A retrospective medical record review was conducted in a general hospital with approximately 500 beds. Review for AE identification was based on the HMPS methodology including a 2-stage examination (Figure 1) [4,6]. Having a primary care service for an acute psychiatric unit appears to help meet the numerous acute and long-standing physical health needs of patients. Previous work by Fisher & Roberts (1998) has highlighted the important role for a primary care doctor among long-stay patients. Our study suggests that patients in acute psychiatric wards can also benefit from primary care expertise. It also suggests that it is feasible to establish a primary care service, and that such a service usefully supplements the physical health care already provided and is welcomed by patients. Potential problems considered when the service was established did not really materialise. Some duplication of work occurred initially when patients were referred to specialist on-call services as well as to the primary care service, but feedback to staff minimised such events. BACKGROUND Patient Advice and Liaison Services (PALS) have been set up in the National Health Service (NHS) in England to enable patients and the public to access information and raise issues they are concerned about their health care. The aim of this study was to examine to what extent, and how, PALS have promoted childrens, young peoples and parents access to and use of the service. METHODS A postal questionnaire was sent to PALS in 553 NHS Trusts and Primary Care Trusts (PCTs). PALS in Ambulance Trusts, and Specialist Trusts that were known not to provide services for children and young people, were excluded. RESULTS Questionnaires were returned by 320 respondents representing 369 trusts - a response rate of 67%. Seventy-seven responses were not valid, giving a final sample of 243 PALS. The autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED) syndrome is a genetic disorder caused by a mutation in the autoimmune regulator (AIRE) gene. Immune deficiency, hypoparathyroidism and Addisons disease due to autoimmune dysfunction are the major clinical signs of APECED. We report on a 21-year-old female APECED patient with two inactivating mutations in the AIRE gene. She presented with sudden onset of periodic nausea. Adrenal insufficiency was diagnosed by means of the ACTH stimulation test. Despite initiation of hormone replacement therapy with hydrocortisone and fludrocortisone, nausea persisted and the patient developed cognitive deficits and a loss of interest which led to the diagnosis of depression.
