A retrospective medical record review was conducted in a general hospital with approximately 500 beds. Review for AE identification was based on the HMPS methodology including a 2-stage examination (Figure 1) [4,6]. First, specific dates in 2007 were randomly selected using a random number table, and admissions of all patients discharged on those dates were selected as index admission. Psychiatric department admissions were excluded. Medical records from the entire duration of hospitalization and 1 year before and after were reviewed using a case review form developed in a previous study [11]. The first-stage review conducted by nurses screened medical records for 41 SC. These criteria encompassed events with high possibility of AE occurrence, such as antidote use, or those highly likely to lead to occurrence of additional AEs. The case review form included 41 SC chosen through the modified Delphi method from a combination of previously used SC [11]. These criteria can be divided into 19 items ...
The global aim of our study is to validate eye movement recording as an early differential diagnostic tool, in order to discriminate as early as possible between neurodegenerative dementias of Alzheimer type and depressive pseudodementias (DPD). The investigators want to put forward idiosyncratic oculomotor characteristics of Alzheimers disease (AD) and DPD respectively. Eye movements are sensitive markers of neurological diseases and can be used in a variety of clinical neurological syndromes.. This study compares 3 groups of 98 patients: patients suffering of AD and DPD and healthy persons. The patients AD will be recruited in the Memory Centre of Resources and Research of Besançon and patients DPD will be selected in the psychiatric department of the University Hospital of Besançon. The control participants will be recruited from the entourage of researchers and patients. The selection of participants in the 3 different groups is based on clinical examinations in psychiatry and neurology ...

However, the alarming lack of any growth model covering the whole campus detracts from the architectural quality introduced in the Psychiatric Department. In the course of the conversion, for example, the temporary Surgery 5 wing was erected overnight on the boundary of the grounds and the wood. By basing it on a steel construction with prefab façade panels in which windows were cut out after installation, the production time was limited to barely three months of working day and night. This quick fix turned out to be necessary so as not to lose the subsidy allotted by the VIPA. The new temporary wing, which in principle must be demolished again after ten years, now forms the new horizon for the Psychiatry Department. In the light of this ad hoc spatial arrangement and disposable architecture it seems more necessary than ever to make an architectural and spatial masterplan part of the compulsory strategic care plan (ZSP). Though a commitment is needed from the Flemish Government Architect that ...

Having a primary care service for an acute psychiatric unit appears to help meet the numerous acute and long-standing physical health needs of patients. Previous work by Fisher & Roberts (1998) has highlighted the important role for a primary care doctor among long-stay patients. Our study suggests that patients in acute psychiatric wards can also benefit from primary care expertise. It also suggests that it is feasible to establish a primary care service, and that such a service usefully supplements the physical health care already provided and is welcomed by patients. Potential problems considered when the service was established did not really materialise. Some duplication of work occurred initially when patients were referred to specialist on-call services as well as to the primary care service, but feedback to staff minimised such events. An initial concern of staff was that the service would introduce an extra layer of complexity in the care of patients. In practice, this did not appear to ...
BACKGROUND Patient Advice and Liaison Services (PALS) have been set up in the National Health Service (NHS) in England to enable patients and the public to access information and raise issues they are concerned about their health care. The aim of this study was to examine to what extent, and how, PALS have promoted childrens, young peoples and parents access to and use of the service. METHODS A postal questionnaire was sent to PALS in 553 NHS Trusts and Primary Care Trusts (PCTs). PALS in Ambulance Trusts, and Specialist Trusts that were known not to provide services for children and young people, were excluded. RESULTS Questionnaires were returned by 320 respondents representing 369 trusts - a response rate of 67%. Seventy-seven responses were not valid, giving a final sample of 243 PALS. This included 122 PALS serving PCTs, 101 serving NHS Trusts, 15 serving a mix of both PCTs and NHS Trusts, and five serving childrens hospitals. The responses suggest that, to date, most PALS have been ...

The institution of an old age consultation/liaison team led to an improvement in service provision; there was an increased case-load but patients were seen more quickly and more often, in keeping with the shift from a consultation-oriented service to a liaison-orientated one; care of the elderly teams more often acted on advice given, and on discharge, fewer onward referrals to the old age CMHTs were made. Patients referred to the service represented the older old (over 80 years), were medically complex, had a greater number of medications at discharge, longer admissions and higher mortality.. The argument for specialist liaison input is strong but, despite this, 73% of services in the UK are provided according to a traditional sector-based model (Holmes et al, 2003).. In keeping with previous work (Scott et al, 1988; De Leo et al, 1989), the 3 Ds (dementia, delirium and depression) of old age psychiatry represented the main diagnoses. However, referral rates in this population still did not ...

Homeless, mentally ill, and three months pregnant, Tanya P. was arrested on New Years Day 1993 for trying to set fire to a shelter and for smoking crack. Police took her to the Bellevue Hospital psychiatric unit, where she voluntarily began in-house drug treatment and anti-psychotic medication. But a month before her baby was due, she demanded release. The hospital refused, under a city policy that allowed involuntary commitment for drug treatment during pregnancy. Represented by the New York Civil Liberties Union (NYCLU), Tanya sued, and persuaded Justice Glen to release her immediately. ...

A diagnosis of major mental illness is associated with a 15-20 year reduction in life expectancy. Individuals with mental illness face many difficulties accessing and receiving healthcare, many of these barriers exist in secondary care. On medical and surgical wards, the majority of mental healthcare is delivered by consultation liaison services. Hospital based psychiatry is increasingly important; well designed services are often cost-effective and can reduce patients length of stay. Some individuals care needs, however, exceed the capacity of such a service. There is a significant unmet need for individuals with severe co-morbid mental and physical illness, due to their increased lengths of stay, costs and readmission rate. In these cases both medical and psychiatric safety features form a prerequisite for the physical settings 4. This paper examines the need for shared care units (SCU), with additional mental health input. A Vision for Change proposes one adult liaison mental health ...

Following a hip fracture, nurse-led fracture liaison services or specialist consultant (orthogeriatrician) input both reduce deaths.
I made a formal complaint to PALS, the Patient Advice and Liaison Service (I later discovered that it was in fact PILS). They organised a meeting for me with the consultant and the Ward 2 nurse-in-charge. When I turned up for the meeting I found my mother distraught, sitting in her own faceas, half-undressed. She had been pressing the buzzer for 45 minutes but no one had come. I repeatedly shouted for a nurse but there were none. After a further 5 minutes the consultant arrived, found a nurse to clean my mother and took me into a meeting room. I was satisfied that he had put right all my complaints regarding her medical care but the Ward 2 nurse-in-charge did not attend the meeting and by this time I was even more concerned about my mothers care ...

I have bipolar disorder and Kaiser insurance. I have been with Kaiser for most of my life. The health care that is offered is very convenient for physical ailments. However, the Psychiatric Unit is horrendous. It is near impossible to get an appointment with a psychiatrist or a therapist. I have had to wait ridiculously long to see my psychiatrist. The wait is typically three months. They book their schedule three months in advance and they will put you on a waiting list if there are no more appointments planned. Once you finally get an appointment, you meet with your psychiatrist for 30 minutes max. My psychiatrist knows nothing about me, he doesnt make observations when I am there. I have to play doctor but I am not the one with a PHD in psychiatry. I say what is wrong and he prescribes me meds in excess. It is very important that people with mental disabilities have doctors that make judgment calls. They need to notice if something is out of the ordinary. Unfortunately, he doesnt know what ...

I first met Ron 10 years ago, when we were both hospitalized on a psychiatric ward. It was my first hospitalization for depression, whereas he had schizophrenia and had been hospitalized multiple times before. We connected instantly, despite the strong disapproval of the hospital staff, and formed an intense bond that would last for years.
Maybe someone messed up somewhere. Im not sure how it happened, but here I am fairly confident Im as omniscient as practically attainable, along the axes that omniscience usually refers to. Yet I have not spent enough time cultivating anything that I necessarily must defend; yet somehow Im here anyway.. So this body is especially vulnerable to suicide. There are many moments Id not hesitate to painlessly die if the option were materially available-but even that is a difficult thing. For whatever reason, Im still here. In the psychiatric ward, I was in a particularly hopeful and happy phase, perhaps from the novelty and various other very valuable things gained in the experience and interactions; so the staff asked me incredulously why I was there to begin with, and when I told the truth, nobody understood me, though there was a sense that they really really wanted to help, whatever that means.. Gandhis fundamental problems are lies, of course, so far as they are established in form. ...

The second edition of Handbook of Medicine in Psychiatry edited by Peter Manu and Corey Karlin-Sysman is in fact more a re-writing than an edition. Several new and highly skilled authors have joined the group and a useful grouping of chapters has been introduced. The handbooks obvious first priority is usability for the clinical psychiatrist. As it is further thoroughly updated and easily used because of a very specific build up of chapters this handbook will shortly be found on the shelves in every psychiatric department and clinic next to the diagnostic manuals where it can be reached by any psychiatrist, physician, medical student and nursing staff dealing with psychiatric patients. The book makes it totally clear to the reader that a psychiatrist must be a physician first, secondly a psychiatrist just as the book documents that psychiatry is truly holistic - mental illness is not only mind and brain but also soma. The pronounced excess mortality from physical diseases in mentally ill ...
The autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED) syndrome is a genetic disorder caused by a mutation in the autoimmune regulator (AIRE) gene. Immune deficiency, hypoparathyroidism and Addisons disease due to autoimmune dysfunction are the major clinical signs of APECED. We report on a 21-year-old female APECED patient with two inactivating mutations in the AIRE gene. She presented with sudden onset of periodic nausea. Adrenal insufficiency was diagnosed by means of the ACTH stimulation test. Despite initiation of hormone replacement therapy with hydrocortisone and fludrocortisone, nausea persisted and the patient developed cognitive deficits and a loss of interest which led to the diagnosis of depression. She was admitted to the psychiatric department for further diagnostic assessment. An EEG showed a focal epileptic pattern. Glutamic acid decarboxylase (GAD) antibodies, which had been negative eight years earlier, were now elevated in serum and in the cerebrospinal ...
To our knowledge, this is the first study describing pediatric mental health hospitalizations in a nationally representative sample from all hospitals admitting children and in the regionalized system of free-standing childrens hospitals. We found that mental health hospitalizations are common and costly nationally, with nearly 1 in 10 pediatric admissions having a primary diagnosis of a mental health condition. This finding exceeds national estimates of hospitalizations for asthma (7.1% for 1- to 17-year-olds).23 Aggregate charges for depression alone ($1.33 billion) are comparable to the $1.50 billion in aggregate charges for asthma in 2009.23. Our findings of a substantially lower proportion of mental health hospitalizations in free-standing childrens hospitals (3.3%) compared with hospitals admitting children nationally (9.6%) indicate that general hospitals deliver more inpatient psychiatric services than free-standing childrens hospitals. This difference in mental health care delivery ...

What Is Psychodiagnostic Testing? What is the difference between a psychodiagnostic assessment and a psychiatric consultation?. A psychiatric consultation consists of a thorough clinical interview, review of records, and observation of the patients behavior by a psychiatrist or psychologist. For many psychiatric concerns, this is the most appropriate referral and connects the patient with a mental health provider. Psychodiagnostic testing is a specialized diagnostic procedure that identifies and quantifies degrees of psychopathology. In contrast to a psychiatric consultation, it uses written, oral and projective instruments to evaluate a patients mental processes and to assess how their thinking and emotions are likely to impact their behavior. Therefore, psychodiagnostic testing provides objective data on a patients psychological functioning and is a useful tool for clarifying confusing clinical presentations.. How would psychodiagnostic testing help my patients? Psychodiagnostic testing ...
Chemical dependency is a common, chronic disease that affects up to 25 percent of patients seen in primary care practices. The treatment goal for patients recovering from chemical dependency should be to avoid relapse. This requires physicians to have an open, nonjudgmental attitude and specific expertise about the implications of addiction for other health problems. First-line treatment for chemical dependency should be nonpharmacologic, but when medication is necessary, physicians should avoid drugs that have the potential for abuse or addiction. Medications that sedate or otherwise impair judgment also should be avoided in the recovering patient. Psychiatric illnesses should be aggressively treated, because untreated symptoms increase the risk of relapse into chemical dependency. Selective serotonin reuptake inhibitors may help to lower alcohol consumption in depressed patients, and desipramine may help to facilitate abstinence in persons addicted to cocaine. If insomnia extends beyond the acute or

The use of the psychiatric care days flag in the Admitted Patient Mental Health Care NMDS enables identification of the subset of separations from hospitals that received treatment and care in a specialised psychiatric unit. While the flag does not distinguish acute and non-acute units, the vast majority of separations are attributable to acute units. A trimming process to isolate separations with extreme length of stay (e.g. , 365 days) can be used to approximate acute units. However, the data source cannot disaggregate acute psychiatric units by target population ...

The Norwegian Mental Health Care Act states that patients who are involuntarily admitted to a hospital must be reevaluated by a psychiatrist or a specialist in clinical psychology within 24h to assess whether the patient fulfills the legal criteria for the psychiatric status and symptoms. International research on the use of coercive hospitalization in psychiatry is scarce, and an investigation of Norways routine re-evaluation of involuntarily referred patients may expand knowledge about this aspect of psychiatric treatment. The aim of this study was to investigate the extent to which Involuntarily Hospitalized (IH) patients were converted to a Voluntary Hospitalization (VH), and to identify predictive factors leading to conversion. The Multi-center Acute Psychiatry study (MAP) included all cases of acute consecutive psychiatric admissions across twenty Norwegian acute psychiatric units in health trusts in Norway across 3months in 2005-06, representing about 75% of the psychiatric acute ...

Hospital-based programs that provide diagnostic and treatment services for individuals who have acute psychiatric disorders, who require hospitalization on a voluntary or involuntary basis for maximum benefit, and who might be a threat to themselves, to their families or to others if left in the community or placed in a less restrictive treatment setting. The objective of inpatient mental health programs is to stabilize and then return the individual to the community for ongoing treatment as quickly as possible, and to facilitate the transition by preparing the individuals family to cope with the limitations imposed by the illness and by arranging for other supportive services as required. My parents both tattooed me in August last year. Which Im so grateful for.. Can you tell us a little bit about your decision to let your mum go? Oh man this is a hard one. On February 17th I was working the London Tattoo Collective. At 10.30am, my phone rings and its Mums ward. She was on her way to hospital as they couldnt wake her up, she was unconscious from going into a diabetic hypo. She stayed in hospital for two weeks where they got her eating, they then sent her back to the psychiatric ward where within days she was rushed back into hospital as she was unconscious again.. Since about January, Mum had stopped walking and being able to feed herself through the meds not working and lack of support in the ward. So by this time she had been bed bound for a month. The hospital where mum now was ran test after test and found nothing, she was a little more conscious but she wasnt talking or opening her eyes. We celebrated her birthday on March 10th, she was 73. She was now being fed ...

Is the awkward silence something normal for them and their dad? How long has it been since theyve seen their mom? This is all stuff you could give us in your protagonists POV that could help us connect to your MC. ...
