The effect of treatment with propylthiouracil early in life (PTU; 8 mg k-1 day-1, ip, from day 17 of gestation to postnatal day 42) on the susceptibility of the cerebral cortex to spreading depression (SD) was studied in 13 adult Wistar rats (90-100 days of age). We experienced a case of thyroid storm associated with sepsis caused by pneumonia, which had a catastrophic course including recurrent cardiac arrest and subsequent multiple organ failure (MOF). A 22-year-old female patient with a 10-year history of Graves disease was transferred to our emergency department (ED). She had a cardiac arrest at her home and a second cardiac arrest at the ED. Her heart recovered after 20 min of cardiac resuscitation. She was diagnosed with thyroid storm associated with hyperthyroidism complicated by pneumonia and sepsis. We present a previously undocumented complication of carbimazole treatment occurring in a 28 year old woman with Graves disease. Four months after starting carbimazole (40 mg, dropping to 20 mg daily after six weeks) she developed left sided otalgia and presented with high pitched tinnitus. An audiogram showed a unilateral 25 dB high frequency loss which could not be accounted for by family or occupational history. Crescents and scleroses were noted in 92.9% and 85.7% of these cases, respectively. The most frequent extrarenal organs involved were lungs, followed by the central nervous system (CNS), skin, and digestive system. Ninety percent of patients were positive for perinuclear antineutrophil cytoplasmic antibody, 94.1% were positive for myeloperoxidase, and 88.2% were positive for both.
