Factors associated with patients self-reported adherence to prescribed physical activity in routine primary health care | BMC...
Written prescriptions of physical activity have increased in popularity. Such schemes have mostly been evaluated in terms of efficacy in clinical trials. This study reports on a physical activity prescription referral scheme implemented in routine primary health care (PHC) in Sweden. The aim of this study was to evaluate patients self-reported adherence to physical activity prescriptions at 3 and 12 months and to analyse different characteristics associated with adherence to these prescriptions. Prospective prescription data were obtained for the general population in 37 of 42 PHC centres in Östergötland County, during 2004. The study population consisted of 3300. The average adherence rate to the prescribed activity was 56% at 3 months and 50% at 12 months. In the multiple logistic regression models, higher adherence was associated with higher activity level at baseline and with prescriptions including home-based activities. Prescription from ordinary PHC staff yielded adherence in half of the

UKs largest primary healthcare centre to open next year | Nursing in Practice
The largest primary healthcare centre in Britain - and probably Europe - took another step towards completion when it was topped out with a final trowel-full of concrete by Professor Jonathan Montgomery, chair of Hampshire Primary Care Trust.. When it opens next year, the £30m Aldershot Centre for Health will provide primary healthcare for a local population of 45,000, as well as specialist services to 250,000 people in the wider area. Uniquely, the facilities will be shared by the 5,000-strong Army garrison. At 138,500 sq ft/12,866 m², the building is many times bigger than a conventional health centre. It will have more than 750 rooms for 47 separate user groups. The concept is to provide economies of scale and meets objectives set out in the NHS Plan of 2000.. The developers for the Centre created a special funding package for Hampshire PCT, itself the largest PCT in Britain, and the Ministry of Defence. The PCT and MoD will own the Centre outright when the 30-year lease expires.. It will ...
Mental health training for primary health care workers and implication for success of integration of mental health into primary...
Mental disorders are always remained a neglected public health problems in low and middle-income countries (LMICs), most people with mental disorders never receive effective care and there is a large treatment gap. In order to solve the problem integration of mental health into primary health care is recommended and in Ethiopia implementation of the scale of mental health services at primary health care level was started in 2014. For the success of the integration of mental health into primary health care, primary care health professionals are the key personnel who are responsible for the management of mental, neurologic and substance use disorders. However, proper training and education of primary care health professionals is mandatory for an optimal performance and success of integration. This interventional study was conducted to assess the effectiveness of mental health training course for scale up of mental health services at primary health care level in Ethiopia. Choices for Change: The Path for Restructuring Primary Healthcare Services in Canada
This policy synthesis identifies four major models for organizing primary healthcare that are relevant to the Canadian context. Two models fall into what can be broadly called a communityoriented approach, while the other two fall into what can be called a professional approach. The vision of community primary healthcare is to improve the health of specific geographically defined populations and to contribute to community development by providing a set of required medical, health, social, and community services. Within this vision, there are two types of models: the integrated community model and the nonintegrated community model. These models differ by their degree of integration into other parts of the healthcare system. Several characteristics of their resources, organizational structure, and practices reflect their varying degree of integration. The vision of professional primary healthcare is to deliver medical services to patients who seek these services, or to people who choose to ...

Primary Care-Based Interventions to Reduce Alcohol Use Among HIV Patients - Full Text View - ClinicalTrials.gov
This application responds to RFA-AA12-009, Interventions to Improve HIV/AIDS and Alcohol-Related Outcomes (U01). The proposed study takes place in a HIV primary care clinic and uses the health plans electronic medical record (EMR) for screening; it has the potential to provide a significant benefit to HIV-infected individuals by reducing hazardous drinking and the associated complications. Prior studies have identified high rates of co-occurrence of HIV and hazardous drinking (defined as drinking over threshold limits, i.e., 5+ daily or 14+ weekly drinks for men and 4+ daily or 7+ weekly drinks for women). Drinking at these levels can compromise antiretroviral (ART) treatment, and increase rates of depression, unsafe sex, and mortality. The proposed randomized trial examines the comparative effectiveness of two highly implementable behavioral interventions for reducing hazardous drinking, each with an adaptive, stepped-care component: 1) Motivational Interviewing (MI), consisting of one ...

RACGP - Living well with osteoarthritis
Although general practitioners (GPs) describe favourable attitudes towards clinical practice guidelines, their familiarity with, and application of, OA management guidelines reveals an important implementation gap.5 The CareTrack study reported a median of 43% (95% confidence interval: 35.8, 50.5) of primary care-based healthcare encounters provided appropriate OA care.6 The Bettering the Evaluation and Care of Health (BEACH) study reported that only 17% of patients who consulted their GP for hip/knee OA were referred for lifestyle interventions.7. Previous research into barriers to the use of lifestyle interventions found that primary care practitioners (GPs, nurses, pharmacists, physiotherapists) feel underprepared to deliver these.8 Previous studies have identified a lack of confidence and knowledge of primary care practitioners to effectively deliver lifestyle interventions.9-12 Some practitioners continue to prescribe pharmacological agents7,13 with small therapeutic effects (eg ...
Scaling Up (Revitalising) the Primary Health Care Delivery System - HSCL
The Primary Health Care delivery remains the weakest link in the health care system - only approximately 1 out of every 5 primary health care center is functional - without significant and drastic intervention at this level, Nigeria is not likely to achieve the health-related sustainable development goals.. The Government of Nigeria as part of its commitment to achieving universal health coverage is set to revitalise the primary health care system by ensuring the functionality of 10, 000 primary health care centers - at least 1 functional primary health care center per political ward in Nigeria - to provide quality primary health care to 100million Nigerians. The scale-up of the 10,000 primary health service across the country was based on the key principle of long-term sustainability. Thus, the States will play a critical role in implementing the project and ensuring the expected outputs and outcomes are achieved.. ...

Objective: This study aims to examine the circumstances associated with patients tendencies to recommend a primary care centre, based on four hypotheses, the initial contacts quality, care relationship continuity, treatment encounter responsiveness and whether the significance of encounter responsiveness differs depending on whether the patient has been seeing a nurse or physician. Design: The study is based on the patient self-reported responses, retrieved from the Swedish National Patient Survey. The design is cross-sectional, and data were analysed using a binary logistic regression. Setting: Data were collected from three primary healthcare centres in the region of Västra Götaland, Sweden. Participants: A total of 362 patients (62% females) having visited any of three publicly run healthcare centres in September 2010 constitute the analytical sample. Participants were fairly evenly distributed across all age groups. Main Outcome Measures: Recommendation was captured by patients binary ...

Effective Communications for Nutrition in Primary Health Care (UNU, 1988, 208 pages): 1. Nutrition in primary health care
Ministry of Public Health, Bangkok, Thailand PRIMARY HEALTH CARE IN THAILAND The problem of under-utilization of health centres and hospitals in rural Thailand led to the initiation of a pilot project on primary health care (PHC) in Sarapee District, Chiang Mai Province, in 1969. The project was designed mainly to extend various health services to cover a greater portion of the population in the area. The initial estimation for coverage of existing health services was below 20 per cent of the total population at that time. Recognizing that the health-service delivery structure was not covering all the health and medical demands of the community, the project organizers strategy was to train people selected by the community to function as intermediaries between the peripheral tambon (community of villages) health officers and the community, adding to the existing health-service delivery on a voluntary basis. Evaluation of the pilot project revealed that there was increased coverage of the ...
Nigeria announces policy proposal to restructure inefficient primary healthcare system - NextBillion
Tuesday, December 20, 2016. To make Nigerias inefficient primary healthcare system effective, the government plans an overhaul that could lead to some sort of financial autonomy for individual health centres.. The Minister of Health, Isaac Adewole, who stated this on Monday, said the move could lead to primary healthcare centres, PHCs, having full control of how they utilize the money they are allocated.. Represented by the Director of Public Health at the ministry, Evelyn Ngige, Mr. Adewole spoke at the 2nd Annual Primary Health Care Service Lecture, held in Abuja. He said the current administration would want to be remembered for putting in place a functional primary healthcare system.. The administration of President Muhammadu Buhari would love to be remembered in making basic healthcare services qualitative, accessible and affordable to all Nigerians, he said.. Financial autonomy at the PHC facility level, making sure money gets to the primary healthcare facilities directly with ...
Comprehensive review of the evidence regarding the effectiveness of community-based primary health care in improving maternal,...
Our analysis provides strong evidence that CBPHC can improve neonatal health in low-income settings. Of the studies with strong experimental research designs, over 70% showed a positive neonatal health impact. Although many of these studies were smaller scale pilots or efficacy studies, it demonstrates that CBPHC can be an essential tool where access to facilities is limited and many births take place at home. In these settings, access to antenatal care is often limited; for example, only 49% of pregnant women in sub-Saharan Africa obtain four antenatal care visits [1]. Furthermore, among the 75 countries with the greatest burden of neonatal mortality, the median national coverage of interventions that are important for improving neonatal mortality is quite low: 65% for skilled attendant at delivery, 28% for postnatal visits for newborns, and 50% for early initiation of breastfeeding [14]. Community-based approaches will be essential for the near term in order to achieve universal coverage of ...

The Challenge of Changing Practice : Applying Theory in the Implementation of an Innovation in Swedish Primary Health Care
Background: The translation of new knowledge, such as research findings, new tools or methods into health care practice has gained increased interest in recent years. Important factors that determine implementation outcome have been identified, and models and checklists to be followed in planning as well as in carrying out an implementation process have been produced. However, there are still knowledge gaps regarding what approach should be used in which setting and for which problems. Primary health care (PHC) in Sweden is an area where there is a paucity of research regarding implementation of new methods into practice. The aim of the thesis was to apply theory in the study of the implementation of an innovation in Swedish PHC, and identify factors that influenced outcome.. Methods: The study was performed using a quasi-experimental design, and included six PHC units, two from each one of three county councils in the southeast part of Sweden. A computer-based lifestyle intervention tool (CLT) ...
Knowledge, attitude and behaviors related to HIV/AIDS amongst female adolescents who are accessing the primary health services...
Background: In Namibia, studies showed that HIV/AIDS affects youth, especially the under 24 years age group. At the same time the pregnancy rate is also high by age 19. Interestingly, in Andara district several reports from staff working in the reproductive services have indicated that adolescent girls, who would seem to be taking responsibility in one sphere of their sexual lives by protecting themselves against unwanted pregnancy, were however not using condoms to protect themselves from HIV infection. Study Aim and Objectives: To describe the knowledge, attitude and behaviour related to HIV/AIDS amongst female adolescents who are accessing the primary health care (PHC) services for contraception. Specific objectives were to describe the knowledge of female adolescents who are accessing the PHC services for contraception about the modes of transmission and prevention of HIV/AIDS, to assess their attitude with regards to condom use, abstinence and being faithful to one uninfected partner, also ...
Development of a primary care-based complex care management intervention for chronically ill patients at high risk for...
Therefore, it is crucial to identify as precisely as possible patients most likely to benefit from these programs. Finding high-risk patients in computerized medical record systems, using predictive modelling, has been evaluated in care management trials in the USA and is seen to have better results than case finding by doctors or patient surveys [12, 13]. These software models rely on clinically- and cost-similar disease categories called diagnostic cost groups (DCG) [14] or adjusted clinical groups (ACG) [15] that are generated from insurance claims data.. In Germany, chronic heart failure (CHF), chronic obstructive pulmonary disease (COPD), and type 2 diabetes mellitus (DM) were among the 20 most frequent causes for hospital admission in 2008 [16]. All three conditions are stated as being ambulatory care sensitive conditions (ACSC), meaning that primary care has a dominating role in preventing hospital admissions for these conditions [17]. Hospitalisations may be avoidable by coordinated ...

The key to better primary health care? Human-centered design | Better Immunization Data Initiative
People-centered care means shifting care to the community, and even bringing it directly to individuals when possible. And the Sayana® Press (DMPA-SC) puts health directly in womens hands. Its an all-in-one contraceptive device that combines a plastic bulb of medication with a single-use needle. A single injection-which can be administered by women themselves-prevents pregnancy for three months.. Previous research by PATH and partners has found that women can self-administer DMPA-SC safely and effectively, that they like doing so, and that self-injection enables them to continue using the method longer than injections from providers.. But how do women best learn to self-inject outside of research studies, in routine primary health care settings? Uganda paved the way, becoming the first country in the world to offer contraceptive self-injection outside of a research setting. Taking a human-centered design approach to identify best practices and user needs, as well as health system realities, ...

Primary health care research needs leadership, not pampering | CMAJ Blogs
at Radboud University Nijmegen in The Netherlands; Professor of Primary Health Care Research at the Australian National University in Canberra, Australia; and Past President of WONCA. On a global scale, research in family medicine and primary health care appears to develop slowly and hesitantly, certainly in light of its contribution to education. This difficult state of affairs is mirrored in substantial international differences in research facilities: few countries can boast the visionary funding policies of Canada or Australia, or the inclusive research programming of universities in the Netherlands and the UK. The research grass often looks greener on the other side of a border, and this tends to trigger contemplation of what might have been - along with lamentations of deprived opportunities.. Yet, there IS a parallel primary health care research reality to enjoy. Italian family physicians have been able to publish their research in the highest impact journals, Brazilian universities are ...

Objective- To evaluate the practicability and efficacy of a structured treatment and teaching program for non-insulin-treated type II diabetic patients in routine primary health care.. Research Design and Methods- All physicians (n = 139) and their office staffs in Hamburg, Germany, who had participated in a special training course from 1 April 1991 to 31 December 1991 were contacted for a standardized interview. A random sample of 17 of these offices was selected for office visits during which the documented data of all patients who had received the standardized treatment and teaching in the same period were collected and evaluated.. Results- The program was well received by the physicians, and the data collected on 179 patients (5.1 mo median after the intervention) demonstrated the efficacy of the program at the treatment level: reduction of body weight (mean 2.8 kg, P , 0.0001) and HbA1c levels (from 8.11 ± 1.68 to 7.47 ± 1.64%, P , 0.0001) was substantial. The individual prescribed volume ...

Myocardial infarction is difficult to detect early. Due to delayed and uncertain diagnosis, physicians are unable to take treatment decisions during the golden hour, i.e. before commencement of necrosis. These delays cause incapacitation and death of the patient. We propose to build a smart instrument that will rapidly diagnose myocardial Infarction. We will employ a microcantilever-based bio-sensing system, to detect early cardiac markers (eg. myoglobin) that are released in the blood after an MI event. The instrument will be highly sensitive, highly selective and would provide a rapid diagnosis. The instrument can be used even by a qualified nurse at a rural primary healthcare centre in India.. ...

Observational studies of depression in primary care: what do we know? | BMC Family Practice | Full Text
Understanding the complex interplay between the development and persistence of depression over the longer term, psychological, social and physical factors and the health service use and treatment patterns is crucial if we are to plan better models of care to cope with the increasing burden that depression and related disorders is placing on people experiencing the condition, their social networks and the health care system.. Despite the growing interest in depression being managed as a chronic illness in primary care; this review identified only 17 observational studies of depression in primary care, most of which have been conducted in Europe or the US. The striking finding of this review is the small sample size of many studies, the small numbers in the cohort with depression and the short length of follow-up. The studies provide information on the nature and course of depression for around 7,500 people receiving routine primary care. Of the 17 studies, nine studies followed almost 3,000 ...
Emergency room closure labeled slimy by critics - Nelson Star
This is a hard letter to write, because most of us, including myself, would like to see government as on our side, especially where it comes to health matters.. Over the past few months it has become clear that the IHA has carried out a deliberate and long term plan to deprive Kaslo of not only its doctors and emergency services, but most likely the final goal for the IHA will be the elimination of our hospital altogether.. Two weeks before the meeting on November 12, called by the IHA, our family received a letter from the Ministry of Health saying that by mutual agreement Kaslo and the IHA have decided that it would be in the best interests of Kaslo to have a robust primary healthcare centre from 9 to 5, Monday to Friday.. This was a lie. There was no agreement from Kaslo on that, but that letter (which was sent by the Ministry to many people in the area) exposed the IHAs intentions such that they felt it necessary to call a meeting in November canceling our ER services.. Its not the IHA ...
Sequelae Of Septic Arthritis Of The Hip - Global HELP
While the majority of cases may not be preventable, significant morbidity can be averted by early recognition and treatment, highlighting the need to strengthen the delivery of health care at primary health centers in such resource challenged environments. Practitioners caring for musculoskeletal problems in such environments will necessarily be confronted with neglected hip sepsis, and the hope is that the information in this monograph will help to formulate a treatment plan for these challenging cases ...

INTRODUCTION. Diabetes is a worldwide epidemic, causing a high disease and economic burden. Type 2 diabetes, the most common form of diabetes, is associated with overweight and obesity and an unfavourable lifestyle, including unhealthy diet and physical inactivity. Over the last two decades, many large-scale experimental trials have shown that type 2 diabetes can be delayed or prevented by lifestyle modification in high-risk subjects. This evidence has been translated and implemented in interventions in real-world settings, however, no (cost)effective diabetes prevention programme in Dutch primary health care was available at the start of the current project in 2008. Therefore the SLIMMER study (SLIM iMplementation Experience Region Noord- en Oost-Gelderland) was started in which the SLIM intervention, revealing a 47% diabetes risk reduction, was translated to Dutch primary health care. The aim of this thesis was to implement the SLIMMER intervention in Dutch primary health care and to evaluate ...
Critical shortages of personnel and data jeopardize primary health care systems | PHCPI
Primary health care is one of the most cost-effective ways to equitably reduce morbidity and mortality and improve population health, but the realization of these potential benefits is dependent on having adequate numbers of competent, motivated human resources to deliver primary health care services. Assessing health needs in developing countries | The BMJ
This is the last in a series of six articles describing approaches to and topics for health needs assessment, and how the results can be used effectively. In most developing countries, the evolution of health services has been dominated by Western models of health care. These have rarely taken into account how local people explain illness, seek advice, or use traditional healing methods. The emphasis has been on hospitals and curative care rather than on trying to address local health needs equitably and effectively. Since the Alma Ata declaration on primary health care, more attention has been given to increasing coverage of basic services and preventing common diseases. However, the bias in resource allocation towards secondary care and urban areas remains. ...

Background: In primary care, patients with multiple chronic conditions are the rule rather than the exception. The Chronic Care Model (CCM) is an evidence-based framework for improving chronic illness care, but little is known about the extent to which it has been implemented in routine primary care. The aim of this study was to describe how multimorbid older patients assess the routine chronic care they receive in primary care practices in Germany, and to explore the extent to which factors at both the practice and patient level determine their views. Methods: This cross-sectional study used baseline data from an observational cohort study involving 158 general practitioners (GP) and 3189 multimorbid patients. Standardized questionnaires were employed to collect data, and the Patient Assessment of Chronic Illness Care (PACIC) questionnaire used to assess the quality of care received. Multilevel hierarchical modeling was used to identify any existing association between the dependent variable, ...

Obesity is a chronic and multi-factorial disease and one of the most important causes of morbidity and premature mortality worldwide.. Currently, over a billion people are overweight and half a billion are obese. In more than half of the EU countries one in two individuals are overweight or obese. The epidemic is still increasing in many European countries, whereas in some it seems to have slowed down. In the United States, obesity has been declared the number one health threat.. Non-surgical and non-pharmacotherapeutical treatment options include diet, exercise, behaviour modification and psychological support. The effect size has been reported with a single digit weight loss in kilograms which can be maintained. In contrast to experimental settings, implementing life-style interventions in routine primary care that reduce morbidity at population level have proven to be difficult.. There are only very limited pharmacotherapeutic treatment options. Overall, pharmacotherapy has played a minor ...

All maintained school governing bodies and academy trusts have a legal duty to provide all of the governance information requested on this page in so far as it is available to them. This will increase the transparency of governance arrangements. It will enable schools and the department to identify more quickly and accurately individuals who are involved in governance, and who govern in more than one context. The information requested says nothing in itself about a persons suitability to govern. However, it is essential information for the department to be able to uniquely identify an individual and in a small number of cases conduct checks to confirm their suitability for this important and influential role. You should provide all of the information requested insofar as it is available to you. A minimum set of information needs to be provided before a record can be saved.
