TY - GEN. T1 - EBCPG. T2 - 9th International Congress on Nursing Informatics, NI 2006. AU - Yang, Wen Wen. AU - Chiang, I-Jen. PY - 2006. Y1 - 2006. N2 - Evidence-based clinical practice guidelines (CPG) have been recognized as an important guide to physicians for decision-making. These guidelines can assist physicians in giving good clinical care to their patients. Based on the overall healthcare improvements, medical care quality can be preserved while practice discrepancies and errors can be eliminated. Clear representation and accessible guideline content at the point care are also critical. One way to make implementation easier is through computerized and visualized guidelines combined with evidence-based literature. Therefore, this paper presents a computerized guideline system, EBCPG, which is composed of three main components: CPGs exploration, EBM exploration and Web-base management to accommodate clinician decision-making and maintainability. Two tasks based on CPGs exploration and ...
STS Clinical Practice Guidelines are intended to assist physicians and other health care providers in clinical decision-making by describing a range of generally acceptable approaches for the diagnosis, management, or prevention of specific diseases or conditions. These guidelines should not be considered inclusive of all proper methods of care or exclusive of other methods of care reasonably directed at obtaining the same results. Moreover, these guidelines are subject to change over time, without notice. The ultimate judgment regarding the care of a particular patient must be made by the physician in light of the individual circumstances presented by the patient.. STS Systematic Reviews are intended to compliment clinical practice guidelines in assisting physicians and other health care providers in clinical decision-making by evaluating the strength of evidence regarding particular treatments, and determining whether the level is sufficient enough to make recommendations for clinical ...
A database of clinical guidelines, the NGC is maintained by the Agency for Healthcare Research and Quality of the U.S. Department of Health and Human Services. Guidelines must meet stringent standards to be accepted, and must provide robust evidence-based recommendations or action plans for patient care. Posting to the clearinghouse means the adult eye and vision examination guideline is publicly available for other health care professionals, institutions and agencies to reference with knowledge that it has met NGC standards, said Diane Adamczyk, O.D., chair of the AOA Evidence-Based Optometry (EBO) Committee, which released the guideline in September 2015. The rigorous process of developing this guideline has now met the high standards of the National Guideline Clearinghouse, which, like AOAs Eye Care of the Patient with Diabetes Mellitus, is a testament to the exacting work by the guidelines authors, Dr. Adamczyk said.. What the adult eye and vision examination guideline offers Revised ...
An evidence-based clinical practice guideline1 to inform physicians on the management of uncomplicated acute otitis media (AOM) among children ages two months through 12 years is now available. It was developed by the multidisciplinary Subcommittee on Management of Acute Otitis Media, which was composed of representatives from the American Academy of Pediatrics (AAP), the American Academy of Family Physicians (AAFP), and other relevant professional organizations. A summary of this clinical practice guideline appears in this issue of American Family Physician.2. Using a methodology similar to that used for the recent guideline on the management of otitis media with effusion,3 this AOM guideline is based on the best available published data as summarized in the Agency for Healthcare Research and Quality report on AOM4 and literature subsequently published through September 2003. Evidence-based statements contained in the AOM guideline1 follow AAP definitions, reflecting both the quality of ...

Background: Clinical practice guidelines (CPG) are thought to be an effective tool in improving efficiency and outcomes of clinical practice. Physicians adherence to guidelines is reported to be poor. We evaluated the relationship between generalizability of guidelines on hypertension and physicians adherence to guidelines recommendations for pharmacological treatment. Methods: We used the Sixth Joint National Committees (JNC VI) guidelines on hypertension to evaluate our hypothesis. We evaluated the evidence from controlled clinical trials on which the JNC VI bases its recommendation, and compared the population enrolled in those trials with the American hypertensive population. Data on this population came from the National Health and Nutritional Examination Survey III. Results: Twenty-three percent of the NHANES population had a diagnosis of hypertension, 11% had hypertension requiring drug treatment according to the JNC VI. Only half of the population requiring treatment would have been ...
Patients with hip fractures have high morbidity and mortality, which has not changed significantly since last two decades. There are various national guidelines in UK, which give guidance to help improve outcomes in these patients such as National Institute for Health and Clinical Excellences guidelines (NICE), Guidelines from Anaesthetists of Great Britain and Ireland (AAGBI) and Scottish intercollegiate guideline network (SIGN). NICE guidelines recommend total hip replacement (THR) rather than hemiarthroplasty in patients with a displaced intracapsular hip fracture in selected patients. AAGBI has produced guidelines for Management of Proximal Femoral Fractures 2011.
During the first decade of the ACC/AHA Guidelines effort, other organizations often played a collaborative role. This included providing representatives to the writing committee, designating reviewers from their organization, formally endorsing the guideline, and, occasionally publishing the guideline in their own journal. Although this was a useful model, the ACC/AHA Task Force on Practice Guidelines gradually recognized that it was an inadequate one. Patients with cardiovascular disease often receive care from general internists and family medicine physicians. Thus, it was imperative that those organizations be included to a greater degree. In addition, there was a clear need for medicine to speak with one voice. Other organizations, including the cardiovascular subspecialty organizations and the ESC, were publishing an increasing number of clinical practice guidelines that overlapped in content with the ACC/AHA Guidelines.. The initial effort to involve other organizations in a full ...

The Guidelines Panel is comprised of experts from NCCN Member Institutions across the United States, including oncologists, HIV specialists, ...
Physical attractiveness or unattractiveness wields a tremendous impact on the social and psychological components of life. Many individuals with facial deformities are treated more negatively than normal individuals, which may affect their self-image, quality of life, self-esteem, interpersonal encounters, and ultimately, success in life. Malformations that do not create physiological problems and whose major health impact is to degrade physical attractiveness and engender psychosocial consequences are insufficiently understood and not considered functional problems by medical insurance companies. As part of a clinical practice guideline development process for psychosocial concerns in Freeman-Burian syndrome, manuscripts describing psychosocial considerations related to the presence of non-intellectually impairing craniofacial malformation conditions or associated clinical activities are sought, especially focusing on epidemiology, prevention, symptoms, diagnoses, severity, timing, treatment,
AbstractIntroduction: Two 2-day continuing education seminars were developed to address the orthopedic physical therapy deficits in the country of Guyana. Material was presented in a way to address all stages of behavior change. Methods: Surveys evaluating pre- and post-seminar knowledge was evaluated. Chart reviews to establish adherence to clinical practice guidelines were performed. Results: Pre-seminar surveys revealed minimal knowledge of clinical practice guidelines which was consistent with pre-seminar chart review data. Post-seminar data indicate improvements in both knowledge and adherence to guidelines.Discussion: A brief series of two 2-day seminars utilizing behavior change strategies to improve adherence to clinical practice guidelines shows promise for countries and regions that rely on international health volunteers to provide clinical instruction. Because this study is limited to one situation, further studies with longer follow-up in a variety of clinical settings are recommended to
In a report published May 27 in the Journal of the American Medical Association, Mark Neuman, MD, MSc, Department of Anesthesiology and Critical Care, Perelman School of Medicine, University of Pennsylvania, and his colleagues examined the lasting strength of class I - i.e. procedure/treatment should be performed/administered - recommendations across serial versions of selected American College of Cardiology/American Heart Association guidelines, and found that durability of the guidelines varied across individual guidelines and levels of evidence.. Identifying 619 class I recommendations in 11 index guidelines published between 1998 and 2007, the investigation found that 495 recommendations (80.0 percent) were retained in the subsequent guideline version, 57 (9.2 percent) were downgraded or reversed, and 67 (10.8 percent) were omitted. The ultimate percentage of recommendations that were retained varied across guidelines from 15.4 percent to 94.1 percent.. The endurance of individual ...
A multidisciplinary task force developed and implemented a skincare programme. Two key initiatives ran concurrently: design and implementation of skincare programmes on the clinical units and systematic assessment of pressure ulcer prevalence. The skincare task force, led by the director of nursing research and quality improvement, reviewed the research and existing evidence-based clinical guidelines such as the 1992 US Agency for Health Care Policy and Research (AHCPR) Clinical Practice Guideline on pressure ulcers in adults.2. The task force considered the AHCPR practice guideline to be a credible source. Convened by a prominent health association, the guideline was developed by a multidisciplinary panel which reviewed the research and graded the evidence. It offered a standard for evidence-based practice. Although fortunate to have a current clinical practice guideline available, most of the guideline recommendations were supported by level C evidence indicating that they were drawn from ...
Background:. The purpose of this study is to design and evaluate targeted implementation strategies to fully integrate the VHA clinical practice guidelines for ischemic heart disease into VHA clinical practice. Effectively implementing the guideline will enhance the quality, appropriateness, timeliness, and cost effectiveness of care delivered to veterans with ischemic heart disease.. Objectives:. The long-term objective of this study is to identify the best strategies for implementing the IHD guidelines to improve guideline adherence and provider acceptance. The specific objectives of the study are to: 1) describe the temporal aspects of guideline acceptance and adherence over three periods of time: pre-implementation, post-implementation of general strategies, and post- implementation of targeted strategies; 2) assess the relative effectiveness of targeted intervention strategies on guideline adherence; 3) identify the relationships among provider beliefs, attitudes, and their intentions to ...
The in-patient treatment unit is based in a large psychiatric hospital that is currently being redeveloped, and there has been considerable discussion regarding the future location of the unit, including the level of medical care required. Our research objectives emerged from informal discussions among medical and nursing staff, reflecting on an anecdotal impression that the in-patient unit was treating patients with ever-increasing medical complexity. It is not uncommon for patients to be transferred directly to a general hospital at the point of admission, or shortly afterwards, owing to the severity of their physical condition. The data presented here support this impression.. The unit has based its admission criteria on a since-withdrawn guideline from the Scottish Intercollegiate Guidelines Network (SIGN) - guideline no. 74.9 It outlines the patient characteristics that are likely to make community detoxification suitable and, conversely, situations where in-patient detoxification would be ...
In the United Kingdom, there is very little formal competency based training in the techniques of paediatric procedural sedation except in the specialty of dentistry.1,2 Procedural sedation is often delegated to junior medical staff using a local protocol. The procedures for informed consent, documentation, monitoring, recovery, follow up, and audit are varied and often non-existent. This situation is changing as new clinical governance and training systems are put in place. In Scotland, the Scottish Intercollegiate Guidelines Network (SIGN) has produced a multidisciplinary, evidence based, peer reviewed guideline for the management of paediatric procedural sedation.3 The consensus view of this group was that, in Scotland, ketamine (and agents such as propofol, fentanyl, alfentanil, and remifentanil) should only be used by those trained in and with adequate on-going experience of paediatric anaesthesia and/or intensive care. It should only be used in a hospital setting. The product licence (and ...

The aim of the clinical practice guidelines is to develop recommendations that are based on good research evidence.[11,12] The majority of the recommendations in this area are based on fair research evidence or on expert consensus. This was evident also in the ASPEN Guidelines in Nutrition Support5 since only 16% of the recommendations were judged to be based on good research-based evidence (meta-analysis of RCTs or single RCTs), 29% on fair researchbased evidence (well-designed controlled or non-controlled studies without randomisation, non-experimental studies such as comparative, correlational or case studies) and finally 55% on expert opinion alone. A similar percentage also applies to the present ESPEN clinical practice guidelines. However, this should not classify the recommendations of these guidelines as poorquality statements. It simply reflects the fact that the gold standard methodology to scientifically validate a drug or a therapy (the RCT) cannot always be used to investigate the ...
Background: Acute diarrhea in children is associated with high morbidity globally and with high mortality in undeveloped countries. The Colombian Ministry of Health included childhood diarrhea among the conditions for which new national guidelines are being developed. Before developing a clinical practice guideline (CPG) it is advisable to assess published guidelines to consider its adaptation. The quality of published guidelines on childhood diarrhea has not been assessed.. Objective: To assess its quality of CPG on acute diarrhea or gastroenteritis in children using the AGREE-II instrument.. Methods: We searched electronic databases, national clearinghouses and non-electronic sources until august 2011. Each guideline retrieved was independently assessed using AGREE-II (Spanish version) by three clinical epidemiologists.. Results: We found 63 guidelines, 17 of which (26.9%) were evidence-based. Based on AGREE-II, domains 1 (scope and purpose) and 4 (clarity of presentation) had the highest ...
In this work, we described the development of evidence-based guidelines for the care of patients with diabetes by primary care practitioners, using glycemic and blood pressure control and retinal screening as examples. The careful review and analysis of an independently prepared, evidence-based synthesis by a multispecialty panel of experts provided the best available evidence to the working group. Guidelines are meant to be applicable to large and complex health care systems and yet to empower the process of individual patient-practitioner counseling and goal setting based on individual risk stratification. The VA/DoD Clinical Guidelines for Treatment of Diabetes are intended to be a unifying standard of medical practice for the two largest agencies in the federal health care system that provide direct health care.. Some may criticize certain recommendations as being economically motivated with a design to minimize short-term costs. However, any target value endorsed by a guideline obligates ...

These guidelines are intended as an update to those published in the Southern African Journal of HIV Medicine in 2012. Since the release of the previous guidelines, the scale-up of antiretroviral therapy (ART) in southern Africa has continued. Cohort studies from the region show excellent clinical outcomes; however, ART is still being initiated late (in advanced disease) in some patients, resulting in relatively high early mortality rates. New data on antiretroviral drugs have become available. Although currently few, there are patients in the region who are failing protease-inhibitor-based second-line regimens. To address this, guidelines on third-line therapy have been expanded.. ...

Diet regimens that are low in hydrogenated fat and also cholesterol, high in fiber, and also low in salt are the best choices for a healthy and balanced diet plan. Lowering cholesterol via diet regimen alone can minimize the threat of heart diseases such as coronary artery illness (CAD) and stroke. Individuals with hypertension need to also make an effort to keep their high blood pressure low. High blood pressure boosts the threat of creating cholesterol-related problems, so it should be managed also. Ada Blood Cholesterol Guidelines. Smoking cigarettes or using various other cigarette items has been shown in countless research studies to increase LDL cholesterol levels and decrease HDL cholesterol levels. This is why giving up smoking is so essential. Other factors to cholesterol include excessive weight, physical inactivity, alcohol consumption, and the existence of other threat elements, such as diabetes, cardiovascular disease, and cancer. OPTIONS: The following interventions were systematically reviewed for inclusion in the guidelines: enteral nutrition (EN) versus parenteral nutrition (PN), early versus late EN, dose of EN, composition of EN (protein, carbohydrates, lipids, immune-enhancing additives), strategies to optimize delivery of EN and minimize risks (ie, rate of advancement, checking residuals, use of bedside algorithms, motility agents, small bowel versus gastric feedings, elevation of the head of the bed, closed delivery systems, probiotics, bolus administration), enteral nutrition in combination with supplemental PN, use of PN versus standard care in patients with an intact gastrointestinal tract, dose of PN and composition of PN (protein, carbohydrates, IV lipids, additives, vitamins, trace elements, immune enhancing ...
In 2011, the American Academy of Pediatrics released a revision of its 1999 clinical practice guideline on urinary tract infections in febrile infants and young children two to 24 months of age. The new clinical practice guideline has several important updates based on evidence generated over the past decade. The updated guideline includes clinical criteria for collecting urine specimens. Diagnosis now requires evidence of infection from both abnormal urinalysis results and positive urine culture results (the criterion for a positive culture has been reduced from at least 100,000 colony-forming units per mL to at least 50,000 colony-forming units per mL). Oral treatment now is considered to be as effective as parenteral treatment. Renal and bladder ultrasonography is still recommended, but the biggest change in the current guideline is that routine voiding cystourethrography is no longer recommended after the first urinary tract infection. Follow-up is based on evaluating children for urinary tract

ASCOs Guidance Products Clinical Practice Guideline: Addresses specific clinical situations (disease- oriented) or use of approved medical products, procedures, or tests (modality-oriented). Systematic reviews serve as the evidentiary basis for drafting principles of clinical care. Interpretation and extrapolation of evidence are often necessary. Consensus Guideline: A systematic review is conducted. If evidence identified is limited, inconsistent, indirect, or of poor quality, then the formal consensus-based methodology may be considered. While the decision to incorporate consensus recommendation(s) may vary, the common thread is lack of sufficient evidence. ASCO utilizes a modified Delphi process for creating consensus guideline recommendations. Several factors contribute to the high mortality attributed to severe infections in resource-limited settings. While improvements in survival and processes of care have been made in high-income settings among patients with severe conditions, such as sepsis, guidelines necessary for achieving these improvements may lack applicability or have not been tested in resource-limited settings. The World Health Organizations recent publication of the Integrated Management of Adolescent and Adult Illness District Clinician Manual provides details on how to optimize management of severely ill, hospitalized patients in such settings, including specific guidance on the management of patients with septic shock and respiratory failure without shock. This manuscript provides the context, process and underpinnings of these sepsis guidelines. In light of the current deficits in care and the limitations associated with these guidelines, the authors propose implementing these standardized best practice guidelines while

A medical guideline (also called a clinical guideline or clinical practice guideline) is a document with the aim of guiding decisions and criteria regarding diagnosis, management, and treatment in specific areas of healthcare. Such documents have been in use for thousands of years during the entire history of medicine. However, in contrast to previous approaches, which were often based on tradition or authority, modern medical guidelines are based on an examination of current evidence within the paradigm of evidence-based medicine. They usually include summarized consensus statements on best practice in healthcare. A healthcare provider is obliged to know the medical guidelines of his or her profession, and has to decide whether to follow the recommendations of a guideline for an individual treatment. Modern clinical guidelines identify, summarize and evaluate the highest quality evidence and most current data about prevention, diagnosis, prognosis, therapy including dosage of medications, ...
The best way to avoid being sued for malpractice is to make certain that all your patients are happy and all their outcomes are good.. Reality is seldom so rosy. Patients arent necessarily happy even when their clinical outcomes are as good as they can get. In the event of an undesired outcome, an unhappy patient may easily become a litigious one. A 2011 study in the New England Journal of Medicine estimated that 36 percent of physicians in low-risk specialties such as pediatrics, and 88 percent of physicians in high-risk surgical specialties, would face a malpractice claim by the age of 45. Those percentages climb to 75 percent of physicians in low-risk specialties and 99 percent of physicians in high-risk specialties by the age of 65.. Flaws in clinical practice guidelines. Can clinical practice guidelines protect us? We are all beset by the proliferating standards and guidelines of evidence-based medicine. Its comforting to think that a court may consider adherence to a legitimate clinical ...

Peripheral artery disease (PAD) is often asymptomatic but increases the risk of developing cardiovascular events. Due to the uncertainties regarding the quality of related guidelines and a lack of clear-cut evidence, we performed a systematic review and critical appraisal of these guidelines to evaluate their consistency of the recommendations in asymptomatic PAD population. Guidelines in English between January 1st, 2000 to December 31th, 2017 were screened in databases including Medline via PubMed, EMBASE, the G-I-N International Guideline Library, the National Guidelines Clearinghouse, the Canadian Medication Association Infobase and the National Library for Health. Those guidelines containing recommendations on screening and treatment for asymptomatic PAD were included, and three reviewers evaluated the quality of the guidelines using Appraisal of Guidelines Research and Evaluation (AGREE) II instrument. Related recommendations were then fully extracted and compared by two reviewers. Fourteen
The key aim of this guideline is to make oxygen use in emergency and healthcare settings safer, simpler and more effective. Oxygen is probably the commonest drug used in the care of patients who present with medical emergencies. Prior to the publication of the first British Thoracic Society Guideline for Emergency Oxygen Use in Adult Patients in 2008,1 ambulance teams and emergency department teams were likely to give oxygen to virtually all breathless or seriously ill patients and also to a large number of non-hypoxaemic patients with conditions such as ischaemic heart disease or stroke based on custom and practice. About 34% of UK ambulance journeys in 2007 involved oxygen use.4 This translated to about two million instances of emergency oxygen use per annum by all UK ambulance services, with further use in patients homes, GP surgeries and in hospitals. Audits of oxygen use and oxygen prescription have shown consistently poor performance in many countries, and most clinicians who deal with ...
The American Thoracic Society, Centers for Disease Control and Prevention, European Respiratory Society and IDSA have published an official clinical guideline on the treatment of drug-resistant tuberculosis in the Nov. 15 American Journal of Respiratory and Critical Care Medicine.. The guideline makes new recommendations for the choice and number of drugs, as well as the duration of treatment for drug-resistant tuberculosis. These recommendations prioritize the use of medications that can be administered orally. The guideline makes clear that treatment should be tailored based on drug-susceptibility testing, and that individuals should not receive medicines to which the Mycobacterium tuberculosis strain is resistant.. The guideline recommends treatment with a later-generation fluoroquinolone of all infected contacts of multidrug-resistant tuberculosis patients, rather than watchful observation, and it provides evidence-based guidance for the treatment of pregnant women with MDR-TB for the first ...
Provides clinical descriptions, diagnostic guidelines, and codes for all mental and behavioural disorders commonly encountered in clinical psychiatry. The book was developed from chapter V of the Tenth Revision of the International Statistical Classification of Diseases and Related Health Problems (‎ICD-10)‎. The clinical descriptions and diagnostic guidelines were finalized after extensive field testing by over 700 clinicians and researchers in 110 institutes in 40 countries, making this book the product of the largest ever research effort designed to improve psychiatric diagnosis. Every effort has been made to define categories whose existence is scientifically justifiable as well as clinically useful. The classification divides disorders into ten groups according to major common themes or descriptive likeness, a new feature which makes for increased convenience of use. For each disorder, the book provides a full description of the main clinical features and all other important but less ...
The 2017 American College of Cardiology/American Heart Association guideline defines hypertension as a blood pressure ≥130/80 mm Hg, whereas the 2018 European Society of Cardiology (ESC) and 2019 National Institute for Health and Care Excellence (NICE) guidelines use a ≥140/90 mm Hg threshold. Our objective was to study the associations between isolated diastolic hypertension (IDH), diagnosed using these 2 blood pressure thresholds, and cardiovascular disease (CVD) in a large cohort of UK adults. We analyzed data from UK Biobank, which enrolled participants between 2006 and 2010 with follow-up through March 2019. We excluded persons with systolic hypertension or baseline CVD. We defined incident CVD as a composite of nonfatal myocardial infarction, nonfatal stroke, or cardiovascular death. We used Cox regression to quantify associations between IDH and CVD, as well as the individual outcomes included in the composite outcome. We studied 151 831 participants with normal systolic blood ...

To address the lack/limited local evidence base, a model of contextualization of global evidence process to inform local practice decisions in the form of clinical practice guidelines was presented and the processes undertaken. Local clinical practice guidelines on stroke and low back pain were developed using this model of contextualization. This innovative process is reported in the paper by Suarez et al 2012 and described in the following:. Contextualization of Western Guidelines for Stroke and Low Back Pain Rehabilitation (http://www.dovepress.com/contextualizing-western-guidelines-for-stroke-and-low-back-pain-to-a-d-peer-reviewed-article-JHL). The medical associations in the Philippines have been asked to write clinical practice guidelines on the most common conditions which have the highest cause of morbidity and mortality. Evidence-based practice is a new concept in the Philippines and there is a limited local capacity or knowledge regarding guideline development. In 2011 Philippine ...
TY - JOUR. T1 - A New Guideline on Treatment of Hypertension in Those with Coronary Artery Disease. AU - Jennings, Garry L.R.. PY - 2015/11/1. Y1 - 2015/11/1. N2 - Hypertension is a major risk factor for coronary artery disease (CAD) and the two frequently coexist. The peak cardiology and hypertension societies of the United States recently published new guidelines on the treatment of hypertension in people with CAD. The guidelines update those previously issued eight years previously in the light of new trial data. However, for the most part they will validate what cardiologists are already doing in these patients. The major change is resetting of the blood pressure general treatment target for most people with hypertension and CAD from 130/80 mm Hg to 140/90 mm Hg. It is arguable that the ...

The American Cancer Society (ACS) and the American Society of Clinical Oncology (ASCO) have developed new guidelines on breast cancer survivorship care. The guidelines are to help primary care doctors identify and manage possible physical and psychosocial long-term and late side effects of cancer and its treatment, as well as other aspects of post-treatment survivorship care for adults whove been diagnosed with cancer. The guidelines were published on Dec. 7, 2015 in CA: A Cancer Journal for Clinicians. Read American Cancer Society/American Society of Clinical Oncology Breast Cancer Survivorship Care Guideline. ASCO is a national organization of oncologists and other cancer care providers. ASCO guidelines give doctors recommendations for treatments and testing that are supported by much credible research and experience. These breast cancer guidelines are the third in a continuing series of survivorship. The earlier guidelines addressed the needs of prostate and colorectal cancer survivors. To ...
Standardization of the lens Various studies support the concept of standardization of the size of hpf of lens which depends on the model and the maker of the microscopes. It varies from 0.12 to 0.44mm. It is possible that a count that falls below the diagnostic threshold in one microscope may be above the threshold in a different one [11].The lack of consensus on the size of hpf, can result in 23 fold variability in the description of esinophil density and therefore can affect the diagnosis and the follow-up [12]. Gastro duodenal biopsies To my mind, researchers working on EoE should take biopsies at least from the stomach and the duodenum in addition to the esophagus and so should be aware of the diagnostic guidelines of Eosinophilic gastroenteritis (EGE) which comes next to EoE in prevalence. The guidelines of EGE states that the diagnosis depends on infiltration of the mucosa by dense aggregates or sheets of esinophilis, with potential extension into the muscularis mucosa and submucosa, and ...
The International Liaison Committee on Resuscitation uses the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) working group method to evaluate the quality of evidence and the strength of treatment recommendations. This method requires guideline developers to use a numerical rating of the importance of each specified outcome. There are currently no uniform reporting guidelines or outcome measures for neonatal resuscitation science. We describe consensus outcome ratings from a survey of 64 neonatal resuscitation guideline developers representing seven international resuscitation councils. Among 25 specified outcomes, 10 were considered critical for decision-making. The five most critically rated outcomes were death, moderate-severe neurodevelopmental impairment, blindness, cerebral palsy and deafness. These data inform outcome rankings for systematic reviews of neonatal resuscitation science and international guideline development using the GRADE methodology. ...
The new edition of Therapeutic Guidelines: Palliative Care builds on the excellent first edition. This small pocket-sized text is a vital part of the Therapeutic Guidelines stable. The published version is very user-friendly and I am looking forward to loading the mini computer version, which is now available, onto my personal organiser.. The Palliative Care second version has some changes in format and a tightening of the overall presentation. The order and format of chapters has been streamlined and minor changes only add to the usefulness of this text. The order of chapters reflects the challenges of caring for people with life-limiting illnesses. There is considerable space given to principles, care of the provider of palliative care, ethical issues and communication. Then follow important guidelines regarding community care and other practical factors. The major symptom groups in order of significance and prevalence are then covered with comprehensive consideration of not only ...
The decision to develop a NICE guideline or quality standard on a particular health, public health or social care topic is influenced by a number of factors.. Generally, the topics selected come either from libraries of quality standard topics already referred to NICE, or from updates of previously published guidelines.. The focus on topic selection at NICE therefore is largely on prioritising existing topics. Alongside this portfolio of topics, NICEs commissioners(NHS England and the Department of Health) are likely to continue to identify a small number of new topics, which will have to be similarly prioritised.. On this page the approach to prioritising referred topics for guidelines and quality standards is set out, along with the mechanism for agreeing any additional new topics. The core elements of this process will be included in the new unified guideline manual.. This process applies only to guidelines for public health, social care and clinical topics, not to NICE guidance on new ...

Our interviewees were mostly used to serving as clinician methodologists on a guideline panel. Hence, their views might be biased from a dominantly methodological point of view. Patient or other health professional guideline panelists might have different views on the challenges experienced in guideline development for medical tests that the findings of our study may not include. Despite this limitation, the views expressed in our study are in line with other research that call for better regulatory frameworks for medical test approval and corroborate our findings that health professionals in general find it difficult to understand test accuracy measures and relate that to downstream patient consequences.19-22 Our sample of interviewees play an intricate role, as methodologists, in assisting all panelists with issues they may encounter in the guideline process, hence they have a good understanding of the challenges faced by different types of panelists.. In this study as in other qualitative ...

BACKGROUND: Different strategies have been proposed for the cardiovascular risk management of patients with psoriasis. OBJECTIVE: To estimate the cardiovascular risk and evaluate two cardiovascular prevention strategies in patients with psoriasis, analyzing which proportion of patients would be candidates to receive statin therapy. METHODS: A retrospective cohort was selected from a secondary database. All patients >18 years with psoriasis without cardiovascular disease or lipid-lowering treatment were included. The atherosclerotic cardiovascular disease calculator (2018 American College of Cardiology/American Heart Association guidelines) and the Systematic Coronary Risk Evaluation risk calculator (2016 European Society of Cardiology/European Society of Atherosclerosis guidelines) were calculated. The SCORE risk value was adjusted by a multiplication factor of 1.5. The recommendations for the indication of statins suggested by both guidelines were analyzed. RESULTS: A total of 892 patients ...
NICE has renewed accreditation of the process used by the British Association of Dermatologists to produce clinical guidelines. The accreditation recognises the stringent protocol that the BAD employs in producing clinical guidelines.. The renewed accreditation is valid until 31st May 2021 and applies to guidance produced using the processes described in Updated guidance for writing a British Association of Dermatologists clinical guideline - the adoption of the GRADE methodology 2016.. The original accreditation term began on 12th May 2010 and retrospectively covered guidelines that were accepted for publication from September 2008.. The accreditation means that future BAD guidelines will not only need to follow the accredited process manual strictly, but also fulfill all of the 25 criteria set by NICE in order to maintain the BADs status as an accredited guidance producer (please click here for the NICE guidelines checklist).. Further information on the BADs accreditation decision can be ...

Measuring clinical performance within a framework of clinical governance is increasingly important for monitoring improvements in patient care. Clinical audit often reveals wide variation and reducing this is one way of increasing consistency, which should improve quality of care and, ultimately, health. Practice guidelines are instruments used to give assistance about the action to take for a specific clinical situation. They usually contain a number of quality indicators and specify standards that must be met. While there are several sources of guidance about the process of developing evidence based practice guidelines,1,2 it is more difficult to find out how to develop quality indicators.3,4 In addition, although it is widely agreed that standards should be evidence based there is little information about how to synthesise the results of different studies to define exact standards for the guidelines. Evidence from randomised controlled trials should be used to define standards for clinical ...

Monitoring for new thrombosis, acquired VWD, and/or disease-related major bleeding (in patients with ET) and management of cardiovascular risk factors are recommended for all patients. After initiation of low-dose aspirin (and phlebotomy for patients with PV), the guidelines recommend monitoring symptom status using the MPN Symptom Assessment Form Total Symptom Score (MPN-SAF TSS), evaluating for signs and symptoms of disease progression every 3 to 6 months, and assessing for potential indications for cytoreductive therapy. Bone marrow aspirate and biopsy should be performed as clinically indicated (if supported by increased symptoms and signs of progression).. The development of new thrombosis or disease-related major bleeding, frequent or persistent need for phlebotomy, symptomatic or progressive splenomegaly, symptomatic thrombocytosis, progressive leukocytosis, or progressive disease-related symptoms are considered potential indications for cytoreductive therapy. Abstract:. Dialysis vascular access management in the United States changed significantly after National Kidney Foundation-Kidney Disease Outcome Quality Initiative (NKF-KDOQI) clinical practice guidelines were first published in 1997. The Centers for Medicare and Medicaid Service adopted these guidelines and in collaboration with the End-Stage Renal Disease Networks established the Fistula First Breakthrough Initiative (FFBI) in 2003 to improve the rate of arteriovenous fistula use over arteriovenous graft and central venous catheter in the dialysis population. The implementation of guidelines and FFBI has led to a significant increase in the arteriovenous fistula use in the prevalent dialysis population. The guidelines are criticized for being opinion based and often impractical. Over the past 2 decades, the patient population undergoing dialysis has become older with complex comorbidities and challenges for creating an ideal vascular access. Advancing knowledge about access pathophysiology, ...

In the general population, the majority of cardiovascular events occur in people at the low to moderate end of population risk distribution. The 2013 American College of Cardiology/American Heart Association guideline on the treatment of blood cholesterol recommends consideration of statin therapy for adults with an estimated 10-year atherosclerotic cardiovascular disease (ASCVD) risk ≥7.5% based on traditional risk factors. Whether use of nontraditional risk markers can improve risk assessment in those below this threshold for statin therapy is unclear.. ...

The Task Force recommends against the use of RT-CGM alone for glucose management in the intensive care unit (ICU) or operating room until further studies provide sufficient evidence for its accuracy and safety in those settings (1 , +OOO).. RT-CGM in Children and Adolescent Outpatients. The Task Force recommends that RT-CGM with currently approved devices be used by children and adolescents with type 1 diabetes mellitus (T1DM) who have achieved glycosylated hemoglobin (HbA1c) levels below 7.0% because it will assist in maintaining target HbA1c levels while limiting the risk of hypoglycemia (1 , ++++).. The Task Force recommends RT-CGM devices be used with children and ...
Management of patients with low-gradient AS is very challenging because, at resting echocardiography or catheterization, these patients have the combination of a small AVA with a low gradient, which raises uncertainty about the actual stenosis severity and thus the indication of AVR (Figure 1) (2). Additional tests such as assessment of aortic valve morphology and motion by 2-dimensional (2D) transthoracic or transesophageal, dobutamine stress echocardiography, or aortic valve calcium scoring by multidetector computed tomography are often required in these patients to confirm AS severity and the need for AVR (Figure 1) (2-4). Furthermore, the presence of a low gradient is often the marker for a low-flow state and associated comorbidities mentioned previously. These factors may increase the risk of complications and cardiac events during and after AVR. In the 2014 American College of Cardiology/American Heart Association guidelines (1), high-gradient AS is classified as a D1 stage of the disease, ...
