Selective reduction, multifetal pregnancy reduction (MFPR), or selective termination is the practice of reducing the number of fetuses in a multifetal pregnancy, say quadruplets, to a twin or singleton pregnancy. When the fetus targeted for termination displays signs of a serious disease, the term selective termination is used; otherwise terminology involving the word reduction is used to describe the procedure. Selective reduction is done for both medical and non-medical reasons. Medical issues generally related to multiple births include premature births, low birth weights, and associated medical problems. Non-medical reasons include that the couple or the mother do not feel that they are emotionally or financially ready to handle more than one child.[medical citation needed] Selective reduction can also be used to reduce a twin pregnancy to a singleton one. This is a less common but growing practice as the risks in twin pregnancies, while existent, are much lower than in higher-order multiple ...
History and Objectives: The present study was carried out to study multifetal birth and factors that influence multifetal gestation at Mahdieh Medical Center in 1992. Materials and Methods: A descriptive study was carried out on 11775 single and multifetal gestational deliveries. Personal information on women with multiple gestation, methods of contraception, embryonic development, gestational period, weight of embryo and prevalence of multifetal birth and confidence interval (CI) was recorded. Results: The prevalence of double gestational delivery was 6.2 in thousand (CI 95) and its prevalence in the population is 5.9-6.7 in thousands. Mean age and average number of pregnancies were 27.7±6.8 and 2.3±1.9 respectively. Double and single oocyte double gestation was 69.4 and 30.6 percent respectively. The prevalence of preterm delivery was 36.6. Cesarean section was carried out on 14.7 and 51.3 of single and double gestational pregnancies. The prevalence of the underweight (Below 2500 grams) ...

A study from the Japanese vital statistics database showed the relative risk for perinatal mortality to be 5-fold and 12-fold higher for twins and triplets, respectively, compared with singletons [20]. Certain authors have claimed that aggressive modern perinatal and neonatal care may close the gap between the mortality risks of singletons, twins, and triplets, however. Single-center studies by Collins et al [21], Gonen et al [22], Sassoon et al [23], Angel et al [24], Ron-El et al [25], and Boulot et al [26] have reported excellent outcomes for infants from twin and higher-order multiple pregnancies. Methods, apparatuses and systems directed to refreshing signal information in an infrastructure wireless node location mechanism. The wireless node location mechanism selectively terminates connections with wireless clients to refresh signal strength information used to compute an estimated location for the wireless clients. The wireless node location mechanism terminates the connection between a WLAN and a given wireless node, causing in typical WLAN protocol implementations, the mobile station to transmit frames or packets on all available operating channels in a given band. This allows access points and other WLAN elements, operating on different frequency channels, to detect frames transmitted by the mobile station and provide refreshed signal strength information to a wireless node location mechanism.
TY - JOUR. T1 - Delayed delivery of surviving triplet after fetal reduction and spontaneous abortion. AU - Chang, Hung Chi. AU - Wu, Ming Yih. AU - Ho, Hong Nerng. AU - Yang, Yu Shih. PY - 1996/11/1. Y1 - 1996/11/1. N2 - In multiple pregnancies, survival of remaining fetuses after premature death and delivery of one fetus is uncommon. We report a case of a triplet pregnancy that was reduced to twins at the 14th gestational week and then had preterm premature rupture of membranes and intrauterine fetal death of one twin at the 17th gestational week. To save the surviving fetus, delivery of the dead fetus and ligation of the umbilical cord at the cervical level were performed. We also performed McDonald cervical cerclage to keep the placenta of the dead fetus as well as that of the surviving one in the uterine cavity. After a series of aggressive procedures, including immediate administration of tocolytic agents, and antibiotic prophylaxis to prevent infection and preterm labor, the surviving ...
O31.31X1 is a billable code used to specify a medical diagnosis of continuing pregnancy after elective fetal reduction of one fetus or more, first trimester, fetus 1. Code valid for the year 2020
From a medical perspective, this term refers to modalities to induce fetal demise (1). Feticide is most commonly used for selective termination of higher order gestations to twins or singletons. It is also used by some providers before medical and surgical abortion in the second and third trimesters to avoid signs of life at induction or in the belief that it makes the procedure easier and safer. Several methods have been described including intra-cardiac injection of potassium chloride, intra-amniotic injection of digoxin, and transection of the umbilical cord (2). Older methods of medical abortion employed instillation of hyperosmolar solutions such as urea, which also variably induced fetal demise. ...
The high rate of preterm delivery in the United States is the greatest contributor to the relatively high infant mortality rate in this country. Despite much effort, no effective method exists for prevention of preterm delivery. Progesterone has been studied as a prophylactic agent for preterm delivery with conflicting results. A meta-analysis of older trials using only 17 alpha-hydroxyprogesterone (17P) showed a significant protective effect. Meis and colleagues designed a multicenter modern retrial of 17P for prevention of pre-term delivery.. The authors screened patients for enrollment by reviewing prenatal charts of pregnancies at 15 to 20 weeks of gestation for a history of preterm delivery. Initial screening identified 2,980 women, of whom 1,039 were eligible after exclusion criteria, which included lack of documentation for prior preterm delivery, ultrasound dating of more than 20 weeks gestation, planned cerclage, multifetal pregnancy, or prior progesterone use during the current ...
Background: Vitamin D status is hypothesised to play a role in gestational glucose control. No studies to date have examined vitamin D in relation to changes in blood glucose in pregnancy. Thus, the aim was to examine if vitamin D in early pregnancy and vitamin D trajectory associate with blood glucose trajectory over pregnancy in a Swedish cohort. We also investigated the relation between maternal vitamin D status and excessive fetal growth. Methods: In 2013-2014, pregnant women were recruited to the GraviD cohort study when registering at the antenatal clinics in south-west Sweden. In the present analysis, 1928 women were included. Women with preexisting diabetes and multifetal pregnancy were excluded. Random blood glucose was assessed according to routine practice, in first trimester (T1, gestational week 4-16), second trimester (T2, gestational week 17-27), early (T3a, gestational week 28-35) and late third trimester (T3b, gestational week 36-41). In Ti and T3a, serum 25-hydroxyvitamim D ...

To the Editor.-. The American Academy of Pediatrics Committee on Adolescence policy statement on emergency contraception1 reports the effectiveness of the Yuzpe regimen (ethinyl estradiol and levonorgestrel) in terms of a pregnancy reduction of 70% to 80% and of levonorgestrel-only emergency contraception of 85%. These estimates are outdated. Using current methods for estimating effectiveness, the effectiveness rates seem to be in the range of 50% to 66% and 72% to 80%, respectively.2-5 Because there are no randomized trials with a placebo arm, considerable uncertainty remains about the effectiveness of emergency contraception.3,5. The policy statement also proposes that [e]mergency contraception has tremendous potential to reduce unintended pregnancy rates in teens and adults. This statement remains, as yet, a hypothesis that is unsupported by empirical evidence. Several studies have failed to document a decrease in rates of unintended pregnancy or abortion in populations that are provided ...

Article ID: DD135 , By: Scott B. Rae. Summary The new reproductive technologies give great hope to infertile couples and make many new reproductive arrangements possible. They also raise many difficult moral issues. Artificial insemination by husband is considered moral, but artificial insemination by donor raises questions about a third party entering reproduction. In vitro fertilization is acceptable within limits: the couple should ensure that no embryos are left in storage and that the risk of selective termination is avoided. Commercial surrogate motherhood raises problems because it is the equivalent of selling children, can be exploitative of the surrogate, and violates a mothers fundamental right to raise her child. Even altruistic surrogacy raises questions about the degree of detachment the mother must have from her unborn child to successfully give it up after birth. On March 27, 1986, Mary Beth Whitehead gave birth to a little girl whom she named Sara. That same day, Elizabeth and ...

TY - GEN. T1 - Synchronization of Ghostburster neurons using high order sliding mode control. AU - Han, Chun Xiao. AU - Wang, Jiang. AU - Che, Yanqiu. AU - Zhou, Si Si. PY - 2010/8/13. Y1 - 2010/8/13. N2 - In this paper, high order sliding mode control is proposed to realize the synchronization of two Ghostburster neurons under external electrical stimulation. Being a motion on a discontinuity set of a dynamic system, the sliding mode is used to keep accurately a given constraint and features theoretically-infinite-frequency switching. As the high order sliding mode technique employed in this paper, it considers a fractional power of the absolute value of the tracking error coupled with the sign function. This structure provides several advantages such as the simplification of the control law, higher accuracy and chattering prevention. Firstly we analyze the periodic and chaotic dynamics of individual Ghostburster neuron under different external electrical stimulus, then high order sliding mode ...
Over the years that our organization, the Center for Loss In Multiple Birth (CLIMB), Inc. has been hearing the experiences of countless bereaved parents of twins and higher multiples, we have heard from an increasing number who have undergone the selective reduction of one or more of their potential babies, or chosen not to, with or without later loss, as well as from many considering selective reduction. Some of these have included:. 1) a CLIMB member who, after many years of infertility, conceived 4 babies. Devoutly religious, she reduced to two for the sake of the babies; they were born at 26 weeks, one dying at 4 months old and the other blind and developmentally delayed, now age 10 and the familys only living child.. 2) another CLIMBer who revealed that her twins, conceived after 10 years of infertility and in-vitro fertilization, had actually been 3. She made the decision to reduce to two; then one of the remaining babies died in-utero and the other was born at 26 weeks and died three ...
Loss in placentome number paralleled the observed drop in fetal and organ weights. The study found increased placentomal weight representing better placental efficiency, but similar to what we found in more extreme space restriction (Meyer et al. 2010; Meyer‐Gesch et al. 2013), after GD120, the ability to compensate in higher‐order multifetal gestation was limited. Thus, in this situation, placental efficiency does not accurately reflect functional indices including placental clearance or delivery of nutrients and metabolites. As anticipated, brain weights (g) did not fall, but rose when expressed as ratio of brain weight (g) per body weight, (kg) as fetal number per ewe rose, defining head‐sparing asymmetric growth restriction, with other organ weights proportionate to body weight. As anticipated, males weighed more than females. Brain weight fell slightly (86% of singletons) when fetal number reached four. Many of the fetal body weights fell below our flocks 10th percentile for ...

Experimental paradigms using the pregnant sheep have been widely used to identify the nutritionally sensitive periods of conceptus development. When quintuplets are born, its often something that doctors and nurses talk about as its not something that happens all the time. When a set of five babies were born, they appeared to be healthy. However, when doctors started looking closely at them, they noticed something shocking.. Adam and Danielle heard the news that they were unable to have children, so they decided to enlist the help of a doctor who might be able to help them get pregnant by using various medications before they went through IVF. They tried one last time to get pregnant and left everything in the hands of God.. The couple soon had a little girl. A few years later, they knew they wanted other children. They followed the same medical routine and learned that they were pregnant with five babies. The set of quintuplets is the first that are all girls. They are happy and healthy, growing like normal. Doctors were surprised when they found out they were all girls. The babies were born at 28 weeks, but they were healthy when ...

In this thesis, we have analyzed the accuracy and stability aspects of weak boundary and interface conditions (WBCs) for high order finite difference methods on Summations-By-Parts (SBP) form. The numerical technique has been applied to wave propagation and flow problems.. The advantage of WBCs over strong boundary conditions is that stability of the numerical scheme can be proven. The boundary procedures in the advection-diffusion equation for a boundary layer problem is analyzed. By performing Navier-Stokes calculations, it is shown that most of the conclusions from the model problem carries over to the fully nonlinear case.. The work was complemented to include the new idea of using WBCs on multiple grid points in a region, where the data is known, instead of at a single point. It was shown that we can achieve high accuracy, an increased rate of convergence to steady-state and non-reflecting boundary conditions by using this approach.. Using the SBP technique and WBCs, we have worked out how ...

Abstract , References , Similar Articles , Additional Information Abstract: This paper is concerned with the development of high order methods for the numerical approximation of one-dimensional nonconservative hyperbolic systems. In particular, we are interested in high order extensions of the generalized Roe methods introduced by I. Toumi in 1992, based on WENO reconstruction of states. We also investigate the well-balanced properties of the resulting schemes. Finally, we will focus on applications to shallow-water systems. ...
The type of placentation in multifetal gestation is predictive of many complications and pregnancy outcome. It depends upon the type of placentation whether the triplet pregnancy is trichorionic, dichorionic or monochorionic. Trichorionic triplets have three foetuses with separate placentas and amniotic cavities. Dichorionic triplets have a singleton pregnancy with its own placenta along with monochorionic twins. Monochorionic triplets have three foetuses with one common placenta and three separate amniotic cavities as was seen in our case. Multiple pregnancy is associated with higher risks and complications for both mother and babies as compared to singleton pregnancies. Over 60 percent of twins and almost all higher order multiples are premature. The expected average duration of pregnancy decreases with increasing number of foetuses. The average duration of pregnancy is 35 weeks for twins, 32 weeks for triplets and 30 weeks for quadruplet. The risk of spontaneous loss of entire pregnancy is 8% ...
