The pleural cavity is the thin fluid-filled space between the two pulmonary pleurae (known as visceral and parietal) of each lung. A pleura is a serous membrane which folds back onto itself to form a two-layered membranous pleural sac. The outer pleura (parietal pleura) is attached to the chest wall, but is separated from it by the endothoracic fascia. The inner pleura (visceral pleura) covers the lungs and adjoining structures, including blood vessels, bronchi and nerves. The pleural cavity can be viewed as a potential space because the two pleurae adhere to each other (through the thin film of serous liquid) under all normal conditions. In humans, there is no anatomical connection between the left and right pleural cavities. Therefore, in cases of pneumothorax, the other lung will still function normally unless there is a tension pneumothorax or simultaneous bilateral pneumothorax, which may collapse the contralateral parenchyma, blood vessels and bronchi. Purpose: The pleural volumes will deform during surgery portion of the pleural photodynamic therapy (PDT) of lung cancer when the pleural cavity is opened. This impact the delivered dose when using highly conformal treatment techniques. In this study, a finite element-based (FEM) deformable image registration is used to quantify the anatomical variation between the contours for the pleural cavities obtained in the operating room and those determined from pre-surgery computed tomography (CT) scans. Methods: An infrared camera-based navigation system (NDI) is used during PDT to track the anatomical changes and contour the lung and chest cavity. A series of CTs of the lungs, in the same patient, are also acquired before the surgery. The structure contour of lung and the CTs are processed and contoured in Matlab and MeshLab. Then, the contours are imported into COMSOL Multiphysics 5.0, where the FEM-based deformable image registration is obtained using the deformed mesh - moving mesh (ALE) model. ...

Case History: 4-years old normal short haired European type of female cat was presented to the clinic with sever dyspnoea and cyanotic mucous membranes. At the time the cat hospitalized she showed a severely disturbed general state. The cat was so apathetic and during percussion horizontal dullness was detected. Other organs out of thorax didnt show abnormalities. An intravenous catheter was installed for medication and blood sample were taken for hematology examination. Additional clinical examination like Thoracic radiography revealed lift-sided blurring of the thorax indicative for pleural effusion. By thoracocentesis purulent greenish fluid was obtained. The result of the cytological examination was septic exudates in pleural cavity. Diagnosis: Septic pleuritis (pyothorax) which is characterized by the presence of septic exudates and consequently bacterial contamination of the pleural cavity.. Devices and methods for providing access to a body cavity without substantial loss of inflation gas therein. The device comprises an access assembly 10 that may have a tubular member 12, at least one sleeve 16 coaxially disposed over the tubular member, a pneumostasis valve 13 attached to the tubular member, and an obturator 11 removably received in the tubular member. Sleeve 16 comprises expandable region 19 capable of forming an anchor 30 upon axial compression. A method to provide access using assembly 10 comprises insertion of the assembly 10 into the body cavity, expanding region 19, and insufflating the cavity with gas. Expandable region 19 on assembly 10 is axially compressed to form anchor 30 to seal against an interior wall of the body cavity. Due to the high yield of material for analysis from individual blastocysts, rabbit embryos will be used in the project. Non-enzymatic age-dependent protein modifications in blastocysts and cavity fluids of blastocysts from young (20 weeks) and old (130 weeks) females, diabetic and healthy donors will be analysed by two-dimensional (2D) gel electrophoresis followed by immunoblotting for AGEs. Modified proteins will be identified by mass spectroscopy and assessed by bioinformatics for functional networks (collaboration with SP5). We will continue collaborating with SP1, SP8 and SP2 on analyses of altered glycation and sialylation of embryonic tissues and fluids. The apparatus incorporates an exhaustion device having a standard luer lock lug receptacle enabling use of a multiple of different size and models of pleural cavity entrance devices having standard luer lock connecting lugs. The exhaustion device also incorporates a one-way valve on the luer lock lug receptacle for exhausting fluid from the pleural cavity and preventing fluid from flowing into the pleural cavity. I am a time bomb. Yes, he really dis say that lol. He said that he couldnt "clear" me for surgery but just assess the risks. My respiratory function is down. They prefer not to do surgery on anyone under a 1 and they definitely wont do it on anyone lower than a .75. Right now, i am a .83. At some point, he wants to do more testing, i believe. Im sorry but it is something of a blur. One of the concerns is that if they did the surgery i would end up permanently on a respirator. Personally, I wont have the surgery done if they the risk of it being permanent is high and i dont think they will do the surgery either. He sent me for more x-rays afterwards and it showed that there was more pleural effusion (fluid in the pleural cavity) but that it wasnt as bad as last time. At this point, no one has called to say that they want to drain me again and so they may wait to see how it goes. We came away feeling that there is no real answer at the moment, but it looks slimmer that they will be able to ...

Pleural fluid is a serous fluid produced by the serous membrane covering normal pleurae. Most fluid is produced by the parietal circulation (intercostal arteries) via bulk flow and reabsorbed by the lymphatic system. Thus, pleural fluid is produced and reabsorbed continuously. In a normal 70 kg human, a few milliliters of pleural fluid is always present within the intrapleural space.[2] Larger quantities of fluid can accumulate in the pleural space only when the rate of production exceeds the rate of reabsorption. Normally, the rate of reabsorption increases as a physiological response to accumulating fluid, with the reabsorption rate increasing up to 40 times the normal rate before significant amounts of fluid accumulate within the pleural space. Objective: To report the cases of 6 patients with fungus ball caused by Aspergillus fumigatus (aspergilloma) in the pleural cavity. Methods: Between 1980 and 2009, 391 patients were diagnosed with aspergilloma at the Santa Casa Hospital Complex in Porto Alegre, Brazil. The diagnosis of aspergilloma in the pleural cavity was made through imaging tests revealing effusion and pleural thickening with air-fluid level; direct mycological examination revealing septate hyphae, consistent with Aspergillus sp.; and positive culture for A. fumigatus in the surgical specimen from the pleural cavity. Results: Of the 391 patients studied, 6 (2%) met the established diagnostic criteria. The mean age of those 6 patients was 48 years (range, 29-66 years), and 5 (83%) were male. The origin of the myofibroblast in fibrotic lung disease is uncertain, and no effective medical therapy for fibrosis exists. We have previously demonstrated that transforming growth factor-β1 (TGF-β1) induces pleural mesothelial cell (PMC) transformation into myofibroblasts and haptotactic migration in vitro. Whether PMC differentiation and migration occurs in vivo, and whether this response can be modulated for therapeutic benefit, is unknown.
