The low rates of adoption of the CSP can be attributed to the physicians perceptions that the programs complexity and incompatibility exceeded its relative advantages. Furthermore, womens knowledge, attitudes and practices, identified as barriers by physicians, were also barriers to physicians adopting the CSP. There is growing evidence suggesting that prolonged sitting has negative effects on peoples weight, chronic diseases and mortality. Interventions to reduce sedentary time can be an effective strategy to increase daily energy expenditure. The purpose of this study is to evaluate the effectiveness of a six-month primary care intervention to reduce daily of sitting time in overweight and mild obese sedentary patients. The study is a randomized controlled trial (RCT). Professionals from thirteen primary health care centers (PHC) will randomly invite to participate mild obese or overweight patients of both gender, aged between 25 and 65 years old, who spend 6 hours at least daily sitting. A total of 232 subjects will be randomly allocated to an intervention (IG) and control group (CG) (116 individuals each group). In addition, 50 subjects with fibromyalgia will be included. These patients were by definition unstable as they were hospitalised due to heart failure and more symptomatic (ill) with a lower QoL at enrolment, while not necessarily more diseased, as indicated by the past medical history and substantial comorbidities[46, 47]. The aim of this study is to identify ways to build research capacity within primary health. A consultation was undertaken in late 2004 using a combination of a one-page survey and a guided meeting format, in a primary health setting in rural NSW. Most (81.3%) of the 134 individuals consulted were part of an Area Health Service, with 12.7% from non-government settings. Most (80.6%) were clinicians, with a third (31.5%) nurses, 8.3% in medicine, and the remainder from a range of allied health professions. Eleven organisations were represented. The main outcome measures were identification of support needs, processes to enhance research engagement, and barriers and enablers to clinicians research involvement. The results showed that popular delivery modes for research training and support were courses and one-to- one advice. Writing topics were generally more popular than others. BACKGROUND: The Diabetes Manual is a type 2 diabetes self-management programme based upon the clinically effective Heart Manual. The 12 week programme is a complex intervention theoretically underpinned by self-efficacy theory. It is a one to one intervention meeting United Kingdom requirements for structured diabetes-education and is delivered within routine primary care. METHODS/DESIGN: In a two-group cluster randomized controlled trial, GP practices are allocated by computer minimisation to an intervention group or a six-month deferred intervention group. We aim to recruit 250 participants from 50 practices across central England. Eligibility criteria are adults able to undertake the programme with type 2 diabetes, not taking insulin, with HbA1c over 8% (first 12 months) and following an agreed protocol change over 7% (months 13 to 18). Following randomisation, intervention nurses receive two-day training and delivered the Diabetes Manual programme to participants. Deferred intervention nurses
