This essay explores the origins and the implications of this relative silence on ethical matters. This study represents a program evaluation of Swim for Health: a multiagency partnership in two local authorities in the United Kingdom that sought to increase aquatic activity participation among four target groups. A theory-driven, scientific program evaluation model was used to assess if the program achieved its stated aims (Rossi et al 2003). Chronological records were maintained. Participation figures were collected and triangulated with 20 semistructured interviews with program stakeholders. Barriers to program implementation included a lack of prior needs-analysis of service provision and the goals of the program did not always match those of stakeholders. However, Swim for Health increased participation in three of four target groups. Program developments were also limited by availability of staff able to deliver novel activities other than aqua aerobics. Consequently participants were primarily women. The conventional common parameter evaluation model widely used in econometrics is one case where experiments uniquely determine joint the distribution of program impacts. STUDY OBJECTIVE--To examine the difficulties of developing and maintaining outcome evaluation designs in long term, community based health promotion programmes. DESIGN--Semistructured interviews of health promotion managers. SETTING--Wales and two reference health regions in England. PARTICIPANTS--Nine health promotion managers in Wales and 18 in England. MEASUREMENTS AND MAIN RESULTS--Information on selected heart health promotion activity undertaken or coordinated by health authorities from 1985-90 was collected. The Heartbeat Wales coronary heart disease prevention programme was set up in 1985, and a research and evaluation strategy was established to complement the intervention. The Helping Young Smokers Quit (HYSQ) initiative was a multi-phase project that addressed the critical need to disseminate effective, developmentally appropriate cessation programs for young smokers. Phase I identified and described tobacco treatment programs available for youth in the United States, Phase II evaluated smoking secession programs tailored for youth to help understand what works, and Phase III identified factors associated with program sustainability. Phase II collected data from five sources: (1) program participants, (2) program providers, (3) program curricula, (4) organizational leaders, and (5) community leaders and community ordinances. In recent years, there has been an increased interest within the program evaluation field for introducing systems thinking concepts. However, most of these attempts have been primarily directed towards supporting the practice of evaluation and not towards making theoretical advancements. This article is focused on introducing systems thinking, and specifically perspectives and concepts from the work in critical systems thinking (CST), at a theoretical level in the program evaluation field, towards a reframing of the Fourth Generation Evaluation methodology. The process for carrying out such reframing is introduced, as well as describing the major changes produced in the evaluation methodology by incorporating the CST perspective. A significant nursing shortage exists in the United States, which is compounded by high turnover rates. Skilled nurse preceptors are needed to train and assimilate nurses, which would improve retention of nurse preceptors, facilitate transition of nurses to the practice environment, and increase the number of preceptors within the organization. A preceptor preparation program in a 300-bed hospital was developed to promote nurse preceptor success while enhancing learning outcomes and creating a supportive work environment. Our review indicates that current evaluation methods evaluate different aspects of HIS and they can be improved upon. A new evaluation framework, human, organization and technology-fit (HOT-fit) was developed after having conducted a critical appraisal of the findings of existing HIS evaluation studies. HOT-fit builds on previous models of IS evaluation--in particular, the IS Success Model and the IT-Organization Fit Model.
