The complex issues surrounding hormone replacement therapy have been escalating over the past decade. Initial results of the Womens Health Initiative (WHI) trial released in 20021 have served to further complicate the decisions made by prescibers and potential consumers of hormone therapy. The WHI report by Chlebowski et al provides specific details about breast cancer outcomes for the oestrogen plus progestogen and placebo groups. Similar results regarding hormone therapy and breast cancer have been reported in European studies.2,3. It was previously believed that the increased risk of breast cancer with hormone therapy was seen in women with longer term (,5 y) use. These recent results reveal unexpected findings of early development of invasive breast cancers with the use of oestrogen plus progestogen. Additionally, the use of combined hormone therapy resulted in higher rates of mammographic abnormalities, adding to the emotional and economic burden for both patients and providers.. Future ...
Venous thrombosis occurred in 167 women taking estrogen plus progestin and in 76 taking placebo (twice the risk for venous thrombosis for women taking hormone therapy). Compared with women between the ages of 50 and 59 years who were taking placebo, the risk associated with hormone therapy was higher with age: 4.3 times the risk for women aged 60 to 69 years and 7.5 times the risk for women aged 70 to 79 years. Compared with women who were of normal weight and taking placebo, the risk associated with taking estrogen plus progestin was increased among overweight (3.8 times the risk) and obese women (5.6 times the risk). Participants with the hereditary blood coagulation disorder Factor V Leiden had a 6.7 times increased risk of thrombosis compared with women in the placebo group without the genetic mutation ...
a. Breast Cancer The most important randomized clinical trial providing information about breast cancer in estrogen plus progestin users is the Womens Health Initiative (WHI) substudy of daily CE (0.625 mg) plus MPA (2.5 mg). After a mean follow-up of 5.6 years, the estrogen plus progestin substudy reported an increased risk of invasive breast cancer in women who took daily CE plus MPA. In this substudy, prior use of estrogen-alone or estrogen plus progestin therapy was reported by 26 percent of the women. The relative risk of invasive breast cancer was 1.24 (95 percent nCI, 1.01-1.54), and the absolute risk was 41 versus 33 cases per 10,000 women-years, for CE plus MPA compared with placebo.. Among women who reported prior use of hormone therapy, the relative risk of invasive breast cancer was 1.86, and the absolute risk was 46 versus 25 cases per 10,000 women-years, for estrogen plus progestin compared with placebo. Among women who reported no prior use of hormone therapy, the relative risk ...

May 25, 2005 -- WASHINGTON -- The U.S. Preventive Services Task Force (USPSTF) recommends against the routine use of combined estrogen and progestin for the prevention of chronic conditions in postmenopausal women. The USPSTF found good evidence that the use of combined estrogen and progestin results in both benefits and harms. Benefits include reduced risk for fracture (good evidence) and colorectal cancer (fair evidence). Combined estrogen and progestin has no beneficial effect on coronary heart disease and may even pose an increased risk (good evidence). Other harms include increased risk for breast cancer (good evidence), venous thromboembolism (good evidence), stroke (fair evidence), cholecystitis (fair evidence), dementia (fair evidence), and lower global cognitive function (fair evidence). Because of insufficient evidence, the USPSTF could not assess the effects of combined estrogen and progestin on the incidence of ovarian cancer, mortality from breast cancer or coronary heart disease, ...

Drospirenone is what is known as a 4th generation progestin, which is an essential component in the newer and most popular oral contraceptives Yaz and Yasmin. Oral contraceptives that contain 4th generation progestins are accordingly referred to as 4th generation as well. That sounds simple enough, but the question is why the distinction?. First of all, progestins are synthetic (man-made) forms of progesterone, the presence of which inhibits ovulation among other physiological effects, which makes it the active ingredient for contraception. Naturally occurring progesterone is poorly absorbed by the body and rapidly degrades. Progestins, on the other hand, are more stable. The first generation of progestins was developed in 1951 and was first approved for use in the US as an oral contraceptive in 1960. These were estranes (steroids) derived from testosterone.. As the science improved, so did progestins. The aim for use in oral contraception is for high progestational selectivity and low ...
Progesterone is a steroid hormone that is essential for the regulation of reproductive function. The main physiological roles of this hormone have been widely described. Progesterone and progestins have been approved for a number of indications including the treatment of irregular and anovulatory menstrual cycles and, when combined with estrogen, for contraception, and the prevention of endometrial hyperplasia in postmenopausal hormonal replacement therapy (HRT) regimens. Lack of understanding between the differences in categories of the progestins as well as with the physiological hormone has resulted in considerable controversy surrounding the use of progestins for HRT regimens. Newer evidence suggests that there are distinct differences between the molecules and there is no progestin class effect, with regard to benefits or side-effects. In addition to its role in reproduction, progesterone regulates a number of biologically distinct processes in other tissues, particularly in the nervous ...
The 2 meta-analyses by Humphrey and colleagues and Nelson and colleagues, and much of the medical literature, do not clearly define postmenopausal HRT. Although authors often claim that many observational studies show the benefits of HRT on CAD risk, the data almost completely relate to unopposed estrogen (1). Few epidemiologic studies have evaluated estrogen plus progestin administered as continuous combined therapy. A limited number of studies were included in the meta-analyses by Humphrey and Nelson and their colleagues, and the included studies sometimes failed to specify type of HRT. Studies published in the 1980s and earlier likely refer to unopposed estrogen therapy. The lack of specificity in type of HRT is problematic because of the potential for different effects of estrogen alone and estrogen plus progestin. It is important to note that the meta-analyses used a common protocol to apply standard criteria for quality (2) and included only studies of higher quality. For studies assessing ...

Breast cancer was not associated with combined estrogen-progestin hormone replacement therapy. Although clinicians may despair at the seemingly contradictory conclusions of the studies by Colditz and Stanford and their colleagues, these studies provide important new information. In keeping with previous research, both studies support the conclusion that short-term HRT (, 5 years) has no important effect on the risk for breast cancer. Similarly, neither study found evidence that progestins either protect against or dramatically increase the effects of estrogen on the breast. The former claim was made on the basis of an earlier study that has since been discounted because of methodologic problems; the latter concern, raised in a large study done in Sweden, has diminished with longer follow-up from the study (1). Still in dispute are the effects of long-term HRT, which are now commonly prescribed to protect against osteoporosis and coronary heart disease. An increase in breast cancer was shown only ...
Estrogen alone and estrogen/progestin have opposite effects on breast cancer risk; adding progestin may lead to a lifetime of increased risk.
Main results. After a mean 11 years and 170 166 person-years of follow-up, HT increased risk for invasive breast cancer more than placebo (Table). More cases of breast cancer in the HT group were node-positive (24% vs 16%, P = 0.03), and fewer were localized (75% vs 81%; regional, 23% vs 16%; distant, 1% vs 2%; P = 0.05); groups did not differ for other tumor characteristics. HT was associated with a borderline significant increased risk for breast cancer mortality (Table). ...
Equine Disease Quarterly - July 2016". Progestogens are a class of steroid hormones largely responsible for sustaining the embryo and maintaining uterine quiescence. In horses, at least 10 known progestogens are present in maternal circulation during gestation. To date, only a few of them are known to be biologically active. Progesterone, the most renowned of this class of steroid hormones, is the only one with clinical diagnostic application. During early pregnancy, progesterone is produced in the equine ovary by the corpus luteum (CL), and its concentrations remain elevated and peak between 60 and 120 days of gestation. From that point on, progesterone slowly decreases until it becomes nearly undetectable around 180 to 200 days of gestation. During late gestation, other progestogens produced by the feto-placental unit are responsible for maintaining the pregnancy. These are first detectable by day 60 of gestation and are completely capable of maintaining pregnancy from around 120 to 140 days ...
As with all medicines, hormonal therapies have possible side effects. Side effects for the medicines are different and vary from woman to woman, and medicine to medicine. While side effects may be minor in some women, other women may want to stop treatment because of unpleasant or serious side effects. Some women find a gradual return of symptoms some months after treatment stops.. Treatment with the contraceptive pill. The contraceptive pill can help to relieve the pain of endometriosis. This is especially so if the tablets are taken continually for several months so that a period does not occur. This method is not suitable for women who are trying to become pregnant, as the contraceptive pill prevents pregnancy.. Treatment with a Progestagen. Taken every day, Progestagens usually help to relieve symptoms. Side effects are usually minor, but the pills may cause irregular bleeding, weight gain, tiredness and depression in some women. Progestagens can also be given as an injection once every 12 ...
In the past, many women used menopausal hormone therapy (MHT) to relieve hot flashes and other symptoms of menopause. But studies show that use of estrogen plus progestin increases the risk of both developing and dying from breast cancer. Although MHT is approved for the short-term relief of menopausal symptoms, the U.S. Food and Drug…

For many years, estrogen therapy and estrogen-progestin therapy were prescribed to treat menopausal symptoms, to prevent osteoporosis and to improve womens overall health. However, after publication of results from the Womens Health Initiative (WHI) in 2002 and March 2004, the U.S. Food and Drug Administration (FDA) now advises health care professionals to prescribe menopausal hormone therapies at the lowest possible dose and for the shortest possible length of time to achieve treatment goals. Treatment is generally reserved for management of menopausal symptoms rather than prevention of chronic disease.. The WHI was a study of 27,347 women aged 50 to 79 (mean age, 63) taking estrogen therapy or estrogen/progestin therapy. They were followed for an average of five and a half to seven years. The study was unable to document that benefits outweighed risks when hormone therapy was used as preventive therapy, and it found that risk due to hormones may differ depending on a womans age or years ...

Other women have remained loyal to the treatment, citing such benefits as better memory, mood and sleep. How effective really is HRT for treating these symptoms? As part of the Womens Health Initiative (WHI), over 16,000 women ages 50 to 79 were randomly selected to receive either estrogen plus progestin or a placebo (essentially a sugar pill); neither the researchers nor the participants were aware which each had been given. Measures of quality of life were recorded initially, one year later, and after three years (in a smaller subset of these women). Estrogen and progestin pills did not provide any significant improvements in the womens health in any quality-of-life area, including memory; sleep; sex life; depression; pain; energy levels; or emotional health. Hormone therapy was considered effective for treating hot flashes and night sweats only in 50- to 54-year-olds suffering these symptoms, however. In another blow to HRT, this study from The New England Journal of Medicine shows that it ...
HRT, consisting of estrogens alone, or in combination with a progestogen, is widely used for the relief of symptoms in postmenopausal women. Early observational studies have suggested that HRT might be associated with a reduced risk of cardio- and cerebro-vascular events. These encouraging results prompted randomized controlled trials assessing the risks and benefits of HRT in primary and secondary prevention of arterial vascular events. However, these clinical trials and further observational studies did not confirm the protective effect of HRT; it is now established that HRT increases the risk of stroke. This increased risk is mainly related to an increased risk of ischemic stroke. Oral estrogen alone and combined with progestogen are associated with a similar increased risk, which may be dose dependent. Conversely, a low dose of transdermal estrogens with or without a progestogen does not seem to be associated with such an increased risk of stroke, whereas the impact of tibolone, a synthetic ...

These findings build upon a considerable body of evidence pointing to the RANK signaling pathway as being a key mediator of progestin action in the mammary glands.

Heres the deal with my ADD and birth control. I take BC to manage my perimenopause symptoms and it has really helped with my ADD symptoms too. The biggest change is that this BC makes my vyvance work much better, as well as it did before my hormones got all wonky. This is what I take: https://en.wikipedia.org/wiki/Drospirenone. I now consider hormones to be an adjust medical treatment for my ADD. Until I started taking this type of BC I didnt realize how much my hormones were screwing my attention and memory and mood. I dont want to evangelize this BC pill or anything like that, but I mention it because I havent seen my experience reflected elsewhere and I think it may help other people.. My understanding is that this is a new form of progestin, or synthetic progesterone. I wonder if this makes a difference? Perhaps different progestins have different effects both on ADD symptoms and on the effects of stimulant medications? Then there is the ratio of estrogen to progesterone, which changes ...

Im not sure that is true. Recent research from Cancer Research UK and the Medical Research Council (published in the Lancet last week) found this: "If these associations are largely causal, then for women of average weight in developed countries, 5 years of MHT, starting at age 50 years, would increase breast cancer incidence at ages 50-69 years by about one in every 50 users of oestrogen plus daily progestagen preparations; one in every 70 users of oestrogen plus intermittent progestagen preparations; and one in every 200 users of oestrogen-only preparations. The corresponding excesses from 10 years of MHT would be about twice as great.". ...
