The trial, which was run in six lower-income countries, found that giving first-time mothers a daily low-dose aspirin reduced their risk of preterm birth by 11%. Their chances of a very early delivery -- before the 34th week of pregnancy -- were cut by one-quarter.. Infection of vagina and cervix of pregnant women with the bacteria, Group B Streptococcus (GBS), causes chorio-amnionitis that significantly increases the probability of preterm births. We report that, GBS produces small extracellular membrane vesicles (MVs) which are toxic to both fetal and maternal cells. In animal studies, we found that the MVs disrupt the connective tissue of the fetal membrane reducing its mechanical strength which may cause premature rupture of amniotic sac. Further we show that even in absence of the bacteria, the MVs directly led to extensive inflammation in the mouse resulting in chorio-amnionitis, preterm births and still births. Previous research investigating structural neurodevelopmental alterations in individuals who were born very preterm demonstrated a complex pattern of grey matter changes that defy straightforward summary. Here we addressed this problem by characterising volumetric brain alterations in individuals who were born very preterm from adolescence to adulthood at three hierarchically related levels - global, modular and regional. We demarcated structural components that were either particularly resilient or vulnerable to the impact of very preterm birth. To evaluate if mid-pregnancy immune and growth-related molecular factors predict preterm birth (PTB) with and without (±) preeclampsia. Included were 400 women with singleton deliveries in California in 2009-2010 (200 PTB and 200 term) divided into training and testing samples at a 2:1 ratio. Sixty-three markers were tested in 15-20 serum samples using multiplex technology. Linear discriminate analysis was used to create a discriminate function. Model performance was assessed using area under the receiver operating characteristic curve (AUC). Twenty-five serum biomarkers along with maternal age |34 years and poverty status identified |80% of women with PTB ± preeclampsia with best performance in women with preterm preeclampsia (AUC = 0.889, 95% confidence interval (0.822-0.959) training; 0.883 (0.804-0.963) testing). Together with maternal age and poverty status, mid-pregnancy immune and growth factors reliably identified most women who went on to have a PTB ± preeclampsia.

Preterm birth does not have a single identifiable cause, but it has been associated with a number of factors. In 2006, the Institute of Medicine (IOM) published one of the most comprehensive reviews of preterm birth ever compiled. It identified prior preterm birth, incompetent cervix, multiple gestation pregnancy, infection, and smoking as some of the factors associated with preterm birth.
