The European Atrial Fibrillation Trial (EAFT) results represent the first instance of trial-proved efficacy of warfarin in any setting of secondary stroke prevention. The rate of the composite end point for patients in the placebo group is remarkably high at 17%/y. Most events were stroke recurrences at 12%/y. For perspective, the placebo rate of initial stroke in the primary prevention trials in atrial fibrillation was about 5%/y (1). Considering the results of the primary prevention trials, it was not surprising that warfarin was effective in secondary prevention, although, interestingly, overall mortality was not improved. The greater issue was whether it could be used safely because these patients have risks for general and intracranial bleeding, including atrial fibrillation itself, previous stroke, and advanced age (2). Teachers interested in teaching this subject can download for free and use them with the method felt as most appropriate; they consist of text, graphs, tables, images, quizzes and an interactive atlas. According to a personal experience, the use of these educational resources helps teachers in teaching and students in learning about cancer prevention. According to the World Health Organisation, cancer is a leading and growing cause of mortality with 10 and 14 million new cases worldwide respectively in 2000 and 2012. Several risk factors have been identified; if they are avoided, more than 30% of the cancers can be prevented. OBJECTIVE--To determine the effects of lowering cholesterol concentrations on total and cause specific mortality in randomised primary prevention trials. DESIGN--Qualitative (meta-analytic) evaluation of total mortality from coronary heart disease, cancer, and causes not related to illness in six primary prevention trials of cholesterol reduction (mean duration of treatment 4.8 years). PATIENTS--24,847 Male participants; mean age 47.5 years. MAIN OUTCOME MEASURES--Total and cause specific mortalities. RESULTS--Follow up periods totalled 119,000 person years, during which 1147 deaths occurred. Mortality from coronary heart disease tended to be lower in men receiving interventions to reduce cholesterol concentrations compared with mortality in control subjects (p = 0.06), although total mortality was not affected by treatment. Background Suboptimal use of cardiovascular prevention medications has been reported. We report recent trends in secondary cardiovascular disease prevention drug use in Europe. Design We used the 2004-2013 data from the Survey of Health, Ageing and Retirement in Europe (SHARE), a large longitudinal cohort study in middle-aged and elderly Europeans. Methods Cross-sectional and panel data logistic regression models were used to study trends in cholesterol- and blood pressure-lowering drug use and effects of participant characteristics on the likelihood of drug use among participants with histoty of cardiovascular disease in SHARE. Results In SHARE 21,388 participants reported history of cardiovascular disease and provided data on cholesterol- and blood pressure-lowering drug use. At initial report, 40% and 60% of them used cholesterol- or blood pressure-lowering drugs, respectively. Therefore, prevention in the preclinical stage is likely the most effective way to decrease the incidence of this age-associated neurodegenerative condition, and its associated burden for individuals and society. Age, gender, family history, ApoE4, systolic blood pressure, body mass index, total cholesterol level and physical activity are all used as component of dementia risk score. There have been numerous challenges in conducting primary prevention trials in AD. The Lancet has published the results of a meta-analysis that aimed to assess the benefits and risks of aspirin therapy in the primary prevention of vascular events. This study has been reported in the general media (BBC) and builds on previous analyses.. The analysis was conducted by the Antithrombotic Trialists Collaboration and included data from six trials with 95,000 participants. Aspirin therapy produced a 12% relative risk reduction (absolute risk of 0·51% with aspirin versus 0·57% control per year) in serious vascular events mainly driven by reductions in non-fatal heart attacks. Aspirin treatment also increased major gastrointestinal and extra-cranial bleeds (0·10% versus 0·07% per year).
