We studied the effects of temazepam premedication and induction of anaesthesia with thiopentone or propofol on the heart rate power spectrum in 47 patients undergoing elective minor surgery. Eighteen patients received temazepam 20 mg orally as premedication. There was a significant reduction in high frequency power and total power, and an increase in the ratio of low to high frequency power after induction of anaesthesia with either propofol or thiopentone. Patients who had received temazepam premedication had significantly greater low frequency, high frequency and total power than those who were not premedicated. There was no significant difference between premedicated and unpremedicated patients in the ratio of low to ventilatory frequency power.

In this issue of the Journal, Tardif et al. (16) present an interesting study that sought to assess the value of inclacumab, a novel P-selectin antagonist, to reduce myocardial damage during PCI. In the SELECT-ACS trial, a total of 544 patients with NSTEMI were randomly allocated to receive an infusion of 5 or 20 mg/kg of inclacumab or placebo (1:1:1) before coronary angiography. The primary endpoint, the increase of troponin I levels at 16 and 24 h, was evaluated in the 322 patients who eventually underwent PCI. Inclacumab 5 mg/kg had no effect on any efficacy outcome measure. However, placebo-adjusted geometric mean percent changes in troponin with inclacumab 20 mg/kg were −22.4% at 16 h (p = 0.066) and −24.4% at 24 h (p = 0.053). Introduction: Midazolam is commonly used in pediatric patients as a sedative and anxiolytic before the induction of anesthesia. Little is known about clinically resulting plasma levels in the setting of short procedures. Aim of this study was to compare plasma levels of Midazolam at the end of intervention with the corresponding levels at the time point of anesthesia induction for short procedures. Acute adverse reactions to contrast media will occur at a certain rate. Non-ionic iodinated contrast is said to have an adverse reaction rate of about 0.6 to 3.1% [6-10]. This rate is said to be about 0.07 to 0.67% for gadolinium contrast media [6-9].. The ACR Manual on Contrast Media [1] and EUSR Guidelines on Contrast Media [2] recommend premedication for patients at risk for adverse reactions to contrast, but the definition of at-risk has some variation among protocols and studies. The ACR Manual states patients with a history of adverse reactions should be premedicated, but does not elaborate on the degree of the reaction. The ESUR Guidelines, on the other hand, includes patients with a history of moderate or severe reactions. Abstract: Objective To compare two spray administration of intranasal dexmedetomidine (DEX) doses for premedication in children. Methods In this prospective,randomized,double-blind study,41 children aged 3-6 years were enrolled. Children were assigned to receive either spray administration of intranasal DEX 1 μg/kg (Group D1) or 2 μg/kg (Group D2). The Ramsay Sedation Scale scores were evaluated at 5 min intervals. Sedation status at separation from patient was also evaluated. Heart rate (HR) and saturation of peripheral oxygen (SpO2) were recorded before and every 15 min after drug administration. Results The median sedation onset time was 30 min and 20 min in group D1 and group D2,respectively. Compared with the children in group D1,those in group D2 were significantly more sedated when they were separated from their parents (61.9% vs. 95.0%,respectively)(χ2=5.549,P=0.010). In the past week, I saw my first grand mal seizure in a dog (terrifying). Ive learned how to take blood pressures using a Doppler, learned how to set up a CRI (constant rate infusion), watched a classmate remove fluid from a dogs abdomen, and saw my first case of pericardial effusion. I saw a dog who got put on a ventilator to breathe for her after meningitis caused his brain to swell to the point that he lost control of his respirations. We made a lot of sad diagnoses (cancers mostly, or poor outcomes following surgery), but also got to send a lot of our patients home after they made it out of the woods. I helped take care of a litter of kittens post-C-section, and got to send a tiny puppy patient home after he recovered from a pretty major head trauma ...

The use of hydrazine sulfate to treat cancer is discussed extensively in many articles on the internet. The reports are unusually positive which drew me to try to understand its biochemical mechanism. That is the only approach I can take to convince myself as to their validity and how reproducible the results can be expected to be. Such an understanding is also essential for taking the next step, perfecting the treatment to achieve its full potential. I first concluded the conventionally accepted theory, identified in every report, was seriously flawed. Did this mean that the positive results were exaggerated? As an experiment (mental), I decided to assume that the results were valid and try to identify the correct mechanism. I believe I have done that and have concluded the treatment, properly applied, has the potential of being unusually effective. Introduction/Context: A comparative study was carried out to evaluate the analgesic efficacy and side effects of addition of fentanyl to local anesthetic undergoing surgeries on forearm and elbow. All patients were hemodynamically stable, and there were no serious side effects in any of the patients in both the groups. I was a perfectly healthy 45 year old male with no history of any chronic medical conditions. I went to bed feeling perfectly fine and a few hours later woke up with difficulty breathing. Called the ambulance and was taken to the local hospital before being transferred to a major hospital in New York City. My condition became critical and was advised that I would have to undergo a minor surgical procedure. My lung was removed from my chest cavity (taken from between my ribs) and scar tissue was peeled off the lung before it was replaced back into my chest cavity. The surgical scar that resulted spans my entire back. I was advised that my infection was a result of my recent dental work and could have prevented by pre medicating with four 500 mg (2000mg) of Amoxicillin an hour prior to the dental work. Even so we get upset clients. Last week I had someone get very upset because her cat may have had a vaccine reaction. It had diarrhea and vomiting after being in for kitten immunizations, which are potential signs of a reaction. The diarrhea had started while in the exam room, something noted by one of my associates who cautioned the owner about reactions and offered to treat. The next day the client called because the kitten still vomited some. We told them to bring her in, but they went and had lunch instead and by the time they came in our doctor was at lunch. Rather than understanding they got upset because they hadnt been told not to come in at a certain time, and wanted one of the techs just to give a shot. They also didnt want to bother leaving the kitten with us for observation.
