Recalls: Visit us for regular six-month examinations. Often, small problems that are developing around the restorations can be found at an early stage and corrected easily, but waiting for a longer time may require redoing the entire restoration. Inadequate return for examination is the most significant reason for prostheses failure. We will contact you when it is time for your recall appointment ...
In the mid-2000s, before the orthopaedic community was aware of the potentially catastrophic shortcomings of metal-on-metal (MoM) hip implants, nearly half of all hip replacements used MoM components. With so many people walking around today on those prostheses, orthopaedists are looking for rational and effective ways to monitor these patients for adverse reactions to metal debris. Findings from…
This paper examines the determinants of the time it takes for an index options market to be brought back to efficiency after put-call parity deviations, using intraday transactions data from the French CAC 40 index options over the August 2000 - July 2001 period. We address this issue through survival analysis which allows us to characterize how differences in market conditions influence the expected time before the market reaches the no-arbitrage relationship. We find that moneyness, maturity, trading volume as well as trade imbalances in call and put options, and volatility are important in understanding why some arbitrage opportunities disappear faster than others. After controlling for differences in the trading environnement, we find evidence of a negative relationship between the existence of ETFs on the index and the time to ...
The rationale for this result (coincident with other studies) is that Europeans (or our Asian ancestors) went through one or more population bottlenecks when diversity was pretty small, allowing these defects to persist, even many milennia later. In contrast African populations have always been pretty large, forcing these potentially harmful genes to become less frequent through normal natural selection ...
A new Roentgen stereophotogrammetric analysis system, using a biplane technique, has been developed to determine the migration and rotation of total hip replacement (THR) femoral components in three dimensions. Stainless steel marker balls were injected into the femur during the operation. The patients stood within a calibration frame during the X-ray. The two exposures were taken consecutively allowing radio-opaque shutters to be moved in front of the films to prevent fogging. Studies with a model demonstrated that the system was capable of measuring the position of an implant to better than 0.11 mm (2 SD). In vivo measurements demonstrated that the migration rate of the different parts of the femoral component could be determined with an accuracy of 0.25 to 0.50 mm/year. By considering the accuracy determined in different ways, methods for improving the system have been identified. The migration and rotation rate of 58 Hinek cemented femoral components was studied for four years. Migration was three
We performed a clinical and radiological study to determine the rate of failure of the Charnley Elite-Plus femoral component. Our aim was to confirm or refute the predictions of a previous roentgen stereophotogrammetric analysis study in which 20% of the Charnley Elite-Plus stems had shown rapid posterior head migration. It was predicted that this device would have a high early rate of failure. We examined 118 patients at a mean of nine years after hip replacement, including the 19 patients from the original roentgen stereophotogrammetric study. The number of revision procedures was recorded and clinical and radiological examinations were performed. The rate of survival of the femoral stems at ten years was 83% when revision alone was considered to be a failure. It decreased to 59% when a radiologically loose stem was also considered to be a failure. All the patients previously shown in the roentgen stereophotogrammetric study to have high posterior head migration went on to failure. There was a highly

Metallosis is the putative medical condition involving deposition and build-up of metal debris in the soft tissues of the body. Metallosis has been hypothesized to occur when metallic components in medical implants, specifically joint replacements, abrade against one another. Metallosis has also been observed in some patients either sensitive to the implant or for unknown reasons even in the absence of malpositioned prosthesis. Though rare, metallosis has been observed at an estimated incidence of 5% of metal joint implant patients over the last 40 years. Women may be at slightly higher risk than men. If metallosis occurs, it may involve the hip and knee joints, the shoulder, wrist, or elbow joints. The abrasion of metal components may cause metal ions to be solubilized. The hypothesis that the immune system identifies the metal ions as foreign bodies and inflames the area around the debris may be incorrect because of the small size of metal ions may prevent them from becoming haptens. Poisoning ...

We performed this investigation to determine the possible migration starting immediately after surgery and the effect of different weightbearing regimens on the migration pattern of an uncemented hip stem (CLS). Stem migration was determined with radiostereometry analysis with baseline when the patients still were anesthetized. Subsequent examinations were done up to 1 year. Twenty-nine patients (mean age, 55 years; range, 26-63 years) were randomized to either unrestricted weightbearing combined with intensive physiotherapy from the first day after surgery or to partial weightbearing and a conservative training regimen for the first 3 months after surgery. At 1 week, subsidence was -0.03 mm in the unrestricted weightbearing group and 0.01 mm in the partial weightbearing group. At 1 year, subsidence was 1.01 mm in the unrestricted weightbearing group and 0.51 mm in the partial weightbearing group. One patient in the unrestricted weightbearing group had revision surgery because of aseptic ...
Materials and methods: A total of 49 patients, who required at least two implants, had two sites randomised according to a split-mouth design to receive one abutment of each type at seven different centres. The time of loading (immediate, early or delayed) and of prosthesis (provisional crowns of fixed prosthesis) was decided by the clinicians, but they had to restore both implants in a similar way. Provisional prostheses were replaced by definitive ones 3 months after initial loading, when the follow-up for the initial part of this study was completed. Outcome measures were: prosthesis failures, implant failures, complications, pink esthetic score (PES), peri-implant marginal bone level changes, and patient preference ...

Processing techniques for various surface mount modular components provide various structures for single device components (10) and multiple device components (FIGS. 6 and 7) suitable as character displays. The technique beings with a slab of substrate material 12 patterned on both sides. Plated through holes (33, 43) connecting back side terminals (19, 20) to front side connective strips (22, 24) are formed. Devices (15, 16) are mounted to land areas (13, 34) and wire bonded to connecting pads (14). The front side is coated with epoxy to encapsulate the devices in a layer having an outer surface formed into optional lenses (262, 263). The slab is then separated to provide the modular components.

The paper presents an approach to a cost-efficient modularly built non-dispersive optical IR-gas sensor (NDIR) based on a construction kit. The modularity of the approach offers several advantages: First of all it allows for an adaptation of the performance of the gas sensor to individual specifications by choosing the suitable modular components. The sensitivity of the sensor e.g. can be altered by selecting a source which emits a favorable wavelength spectrum with respect to the absorption spectrum of the gas to be measured or by tuning the measuring distance (ray path inside the medium to be measured). Furthermore the developed approach is very well suited to be used in teaching. Together with students a construction kit on basis of an optical free space system was developed and partly implemented to be further used as a teaching and training aid for bachelor and master students at our institute. The components of the construction kit are interchangeable and freely fixable on a base plate. ...

Risk for revision surgery remained higher among women in the study after considering factors specific to the patient, surgeon, surgery, and type of implant.
Not quite sure when its a revision. Is it a revision if the surgeon fuses the same vertebrae over again, probably because something went wrong the first time? Or is the definition broader? E.g., say you have your lumbar fused, then your thorax starts acting up (adding on, taking on new curvature) and your surgeon fuses that as well. Is that considered a revision?

and that yamakasi catleap is not the same for overclocking as the x-star and qnix panels happen to be. did the catleap have multi-inputs? thats going to add latency and hurt response time. there was one very special version of the catleap that was stable up to around 100hz but the price became super inflated on them as it was only a certain early revision that would overclock successfully. since then the newer x-star and qnix panels with single dvi-d inputs have proved to be very decent overclockers. 96hz is pretty much guaranteed and the panels response time is very fast if you go with the single input version. newer panels are popping up everyday so we will have to see if they continue to improve in this area ...

TY - JOUR. T1 - Freeze-dried proximal femur allografts in revision total hip arthroplasty. A preliminary report. AU - Head, W. C.. AU - Malinin, T. I.. AU - Berklacich, F.. PY - 1987/1/1. Y1 - 1987/1/1. N2 - Failure of cemented total hip arthroplasty can emerge from several causes and may be associated with severe bone loss in the proximal femur with a marked diminution in functional capacity of the limb. This is a preliminary report of 14 patients with revision operations for failed total hip arthroplasty. Freeze-dried allograft bone was implanted for restoration of extensive bone loss in the proximal femur. All 14 patients were operated on to salvage failed cemented total hip arthroplasties. Allografts were employed only for large proximal femoral deficiencies. In these patients the alternatives were either an unstable excision arthroplasty or a femoral deficient prosthesis. After revision, all patients had complete bony union and ambulated with the assistance of a cane with greatly increased ...
We report a case of interposition of a broken trochanteric wire in the hip joint. This caused early wear of the prosthesis and dislocation of the Charnley total hip arthroplasty. The patient was treated with a revision total hip arthroplasty. This rare complication should be taken into consideration when performing a trochanteric osteotomy fixation with wiring in Charnley total hip arthroplasty.

Wolfinger, and Chia-Ling Kuo examine if an uncorrected radiographically detected immediate postoperative misfit in immediately loaded conversion prosthesis plays a significant role in early implant or prosthesis failure. In addition, clinical characteristics were analyzed relative to their relationship to early implant or prosthesis failure.. The results showed that misfit in immediately loaded complete‐arch fixed implant‐supported prostheses may not play a detrimental role in the implant survival but may affect survival of the conversion prostheses.. TY - JOUR. T1 - In vivo response to cross-linked polyethylene and polycarbonate-urethane particles. AU - Smith, Richard A.. AU - Maghsoodpour, Anobel. AU - Hallab, Nadim James. PY - 2010/4/1. Y1 - 2010/4/1. N2 - This study was undertaken to examine macrophage response to polycarbonate-urethane, a proposed alternative material to polyethylene in acetabular components of total hip arthroplasty. Polyethylene wear debris from total joint replacements has been linked to osteolysis and implant lifespan. It has been shown in vitro, that polyethylene particles cleaned of endotoxin generate less of an inflammatory cytokine response than endotoxin bound particles. Comparative particle induced effects on implant fixation were tested using endotoxin free cross-linked ultra-high molecular weight polyethylene (x-UHMWPE) and polycarbonate-urethane (PCU) particles with and without intraperitoneal injection (IP) of lipopolysaccharide (LPS) using a Ti-alloy femoral intramedullary nail rat model. No evidence based guidelines on surveillance of cemented total hip arthroplasty exist. We reviewed outcomes of this procedure from 1996 & 1997. Patients were identifed from theatre log books. Follow up data was retrieved from patient records. Evidence of radiological loosening & time of revision were recorded. Data was analysed using a ® statistical software package. We identified 425 primary total hip arthroplasties. Using radiological evidence of loosening as the end point we found an initial peak and another at 8 years. 10 year survival rate was 86%. Using revision surgery as the end point we found an initial peak & another at 8 years. 10 year survival rate was 92%. Once the patient has made it to 1 year, no follow up is required until 8 years; unless symptomatic.. ...
An improved hip prosthesis includes an acetabular cup bearing component constructed from a relatively hard and high strength ceramic material for articulation with a ball-shaped femoral head component which may be constructed from a compatible ceramic or metal material. In one form, the acetabular cup further includes a ceramic porous bone ingrowth surface adhered thereto for secure ingrowth attachment to natural patient bone.
|p|We analyzed by SEM three alumina-on-alumina femoral heads obtained from three patients who underwent revision for an aseptic loosening of the acetabular component. In parallel, the peri-prosthetic tissues were analyzed histologically in search of wear debris coming from the ceramic. Stripe wears, abrasive streaks, craters, and areas with extensive biomaterial removal were evidenced on the three femoral heads by SEM. In the altered area, the structure of the ceramic composed of minute polyhedric grains packed together was evidenced. In the peri-prosthetic tissues, the alumina particles were present in different forms: larges particles appeared transparent and birefringent, small particles engulfed by the macrophages had a light brown tint and were not birefringent. Large particles corresponded to the grains observed by SEM. EDS microanalysis confirmed the presence of aluminum oxide in these particles. Alumina debris are difficult to identify microscopically due to their pleomorphism.|/p|
If a metal ball head is used in combination with a polyethylene (plastic) cup insert, the wear rate is especially high and as a consequence, so is the risk of early loosening of the prosthesis. Clinical studies demonstrate that the polyethylene cup insert can wear by more than 0.2 mm per year. Frequently the surface of the metal head exhibits fine scratches, which can increase the wear rate even further. Laboratory studies using scratched metal ball heads indicated an increase in the amount of polyethylene wear debris of roughly 30 times.. Ceramic ball heads reduce the wear of the polyethylene cup inserts to less than 0.1 mm per year. Moreover, in contrast to a metal surface, the ceramic surface cannot be scratched. Ceramic/polyethylene wear couples (BIOLOX®delta or forte polyethylene wear couples) can reduce the rate of implant loosening, implant revision and associated complications by 2 to 5 times when compared to metal/polyethylene wear couples.. These conclusions are based on scientific ...

Dr. Schmalzried testified that he himself revised 15 out of 66 ASR hip implants, and stated that this revision rate is unacceptable for hip implants. I would not implant the ASR XL today, he said to jurors. According to Law 360, he also agreed with the assertion that the benefits of the ASR do not outweigh its risks. Schmalzried is the first witness to testify for the defense in the trial weighing the lawsuit of Carol Strum, whose case is the second of over 10,000 to go to trial. She alleges that her ASR hip implant is defective and caused injuries such as pain and revision surgery by shedding dangerous amounts of metal debris into the bloodstream and local tissue ...
Results: Fifty-two implants were loaded immediately and 52 early. Three patients with 8 implants dropped out from the immediate group versus two patients with 3 implants from the early loaded group; all remaining patients were followed for at least 10 years after loading. One single immediately loaded implant failed 2 months after placement. Three patients with immediately loaded implants and two with early loaded implants were affected by complications. There were no statistically significant differences for implant/prosthesis failures (Fishers exact test: P = 0.294; difference = 4%, 95% CI: -16% to 24%) and complications between groups (Fishers exact test: P = 0.574; difference = 4.5%, 95% CI: -12% to 21%). Both groups gradually lost peri-implant bone in a highly statistically significant way at 2, 8 and 14 months, and at 4, 5 and 10 years. After 10 years, immediately loaded patients lost an average of 1.34 mm and early loaded patients lost 1.42 mm of peri-implant marginal bone. At 10 years, ...
Registry-based medical research is an important tool in assessing health care interventions in the general population. Observational studies are used to establish effectiveness whereas randomized clinical trials assess efficacy in an experimental manner targeting a carefully selected population of patients. Medical registries may be office or hospital based, state/provincial, national, or more recently, global. Their role in postmarketing pharmacosurveillance is effective in detecting early adverse events and long-term benefits. Surgical disciplines use registry data to discover early device or prosthesis failures. Payers and administrators depend on medical registries to measure cost effectiveness (value). Patients are the ultimate beneficiary in terms of quality care. Current medical registries benefit from the explosion in medical informatics. The growth in the electronic health record for individual patients and hospitals provides an online wealth of data. The pharmaceutical industry has ...

The grinding and abrading of metal joint components releases tiny metal molecules that begin a foreign body reaction and cause dangerously elevated metal levels in the body. Metallosis occurs with titanium-based joint implants as well as those made with chromium and cobalt although the toxicity of these metals varies. When the body detects metal particles, the immune system goes into overdrive causing inflammation, swelling and other painful conditions. In the joints, metallosis causes synovitis, which is the inflammation of the synovial tissue and membranes that line the joints. Synovitis is a painful condition associated with gout that causes unbearable pain and makes joint movement difficult due to swelling and increased pressure.. ...

The DePuy ASR XL Acetabular System first became available in 2005 in the United States. At the beginning of 2010 DePuy Orthopaedics said they were phasing out the ASR Hip Implant because of declining sales, but never mentioned the high failure rate data from an Australian implant registry. In March 2010 the New York Times reported that DePuy issued its first warning to doctors and patients about the high early failure rate. However at this point they still had not issued a recall of the product. In fact, they adamantly denied there was a recall and claimed any statements referencing a recall were false. In turn of the damages being made, the DePuy lawsuit will cover all reasonable and customary costs of monitoring and treatment for services, including revision surgeries, associated with the recall of ASR products. ...

A modular prosthesis has an intramedullary rod element which is to be inserted in a bone. The rod has a shaped proximal portion which is telescoped into one end of a bore in the trochanter element. The mating surfaces of the shaped rod and the trochanter bore form a rotationally immovable connection. A neck element is telescoped into the other end of the trochanter bore permitting rotational adjustment. All the elements are locked together by a bolt through the neck and rod.

Sorry that it has been awhile since I have posted. So this past fall, I had my revision surgery for flatback syndrome. The surgery was an A/P approach and I am now fused to pelvis. At this point, I am a little over 7 weeks post-op and doing quite well. That being said, I remain cautiously optimistic. This was a very difficult decision for me to make. I had received multiple opinions from surgeons and even traveled out of town to do so. Although all of these surgeons agreed on the problem,
TY - JOUR. T1 - Formation of offshore tidal sandbanks triggered by a gasmined bed subsidence. AU - Roos, Pieter C.. AU - Hulscher, Suzanne J.M.H.. PY - 2002. Y1 - 2002. N2 - Offshore gasmining is an example of a human intervention with a morphological impact. On land, it is usually attended with a dish-like bed depression. We show that, if located at sea, such a bed depression can become morphodynamically active by triggering mechanisms related to tidal sand bank formation. To that end, a simple morphological model is considered which describes an erodible bed subject to a tidal wave in a shallow sea. The continuous subsidence is modelled by a sink term in the sediment balance. Then, a linear approximation is carried out to describe the bed evolution after the onset of subsidence. The results, presented in physical space, show that the subsidence triggers the formation of a sand bank pattern that gradually spreads around the centre of subsidence, at a rate that may go up to 160 m year¿1, ...
