Objective: We sought to assess the hazard ratio (HR) of incident overall malignancy and incident malignancy by subtype in patients with chronic pruritus during the 5 years after diagnosis. Methods: A population-based cohort study was performed in the Health Improvement Network. In all, 8744 patients with chronic pruritus were matched with 31,580 patients without chronic pruritus based on sex, age, and practice. Primary outcomes were HR of incident malignancy and HR of malignancy subtypes. Results: The fully adjusted HR for incident malignancy in patients with chronic pruritus was 1.14 (95% confidence ...
The severity of pruritus and the extent and severity of erythema were quantified in 107 dogs presenting with various dermatoses. Pruritus was assessed using a previously validated scale, and erythema was quantified by assessing severity at 72 different body sites. Pruritus scores were either 0, or followed a normal type of distribution, with most dogs having a score in the middle of the range and a few dogs having low or high scores. The median pruritus score was 6.3 ⁄ 10. Erythema scores were heavily skewed towards lower values, with only a few dogs having high scores. The median diffuse erythema score was 6.0 ⁄ 216 and the median score for maculo-papular ⁄ pustular erythema was 0 ⁄ 1080. Pruritus and erythema scores were significantly correlated with a Spearman rank correlation coefficient of 0.4062 (P , 0.001). However, visual assessment of the data representing the two variables revealed that this was not a consistent biological or clinically relevant correlation. Individual dogs ...
Mycosis Fungoides (MF) is a rare malignancy in the United States. It is the most common form of cutaneous T-cell lymphoma (CTCL). Sézary syndrome (SS) is the most severe and leukemic form of CTCL. Severe pruritus is a manifestation of the SS and also affects patients with MF. Severe pruritus is a manifestation of SS and also a common symptom among patients with MF. Pruritus can significantly impact on quality of life. In one national survey of the impact of MF on patients health-related quality of life (HRQOL), the majority of survey respondents (88%) reported being bothered by pruritus.. The pathogenesis of pruritus is complex and not well understood. It affects a majority of patients with mycosis fungoides (MF) and is most severe in Sézary syndrome (SS), the leukemic form of cutaneous T-cell lymphoma. Chemical or physical stimuli that perturb, without damaging, only the epidermis (the outer most layer of the skin), elicit the symptom of pruritus. The perception of itch is abolished if the ...
Background.. Uremic pruritus is still a common phenomenon in patients with end-stage renal failure. It is regarded as one of the most bothersome symptoms in patients on chronic dialysis.. The pathogenesis of uremic itch remains unclear. Several theories have been proposed like dryness of the skin, imbalance in divalent ions, peripheral neuropathy and others.. Many treatment modalities have been tried to reduce uremic pruritus, however , the majority of them produced only temporary improvement. Therefore , every new therapeutic option for uremic pruritus is desirable.. This study will undertaken to evaluate the efficacy of a cream which included olive oil and omega-3 fatty acids.. Olive Omega-3 is an efficient product that soothes itchiness of cracked and very dry skin.. The product is unique and based on patented technology developed by the Technion Research and Development Foundation in Haifa.. Olive Omega-3 is an ointment composed of all natural ingredients. The main active ingredients are ...
If your dog spends a large portion of his time scratching, he may have a condition known as pruritus, or itching, an unpleasant sensation that causes your dog to scratch or bite at himself. It is caused by chemical reactions that occur in the skin and stimulate the nerves, causing the brain to feel the itch. In fact, the act of scratching itself may stimulate these inflammatory reactions in the skin and make the condition worse. Any skin condition that causes inflammation can cause pruritus.. How pruritus affects your dogs health depends on the degree of the pruritus. Mild pruritus may hardly have any effect at all. However, severe pruritus leads to intense scratching, which may result in painful skin lesions that may become infected. Your dog will often whimper or cry out and may have trouble sleeping ...

To our knowledge, this is the first study to investigate the association of pruritus with QOL and disability in SSc. The presence of pruritus on most days in the past month was significantly associated with poorer mental and physical function and greater disability, with a small effect size difference for disability and medium effect size differences for mental and physical health. Typically, a medium standardized difference effect size (approximately 0.50) is considered to be clinically meaningful (41). After controlling for sociodemographic and other disease variables, the presence of pruritus was significantly associated with mental and physical health, but not disability. The magnitude of the independent association of pruritus with QOL outcomes in multivariate analyses was small, but it is notable that it was significantly associated with QOL even after controlling for a large number of variables known to have robust associations with QOL and disability in SSc.. The results of this study ...

Forty-five outpatients with metastatic solid tumors treated with cetuximab, erlotinib, gefitinib, imatinib, or sunitinib were enrolled and treated with a short course of aprepitant. The study showed that aprepitant significantly decreased the severity of pruritus induced by biological anticancer treatments and could be a useful antipruritic agent both as the first-choice treatment or after failure of standard antipruritic therapy (Table?2) [27]. In another retrospective, analytical study, promising ...
Aquagenic pruritus is a skin condition characterized by the development of severe, intense, prickling-like epidermal itching without observable skin lesions and evoked by contact with water. Symptoms occur for up to 35 minutes; duration of an attack is typically between 10 and 120 minutes. However, sensitivity varies among sufferers, and since water is always present to some extent in the air (atmospheric humidity), those with greater sensitivity who live in moister regions are symptomatic almost constantly, while perspiration can cause frequent symptoms even in the driest climates. No definitive medical test is known for aquagenic pruritus. Rather, the diagnosis is made by excluding all other possible causes of the patients itching. Some of which includes polycythemia vera. Since pruritus is a symptom of many serious diseases, it is important to rule out other causes before making a final diagnosis. The name is derived from Latin: aquagenic, meaning water-induced, and pruritus, meaning itch. ...

in nociceptors expressing transient receptor potential vanilloid 1 (TRPV1). BRAFNav1.8 mice showed de novo neuronal responsiveness to pruritogens, enhanced pruriceptor excitability, and heightened evoked and spontaneous scratching behavior. GRP receptor expression was increased in the spinal cord, indicating augmented coding capacity for itch subsequent to amplified pruriceptive inputs. Enhanced GRP expression and sustained ERK phosphorylation were observed in sensory neurons of mice with allergic contact dermatitis- or dry skin-elicited itch; however, spinal ERK activation was not required for maintaining central sensitization of itch. Inhibition of either BRAF or GRP signaling attenuated itch sensation in chronic itch mouse models. These data uncover RAF/MEK/ERK signaling as a key regulator that confers a subset of nociceptors with pruriceptive properties to initiate and maintain long-lasting itch sensation. ...

The mechanisms that generate itch are poorly understood at both the molecular and cellular levels despite its clinical importance. To explore the peripheral neuronal mechanisms underlying itch, we assessed the behavioral responses (scratching) produced by s.c. injection of various pruritogens in PLCbeta3- or TRPV1-deficient mice. We provide evidence that at least 3 different molecular pathways contribute to the transduction of itch responses to different pruritogens: 1) histamine requires the function of both PLCbeta3 and the TRPV1 channel; 2) serotonin, or a selective agonist, alpha-methyl-serotonin (alpha-Me-5-HT), requires the presence of PLCbeta3 but not TRPV1, and 3) endothelin-1 (ET-1) does not require either PLCbeta3 or TRPV1. To determine whether the activity of these molecules is represented in a particular subpopulation of sensory neurons, we examined the behavioral consequences of selectively eliminating 2 nonoverlapping subsets of nociceptors. The genetic ablation of MrgprD(+) ...

Itching (pruritus) is a common symptom associated with numerous skin diseases, as well as a secondary symptom of numerous serious conditions such as renal failure and liver disease. Itching, unlike other skin sensations, is generally a result of CNS activities and typically goes untreated by standard medical therapies.. The endocannabinoid system is acknowledged to play an important role in maintaining skin health[1] and cannabinoids have been regarded as promising agents for the treatment of itch.[2]. A review of the scientific literature identifies at least three clinical trials investigating the use of cannabinoids in the treatment of pruritus. Writing in the August 2002 issue of the American Journal of Gastroentrology, investigators from the University of Miami Department of Medicine reported successful treatment of pruritus with 5 mg of THC in three patients with cholestatic liver disease.[3] Prior to cannabinoid therapy, subjects had failed to respond to standard medications and had lost ...
A recent study carried out by Dr. Sun Yangangs lab at the Institute of Neuroscience of the Chinese Academy of Sciences discovered a central neural circuit that is critical for transmitting the itch signal. By using optogenetic, chemogenetic, patch clamp recording, and in vivo fiber photometry techniques, the researchers demonstrated that the spino-parabrachial pathway plays a key role in transmitting itch signals from the spinal cord to the brain, and identified the parabrachial nucleus as a first central relay for the itch sensation.

Pruritus is a debilitating symptom that can be associated with cutaneous involvement by an underlying malignancy. We report the case of a 68-year-old woman with a history of triple-negative breast cancer who presented with extensive, localised cutaneous metastasis complicated by incapacitating, treatment-refractory pruritus localised to the anatomic regions involved by her metastatic disease. Physical examination revealed an indurated, ecchymotic, ulcerated plaque circumferentially encasing her thorax. Histopathological examination revealed substantial dermal lymphatic involvement and dilation as well as dermal collagen infiltration by tumour cells and nodules. The clinical and pathological findings were consistent with a diagnosis of carcinoma en cuirasse. Mirtazapine, a noradrenergic antidepressant with antiserotonin and antihistamine activity, was started and led to rapid, sustained relief of the patients pruritus. Mirtazapine may be a useful systemic agent for the palliative relief of ...

Itch is an unpleasant sensation that evokes behavioral responses such as scratching the skin. Interestingly, it is conceived that the perception of itch sensation is influenced by age. Indeed, accumulating evidence supports the idea that even children or younger adults show distinctive itch sensation depending on age. This evidence implies the presence of a mechanism that regulates the perception of itch sensation in an age-dependent fashion. Therefore, the purpose of the present study was to investigate a putative mechanism for the age-dependent perception of itch sensation by comparing histamine-induced scratching behaviors in 45-day old (D45) and 75-day old male young adult mice ...
A new study has shown that Gabapentin can be an effective drug against uremic pruritus suffered by patients with advanced chronic kidney disease. The study was also suggested that Gabapentin may also be the only effective drug against it. (1). Uremic pruritus is a disease characterized by continuous bouts of itching throughout the body. It often worsens during the night causing patients sleeplessness or insomnia. Separate studies have shown that almost 46% of patients undergoing dialysis suffer from uremic pruritus.. This comes after the research team claimed that other treatments tested for the disease were weak and limited by small studies with a high risk of bias. The team was led by Claudio Rigatto, MD, MSc, of the University of Manitoba in Winnepeg, Canada. Their statement was reported online ahead of print in the American Journal of Kidney Diseases.. The study was conducted by assessing patients with chronic kidney disease, stage 3 or higher, or undergoing any form of dialysis on 44 ...
Hypothesis: Pruritus or itch is the most common skin symptom in elderly patients and is estimated to affect more than 30% of nursing home patients. Clinical and experimental evidence suggests pruritus in elderly patients may be linked to pemphigoid. Pemphigoid is the most common autoimmune skin blistering diseases and mainly affects the elderly. It is successfully treatable with systemic therapy. However, pemphigoid is often missed as a cause of pruritus in elderly patients. Although nursing home patients and patients with dementia in particular have the highest risk for development of pemphigoid, no study has been performed in this population so far. Including serological screening for pemphigoid in the diagnostic evaluation of chronic pruritus in nursing home patients may lead to the diagnosis of pemphigoid. Furthermore, chronic itch may be an unrecognized cause of neuropsychiatric symptoms in nursing home patients with dementia ...

Here, we identify the BRAF pathway as an upstream regulator of expression of a cohort of itch mediators in mouse sensory neurons. Our data further demonstrate that GRP/GRPR signaling plays an important role in the initiation and maintenance of long-lasting itch in 3 distinct genetic and/or clinically relevant chronic itch models. Finally, our data suggest that a phenotypic switch from nociceptors to pruriceptors underlies the transition from acute itch signaling to pathological itch state.. BRAFNav1.8 mice are a new and unique genetic model for unraveling how pruriceptive transmission is progressively enhanced at the molecular and circuit levels, as increasing scratching behavior correlates well with an expression profile change of itch mediators. The fact that enhanced evoked scratching preceded the manifestation of spontaneous itch (from 3 weeks of age to around 6 weeks of age) suggests the existence of a postnatal period for developmental maturation of itch circuitry as well as a threshold ...
Background Patients who suffer from chronic itch employ creative techniques to alleviate their itch, often using painful thermal stimuli, such as hot and very cold showers, as well as mechanical stimuli, such as scratching.. Objectives The present study examined whether the sensory perception of itch is attenuated by remote interactions between both thermal and mechanical stimuli and afferent information related to itch.. Patients and methods Itch was induced with histamine iontophoresis in 21 healthy young subjects. Repetitive thermal stimuli including innocuous warmth, innocuous cool, noxious cold and noxious heat as well as scratching were applied 3-cm distal to the area of histamine iontophoresis. Subjects rated their perceived intensity of histamine-induced itch with a computerized visual analogue scale.. Results Itch intensity ratings were significantly reduced during each period of scratching and repeated noxious heat and cold. Innocuous cooling and warming did not significantly alter ...

There does not appear to be any one underlying cause for delusional parasitosis, although it probably has to do with fundamental aspects of brain chemistry. This syndrome is often classified into three types according to the underlying condition as below. However, there is increasing literature on itch and chronic pruritus (itch) that indicate that delusional parasitosis may not be a psychiatric syndrome per se and instead may have a fundamental origin in the itch nerve pathways between the skin and the brain. There are many physiological syndromes that cause the symptoms ascribed to delusional parasitosis.. Delusional Infestation and Chronic Pruritus: A Review Primary Psychotic Delusional Parasitosis ...

Cholestatic pruritus is the sensation of itch due to nearly any liver disease, but the most commonly associated entities are primary biliary cirrhosis, primary sclerosing cholangitis, obstructive choledocholithiasis, carcinoma of the bile duct, cholestasis (also see drug-induced pruritus), and chronic hepatitis C viral infection and other forms of viral hepatitis. Cholestasis means the slowing or stopping of bile flow which can be caused by any number of diseases of the liver (which produces the bile), the gallbladder (which stores the bile), or biliary tract (also known as the biliary tree, the conduit that allows the bile to leave the liver and gallbladder and enter the small intestine). When this occurs, conjugated bilirubin and the waste products that usually would be cleared in bile reflux back into the bloodstream. This causes a primarily conjugated hyperbilirubinemia and jaundice; the liver conjugates the bile to make it water-soluble and because the bile has already been processed by ...

Introduction: Neuropathy is a frequent complication of uremic disease, causing motor and sensorial symptoms, mostly localized in the lower limbs. Another symptom related to uremic disease is pruritus, which may be diffuse all over the body, but usually occurring in the lower limbs. In the terminal phase therapeutic measures are taken, targeting the sensitive components of neuropathy, with the use of drugs against pain, and targeting itching, using different substances, the most widely used of which is gabapentin. Methods: The severity of neuropathy is variable, ranging from slight to severe and may or may not be associated with the length of chronic dialysis treatment. The use of measurement scales of pain, VAS type adapted to the itching, may link both conditions of neuropathy and itching. Results: We noted that a severe intensity of pruritus can be related to severity of neuropathy in uremic subjects and may become predictive of progressing neuropathy. Conclusions: In this study we examined a group

Pruritus is a common manifestation of dermatologic diseases, including xerotic eczema, atopic dermatitis, and allergic contact dermatitis. Effective treatment of pruritus can prevent scratch-induced complications such as lichen simplex chronicus and impetigo. Patients, particularly elderly adults, with severe pruritus that does not respond to conservative therapy should be evaluated for an underlying systemic disease. Causes of systemic pruritus include uremia, cholestasis, polycythemia vera, Hodgkins lymphoma, hyperthyroidism, and human immunodeficiency virus (HIV) infection. Skin scraping, biopsy, or culture may be indicated if skin lesions are present. Diagnostic testing is directed by the clinical evaluation and may include a complete blood count and measurement of thyroid-stimulating hormone, serum bilirubin, alkaline phosphatase, serum creatinine, and blood urea nitrogen levels. Chest radiography and testing for HIV infection may be indicated in some patients. Management of nonspecific pruritus
Itching of the anus is a common condition afflicting up to 5% of the population.2 The incidence of primary and secondary pruritus ani varies among different studies. Several studies show that the incidence of idiopathic pruritus ani is 75-95%.9 Other authors have found that its incidence is only 25%.10 Characteristically, pruritus ani is exacerbated by friction or a warm moist perineal environment. Poor anal hygiene or, in contrast, overcleansing with soap is often a contributing factor. Patients with idiopathic pruritus ani have been found to have abnormal transient internal sphincter relaxation11 and an abnormally profound decrease in anal canal pressure during rectal distention, as well as early incontinence on saline continence tests, implying that intermittent seepage from the anal canal may be a causative factor.12 A decrease in resting anal canal pressure with coffee intake has also been reported13 which may explain the documented exacerbation of symptoms with ingestion of caffeine. ...

Itch is triggered by somatosensory neurons expressing the ion channel TRPV1 (transient receptor potential cation channel subfamily V member 1), but the mechanisms underlying this nociceptive response remain poorly understood. Here, we show that the neuropeptide natriuretic polypeptide b (Nppb) is expressed in a subset of TRPV1 neurons and found that Nppb−/− mice selectively lose almost all behavioral responses to itch-inducing agents. Nppb triggered potent scratching when injected intrathecally in wild-type and Nppb−/− mice, showing that this neuropeptide evokes itch when released from somatosensory neurons. Itch responses were blocked by ...
Being one of the most common dermatological problems, pruritus ani is often defined as an unpleasant skin sensation. Though it is not a life threatening ailment, yet the great urge to scratch which occurs due to this problem is highly irritating. Find out in detail about the causes and treatment here. What does the term pruritus means? Basically, it is a medical term which refers to itching… which can make you feel worse at anytime of the day! Do you know it can results in damage and scarring on the skin surface? Yes, due to the damage of the skin surface, it then serves as an entry point for the microbes. Hence, increasing the chances of infection by manifold! As far as the occurrence of pruritus on the skin is concerned, it might be caused due anything ranging from mild skin disorder to a highly serious systemic disease condition. And on the other hand, irritation or itching may be induced due to any material, when the infested come into contact physically. Now the question arises; What is
Lateral habenula (LHb) is a brain region acting as a hub mediating aversive response against noxious, stressful stimuli. Growing evidences indicated that LHb modulates aminergic activities to induce avoidance behavior against nociceptive stimuli. Given overlapped neural circuitry transmitting pain and itch information, it is likely that LHb have a role in processing itch information. Here, we examined whether LHb is involved in itchy response induced by histamine. We found that histamine injection enhances Fos (+) cells in posterior portion within parvocellular and central subnuclei of the medial division (LHbM) of the LHb. Moreover, chemogenetic suppression of LHbM reduced scratching behavior induced by histamine injection. These results suggest that LHb is required for processing itch information to induce histaminergic itchy response.

Its etiology is not known and treatment is difficult. This symptom considerably worsens the patients quality of life. OBJECTIVES: To evaluate the degree of pruritus and its influence on the quality of life of patients on hemodialysis. METHODS: Patients with chronic renal disease on hemodialysis and presenting pruritus in a Hospital of Porto Alegre were enrolled in this transversal study. A visual analogue scale for measuring the degree of pruritus was used and the Dermatology Life Quality Index questionnaire was applied. RESULTS: Of 200 evaluated patients; 69 (34.5%) presented pruritus, of whom 16 (23%) of mild degree, 39 (56.5%) moderate, 14 (20.2%) severe. Quality of life ...
Rifampicin is a semisynthetic antibiotic derived from the rifamycins, a group of antibacterials produced by Streptomyces mediterranei. Rifampicin was initially developed to treat tuberculosis but more recently has been used against other bacteria. The beneficial effect of rifampicin on pruritus was initially discovered serendipitously. Four formal studies (three of which were randomised controlled trials) have suggested a subjective effect of rifampicin on itch severity.10-13 These studies also suggested that rifampicin was better at relieving itch than other hepatic enzyme inducing agents such as phenobarbitone.10 A single small study failed to confirm the effectiveness of rifampicin for subjective itching in cholestatic liver disease (including three patients with PBC).18 All of these studies used subjective measures of itch severity (for example, visual analogue scales). To our knowledge no trial of rifampicin in cholestasis has examined efficacy in terms of objective changes in scratching ...

N2 - Atopic dermatitis (AD) is a common, chronic, relapsing, inflammatory skin disease that affects both children and adults. AD is the cause of considerable morbidity including severe pruritus and impaired quality of life. Treatments for active disease include avoidance of triggering factors, barrier repair, topical medications including topical corticosteroids (TCs) and topical calcineurin inhibitors (TCIs), phototherapy, antibacterial agents, and systemic immunosuppressants including cyclosporine. Until recently, the only Food and Drug Administration (FDA)-approved systemic treatment options for patients with moderate-to-severe AD were steroids and ...

Botanix Pharmaceuticals has today updated the market with plans for its Atopic Dermatitis (AD) clinical program. This follows important recent learnings on drug dose and formulation design of synthetic CBD in dermatitis applications, together with positive data from a new pilot study conducted in canines with AD.. Canine atopic dermatitis is clinically and immunologically similar to human AD, providing an efficient approach to inform progression of further human studies too.. A recent pilot study of Botanixs BTX 1204A product in canines (a new higher dose formulation of synthetic CBD for atopic dermatitis, leveraging PermetrexTM), demonstrated significant reductions on average in both the Enhanced Pruritus Score (ESP) and Canine Atopic Dermatitis Extent and Severity Index (CADESI-04) scores over the 28-day treatment period.. Botanix is now advancing plans for a larger BTX 1204A proof of concept canine study in Australia, with goals to confirm and expand safety and efficacy data. Following this, ...

When Hemorrhoids Itch Are They Healing? Itching is one of the most common symptoms. In most cases, hemorrhoids itch because of mucus, feces and bad hygiene not because they are healing. In most cases, proper early treatment can take care of the condition.
Orphan receptor. Probably involved in the function of nociceptive neurons. May regulate nociceptor function and/or development, including the sensation or modulation of pain. Potently activated by enkephalins including BAM22 (bovine adrenal medulla peptide 22) and BAM (8-22)(PubMed:26582731). BAM22 is the most potent compound and evoked a large and dose-dependent release of intracellular calcium in stably transfected cells. G(alpha)q proteins are involved in the calcium-signaling pathway. Activated by the antimalarial drug, chloroquine. May mediate chloroquine-induced itch, in a histamine-independent manner.
