OBJECTIVES: To compare the hospital costs associated with two fluid resuscitation strategies for cirrhotic ascites: one with human albumin 20% (Vialebex ((R)) 20%) and one with polygeline. METHODS: Multicenter prospective randomized double-blinded comparative trial (that also compared efficacy and tolerance). The economic evaluation was based on direct medical costs throughout the follow-up period: days of hospitalization, hospital consultations, medical procedures, and fluid resuscitation products.. This cost-minimization study had a 6-month follow-up period. Daily costs in euros were adjusted over a 30-day period. The study was interrupted prematurely because of an alert due to the bovine origin of the polygeline, and the inclusion objectives could therefore not be met. RESULTS: The economic analysis included all patients in the efficacy population (group receiving human albumin 20%: n=30, polygeline group: n=38). It found a standardized cost per patient for 30 days of treatment that was ...
BACKGROUND: Balancing the beneficial effects of resuscitation fluids against their detrimental effect on hemostasis is an important clinical issue. We aim to compare the in vitro effects of 3 different colloid resuscitation fluids (4.5% albumin, hydroxyethyl starch [Voluven 6%], and gelatin [Geloplasma]) on clot microstructure formation using a novel viscoelastic technique, the gel point. This novel hemorheologic technique measures the biophysical properties of the clot and provides an assessment of clot microstructure from its viscoelastic properties. Importantly, in contrast to many assays in routine clinical use, the measurement is performed using unadulterated whole blood in a near-patient setting and provides rapid assessment of coagulation. We hypothesized that different colloids will have a lesser or greater detrimental effect on clot microstructure formation when compared against each other. METHODS: Healthy volunteers were recruited into the study (n = 104), and a 20-mL sample of whole ...
BACKGROUND:: Fluid extravasation may lead to myocardial edema and consequent reduction in ventricular function. Albumin is presumed to interact with the endothelial glycocalyx. The authors objective was to compare the impact of different resuscitation fluids (human albumin, hydroxyethyl starch, saline) on vascular integrity. METHODS:: In an isolated perfused heart model (guinea pig), Krebs-Henseleit buffer was augmented with colloids (one third volume 5% albumin or 6% hydroxyethyl starch 130/0.4) or crystalloid (0.9% saline). Perfusion pressure and vascular fluid filtration (epicardial transudate formation) were assessed at different flow rates. After global, stopped-flow ischemia (37 degrees C, 20 min), hearts were reperfused with the same resuscitation fluid additives. In a second series, the authors applied the respective perfusates after enzymatic digestion of the endothelial glycocalyx (heparinase, 10 U over 15 min). RESULTS:: Both 5% albumin and 6% hydroxyethyl starch decreased fluid ...
This is a prospective, controlled, before-and-after study. The baseline pre-intervention period will include collection of data while doctors and nurses are unaware that such collection is taking place. During this time, high chloride fluids (saline, Gelofusine, 4% albumin) will continue to be used according to standard practice with an estimated 30,000 liters of saline as well as 2,000 bottles of Gelofusine® being consumed.. Following a wash out period of education and preparation, there will be a complete shift to a working environment where use of saline, Gelofusine and any other fluids with a high chloride level (,110 mmol/L)will be restricted and substituted with fluids of lower chloride concentration similar to blood; either Hartmanns solution or Plasmalyte® or 20% albumin.. The study will compare a 6 month control period (before) and a six month intervention period (after). ...

CONTEXT: Administration of traditional chloride-liberal intravenous fluids may precipitate acute kidney injury (AKI).. OBJECTIVE: To assess the association of a chloride-restrictive (vs chloride-liberal) intravenous fluid strategy with AKI in critically ill patients.. DESIGN, SETTING, AND PATIENTS: Prospective, open-label, sequential period pilot study of 760 patients admitted consecutively to the intensive care unit (ICU) during the control period (February 18 to August 17, 2008) compared with 773 patients admitted consecutively during the intervention period (February 18 to August 17, 2009) at a university-affiliated hospital in Melbourne, Australia.. INTERVENTIONS: During the control period, patients received standard intravenous fluids. After a 6-month phase-out period (August 18, 2008, to February 17, 2009), any use of chloride-rich intravenous fluids (0.9% saline, 4% succinylated gelatin solution, or 4% albumin solution) was restricted to attending specialist approval only during the ...

Introduction: Strain echocardiography (echo) is increasingly recognized as an effective technique for predicting patient survival in a wide spectrum of diseases. We sought to evaluate the effectiveness of 2D speckle-tracking echo in predicting all-cause mortality in patients with isolated mitral regurgitation (MR).. Methods: Transthoracic echo examinations were retrieved for patients with mild to severe MR from 2010-2012. After excluding patients with previous CABG, other valvular lesions, valve replacement, congenital heart disease, or incomplete echos, conventional and strain echocardiographic parameters were obtained in 80 patients.. Results: Over mean follow up of 1020 ± 617 days, a total of 33 (41%) patients died. Patients had a mean age of 70 ± 15 years. Thirty (38%) had severe MR. Patients who died had significantly worse global longitudinal strain (GLS) (12.6±3.5% versus 15.8±4.2%; p,0.001). In a multivariate model including age, race, sex, BMI, creatinine, HTN, HLD, diabetes, ...

Gastric sodium loading results in an increase in the portal venous concentration of vasoactive intestinal peptide (VIP) and down-regulation of both the intrahepatic and circulating renin-angiotensin systems. In the present study we sought to determine whether an increase in the concentration of VIP in the portal circulation might act to down-regulate the intrahepatic and/or circulating renin-angiotensin systems. Male Sprague-Dawley rats were infused intraportally with haemaccel vehicle or VIP in haemaccel for 60 min. Livers were harvested and blood was sampled. Angiotensin-converting enzyme (ACE) activity and angiotensinogen, angiotensin I, angiotensin II and renin concentrations were measured. VIP infusion decreased hepatic ACE activity (P , 0.05), the hepatic angiotensinogen concentration (P , 0.001) and the hepatic angiotensin I concentration (P , 0.05). The plasma angiotensinogen concentration and serum ACE activity were also decreased by intraportal VIP infusion (P , 0.05 for each). Plasma ...
Coarctation of aorta(CoA), complicated by endarteritis in a children is very rare. Here we present a case of endarteritis in an unoperated CoA in a four year old boy. CoA had been diagnosed in the referring hospital, yet the diagnosis of endocarditis distal to CoA, was made in the tertiary center using modified transthoracic echo windows or focused views. After six weeks of intravenous antibiotic treatment, a coarctectomy and end-to-end anastomosis was performed and he recovered clinically well. This case report concludes that echocardiography remains as the standard diagnostic method for identifying intracardiac manifestations of infective endocarditis/endarteritis. Last but foremost, it delineates the importance of modified transthoracic echo windows or focused views in identifying the unusual position of endocarditis.. ...

Tissue engineering is considered one of the most important therapeutic strategies of regenerative medicine. The main objective of these new technologies is the development of substitutes made with biomaterials that are able to heal, repair or regenerate injured or diseased tissues and organs. These constructs seek to unlock the limited ability of human tissues and organs to regenerate. In this review, we highlight the convenient intrinsic properties of gelatin for the design and development of advanced systems for tissue engineering. Gelatin is a natural origin protein derived from collagen hydrolysis. We outline herein a state of the art of gelatin-based composites in order to overcome limitations of this polymeric material and modulate the properties of the formulations. Control release of bioactive molecules, formulations with conductive properties or systems with improved mechanical properties can be obtained using gelatin composites. Many studies have found that the use of calcium phosphate ...
Protein films possess good gas barrier properties at low to intermediate relative humidity but reduced water vapor barrier properties, which limits their application. The introduction of chemical or enzymatic modifications could be an alternative to improve the cohesion properties of the polymeric matrix due to the formation of cross-linkages, thus improving the barrier characteristics and the mechanical resistance and decreasing the film solubility. The objective of this study was to evaluate and compare the effect of chemical and enzymatic modifications on the solubility, mechanical properties, barrier properties, thermal properties and morphology of gelatin-based films. The results indicated that the treatments resulted in 20% reduction in solubility for all the modified films, whereas the greatest reduction in water vapor permeability was observed for the enzymatic modified films ( congruent to 35%) as compared to the native film. The mechanical properties were only significantly affected by ...

High-intensity cycle training among triathletes has a very detrimental effect on their sperm count, a study has found - and it could all be in the trousers.. Researchers studied a group of 15 Spanish triathletes with an average age of 33 and competing at both national and international level. Their training routines were analysed in detail, and particular note taken of the time they spent cycling each week. Sperm was taken after three days of sexual abstinence and analysed for their shape and form.. "We found a statistically adverse correlation between sperm morphology and the volume of cycling training undertaken per week," said Professor Diana Vaamonde, from the University of Cordoba Medical School, Cordoba, Spain.. "While all triathletes had less than 10 per cent of normal-looking sperm, the men with less than four per cent - at which percentage they would generally be considered to have significant fertility problems - were systematically covering over 300km per week on their ...

The 2015 Surviving Sepsis Campaign Bundle. We all know the "rules" for sepsis resuscitation - fluids, early antibiotics, MAP , 65, lactate normalization, culture every orafice you can find, etc. But its important to remember that ticking the boxes does not translate to adequate resuscitation and high quality patient care. But how did a patient whos management seemed so straightforward turn out to be a set-up for disaster left on the medical ward?. One of the most important concepts that is often left out of the commonly quoted "early antibiotics" mantra is appropriate antibiotic therapy. While it is often difficult to determine what is appropriate for each patient presenting with severe sepsis, there are a number of tools that must be considered what choosing your gorillacillin variant (local antibiogram, patients relationship with the medical community, previous culture data, etc.). Two articles that have been published within the past month highlight the impact of early vs. early AND ...

Rock physics is an essential link connecting seismic data to the properties of rocks and fluids in the subsurface. One of the most fundamental questions of rock physics is how to model the effects of pore fluids on rock velocity and density. Contemporary scientific computing allows geophysicists to conduct extremely complex virtual (computational) experiments on realistic digital representations of complex porous media, and thus directly relate the measurable properties of the media to their microstructure and saturation. Computational (digital) rock physics can also serve as an effective tool in examining new and existing rock physics models. The finite element method (FEM) has been proved effective in simulations of the linear elastic properties of porous rock under static conditions. In this thesis, FEM is used to study the effect of patchy saturation on elastic velocities of digital images of rocks. However, FEM belongs to a group of grid methods, and its accuracy is limited by ...

OBJECTIVE: To investigate the accuracy of measurement of intrathoracic blood volume index by single thermodilution (ITBVI*) and its sensitivity to detect changes in preload after cardiac surgery compared with conventional transpulmonary arterial dye dilution ITBVI and with conventional monitoring (central venous pressure [CVP] and left ventricular end-diastolic area index [EDAI] by transesophageal echocardiography).. DESIGN: Prospective clinical study.. SETTING: University hospital.. PARTICIPANTS: Nineteen patients immediately after cardiac surgery.. INTERVENTIONS: Volume loading was administered with 20 mL of oxypoligelatine (Haemaccel [Behringwerke Aktiengesellschaft Corp, Marburg, Germany]) 3.5% times body mass index over 10 minutes.. MEASUREMENTS AND MAIN RESULTS: Intrathoracic blood volume index was measured by dye dilution (ITBVI) and thermodilution (ITBVI*) immediately before and after volume loading. Measurements of ITBVI and ITBVI* correlated closely (r = 0.94; p , 0.0001). With volume ...
For years, jelly confectionery manufacturing has relied heavily on starch moguls. Rousselots SiMoGel allows manufacturers to replace the traditional starch moguls with silicone or metal moulds or even blisters, making the production process completely hygienic and ideal for use by nutraceutical confectionery producers. However, SiMoGel offers many other significant advantages to the confectionery industry. It can be used to significantly speed up the production process, as gelatin-based gummies without starch depositing require only a few minutes to dry instead of ca 24 hours. SiMoGel also eliminates the starch recycling stage, lowers energy consumption and labor time, while removing the need for storage or gelling rooms before demolding. With its multiple functional and processing benefits, SiMoGel can be used by a wide range of confectionery and nutraceutical manufacturers, to create end products with superior appearance, flavor release and transparency. The solution contains a customized ...
Abstract: Managing hemorrhagic shock is complex and difficult, and hemorrhage remains the leading cause of preventable deaths among trauma patients. There is an on-going debate on the best resusciative strategy. Many intensive care units practice massive transfusion protocols with resuscitation fluids composed of Packed Red Blood Cells (PRBC), Fresh Frozen Plasma and platelets in equal ratios, thus resembling whole blood. Some studies reported a great survival benefit from this relative high proportion of FFP, but most of this benefit has later been shown to be due to a survival bias. Trauma-induced coagulopathy (TIC) is present in 10-34 percent of trauma-patients at admission to the intensive care unit. A feared complication to massive hemorrhage includes the so-called «viscous cycle» of hypothermia, acidosis and coagulopathy, which is associated with increased morbidity and mortality. To understand the pathophysiology behind this, knowledge of hemostasis is needed. Endothelium, platelets and ...
