoside reverse transcriptase inhibitor, has a similar structure to nevirapine and can cause insomnia and psychotic reactions.1 We report three cases of neuropsychiatric sequelae to nevirapine in patients with HIV infection but no history of mental illness. Medline, Embase, and PsychLIT list no reported cases.. Within two weeks of starting nevirapine a 35 year old man developed low mood and had to stop working because of cognitive impairment and clouding of consciousness. He was admitted after taking an overdose of nevirapine and the treatment was stopped. Five days later, fearing that nursing staff would kill him, he leapt through a third floor window. As the temporal connection to his deterioration was unclear, nevirapine treatment was restarted. After a two week ...
Too many bands these days try and fly before they can musically walk, the advent of good quality home recording gear has meant a lot of bands are putting out albums when they should still be a couple of years away from recording their first EP. Tons of bands that fall into this trap end up sounding more like Keith Harris and Orville at Felixstowe Spa Pavilion than Metalica at Donnington Monsters of Rock. Whilst the Psychotic Reaction have an undeniable lo-fi edge they totally validate the whole home recorded field. This album should stand as a shining example of what can be done with skill, talent, song writing ability and a limitless amount of time for recording. This band shits out smart ideas, and nothing seems forced. The whole album has a laid back vibe that (far from getting boring) keeps you very interested. The variety they present makes this album like some sort of journey. Imagine what Badly Drawn Boy would be like if his output was actually interesting. There are shadows of so many ...

The main finding of this study of patients with psychosis in Sweden was the markedly increased frequency of prediabetes, defined as FPG 6.1-6.9 mmol/L, in patients with psychosis compared with control subjects. In participants aged 50-59 years, prediabetes was observed in 15% of patients with psychosis but less than 5% of the control subjects. In addition, 64 (77%) patients with psychosis who had prediabetes satisfied the criteria for metabolic syndrome.. In Sweden, specialized psychosis clinics are responsible for the treatment of the vast majority of outpatients who have chronic psychotic disorders. As part of general health screening, all patients with psychosis in regular clinical treatment from several clinics were asked to participate in the study. Therefore, the results may be generalizable to the entire population of patients with psychosis. The control sample was population-based, but it was refined for family history of diabetes, and individuals with diabetes at baseline were excluded. ...
Another type of depression is bipolar disorder, also called manic-depressive illness. Not nearly as prevalent as other forms of depressive disorders, bipolar disorder is characterized by cycling mood changes: severe highs (mania) and lows (depression). Sometimes the mood switches are dramatic and rapid, but most often they are gradual. When in the depressed cycle, an individual can have any or all of the symptoms of a depressive disorder. When in the manic cycle, the individual may be overactive, overtalkative, and have a great deal of energy. Mania often affects thinking, judgment, and social behavior in ways that cause serious problems and embarrassment. For example, the individual in a manic phase may feel elated, full of grand schemes that might range from unwise business decisions to romantic sprees. Mania, left untreated, may worsen to a psychotic state ...

Psychosis is characterized as disruptions to a persons thoughts and perceptions that make it difficult for them to recognize what is real and what isnt. These disruptions are often experienced as seeing, hearing and believing things that arent real or having strange, persistent thoughts, behaviors and emotions. While everyones experience is different, most people say psychosis is frightening and confusing.. Psychosis is a symptom, not an illness, and it is more common than you may think. In the U.S., approximately 100,000 young people experience psychosis each year. As many as 3 in 100 people will have an episode at some point in their lives.. ...

This is the first reported case of acute steroid-induced psychosis caused by EUS-guided CPB. While steroid-induced psychosis after administration of systemic corticosteroids has been extensively reported since the introduction of corticosteroids in the early 1950s, psychosis from regional administration is much more rare.4 Episodes of acute psychosis following epidural, intramuscular, and intraarticular corticosteroid administration have been reported.5-7 To our knowledge, only two cases of acute psychosis following regional CPB have been reported.8 In both of these cases, the patients received an anesthesiologist-administered CPB with triamcinolone via a posterior approach under fluoroscopic guidance. Additionally, both patients also had a known history of steroid-induced psychosis from previous systemic corticosteroid administration.. Mercury is a neurotoxin that has no value in the human body. The effects of mercury poison on a person may vary from no reaction to nervous system problems such as psychotic reactions, hallucinations, suicidal tendencies and delirium. It is especially dangerous to unborn children and infants. There are ordinary household items that may contain mercury, such as fluorescent light bulbs and appliances, to button cell batteries used in watches and toys. Some materials used in construction also contain mercury, like gymnasium floors, thermostats, and boiler heating systems. To eliminate contamination of the air and water in the environment, mercury should be removed by professionals following all rules and regulations.. Risk Removal professionals are trained and certified in the proper abatement of mercury from your home, school, or commercial property. Post-acute-withdrawal syndrome (PAWS), or the terms post-withdrawal syndrome, protracted withdrawal syndrome, prolonged withdrawal syndromes describe a set of persistent impairments that occur after withdrawal from alcohol, opiates, benzodiazepines, antidepressants and other substances. Infants born to mothers who used substances of dependence during pregnancy may also experience a post-acute withdrawal syndrome. While post-acute withdrawal syndrome has been reported by those in the recovery community, there have been few scientific studies supporting its existence. Because of this, the disorder is not recognized by the Diagnostic and Statistical Manual of Mental Disorders or major medical associations. Drug abuse, including alcohol and prescription drugs, can induce symptomatology which resembles mental illness. This can occur both in the intoxicated state and during the withdrawal state. In some cases these substance-induced psychiatric disorders can persist long after detoxification, such as ...
IMPORTANCE: Longitudinal studies have linked the systemic inflammatory markers interleukin 6 (IL-6) and C-reactive protein (CRP) with the risk of developing heart disease and diabetes mellitus, which are common comorbidities for depression and psychosis. Recent meta-analyses of cross-sectional studies have reported increased serum levels of these inflammatory markers in depression, first-episode psychosis, and acute psychotic relapse; however, the direction of the association has been unclear. OBJECTIVE: To test the hypothesis that higher serum levels of IL-6 and CRP in childhood would increase future risks for depression and psychosis. Prodrome or at risk mental state clinics are specialist services for those with subclinical symptoms of psychosis or other indicators of risk of transition to psychosis. The Pace Clinic[22] in Melbourne, Australia, is considered one of the origins of this strategy,[23] but a number of other services and research centers have since developed.[24] [25] These services are able to reliably identify those at high risk of developing psychosis[26] and are beginning to publish encouraging outcomes from randomised controlled trials that reduce the chances of becoming psychotic,[27] including evidence that psychological therapy[28] and high doses of fish oil[29] have a role in the prevention of psychosis. However, a meta-analysis of five trials found that while these interventions reduced risk of psychosis after 1 year (11% conversion to psychosis in intervention groups compared to 32% in control groups), these gains were not maintained over 2-3 years of follow-up.[30] These findings indicate that ...

The pathophysiology of psychosis and other forms of severe mental illness in HIV infection is complex, and multifactorial causation is likely in most instances. Severe mental illness has been identified as a risk factor for the acquisition of HIV infection and occurs as both a manifestation of opportunistic infections and a result of the neurotropic effects of the virus. A full psychiatric assessment in people living with HIV / AIDS (PLWHA) presenting with psychosis is important but may prove difficult in many parts of South Africa. This paper presents a variety of algorithms to simplify the assessment and management of an HIV-infected patient with psychosis.

Specific talk about psychotic symptoms occurred about 1.4 times (range 0-4) per interaction, lasted on average 67 seconds, and was started by doctors on 21 occasions, by patients on 22 occasions, and by a carer on one occasion. In general, doctors tended to ask about the frequency of these symptoms or to refer to their severity when the patient was on different kinds of drugs, whereas patients actively attempted to talk about their psychotic symptoms. In addition to telling their troubles and describing their symptoms,9 patients also asked the doctors opinion about the cause of their troubles and about others disbelief in relation to their experience, accounted for why they had their symptoms or illness, or discussed the pros and cons of drugs with respect to the severity of their symptoms.. We identified two situations when the patient attempted to talk about their psychotic symptoms. Firstly, when the preceding interaction had created an opportunity for the patient to talk about their ...
